Iontophoretic anaesthesia of the tympanic membrane.

Anaesthesia of the tympanic membrane by iontophoresis of lidocaine is a painless and safe method for myringotomy and grommet insertion. Thirty-nine patients with serous otitis media and 11 patients with Ménière's disease had their tympanic membranes anaesthetized by iontophoresis of lidocaine; analgesia was perfect for all of them. In the patients with serous otitis media, the insertion of grommets was carried out easily and without pain. Major problems or complications have not been encountered during this study.

Distribution of ribonucleic acid coliphages in south and east Asia.

We investigated the distribution of ribonucleic acid (RNA) coliphages in the Philippines, Singapore, Indonesia, India, and Thailand by collecting sewage samples from domestic drainage in November 1976. Of the 221 samples collected from domestic drainage, 50 contained RNA phages (52 strains). By serological analysis, 46 of the 52 strains were found to belong to group III. It can thus be said that the most prevalent RNA phages in Southeast Asia (at least, in the Philippines, Singapore, and Indonesia) were group III phages. Investigations of sewage samples collected from domestic drainage in Japan indicate that the most prevalent RNA phages in mainland Japan (north of Kyushu) are group II phages, whereas group III phages are predominant in the southern part of Japan (south of Amamiohshima Island). We therefore propose a borderline between Kyushu and Amamiohshima Island for the geographical distribution of RNA coliphages in the domestic drainage of South and East Asia. Moreover, one strain (ID2) was inactivated to some extent with the antisera of four groups of RNA phages. This is thought to be significant from the evolutionary viewpoint.

Synthesis of 5''-amino-3,5''-dideoxybutirosin A.

The titled compound was prepared by condensation of 3'-deoxyparomamine derivative (5) with 2,3-O-bis(p-nitrobenzoyl)-5-O-tosyl-D-xylofuranosyl bromide followed by 1-N-acylation with the active ester of (S)-4-benzyloxycarbonylamino-2-hydroxybutyric acid. The compound was slightly more active than 3'-deoxybutirosin A against Pseudomonas.

Fatal measles (rubeola) pneumonia in adults.

Fatal measles (rubeola) pneumonia, confirmed by viral culture and electron microscopy, occurred in a previously healthy 19-year-old man. Autopsy disclosed measles pneumonia but no recognizable disease that would predispose to such an infection. However, the prolonged course of the measles infection and very low levels of serum antibodies to measles indicate that an immune deficient state existed. Nearly all other adults dying of measles pneumonia have had impairment of the immune system, typically due to a lymphatic or hematologic malignant neoplasm treated with chemotherapy. An immunodeficient state may be a precondition for death from measles pneumonia in adults. Enlargement of air spaces with fibrosis in the anterior portions of both lungs is suspected to be due to tissue necrosis and high mechanical ventilatory pressures.

Pathogenesis of intracranial arachnoid cysts.

The pathogenesis of intracranial arachnoid cysts is reviewed. Light and electron microscopic features of arachnoid cysts are presented in support of the hypothesis of Starkman et al, that these cysts are truly intraarachnoid in location and are formed by splitting or duplication of the arachnoid membrane. Documentary evidence is provided to prove that Richard Bright's original description in 1831 contains these concepts, which have passed unnoticed for nearly a century and a half. A brief description of the meninges and the subarachnoid spaces is given.

Frequent appearance of radiation-induced transformation in junctional areas of colonies.

Morphological observations on induction of in vitro transformation were made in X-irradiated mouse embryonic 10T-1/2 cell line. Morphology of the cells growing in interior of the developing colony was almost uniform, whereas giant, morphologically altered, and transformed cells appeared in the contact area between two and three neighboring colonies. All of 10 transformed foci appeared in the contrast area between 2 and 3 neighboring colonies. This suggests that the contact of colonies was an important process for the expression of radiation-induced transformation.
