Papillary and cystic tumor of the pancreas. Two cases indistinguishable from islet cell tumor.

Two cases of large cystic tumor of the pancreas in two young women are reported. Gross features and conventional light microscopic appearances of these tumors were consistent with those described as being the so-called papillary and cystic tumor (PCT) or solid and cystic tumor. Immunohistochemical staining for neuron-specific enolase was positive diffusely in both cases. Special stainings with Grimelius' silver impregnation, and stainings for insulin, glucagon, and somatostatin also were positive, although the population of positive cells was different in each case. Ultrastructural examinations confirmed the presence of numerous granules, probably neurosecretory, within the cytoplasm of many tumor cells in each case. Therefore, current cases were indistinguishable pathologically from the cystic nonfunctioning islet cell tumor. This study suggests that some PCT of the pancreas consist of endocrine cells predominantly.

Tricuspid and mitral regurgitation detected by color flow Doppler in the acute phase of Kawasaki disease.

Valvular lesions in the acute phase of Kawasaki disease were studied in 19 children. The patients were intensively observed by color flow Doppler every day from the day of hospitalization up to 12 days after the onset of the disease and 2 or more times a week thereafter, for up to 28 days. Mitral regurgitation (MR) was found in 9 patients (47%) and tricuspid regurgitation (TR) in 10 (53%). MRs were of transient type and confirmed from 7.5 +/- 1.6 (mean +/- standard deviation) to 13.1 +/- 6.5 days after the onset of the disease. Both types of valvular regurgitation were mild. The direction of regurgitation was from the center of valvular coaptation toward the posterior wall of the atrium. Neither valvular prolapse nor valvular deformity was noted. In patients with MR, left ventricular ejection fraction on M-mode echocardiography was significantly lower in the acute phase than in the convalescent phase of the disease (p less than 0.05). Using gallium-67 scintigram, the positive uptake of the isotope was noted in 7 (88%) of 8 patients with MR, but not found at all in 8 patients free of MR. These results suggest that MR and TR are often transient in the acute phase of Kawasaki disease and could be attributed to myocarditis.

Hepatic cysteine dioxygenase activity and sulfur amino acid metabolism in rats: possible indicators in the evaluation of protein quality.

Experiments were carried out to examine the possibility that the sulfur amino acid metabolism of rats may be an indicator of the nutritional value of dietary protein. Rats were fed diets containing 8, 16 or 24% of gluten, soy protein or casein for 3 wk. Hepatic cysteine dioxygenase activity, hepatic concentration of glutathione, cysteine and taurine and urinary taurine were examined. In addition, the sulfur amino acid metabolism of rats fed these diets fortified with the appropriate first limiting amino acid for 7 d was also examined. High urinary taurine excretion was observed in the three gluten groups, whereas very low urinary taurine excretion was observed with up to 24% soy protein or up to 16% casein. The hepatic hepatic cysteine dioxygenase activities of the gluten diet groups were higher than those of corresponding soy protein or casein diet groups, except that of rats fed the 24% casein diet. The hepatic concentrations of both glutathione and cysteine in gluten diet groups were also higher than those of corresponding soy protein or casein diet groups, except 24% soy protein and 16 and 24% casein diet groups. In rats fed the casein or soy protein diets urinary taurine excretion and hepatic cysteine dioxygenase activity increased with increasing methionine supplementation, the first limiting amino acid. Conversely, in rats fed the gluten diet both urinary taurine excretion and hepatic cysteine dioxygenase activity decreased with increasing lysine supplementation, the first limiting amino acid. These findings suggest that urinary taurine excretion and hepatic cysteine dioxygenase activity may be useful as sensitive indicators of the nutritional value of dietary protein.

Narrow QRS complex tachycardia with atrioventricular dissociation.

We describe the case of a 22-year-old man who had frequent episodes of narrow QRS complex tachycardia with atrioventricular dissociation. The ECG during sinus rhythm showed normal PR and QRS intervals, but it showed a left bundle branch block configuration during atrial pacing or after injection of verapamil. An electrophysiological study demonstrated that the patient had nodoventricular Mahaim fibers. The narrow QRS complex tachycardia was explained by a circuit involving antegrade conduction via the atrioventricular nodo-His axis and retrograde conduction via the nodoventricular bypass tract.

Idiopathic sustained left ventricular tachycardia: clinical and electrophysiologic characteristics.

Electrophysiologic studies were performed in 16 patients 11 to 45 years old (mean 33 years) with idiopathic sustained (lasting more than 5 min) ventricular tachycardia (VT) originating from the left ventricle. Endocardial mapping during VT showed that the earliest site of activation was at the apical inferior portion of the left ventricle in 14 patients whose QRS morphology during VT showed a right bundle branch block pattern and left-axis deviation, but at the apical anterosuperior portion of the left ventricle in two patients whose QRS morphology during VT showed a right bundle branch block and right-axis deviation. Single programmed ventricular stimulation induced VT in 13 patients, and rapid ventricular pacing induced VT in the remaining three patients. Rapid ventricular pacing terminated VT in all patients. The relationship between the coupling interval and the echo interval was inverse in all eight patients with a wide VT inducible zone. Entrainment was recognized in three of six patients. The initiation of VT by constant pacing depended on the number of pacing beats but not the duration of pacing in all four patients tested. Intravenous verapamil terminated the VT in 13 of 14 patients. Long-term oral verapamil was also effective in all five patients who required long-term oral therapy for their symptoms associated with VT. In conclusion (1) idiopathic left ventricular tachycardia has unique electrocardiographic, electrophysiologic, and electropharmacological properties, (2) the electrophysiologic characteristics suggest that the mechanism is reentry, and (3) verapamil is effective in both the short- and long-term treatment of VT.

Improved accuracy of the exercise electrocardiogram in detection of coronary artery and three-vessel coronary disease.

The study objective was to assess the widespread applicability of ST/HR slope for the modified Bruce exercise test using a computerized electrocardiogram (ECG); compare the usefulness of the ST/HR slope with standard ECG criteria in detection of coronary artery disease (CAD) and identification of three-vessel or left main CAD; and then develop a new, modified ST/HR score (MSHS) for improving the diagnostic accuracy of ST/HR slope. The studies were retrospective and prospective in design, conducted in referral-based cardiology clinics at a national cardiovascular center. A selected sample of 142 patients underwent exercise ECG and coronary angiography, as did a normal control group of 402 patients who were apparently free from CAD. Sixty three other patients who underwent coronary angiography were also studied prospectively. No limitations of medical treatment were exacted for the test except digitalis treatment. Linear regression analysis, from which ST/HR slope was derived, was done with seven measurements of HR and ST displacement at 60 ms from J point in leads a VF and V5 during 6 min before the end of exercise. MSHS was derived from a multiple regression model with peak HR (% maximum HR), ST index (ST depression + ST slope), and ST/HR slope. Although the usual ST criteria (sensitivity = 63 percent, specificity = 73 percent), ST index (71 percent, 80 percent), and ST/HR slope (70 percent, 97 percent) were equally accurate in detection of CAD, MSHS showed significantly improved sensitivity (88 percent) with similar specificity (81 percent). In identification of three-vessel or left main CAD, when compared with other criteria, ST/HR slope and MSHS provided improved diagnostic accuracy: sensitivity (74 and 78 percent, respectively), specificity (88 and 93 percent) and overall test accuracy (85 and 89 percent). The improved accuracy of ST/HR slope and MSHS was prospectively validated in 63 other patients. ST/HR slope was applicable to computerized ECG data for the standard treadmill test, and showed improved accuracy in detection of three-vessel or left main CAD. The new, modified ST/HR score more accurately predicted not only the presence but also the severity of CAD.

Effect of hypophysectomy on 17 alpha-hydroxylase activity in theca cells of the ovarian follicle in the Japanese quail.

The activity of 17 alpha-hydroxylase, a key enzyme for progesterone metabolism in avian follicles, was measured in cell-free homogenates of isolated theca cells of Japanese quail. Kinetic studies revealed that differences in enzyme activity in the largest and the second largest follicles were mainly due to differences in Vmax values. In order to investigate the mechanism of regulation of 17 alpha-hydroxylase activity, quail were hypophysectomized at various time periods during the ovulatory cycle, and 17 alpha-hydroxylase activity in theca homogenates was measured 4 h after surgery. Hypophysectomy at 8 h before the predicted time of ovulation, which is the time when the preovulatory luteinizing hormone (LH) surge is initiated, resulted in a significant decrease in enzyme activity. In contrast, activity was increased when the pituitary was removed 20 h before ovulation. Hypophysectomy 24 h before ovulation had no effect on enzyme activity.

[Clinical study of primary pontine hemorrhage].

Forty-three cases of primary pontine hemorrhage were seen in our hospital from 1979 to 1986. We studied the correlations between clinical signs, CT, ABR findings and their outcomes, and then reported surgical results. The case consisted of 30 males and 13 females between 32 to 73 years with an average age of 54.5. Thirty cases were confirmed to have had hypertension prior to the hemorrhage. In ten other cases hypertension was suspected, although their past histories were not obtained. In the remaining three cases, no hypertension was detected. On admission, comatose state, ocular fixation, absence of light reaction, tetraplegia, decerebrate posture, respiratory disturbance, tachycardia and hyperthermia were the signs of unlikely recovery. On the CT, the hematomas of the group of likely recovery patients were less than 25% of the cross section of the pons and lower midbrain in vertical. Greater size of hematomas were seen exclusively among the groups of death and severe disability cases. Acute stage ABR and CT findings showed discrepancy. We suggest, if ABR, CT findings and clinical symptoms were studied more in depth, it is possible to determine a patient's prognosis more precisely. Three cases were treated by Stereotactic Aspiration, three cases by ventricular drainage and the remaining thirty-seven cases conservatively. There was, however, no significant difference in recovery between surgically treated cases and conservatively treated one. We think that surgical indication is doubtful except for limited cases.

Prevention of pulmonary metastasis of Lewis lung carcinoma and activation of murine macrophages by a novel organic germanium compound, PCAGeS.

The pulmonary metastasis of Lewis lung carcinoma was strongly blocked by daily intraperitoneal (i.p.) treatment with 0.5 mg of PCAGeS/kg/day for 7 days after tumor implantation. The metastasis-preventive activity of PCAGeS was markedly reduced when mice were treated with carrageenan, a macrophage blocker. On the other hand, treatment with antiasialo GM1 antiserum did not significantly affect the percentage of inhibition of metastasis by the compound. These results suggest that macrophages rather than natural killer (NK) cells play an important role in the suppression of metastasis by PCAGeS. PCAGeS induced tumoristatic and tumoricidal activities in the peritoneal macrophages of mice by oral administration. The activity of NK cells was also augmented by i.p. treatment with the compound. These results suggest that PCAGeS is a useful substance for preventing pulmonary metastasis.
