Vaginal stenosis due to bullous erythema multiforme (Stevens-Johnson syndrome). Case report.

Bullous erythema multiforme is a disorder in which oral and genital erosions, ocular inflammation and cutaneous bullae may follow a viral or bacterial infection or the ingestion of certain drugs, such as sulphonamides. Unpleasant sequelae have been reported including residual scarring, and death may occasionally result from infection or renal failure; severe scarring can cause blindness and infrequently the genital tract is involved. We report a patient who developed complete vaginal stenosis following bullous erythema multiforme in pregnancy.

Lichen scrofulosorum with tuberculous dactylitis.

A patient with tuberculosis of cervical lymph nodes, lichen scrofulosorum on the trunk and limbs and tuberculous dactylitis of the hands is reported. Both the clinical and histological features of her lichen scrofulosorum conformed to the characteristic pattern described in other recently reported cases and the association with extra-pulmonary tuberculosis was again evident. There have been no other recent reports of tuberculous dactylitis, which was always said to be due to underlying tuberculous bony involvement. In our patient tuberculous dactylitis, which was clinically indistinguishable from previous cases, was found to be due to granulomatous involvement of the overlying skin with no associated bony abnormality.

Blood flow changes with naftidrofuryl in systemic sclerosis and Raynaud's phenomenon.

Blood flow was measured in patients with Raynaud's phenomenon, systemic sclerosis and in normal controls by venous occusion plethysmography on the forearm and calf, and changes in blood flow in the finger were followed by calorimetry. Intravenous naftidrofuryl oxalate (praxilene) improved blood flow to the skin of the finger as measured calorimetrically in seven of sixteen patients with Raynaud's phenomenon and systemic sclerosis. The drug markedly reduced blood flow in the finger in all ten normal control subjects. However, naftidrofuryl increased blood flow as measured by venous occlusion plethysmography in the leg both of normal subjects and of those with Raynaud's phenomenon. In addition to these findings, no significant change in blood flow in response to the drug was observed in the forearm. The reasons for these regional differences are discussed and considered to be due to redistributuion of blood flow as a result of differences in pharmacological receptor activity in different parts of the body and possible also due to effects of central control.

Pretibial myxoedema: stimulation of mucopolysaccharide production of fibroblasts by serum.

We have shown that sera from normal individuals and from patients with pretibial myxoedema contain a factor which simulates mucopolysaccharide biosynthesis in normal human skin fibroblasts cultured in vitro. This factor was present in larger amounts in sera of patients with pretibial myxoedema. The role of growth stimulating factors in serum is reviewed and a hypothesis is put forward that the fibroblast stimulating factor is somatomedin and that its presence in increased amounts in thyroid disease may lead to pretibial myxoedema.
