[Trace elements and endemic goiter].

The effects of xenobiotics on morphological structure and function of the thyroid gland (TG) were analysed. Salts of heavy metals (zink, lead, copper, cadmium, surma etc.) were measured in TG of 152 citizens of Kama region who had died of trauma in the 1960s (n = 73), 1990s (n = 20) and 2000s (n = 52). TG is shown to accumulate xenobiotics. As the examinees had no occupational contact with heavy metal salts (HMS), their deposits in TG is explained by anthropogenic pollution of the environment. HMS in the thyroid tissue was much lower than in later decades indicating aggravation of environmental pollution in the Urals. Being protoplasmic poisons, HMS bind proteins and provoke immune reaction. All the examinees who died after 2000 had histologically proved deposits of immunocompetent cells in TG tissue--lymphoid and plasmic TG infiltration. In the 1960s these TG alterations were absent. Goiter endemia can be stopped by improvement of environmental protection in each ecopolluted region and mass iodine prophylaxis.

[Trends of mental and somatic diseases during therapy of endemic goiter].

The trend of mental and somatic diseases was studied in 70 patients with epidemic goiter (EG) treated with L-thyroxine or iodides during 1-5-year follow-up. Borderline mental disorders (diminished memory and attention, emotional and intellectual disorders) were found to be stable in these patients. The most delicate functions of the brain, such as the capacity for analysis and synthesis, are particularly vulnerable. Irrespective of the age of a patient, his/her educational level, occupation, the size of goiter, concrete rather than abstract logic thinking are prevalent in patients with EG. Drug treatment of EG only improves the patients' health status, but fails to eliminate evolving mental disorders even if the sizes of the thyroid can be brought to the normal values, the use of L-thyroxine is more effective that that of iodides.

[Rational antibiotic therapy of pneumonia].

The aim of the study was to analyse antibacterial treatment in patients with extrahospital pneumonians in a standard pulmonological department. The analysis covered 80 patients with EHP treated in 2000. In the 100% cases the choice of the antibiotics was random and did not depend on pneumonia severity, age, sex, occupation of the patient, concomitant pathology, the disease duration, previous treatment. Gentamycin was prescribed most frequently. The course doses of the drugs were often violated. The mean hospital stay was 22.5 days, complete resolution was observed only in 8.75% patients.

[Autonomic system disorders in endemic goiter].

Vegetative tonicity, reactivity and provision were studied in 88 patients with endemic goiter (EG) aged 18 to 40 years. Vegetative dystonia syndrome (VDS) in EG develops as parasympathicotonia. Subjective symptoms of vegetative dysregulation are more pronounced in high reactive and personality anxiety, in neurotism, they do not depend on extra- or introversion. VDS in EG deteriorates mental and physical performance, attention. Vegetative reactivity in EG becomes hypersympathicotonic. Vegetative dystonia affects reaction to the disease. Prolapse of heart valves often seen in EG patients results rather from hypothyroid edema of the myocardium than from VDS. Therefore, treatment of such patients with beta-adrenoblockers is not grounded. Vegetative dysregulation gradually attenuates in response to replacement hormone therapy with thyroxine.

[Investigation of mutations of BRCA1 and BRCA2 genes in 52 breast cancer patients].

The proportion of BRCA1/2 genes varies widely among populations. We investigated 52 cases of breast cancer: 24 cases of familial breast cancer, 22--early-onset (till 40 years) and 6--bilateral cancer. The frequency of carriers of mutations among familial cancer cases was 16%, among cases of disease till 40 years--9%. 538insC BRCA1 was the most common genetic mutation among hereditary cases of breast cancer.

[Role of thyroid pathology in pathogenesis of arterial hypertension in diabetes].

190 patients with diabetes mellitus (DM) were examined: 24 patients with DM type 1 and 166 with DM type 2. Frequency of arterial hypertension (AH) in diabetics is higher than in population. AH in different DM types varies by pathogenesis. Symptomatic renal AH is typical for DM1, essential AH combining with renal one in 1/3 of cases--for DM2. Treatment of AH and DM often provokes hypothyroidism which can be iatrogenic because of continuous intake of sugar reducing and antihypertensive drugs with antithyroid and strumogenic actions. Hypothyroidism aggravates an AH course: arterial pressure becomes high and resistant to hypotensive therapy. DM makes difficulties in selection of hypotensive drugs as many of them alter metabolism and due to negative attitude of the patients to continuous intake of sugar reducing, antihypertensive drugs, thyroid hormones. Therefore, hypertensive diabetics should be prepared for treatment psychologically and receive only prolonged hypotensive drugs. In DM with AH medication of choice is ACE inhibitors as they are nephroprotective, had no negative effect on carbohydrate, fat metabolism and thyroid system.

[Echocardiographic evidence of heart state in patients with hypothyreosis].

Echocardiographic examination of 41 patients has demonstrated that mitral prolapse (MP) of the 1st and 2nd degree is frequently associated with latent or apparent hypothyroidism and occasionally with prolapse of the tricuspid and/or aortic valve. As a rule, MP has a silent course without regurgitation and is diagnosed at ultrasonography. Its correction is made by replacement with thyroid hormones. Beta-adrenoblockers for MP treatment are contraindicated in hypothyroidism. MP complications (sudden death, thromboembolism, infectious endocarditis) are rare in patients with hypothyroidism. Thyroid insufficiency sometimes is accompanied with hydropericardium which is symptomless and is detected only at echocardiography.

[Mental status of patients with iodine-deficiency goiter].

Personality and intellectual characteristics were examined in 76 patients with iodine-deficiency goiter (IDG). Subclinical hypothyroidism forming as early as IDG of the 1st degree impairs abstractive and integral thinking, psychological maladaptation to the disease, high anxiety, neurotism, diminished intellectual performance, deterioration of memory, attention, predominance of thinking of concrete-action type. To prevent intellectual disturbances, IDG patients should receive replacement therapy with adequate doses of thyroid hormones preparations.

[The effect of kordaron on thyroid function in patients with ischemic heart disease].

Thyroid function has been assessed in 56 patients with arrhythmic coronary heart disease (CHD) prior to and in the course of cordaron treatment. TTH level made up initially 1.39 +/- 0.27 IU/ml which was normal. T3 blood concentration was low--1.28 +/- 0.06 nmol/l; T4 was normal--128.26 +/- 4.9 nmol/l. It is stated that CHD patients with arrhythmia develop latent hypothyrosis of T3-low-syndrome type. Arrhythmia treatment with cordaron aggravates T3 deficiency as the drug inhibits T4 conversion into T3 in peripheral tissues. It also causes toxic damage to thyroid parenchyma resulting in higher TH levels in the blood. CHD patients receiving long-term cordaron (1 to 8 years) have clinical picture of manifest hypothyrosis. Cordaron molecules includes iodine macro-doses, but these fail to initiate autoimmune thyroid process.

[Clinical, metabolic and hormonal changes in patients with obesity on low-calorie diet therapy].

Low-calorie diet therapy (LCDT) was used to treat obesity in 74 females aged 18-49 in four regimens: short-term (up to 7 days), of middle duration (10-14 days), long-term (up to 21-24 days) and fractionated. Middle duration of LCDT proved most favourable. Long-term fasting may bring unstable effects or complications. LCDT results in electrolyte unbalance, hypokalemia, hypokaligestia. Relevant correction is necessary on the diet day 5-6. Red cell count, blood levels of sugar and protein, lipidogram, nitrogenous metabolism, coagulation activity changed insignificantly. Obese patients on LCDT exhibited reduced hyperinsulinism responsible for body mass decrease, hyperproduction of hydrocortisone and prolactin enhancing fat synthesis. They also developed T3-hypothyrosis reducing thermogenesis. The changes in the body mass depended on these factors.

[Subclinical hypothyroidism in patients with ischemic heart disease].

A total of 187 patients with coronary heart disease (CHD) were examined. Out of them 60% were found as having subclinical hypothyroidism. The serum levels of T3 were significantly lower, particularly in arrhythmic CHD, the level of T4 was normal and even increased, thus making the concentrations of TTG normal, i.e. the T3-low-syndrome was detected. Hypothyroidism in CHD patients decreased tissue oxygen consumption, by reducing the incidence of acute myocardial infarction, heart failure, the atherosclerotic process becoming disseminated and progressive. Cordarone therapy was found to promote the progression of T3-hypothyroidism, i.e. in CHD patients with suspected hypothyroidism, the drugs inhibiting the thyroidal function should not be used. T3-Hypothyroidism should be corrected with adequate doses of triiodothyronine in CHD patients. The use of thyroxine is ineffective as the conversion of T4 to T3 is impaired in the body.

[Comparative effectiveness of the treatment od patients with hyperglycemic coma in diabetes mellitus with small and large doses of insulin].

The results of the study involving 123 patients suffering from hyperglycemic diabetic coma lead to the conclusion on ineffectiveness of high-dose insulin. Treatment of hyperglycemic coma by low-dose insulin is more preferable as warranting continuous decrease of glycemia without its transformation into hypoglycemia, ++post-hypoglycemia hyperglycemia, brain edema.
