Arthrosis: new diagnostic procedures.

The authors emphasize two new methods for determining the real state of the cartilage and bone structure. These methods permit the early diagnosis of arthrosis and necrosis before X-ray changes appear and perhaps a better management and prognosis.

[Necrosis and ischemia of the femoral head during arteritis of the lower extremities. Survey of 138 patients with arteritis and 159 aortographies].

Among 138 subjects suffering form arteritis of the lower limbs, the authors found clinical signs of lesions in one or both hips in 47.6 percent of cases and radiological signs in one or both hips in 42.8 percent of cases. Five patients presented a distinct flattening of the femoral head indicating necrosis at stage III, 19 showed voluminous or multiple dense islets in the femoral collar or head indicating necrotic lesions at stage II, and 26 showed narrowing of the superior joint space mostly on the internal side. The authors think ihat the latter sign indicates ischaemic chondropathy. Eleven of the 138 patients had undergone pertrochanteric phlebography : this was abnormal in all 11 cases. At the same time the authors studied the coxofemorals on 159 aortograms and found, among other anomalies, 2 cases of advanced necrosis at the stage of rupture (stage III). This study tends to confirm the occurrence of necrotic lesions of the femur that are secondary to arteriopathies.

[Circulation in bone in Paget's disease. General review and personal results].

On the basis of a review of work concerned with the circulation in bones affected by Paget's disease and from their own results, the authors show that the vascularization of affected by Paget's disease is completely different from that of normal bone. The periostal vessels are abnormally developed and result in a high flow in the affected limb. The cortical vessels are numerous and of all types, forming a system of canalization of considerable volume. On the other hand the medullary circulation is encumbered by stasis, oedema, and fibrosis. The earliness of the vascular lesions makes it possible to envisage a vascular theory in connection with Paget's disease.

[Lumbar arachno-epiduritis. Study of 20 cases].

The authors report twenty cases of lumbar arachnitis, from the clinical, biological, and neuroradiological points of view; a discussion is developed in connexion with various etiological factors centred, on the one hand, on the responsibility of the contrast media used in neuro-radiology and, on the other hand, on that of the act of surgery, a factor that was encountered in half of the cases reported. The possibilities of therapy are also discussed.
