Radiological manifestations of ritual-enema-induced colitis.

Most South African urban Blacks consult witchdoctors. In order to effect cures the witchdoctors often use enemas containing various ingredients. These enemas may result in complications, some of which may be fatal. The radiological manifestations of ritual-enema-induced colitis are described. The acute radiological abnormalities include segmental toxic dilatation, necrotising colitis, pseudopolypoid formation, ulcer formation, penetration and perforation. The chronic manifestations are loss of bowel calibre, skip stricture formation, diffuse stricture formation and loss of the normal haustral pattern. The spectrum of radiological appearances is both wide and kaleidoscopic, and may mimic many other colonic diseases.

Familial 5/14 translocation with triple X and 47,XY + 14q.

A family with 5/14 balanced translocation was investigated. The father and both his daughters had the same balanced translocation. One of the daughters had four spontaneous abortions, the last one with a karyotype of 47,XY + 14q-. The other daughter had two children, one of them a normal 46,XX girl, the other with the karyotype 47,XXX;5/14.

Polyps and colorectal cancer in South African Blacks.

This study reflects the fact that cancer of the large bowel is uncommon in South African Blacks, and that colorectal polyps do not appear to constitute a precursor to most colorectal cancers. Furthermore, it is shown that dietary factors associated with this cancer in Western populations are not evident in the Black population. In the South African White population, however, the disease behaves in a similar way to that observed in Western countries. Other definitive differences found were the absence of multiple synchronous cancers and diverticular disease in the Blacks with colorectal cancer. It is thus postulated that dietary factors are absent, or have not been present for a sufficient length of time to influence the development of polyps or polyp--cancer sequence in this population. It is also possible that the adenoma--carcinoma progression observed in Western countries may not be relevant to the development of all colorectal carcinomas in communities such as those reported here.

Pulmonary function abnormalities in Sjögren's syndrome and the sicca complex.

The frequency of pulmonary involvement in a group of 20 patients with Sjögren's syndrome or the sicca complex was evaluated with pulmonary function studies. In 12 patients pulmonary functional abnormalities were demonstrated. The most common abnormality was airway obstruction. Nine out of 13 patients with the limited variant of the disease (sicca complex) and three out of seven patients with the complete syndrome had abnormal pulmonary function.

Pitfalls in the diagnosis of gastrointestinal tuberculosis.

Gastrointestinal tuberculosis is difficult to diagnose since it mimics many other abdominal conditions and has protean manifestations. The disease can present as an abdominal mass, bowel perforation, Crohn's disease and dysentery. These presentations are discussed with representative cases to illustrate the diagnostic difficulties. It is emphasized that to obviate the diagnostic pitfalls, tuberculosis should always be considered in the differential diagnosis of unusual gastrointestinal presentations particularly because radiological and laboratory tests may be noncontributory.

Tonsillectomy prevalences in South African schoolchildren.

Tonsillectomy prevalences, using questionnaires, were determined on series of school pupils (total number 10,271), aged 16-18 years, in four South African ethnic groups. Prevalence were, approximately: rural Blacks 2%, urban Blacks 3%, Indians 8%, Coloureds (Eur-African-Malay) 9% and Whites 44%. The inter-ethnic profile of differences is similar to the inter-ethnic profiles of other differences (e.g. in appendicectomy prevalences).

Foreign-body ileitis. A case report.

A patient who ingested a soft-drink bottle top and stones, which resulted in a chronic active ileitis, is described. Histological examination revealed a chronic active inflammatory process, probably indicating long-term ingestion of foreign material. The histological features in some respects resembled those of Crohn's disease.

Hiatus hernia in Johannesburg blacks.

With westernization of diet and lifestyle among South African Blacks, particularly urban dwellers, hitherto unknown gastro-intestinal diseases are becoming evident. Previously unreported is hiatus hernia, now a definite disease entity, although still very uncommon. At Baragwanath Hospital, Johannesburg, over 17 months in 1977-1978, 46 patients with hiatus hernia were detected from barium meal examinations on 1392 persons, i.e. 3,3%, or 0,07% of total hospital admissions of adults. The disease affected mainly middle-aged and elderly women. Prevalences of obesity, comparative physical inactivity and hypertension appeared commoner than in the general urban adult population. Just as appendicitis is a marker of westernization in young urban Blacks, hiatus hernia may be regarded as a marker in the middle-aged and elderly.

Unilateral increased transradiancy of the lung caused by bronchial carcinoid tumour.

Two patients with an intraluminal carcinoid tumour obstructing the right main bronchus in one, and the left main bronchus in the other, resulting in unilateral increased transradiancy from hypoperfusion of the lung, are described. Resection of the tumours and reconstruction of bronchial continuity was followed by return of perfusion to normal between three and 18 months after the operation. The patient in whom the diagnosis was delayed showed a much slower rate of return of perfusion and a small lung resulted. The finding of increased transradiancy of lung even in an asymptomatic patient should be an indication for bronchoscopy.
