Pregnancy after chemotherapy for trophoblastic diseases and psychosomatic development in the progeny.

We treated 542 patients with trophoblastic diseases from 1965 through 1988, and succeeded in preserving fertility in 269 of them (240 cases of invasive mole and 29 cases of choriocarcinoma). Of the 269 patients, 198 (79.5%) conceived after completing treatment. A total of 396 pregnancies in these women resulted in 295 term live births (74.5%), 51 spontaneous abortions (12.9%), and 4 recurrences of moles (1.0%). Four cases (1.32%) of malformations were discovered. The growth conditions of 101 children born in 1976 or later were followed up until the age of 18 months. Their heights or weights did not differ significantly from those of children measured in national infantile-growth surveys. The psychoneurological development of 54 children up to the age of 7 years who were studied by using infantile mental-development questionnaires showed no marked mental retardation. Our results provide strong evidence of the safety of chemotherapy for the treatment of trophoblastic diseases.

Changes in cystathionine gamma-lyase levels in rat liver during lactation.

Rat liver cystathionine gamma-lyase activity increased rapidly with the onset of lactation, reaching a maximum value at week 2 and slowly decreasing thereafter. In non-lactating rats, from which the litters had been removed on the day of birth, the activity of this enzyme increased only minimally. The enzyme activity in the presence and absence of pyridoxal 5'-phosphate (PLP) did not differ significantly, indicating that the cystathionine gamma-lyase in lactating rat liver was also saturated with PLP as control rat. We then investigated whether the increased activity was caused by increases in the enzyme concentration. An antibody, prepared form purified rat liver cystathionine gamma-lyase was utilized for the detection of enzyme content by immunoblot analysis and enzyme immunoassay. On immunoblot analysis, this antibody stained purified cystathionine gamma-lyase, as well as staining a single polypeptide from liver cytosolic proteins that co-migrated with the purified enzyme at about 40 kDa. Changes in the staining intensity of the antibody paralleled the activity of the enzyme. The enzyme immunoassay showed that the content of liver cystathionine gamma-lyase at weeks 0, 1, 2 and 5 of lactation was 1.67, 2.33, 5.19, and 2.17 mg per g of liver, respectively, whereas in the non-lactating rats, the enzyme concentration at week 2 was found to be 2.20 mg per g of liver. The changes in enzyme concentration detected by enzyme immunoassay were also consistent with enzyme activity. We concluded that the increased cystathionine gamma-lyase activity in the lactating rat liver was a result of an increase in enzyme concentration.

Phenylmethylsulfonyl fluoride stimulates proteolysis of nuclear proteins from chick liver.

The degradation of H1 histone and high mobility group (HMG) nonhistone proteins was stimulated when the homogenate from chick liver was incubated in the presence of phenylmethylsulfonyl fluoride (PMSF). Two proteinase inhibitors, elastatinal and chymostatin, significantly inhibited the PMSF-stimulated degradation of H1 histone and HMG proteins. On the contrary, other proteinase inhibitors like leupeptin, pepstatin, trypsin inhibitor, antipain, o-phenanthroline and EDTA had no effect on the degradation of the nuclear proteins. These results warn the researcher to be cautious while using PMSF for preparation of nuclear proteins such as H1 histone and HMG proteins.

Molecular cloning of chick liver HMG 2a cDNA and developmental expression of HMG 2a mRNA.

The cDNA clone encoding HMG 2a of chick liver was isolated from a lambda gt11 expression library using polyclonal antibodies. DNA sequence analysis revealed an open reading frame of 201 amino acids. Comparison of the nucleotide sequences of cDNA coding for chick liver HMG 2a with pig thymus HMG 2 and human monocytic leukemia cell HMG 2 showed 70% homology. 2.0 kb and 1.2 kb mRNAs were found in newly hatched chick liver and decreased during postnatal development of chicks.

A case of accessory mitral valve leaflet associated with solitary mitral cleft.

Accessory mitral valve leaflet is a rare congenital anomaly. More than half of the cases show other congenital cardiac defects and almost all of the cases show subaortic obstruction. We report a case of an accessory mitral valve tissue without outflow obstruction associated with mitral cleft of the posterior mitral leaflet. To our knowledge, this is the first reported case of the combination of these two congenital anomalies.

Mesial temporal subdural electrode as a substitute for depth electrode.

As a substitute for depth electrodes, mesial temporal subdural electrodes were devised. This electrode has a slender trapezoid shape and is easily introduced to the inner uppermost portion of the parahippocampal gyrus. Our results have shown that mesial temporal electrodes can detect not only interictal spikes but also subclinical and clinical seizure discharges from the mesiolimbic structures, and they have excellent capability for lateralization of the mesial temporal epileptic focus. If mesial temporal subdural strips are used in combination with lateral temporal subdural grids, comprehensive understanding of the focus distribution throughout the temporal lobes will be possible without using depth electrodes.

Oral administration of a streptococcal agent OK-432 activates alveolar macrophages in mice.

The effect of orally administered OK-432, a streptococcal preparation, on the functions of alveolar macrophages in mice was examined. The oral administration of OK-432 (1, 2 or 4 KE, four times every 3 days) augmented phagocytic activity, lysosomal enzyme activity and interleukin 1 (IL-1) production of murine alveolar macrophages recovered 5 days after the final administration while it did not augment H2O2 production. The number of alveolar macrophages was not affected by the same treatment. These results suggested that oral administration of OK-432 activates alveolar macrophages qualitatively to protect the lung from the metastasis of cancer cells and infectious diseases by pathogenic micro-organisms.

Enhancement of murine alveolar macrophage functions by orally administered beta-glucan.

The effect of orally administered SSG, a beta-1,3-glucan obtained from the culture filtrate of the fungus Sclerotinia sclerotiorum IFO 9395, on alveolar macrophage (AM) functions of CDF1 mice was examined. SSG administered orally (20, 40, 80 or 160 mg/kg) for 10 consecutive days enhanced the lysosomal enzyme activity of AM. The greatest enhancing effect was observed at 80 mg/kg of SSG. Multiple oral administrations of SSG (10 consecutive days) were needed to induce significant enhancing effects. Phagocytic activity and interleukin-1 (IL-1) production of AM were also augmented by oral administration of SSG, and the kinetics of the activated state differed depending on the kind of activity. However, H2O2 production of AM was not affected by SSG. Orally administered SSG also (40 or 80 mg/kg, 10 consecutive days) increased the number of AM and the greatest increment was observed 14 days after the first administration. On the other hand, the supernatant of Peyer's patch (PP) cells from mice administered SSG (80 mg/kg) orally stimulated the lysosomal enzyme activity of AM in vitro, and enhanced colony stimulating activity (CSA) was detected from this supernatant. These results demonstrate that SSG given by the oral route can activate murine AM both qualitatively and quantitatively, and it would mediated, at least in part, by the activation of PP cells in the intestine.

Induction of cystatin S in rat submandibular glands by papain.

1. Papain (a cysteine proteinase) were administered into the oral cavity of rats twice daily for 5 days. This treatment caused a dramatic increase in the level of cystatin S (a cysteine proteinase inhibitor belonging to family 2 of cystatin superfamily) in enlarged submandibular glands. 2. Immunochemical analysis using antibody against rat cystatin S and electrophoretic analysis confirmed that the protein induced by papain was identical to that induced by isoproterenol. 3. Induction of the cystatin S in the submandibular glands by oral administration of papain suggested a biological response which plays a role in preventing injury exogenous proteinase.

Ontogeny of DNA synthetic rhythms in chick (Gallus domesticus) liver.

1. The diurnal pattern of DNA synthesis and mitotic activity in neonatal (1-4-day-old) chick liver were investigated under various feeding and lighting regimens. 2. In the meal-fed chicks under the condition of light-dark cycle, DNA synthesis exhibited a 12 hr cycle; the peaks occurring at 9:00 and 21:00. 3. Fasting caused a gradual decrease in the 21:00 peaks. 4. The changes in the lighting regimen to 24 hr continuous lighting also caused a profound change in the DNA-synthetic pattern, suggesting a complex interplay of feeding and lighting regimens in the manifestation of the DNA-synthetic rhythm in neonatal chick liver.

Adipofascial (anterior rectus sheath) flaps for breast reconstruction.

A flat breast after a subcutaneous mastectomy for removal of a foreign body tumor or a benign tumor is not acceptable for many patients. If patients do not wish to use a silicone bag implant for reconstruction, it is necessary to use autologous tissue for the reconstruction of the breast mound. The use of a latissimus dorsi myocutaneous flap or a rectus abdominis myocutaneous flap for breast reconstruction in most patients who underwent subcutaneous mastectomy is too aggressive. Free fat grafts are also generally not effective for such patients because of the absorption of the grafted fat. We developed a procedure involving a cranial-based adipofascial (anterior rectus sheath) flap from immediately below the inframammary area for breast reconstruction. The skin incision is made at the inframammary line. After extirpation of the foreign body, the inframammary skin area is undermined. A tongue-shaped flap of the fat and the anterior sheath of the rectus abdominis muscle, approximately 7 cm in length, is pulled up, gathered, and inserted to reconstruct the breast mound. We used this technique in 7 patients, and the results were satisfactory.

Glutamine Induces the N-Dependent Accumulation of mRNAs Encoding Phosphoenolpyruvate Carboxylase and Carbonic Anhydrase in Detached Maize Leaf Tissue.

We have used detached leaves to study the N-dependent control of expression of phosphoenolpyruvate carboxylase (PEPC) and carbonic anhydrase (CA) genes in maize (Zea mays L. cv Golden Cross Bantam T51). Following supplementation with an N-source and zeatin, PEPC and CA mRNA levels increased in leaves detached from N-deficient maize plants. Addition of methionine sulfoximine (MSX), a specific inhibitor of glutamine synthetase, inhibited the nitrate-dependent increase of PEPC and CA mRNA but did not affect the glutamine-dependent increase of PEPC and CA mRNA levels. Glutamine levels in detached maize leaves treated with various N sources in the presence or absence of MSX correlated with the levels of PEPC and CA mRNA. We conclude that glutamine is the most likely effector for controlling the N-dependent expression of PEPC and CA in maize plants.

Aphasic seizure caused by focal epilepsy in the left fusiform gyrus.

We report a patient with paroxysmal aphasia evoked by ictal epileptiform discharges localized to the left fusiform gyrus, where a small brain tumor existed. The intracranial EEG recordings during other seizures demonstrated a close functional link between the left fusiform gyrus and Wernicke's area. The patient also showed transient aphasia with electrical stimulation of the left fusiform gyrus.

Odontoclastic resorption at the pulpal surface of coronal dentin prior to the shedding of human deciduous teeth.

Histological and histochemical observations of more than 150 extracted human deciduous teeth revealed that, prior to shedding, odontoclastic resorption as a rule takes place at the pulpal surface of coronal dentin. We also found that this phenomenon occurs in all kinds of deciduous teeth. The process of this internal resorption of coronal dentin of deciduous teeth clearly showed time-related histological changes. During the time the roots were actively being resorbed, the pulpal tissue retained its normal structure. However, when root resorption neared completion, inflammatory cells started to gradually infiltrate into the pulp, and odontoblasts began to degenerate. After that, multinucleate odontoclasts appeared, and resorption proceeded from the predentin to the dentin. The odontoclastic activity was initially detected only on the pulpal surface at the bottom areas of the crown. It gradually spread towards the pulpal horn regions along the wall of the pulp chamber. However, this internal resorption of coronal dentin did not continue until the teeth were finally shed. After the elimination of resorption, the resorbed dentin surface was repaired by a cementum-like deposition or covered with fibrous connective tissue.

[A case of transfusion-associated GVHD after total laryngectomy].

We report a case of fatal transfusion-associated graft versus host disease (GVHD) that developed in a patient with laryngeal cancer. After a 39.6Gy irradiation dose, total laryngectomy with right radical neck dissection was performed. The postoperative course was uneventful. However, seven days after blood transfusion, high fever (38.5 degrees C) suddenly appeared. On the ninth day, watery diarrhea and facial erythema were observed. On the 12th day, liver disturbance and pancytopenia developed. The patient died on the 16th day because of overwhelming sepsis. Transfusion-associated GVHD has a mortality rate of more than 90%. Therefore, the most important procedure for preventing GVHD is the use of irradiated blood products. Furthermore unnecessary blood transfusion should be avoided.

[Clinical use of the multi-purpose circuit with a soft reservoir built in inflow side in replacement of the descending thoracic aorta].

The temporary bypass circuit under centrifugal pumping is generally applied for the assisted circulation of the operation for the aneurysm of descending aorta. The estimation of the bypass flow to the abdominal viscera, however, sometimes showed inadequate when excessive bleeding happened during operation. In order to keep constant flow to the lower body, we developed a new circuit, that has soft reservoir built in inflow side of the bypass line. In 8 subjects using this circuit, we studied the usefulness of the circuit and the choice of bypass route based on changes in the hemodynamics during bypass and operative complications in relation to bypass technique. During bypass, perfusion pressure to the lower body was 70-110 mmHg and flow was maintained at more than 21/min even if massive hemorrhage occurred. Comparing arterial bypass with LV bypass, the diastolic pressure in the upper limb was significantly depressed in arterial bypass with reading of 40-50 mmHg. Furthermore, in one case of arterial bypass involving the ascending aorta, false aneurysm is found in the site of cannulation. The rectal temperature dropped with the lapse of time from 35.5 +/- 0.8 degrees C to 33.5 +/- 0.4 degrees c in 120 min. These results indicate that multi-purpose circuit is capable of promptly supplying the circulating volume and is excellent means of providing adequate circulation to the lower body even if massive hemorrhage occurred. And LV bypass should be selected first, because arterial bypass has the hazard of depressing the diastolic pressure in the upper limb and involving complications associated with the insertion of cannula.(ABSTRACT TRUNCATED AT 250 WORDS)
