Sulfisoxazole prophylaxis of middle ear effusion and recurrent acute otitis media.

The efficacy of sulfisoxazole prophylaxis was evaluated in 32 otitis-prone children in a double-blind cross-over clinical trial. During the sulfisoxazole therapy, seven patients (22%) had nine episodes of acute otitis media (AOM) while 20 patients (63%) receiving placebo had 36 episodes of AOM (P = .001). Although sulfisoxazole appeared to be beneficial in patients aged 2 to 5 years, statistically significant efficacy was noted only in children under 2 years of age. Otitis media with effusion persisting for more than five weeks was observed in ten children (31%) during sulfisoxazole therapy and in 14 children (44%) during the placebo period (P greater than .975). Sulfisoxazole therefore appears effective in preventing recurrent symptomatic AOM but not in reducing the frequency of persistent otitis media with effusion. The importance of careful follow-up of children receiving long-term sulfisoxazole therapy for prevention of recurrent AOM is stressed.

Acute Disease↗

Spastic paraparesis, mental retardation, and cutaneous pigmentation disorder. A new syndrome.

Four siblings in a family with a highly consanguineous background presented with an unusual combination of spastic paraparesis, muscle wasting, microcephaly, mental retardation, skeletal deformities, and cutaneous manifestations, ie, hypopigmented and hyperpigmented lesions and graying of the hair. An extensive workup including electromyography, muscle biopsy, and chromosomal analysis was unrewarding. An autosomal recessive inheritance is probable. A similar entity was recently reported from israel. The possibility that this previously unrecognized condition represents a new syndrome is suggested.

Retarded skeletal maturation in children with primary enuresis.

Primary nocturnal enuresis (PNE) is a common paediatric problem of multifactorial aetiology. Growth and skeletal maturation were studied in 35 otherwise healthy children with PNE, 26 boys and 9 girls aged 6-14 years, and comparison was made with a control (CTR) group of 19 boys and 3 girls aged 6-13 years of similar ethnic origin. There was no significant difference between the mean height and weight centiles of the two groups. Bone age (BA) determined by the TW-2 method showed a significant lag behind chronological age (CA); the CA-BA difference being 1.46 +/- 1.56 years in the PNE group and -0.08 +/- 0.8 years in the CTR group (P less than 0.001). In 11 of the PNE group (31%) the BA retardation was greater than 24 months: in 4 it was between 24 to 36 months and in 7 the difference was greater than 36 months. In all these children T4 and TSH were found to be normal. It is hypothesised that the retarded bone age in children with PNE may reflect delayed maturation of regulatory CNS functions.

Reduction of serum theophylline levels by terbutaline in children with asthma.

We studied the effect of oral terbutaline on serum theophylline levels in 12 children with asthma. Sustained-release theophylline (10 mg/kg twice a day) was given with placebo or terbutaline (0.075 mg/kg three times a day) in a chronic, randomized, double-blind, crossover design. The trough serum theophylline concentration fell from 13.8 +/- 4.0 to 10.8 +/- 3.6 micrograms/ml and the peak expiratory flow rate increased from 285 +/- 30 to 310 +/- 29 L/min after terbutaline. Further investigation is needed to clarify the mechanism of action by which terbutaline decreases serum theophylline levels.

Deciduous tooth eruption in Israeli children. A cross-sectional study.

Deciduous tooth eruption (DTE) was studied in 366 Jewish Israeli children: 193 boys and 173 girls. The number of erupted teeth (NET) at different ages was similar in both sexes. No statistically significant correlation was found between NET and anthropometric measurements at birth (weight and length) or subsequently, after eliminating the influence of age, between NET and weight, height, and head circumference. The high normal variability in NET at a given age is demonstrated.

Legg-Calve-Perthes disease following transient synovitis. How often?

Forty-one children with transient synovitis of the hip and 10 children with Legg-Calve-Perthes disease were followed. All 41 cases of transient synovitis had a benign outcome. In this series, nine of the 10 children with Legg-Calve-Perthes disease were diagnosed upon their first roentgenographic examination. One child whose first diagnosis was transient synovitis remained symptomatic for 4 months, when he developed roentgenologic changes consistent with Perthes disease. The literature is reviewed and 455 cases of transient synovitis analyzed. Only one patient with transient synovitis developed Legg-Calve-Perthes disease after having been asymptomatic for a period of 3 months. In 17 other cases, symptoms persisted until Legg-Calve-Perthes was diagnosed. It is suggested that transient synovitis is a benign disease and that only children with protracted symptoms are at risk to develop Perthes disease.

Modulation of T-cell subsets in asthmatic children by THF, a thymic hormone.

Induction of theophylline-sensitive T-suppressor cells with THF, a thymic hormone, resulted in elevation of the low levels of these lymphocytes in non-treated asthmatic children. Evaluation, however, by monoclonal OKT8 antibodies did not show changes in the levels of the OKT8+ cells after THF incubation. In the treated asthmatic children, normal levels of theophylline-sensitive T-suppressor cells as well as OKT8+ cells were found. These normal levels were not changed with THF after in vitro incubation.

Familial multiple naevi flammei.

The third family with multiple naevi flammei and the first description of its occurrence in unidentical twins is presented. Autosomal dominant inheritance is confirmed.

Induction of suppressor T cells in asthmatic children by theophylline treatment.

The absolute T-cell numbers and function, and the T-suppressor-cell subset were tested in eleven children with extrinsic asthma prior to and 1 month after theophylline treatment. Low mean T-suppressor-cell number and function was found in the asthmatic children, which returned to normal levels and function after 1 month of theophylline treatment. A normal mean T-cell number was found in this group of children prior to, and 1 month after, theophylline treatment. An obvious correlation between the clinical severity of the asthma and the number of T suppressor cells was found. It is suggested that theophylline most probably activated the T suppressor cells in the asthmatic children.

Psychogenic cough.

Psychogenic cough is croupy and explosive, never occurs during sleep, and is not affected by antitussive drugs. Physical and radiographic examinations of the respiratory tract and microbiological investigations are normal. Bronchial asthma manifested as chronic cough should be excluded in each patient by lung function testing.

The efficiency of nebulization treatment with water compared to sodium cromoglycate in reducing upper respiratory infections in children.

Twenty-two children aged 1 to 5 years with history of recurrent upper respiratory infections (URI) were investigated in a double blind cross over clinical study. After a three week observation period, patients received, at random, over two alternating periods of six weeks each, either 2 ml placebo (water) or 1% sodium cromoglycate (SC) solution t.i.d. by means of a home nebulizer. A significant decline in the number of illness days during both the nebulization periods as compared to the pretreatment period was observed (a mean percentage of days of 13.7 and 36.1 respectively, p less than 0.001). Both types of nebulization were equally efficient in reducing URI morbidity. The present study indicates that humidification may have some beneficial effect on reducing the recurrence rate of URI. Sodium cromoglycate was not superior to water for this purpose.
