Pepleomycin-induced pneumonitis resulting in death.

Two cases of fatal pneumonitis induced by pepleomycin, a new antitumour antibiotic analogous to bleomycin, are described. In the first case, the patient received radiation in combination with intra-arterial administration of pepleomycin to a total dose of 100 mg. for the treatment of an epidermoid carcinoma of the maxilla. The pulmonary changes were noted two weeks after cessation of the chemotherapy and the patient died of pulmonary insufficiency despite intensive therapy with antibiotics and a steroid. The other case with an epidermoid carcinoma of the cheek and oropharynx was treated by radiation and pepleomycin given to a total dose of 205 mg. Pepleomycin was replaced by predonin soon after bilateral shadows were noticed on a chest radiograph, but the patient died of pneumonitis 10 months later. Various factors which may affect the development of pulmonary toxicity by pepleomycin are discussed.

Inhibition of in vitro neutrophil responses to chemotactic factors by piroxicam.

The effect of piroxicam on polymorphonuclear neutrophils (PMN) functions induced by several stimuli was evaluated in vitro. Preincubation of rabbit or human PMN with piroxicam inhibited the cellular responses elicited by N-formyl-methionyl-leucyl-phenylalanine (FMLP) such as superoxide anion (O2-) generation, granule enzyme release and chemotaxis. The effectiveness of piroxicam on each response was superior to those of indomethacin and ibuprofen. Also when either concanavalin A, zymosan-treated serum or ionophore A23187 was used as stimuli, piroxicam inhibited O2- generation of PMN. The inhibitory effect of piroxicam on FMLP-induced O2- generation was dependent on the concentration of stimuli and was reversed by increasing the extracellular calcium concentration. In addition, piroxicam had no effect on the activity of a chymotrypsin-like esterase, N-acetyl-phenylalanine-beta-naphthyl esterase, isolated from rabbit PMN. These results suggest that at least some of the anti-inflammatory effects of piroxicam may be mediated by affecting PMN functions, and the inhibition of O2- generation of PMN by piroxicam may be related to its capacity to modulate the association of calcium with these cells.

Unusual nerve endings found in the rat palatine mucosa.

The fine structure of an unusual type of nerve ending found in the subepithelial spaces of rat palatine mucosa is described. The endings, with their tortuous courses and terminal bulbous portions, form a complicated plexus. An unusually thick basal lamina of filamentous reticular structure envelops the Schwann cell ensheathing the endings. Several irregular microvillous projections extend from the axon terminals through gaps in the Schwann cell envelope. The nerve terminals present numerous mitochondria, microtubules, neurofilaments, myelin figures and clear vesicles approximately 50 nm in diameter. The laminar cytoplasm of Schwann cells enveloping the nerve terminals shows a large number of caveolae. Desmosome-type junctions are noted between axons and Schwann cells.

[A case of familial breast cancer].

The occurrence of breast cancer in 5 members of a family is reported. No specificity of the location, staging or histological typing of the lesions was observed in these cancers. But the characteristics of "hereditary breast cancer", early onset and the occurrence of metachronous double cancers, were observed. As for heredity, it is suspected to be autosomal dominant trait; on the other hand it is more appropriate to assume a poly genic pattern. Recognizing the importance of familial cancer occurrence, it may be possible to detect early cancerous lesions in family members.

Acute antihypertensive effect of nifedipine by sublingual route in cases with clinically severe systolic hypertension. A study up to 4 h after administration.

In a multi-center study, nifedipine (Bay a 1040, Adalat) (10 mg capsule) was administered, in liquid form and via sublingual route, to 22 cases who were diagnosed to have clinically severe systolic hypertension, and the depressor effect of the treatment was studied over a period of 4 h. In patients of "emergent" admission (cerebral hemorrhage n = 8, cerebral thrombosis n = 4, subarachnoid hemorrhage n = 1, renal failure n = 6, essential hypertension n = 3), 3 bouts of blood pressure measurement at intervals of 10-15 min during the control period were carried out. In case of systolic blood pressure higher than 200 mmHg, at least one time, nifedipine at a dose of 10 mg was sublingually administered. Thereafter, blood pressure and pulse rate were recorded at intervals of 15 min up to the end of the first one hour, then at intervals of 30 min up to the end of 4 h. Results were as follows. In terms of the average values for all cases, blood pressure fell to near lowest levels by the end of about 30 min after the administration and it stayed at near lowest levels up to the end of 120-240 min. During this period of time, pulse rate remained substantially unchanged. In terms of the pattern of the blood pressure fall, all cases could be classified generally into two types, namely (a) the "dip" group (7 cases) in which blood pressure fell to the lowest level to form a "dip" and remained below the control level, although it showed a trend to return to control level for 4 h and (b) the "flat" group (15 cases) in which blood pressure declined gradually for about 1 h and, then, remained low or below the control level, although it showed a trend to return to control level throughout the rest of the observation period of 4 h.

[Somatosensory-visual-vestibular interaction in a case of vermis agenesis].

It is well known that the somatosensory input is important for keep a balance as well as the visual and the vestibular inputs. However the knowledge of somatosensory-visual-vestibular interaction is incomplete at now. We have already made a mathematical model of these interactions by extension of the Raphan-Cohen model (Raphan et al, 1979) for visual-vestibular interaction with a direct and an indirect somatosensory pathway. This indirect somatosensory pathway has access to the common visual-vestibular velocity storage integrative mechanism. So the purpose of the present experiment was to verify whether such an extended model (Fig. 4) can be used to describe the nystagmus due to stimulation of the sensory system apart or in combination in case of vermis agenesis with Dandy-Walker's cyst. It was checked, on the other hand, where the lesion are extended. And moreover it was suspected that each parts of the mathematical model are compared with any central neural regions. As a result, the indirect pathways of visual, vestibular and somatosensory inputs and the direct pathways of visual and somatosensory inputs were disturbed completely. The disturbance of the indirect pathways was caused by lesion in the velocity storage integrator which was though to be included in the vestibular nuclei in monkey. The lesion in epsilon 3-h2 was suspected to explain a blockage of the direct visual pathway, and a disturbance of the direct somatosensory pathway was suspected the lesion in gs. We could explain this cause by a mathematical model for somatosensory-visual-vestibular interaction, so we could conclude that this model was useful for human beings as well as monkey.(ABSTRACT TRUNCATED AT 250 WORDS)

[Parathyroid carcinoma--a case report and a review of the Japanese literature].

The patient, a 42-year-old female, presented with bone pain and a cervical tumor. The tumor was operated at another hospital under a diagnosis of thyroid adenoma and the bone pain subsequently disappeared. Three years later, a cervical tumor was palpated just under the operative scar; the patient experienced nausea, vomiting. The tumor and the left thyroid lobe were removed and modified radical neck dissection was performed. The histological diagnosis of the resected specimen was parathyroid carcinoma.

An immunohistochemical study of hepatoblastoma producing human chorionic gonadotropin.

High tissue concentrations of human chorionic gonadotropin (hCG) and alpha-fetoprotein (AFP) were revealed by radioimmunoassay in a hepatic tumor which was surgically removed from a two-year, eight-month-old boy manifesting sexual precocity. Histologically the tumor showed an embryonal type hepatoblastoma admixed with multinucleated tumor giant cells. The localization of native hCG and hCG-beta subunit was immunohistochemically demonstrated in some of the above giant cells, while that of alpha-fetoprotein was diffusely in the tumor cells of hepatoblastoma of embryonal type. Ultrastructural features of the giant cells were also presented.
