A malignant papillary mesothelioma of the tunica vaginalis testis with retroperitoneal lymph node and pulmonary metastases is reported in a seventy-seven-year-old man. Electron microscopy of a local recurrence documented its mesothelial origin. The clinical and pathologic features of these rare neoplasms are discussed.

A multifocal leiomyosarcoma of the stomach originating from the muscularis mucosae with lymph node and distant metastases is described in a 66 year old man. The electron microscopic features of a representative tumor mass and a metastasis confirmed the smooth muscle histogenesis. The light microscopic appearance consistently suggested malignant fibrous histocytoma. The pathological features of gasttric leiomyosarcomas are reviewed with special emphasis on the problem of practical diagnosis. This case also indicates that not all sarcomas with storiform features are necessarily histiocytic in origin.
An unusual example of Kaposi's sarcoma is reported in which the patient presented with a history of intractable diarrhea of 18 months duration. The roentgenographic and endoscopic findings were those of a segmental colitis. The diagnosis was established following the appearance of skin lesions and after a subtotal colectomy was performed. Kaposi's sarcoma was present in both the skin and colon.
A rare lung tumor composed of contiguous foci of classical blastoma and carcinosarcoma with transitional features is described. Based on these observations and a review of the literature, we propose that pulmonary carcinosarcomas are composed of a heterogeneous population of tumors. This case supports the concept that blastomas are a distinctive subtype of carcinosarcomas. An unusual feature in the clinical presentation was the expectoration of an endobronchial portion of tumor.

Platelet monoamine oxidase (MAO) activity was studied in 31 individuals suffering from early childhood autism and was not significantly different from that found in normal children or adults. In the autistic children, MAO activity decreased with age, and there was a trend toward greater platelet MAO activity in prepubertal and pubertal male autistic children relative to normal male children. Total platelet counts were not elevated in autistic children.
Cystic adventitial degeneration (CAD) of the arteries is an uncommon cause of intermittent claudication in adults. The majority of reported cases have involved the popliteal artery and have occurred predominantly in men, and all had clinical evidence of occlusive vascular disease. An unusual example of CAD of the common femoral artery masqueraded clinically as an aneurysm, but the arteriogram was normal. It involved 5 cm of the artery and half its circumference, and was resected and replaced with a saphenous vein graft. The cyst contained clear mucoid fluid and a fibrous wall without a cyst lining. This is the eighth case reported of CAD involving the proximal segment of a large lower extremity artery. The etiology is unknown, but many authors think such cysts arise from primitive "arthrogenic" mesenchyme in the adventitia of blood vessels.
A 23-year-old patient developed a carcinoma in the wall of a breast cyst during the second trimester of an otherwise uncomplicated pregnancy. A review of the literature revealed 7 reported cases of carcinoma of the breast arising in a cyst wall. However, none of these these cases has been associated with pregnancy. The relationship of fibrocystic disease and carcinoma of the breast is discussed. The possible influence of pregnancy and age on the prognosis of this patient are also considered.
1. Inactivation of beta-phenylethylamine and several of its derivatives was studied in a preparation of rabbit lung perfused with Krebs physiological medium at 37 degrees C. Inactivation or removal of these compounds was calculated as the difference between the concentration of each amine in the perfusion medium and the effluent, collected separately from each lung. The extent of amine metabolic degradation was also measured, by column chromatography, in lung effluent. 2. With this technique the magnitude of amine removal as a function of concentration was determined and an apparent Km and Vmax of removal were calculated for each amine. 3. Percentage removal was highest with phenylethylamine (95%), and decreased, apparently in relation to increasing phenyl- and side chain-hydroxylation (and therefore likely increased hydrophilicity), and 5-hydroxytryptamine (64%), tyramine (53%), octopamine (35%), dopamine (32%) and noradrenaline (23%). 4. Inactivation of each amine could be accounted for by metabolic degradation to deaminated products, which appeared in lung effluent within 90 s of beginning amine perfusion. 5. When intrapulmonary metabolism of phenylethylamine was inhibited by simultaneous perfusion with semicarbazide (10 mM) and pargyline (10 micronM), the removal rate was unaltered, establishing that uptake of the amine from the vascular space is not dependent on metabolism at least for 4 min infusions.
Extensive hormonal evaluation was performed in a girl with adrenal carcinoma during the primary tumor stage, following adrenalectomy, during the period when metastases were evident and while on treatment with o,p'-DDD. At the age of 14 months a diagnosis of congenital adrenal hyperplasia was made and treatment with dexamethasone (0.125 to 0.25 mg/day) resulted in a fall-off in growth rate, normal advancement in bone age, decrease in virilization and suppression of 17- ketosteroid excretion which continued until 4 3/12 years of age when virilization increased. At five years of age elevated serum and urinary androgen levels unsuppressible with dexamethasone were noted. Following removal of a large right adrenal carcinoma, serum and urinary hormone levels returned to normal. There months following surgery, liver metastases were documented associated with elevated levels of serum androgens. With o,p'-DDD treatment, serum dehydroepiandrosterone sulfate (DS) and urinary 17-ketosteroid (17-KS) excretion fell rapidly while there was a delay in the fall of free androgens. The persistence of free steroid secretion with decreased formation of DS suggests that the o,p'-DDD may have altered sulfatase activity before causing tumor necrosis and total decrease in steroidogenesis.
The transnasal transsphenoidal approach to the sella turcica has been refined and perfected through advances in medicine and technology. Originally performed for excision of pituitary adenomas, its present primary use is pituitary ablation is hormonally dependent diseases. Our study of 55 patients who have undergone this procedure during the years 1970-1975 demonstrates that this microscopic technique gives excellent exposure and visualization of intrasellar contents. Our method gives excellent exposure and visualization of intrasellar contents. Our method with several instrument modifications is discussed. It is technically an easy procedure with a minimum of morbidigy and mortality.
True laryngeal adenomas are considered extremely rare and some doubt their existence. Laryngeal lesions having oncocytes as the predominant cell type have been described in the literature under a variety of different names: oncocytoma, oncocytic papillary cystadenoma, papillary cystadenoma, oxyphilic granular cells adenoma, etc. The oncocytes originate from changes in normal acinic and ductal cells. The significance of these changes is controversial. Whether the oncocytic laryngeal lesions represent true neoplasms or are degenerative in nature remains to be determined. We present clinical and histopathological details of four cases of oncocytic lesions of the larynx and compare their histology with microscopic findings in 30 cases filed as laryngeal cysts at the Minneapolis V.A. and University of Minnesota Hospitals. While the controversy regarding the neoplastic versus degenerative etiology of these lesions could not be resolved from a histopathological standpoint, their clinical behavior is that of a benign neoplasm. We review the literature on the subject and suggest a course of treatment for these lesions.

Metabolism of mescaline by several rabbit tissues was examined in vitro. Mescaline-oxidizing activity (micromoles per milligram of protein/15 min) of lung homogenates was 4 times greater than that of either liver or kidney. Brain and plasma each had comparatively little capacity to metabolize mescaline. Mescaline metabolism in vitro was sensitive to inhibition by semicarbazide. Removal of mescaline from the medium perfusing the isolated rabbit lung was explained by intrapulmonary metabolism. Semicarbazide (10(-3) M pargyline. Semicarbazide-treated lungs accumulated more mescaline than did untreated lungs. Mescaline efflux from lung was slower than that of its metabolite. These results indicate that the intact lung removes perfused mescaline and may be important in the disposition of circulating mescaline in vivo.
