Direct vision internal urethrotomy. A prospective study of 81 primary strictures treated with a single urethrotomy.

The post-operative treatment of urethral strictures as well as the criteria of success have varied considerably. In order to find a solution to this problem, we undertook a prospective evaluation of 81 men treated for their first urethral stricture with a single urethrotomy. Fifty-one patients with a stricture 5 mm or less in length were catheterised post-operatively for 3 days. Patients with strictures longer than 5 mm were catheterised for 3 weeks (14 patients) or 6 weeks (16 patients). All were followed up for a minimum of 24 months. The stricture recurred in 33 patients (41%). We found that 3 days' catheter drainage was sufficient for patients with short strictures. Whether strictures measuring more than 5 mm might be treated similarly needs further investigation.

Nonspecific psychological distress, psychosocial stressors and delusional psychosis. A comparison with psychiatrically symptom free surgical controls.

The purpose of the study was to examine patients with delusional psychosis regarding characteristic personality traits as illustrated by nonspecific psychological distress, and psychosocial stress prior to onset of the psychosis. The design of the study was a comparison of first-admitted psychiatric patients with delusional psychosis and psychiatrically symptom free surgical controls. Patients with delusional psychosis reported significantly more psychosocial stress than controls during the 3 months prior to admission. They also reported significantly higher mean values than controls on the subscales named poor self-esteem, helplessness-hopelessness, dread, sadness, anxiety and approval of rule breaking on the schedule of nonspecific psychological distress. In discriminating patients with delusional psychosis and surgical controls, approval of rule breaking, helplessness-hopelessness, and severity of psychosocial stressors were the most informative variables. Patients with delusional psychosis will be followed up in order to analyze if nonspecific psychological distress and psychosocial stressors are of importance for the prediction of the course and the outcome.

Susceptibility in urinary bladder cancer: acetyltransferase phenotypes and related risk factors.

The excess proportion of slow acetylator phenotypes has previously been found among bladder tumour patients. Among 228 bladder tumour patients in the present study, the slow acetylator phenotype was significantly associated to the unskilled worker and T3-T4 tumour, and to a certain extent to tobacco smoking. It is suggested that slow acetylator subjects are more susceptible to bladder tumour development when exposed to arylamines.

Comparison of splenoportography and transhepatic portography in the diagnosis of portal vein thrombosis.

Accurate diagnosis and localization of thrombosis in the portal venous system is essential for proper surgical treatment. We compared the results of percutaneous transhepatic portography and splenoportography in 66 patients with cirrhosis of the liver. The two methods agreed on absence of thrombosis in 48, and in presence of thrombosis (verified later by surgery/autopsy) in four patients. In 12 patients an apparent thrombosis diagnosed by splenoportography was disproved by transhepatic portography, and vice versa in two patients. Free portal pressure and splenic pulp pressure did not differ significantly irrespective of the 'diagnosis' of thrombosis. We conclude, that transhepatic portography is better than splenoportography in making the diagnosis of thrombosis in the portal venous system although failure to visualize the splenic vein may indicate splenoportography.

Prediction of educational achievement, behaviour and health at school using information from infancy health service.

The study is a 12-year follow-up of all risk-children (652) and a random sample of control-children (626) born during 1968 in a delimited geographical area (Aarhus county). Information from the infancy period was obtained from the health nurses' schematic records and were concerned with family background variables, life events which had occurred during the first year of life, perinatal risk-factors and indices of health care during the first year of life. Information at 12 years of age was obtained through the records in the school health service. Multivariate predictor analyses were performed. Significant educational difficulties, which 16.6% experienced, were pronouncedly predicted from indices of low socio-economic status, child abuse and neglect and male sex. The prevalences at 12 years of age of mental disorders psychosomatic (6.7%), behaviour disorder (3.3%), and mental retardation (1.3%) were predictable from indices of social and psychosocial stress including the health nurses' assessment of inadequate care during infancy period, but with some differences among the diagnostic groups. The results show that unfavourable social factors, psychosocial stress and inadequate care are informative variables for delimitation of risk-groups in infancy.

Urodynamic investigation predicts bladder dysfunction at an early stage after abdominoperineal resection of the rectum for cancer.

In a prospective study, 25 consecutive patients (14 men and 11 women) who underwent abdominoperineal resection of the rectum were urodynamically investigated before surgery and 10 days, one month, and three months after the operation. Nine men and three women (48% and 95% confidence limits; range 28% to 69%) developed voiding disturbances 10 days after the operation. One man and one woman developed incomplete lower motor neuron lesion (8% and 95% confidence limits; range 1% to 26%). At follow-up 3 months after surgery, five patients (20% and 95% confidence limits; range 7% to 41%) had still voiding disturbances: two patients with lower motor neuron lesion and three men with bladder outlet obstruction caused by prostatic hypertrophy. The three men underwent transurethral resection of the prostate 6 months after excision of the rectum. It is concluded that the risk of neurogenic bladder and voiding dysfunction is a minor problem. Cystometry performed before removal of the vesical catheter around the tenth postoperative day will diagnose patients with neurogenic bladder dysfunction. If voiding symptoms persist beyond 3 months after the operation, further urodynamic investigations should be undertaken to find patients with bladder dysfunction not due to lesion of the peripheral parasympathetic nerves to the bladder, which could demand surgery.

N-Acetyltransferase phenotypes in the urinary bladder carcinogenesis of a low-risk population.

N-Acetyltransferase phenotypes were measured in 228 patients with tumours of the urinary bladder and 100 non-cancer controls. 63.6% of the patients and 54.0% of the controls were slow acetylator phenotypes (p less than 0.10). In general, 65% of bladder tumour patients and 59% of controls are slow acetylators (p less than 0.025). N-Acetyltransferases are involved in the metabolism of carcinogenic arylamines and thus could be an important factor in interindividual susceptibility to these agents.

Retention of urine in genital herpetic infection. Survey and case report.

During the last years several cases of acute urinary retention, due to genital herpetic infection, have been described, especially in youngsters. The condition is caused by neurogenic dysfunction due to a sacral meningomyelitis. The clinical picture before the acute attack is characterized by painful genital eruptions; fever and malaise; tender inguinal lymph nodes; paresthesia in the perineum and on the inside of the thigh, and obstipation. Skin and mucous membrane manifestations are often sparse or located so that they are easily overlooked. The treatment is bladder drainage until the patient is able to void spontaneously. Most of the patients regain normal bladder function within 10 days, but bladder dysfunction for 5 weeks has been described. We present a typical case of acute urinary retention due to genital herpetic infection.

Haematuria.

Haematuria due to lesions such as haemangiomas, varicose veins etc. are seldom described. We present two patients with rare lesions causing gross haematuria that caused diagnostic problems since all the clinical investigations were negative. The lesions were diagnosed peroperatively with the use of a pyeloscope. In this way a nephrectomy was avoided.

Gastric cancer with special reference to prognostic factors. A review of 779 cases.

A series of 779 patients with gastric cancer diagnosed in the 20-year period 1960-1979 is presented. Surgery was performed in 86% of the cases, but was judged to be radical in only 28%. The operative mortality in the total period was 14.3%, with a fall in the final 5-year period. Patient age, stage of cancer and radicality of operation were prognostic factors as regards survival time. The overall 5-year survival rate was 12%. The rate was 37% after radical surgery, increasing to 49% when corrected for age and sex. The survival rate was unchanged during the period of the study. The crude 5-year survival among the patients with radical surgery was 41% and was independent of the type of surgery. Median age was significantly lower in the patients with early-stage cancer than in the series as a whole. The numbers of younger patients increased during the last ten years of the study.

Repeated bladder distension by the Cystomat in the treatment of detrusor instability.

Treatment of idiopathic detrusor instability with drugs and prolonged distension of the bladder is often unsatisfactory. Some amelioration of symptoms has previously been described by using the Cystomat. The present study showed that 1 week's treatment with the Cystomat produced convincing subjective improvement as well as an increase in bladder capacity. The improvements were statistically significant and persisted for 3 months. Although the mechanism by which bladder distension exerts its effect is poorly understood, the technique of repeated distension offers an alternative in the treatment of patients with idiopathic detrusor instability.

Acute retention of urine due to benign prostatic obstruction treated with alpha-adrenergic blockers.

Bladder catheterisation is the usual treatment in patients with acute retention of urine due to prostatic hypertrophy. The interference is not without complications why medical treatment would be a progress. The prostatic gland has been found to contain alpha-adrenergic receptors, and it has been argued that some cases of acute retention of urine is caused by over-stimulation of the alpha-adrenergic receptors. In the present study we have investigated the effect of intravenously administered phentolamine ( Regitin ) in patients with acute retention of urine, due to prostatic hypertrophy, and find that the treatment is an alternative to conventional bladder catheterisation.

Alpha-adrenergic blockade in the diagnosis of detrusor instability secondary to infravesical obstruction.

Detrusor instability secondary to infravesical obstruction is probably caused by overactive stretch receptors in the bladder wall. Since recent studies have shown that alpha-adrenergic blockade with phenoxybenzamine reduces the intravesical pressure under micturition, we undertook this investigation in order to see if phenoxybenzamine has an effect on detrusor instability caused by infravesical obstruction. We found that the detrusor instability was abolished by phenoxybenzamine, which supports the 'stretch receptor theory'. Further, phenoxybenzamine is probably a good test in the diagnosis of detrusor instability secondary to infravesical obstruction.

A predictor analysis of patients' assessment of outcome after operation for duodenal ulcer. A one-year prospective study.

During a period of one year data were obtained concerning life events, non-specific psychological symptoms, individual social history and ulcer history in consecutive cases of patients about to undergo elective surgical treatment for duodenal ulcer. At a one-year follow-up, a blind clinical evaluation was performed, and information concerning the patients' assessment of outcome was obtained. Those patients who at the one-year follow-up stated no improvement due to the operation could be predicted to some extent from postoperative complications, partly from a long ulcer history. The patients who stated that the result did not come up to their expectations were predicted from older age, and from certain symptoms, especially severe headache. It is suggested that it is relevant to apply the patients' assessment of outcome for the purpose of evaluation, supplementing the clinical assessment of the more biomedical aspects of outcome.
