[Social tension--a factor in conversion. Case report].
Presented is a case of a girl of Albanian nationality, an adolescent, one of the ill in the group of "mass poisoned" in Kosovo, at the beginning of 1990. The up to then, completely healthy girl had repeated crises of the same type four times, with functional disorders. She was hospitalized at the request of her father in Novi Sad, at the Institute for Neurology and Psychiatry of the Developmental Age. The somatic and laboratory findings were within the average limits. The psychological tests indicated a person prone to conversive reacting in situations which she does not succeed to process emotionally in an adequate manner. Such a personality structure had the role of inducing a conversive atmosphere in the immediate school environment. The presented case has suggested that a low level of general culture and expected school knowledge primarily creates favorable grounds for massive hysterical occurrences.
[Organic factors in behavior disorders in the developmental age].
The problem of bad behavior in the young is as old as the written history of mankind. It is always a current problem in a practical and theoretical sense as well. While everyday life is filled with various forms of criminal offense, drug addiction, the forming of gangs-especially in dynamic societies, theoretical opinions tend to oscillate between biological factors and the psychological-social ones which are always unsuitable for a person in the course of its development. The authors have presented a review of the basic theoretical directions, and after that the results of their own research. Children with behavior disorders were investigated: parallely by psychological-psychiatric investigation as well as with an up-to-date biological approach, examining subclinical semiotics, EEG and REG. On the basis of these comparisons we have come to the following principal opinions: the suggested psycho-neuro-vegetative subclinical syndrome, recommended by the same authors, does not take the behavior disorder upon itself. Also, not every behavior disorder includes the obligatory psycho-neuro-vegetative subclinical syndrome (PNVSS). Nevertheless, PNVSS presents a risk factor for behavior disorder, in the case that the personality in development is growing up under unfavorable psycho-social conditions. Finally, in the younger ages PNVSS, by its primary features, determines the behavior disorder also, while at an older age (after 11) behavior has all the more of an asocial and antisocial character.

[The morphological findings in the myometrial arteries and in the placenta with reference to the age of primaparae].
The authors investigate primiparous women at various ages, searching for eventual changes in uterine blood vessels and in the placenta, which eventually could have unfavourable effect on the fetus. The study established progressive thickening due to fibrosis of the muscular layer of the myometrial arteries (in reduction of smooth muscular elements) as well as delay of maturation and differentiation of placental villi with advancement of age of the pregnant woman. These phenomena independently could be the cause for more frequent unfavourable outcome of pregnancy in elderly pregnant women.
[Relation between the anti-oxidative potential of psoriasis blood plasma in reactivity of granulocytes].
It was revealed a positive correlation between an antioxidative potential (AP) of blood plasma from psoriasis patients and generation of O2- by normal granulocytes, stimulated by zymosan or aggregated human gamma-globulin, in the presence of the same plasma samples. The AP of plasma was measured by means of inhibition of the chemiluminescence arising during photoautoxidation of luminol. The production of O2- after cell stimulation was estimated by means of the reduction of tetrazolium salt. The result is discussed from the viewpoint of informative-regulatory role of the plasma AP for the white blood cells as a part of the whole organism, participating in its common defence reactions.
[The results of a study of frustration and depression in older primiparae].
On the basis of an experimental group of 40 elder nulliparous women (ENP) (over 35 years of age) and a control group of 70 nulliparous women, aged up to 30 years of age, and examined at the chair of obstetrics and gynecology in the town of Pleven, the authors investigated the psychic state of women under the conditions of frustration and searched for hidden or manifest depression. Picture-frustration test of C. Rosenzweig and self-evaluating scale of A. K. Beck for disclosure of depression were used for the purpose of the study. It was established that in the experimental group of women there was diminished reaction to frustrating events and increased exactingness to oneself and to the others for liquidation of frustration. Correlation between elevated autoaggression and light and moderate degree of depression in 20% of ENP was found. ENP are determined as a risk group from a psychological point of view on the basis of the obtained results. This is of interest in determining the prophylactic and therapeutic approach for formation of the most correct regimen (programme) for the protection of pregnancy.
[Equivalence values of the antioxidative capacity in serum of children with chronic renal failure, chronic hemodialysis and kidney transplantation].
It is reported on the results of measurements of the antitoxydative capacity (AC) in the serum of children suffering from chronic renal failure, during hemodialysis and after kidney Transplantation. The results show in comparison with healthy children (19.95 +/- 5.22 mm, n = 19) that children suffering from chronic renal failure may develop an increased AC (66.18 +/- 29.14 mm, n = 24). In dialyzed children different values are found previous and after hemodialysis (58.11 +/- 9.25 vs. 17 +/- 3.6 mm). Children show after transplantation the highest values (78.9 +/- 26.04 mm, n = 13). If the increase of the AC is a compensatory and regulatory mechanism or the result of accumulation of antioxydative substances should realized in further investigations. The measurements of vitamin-E-supplemented sera may show that an oral application of vitamin E increase the AC in vivo.
[The effect of maternal age on the incidence of and indications for cesarean section in primiparae].
The authors aim to evaluate the effect of the maternal age on the frequency and indications for cesarean section in primiparas. Indications for abdominal deliveries of 1020 women are analyzed. Three age groups are formed: first group--up to 29 years of age, second group--up to 30-34 years of age, and third group--up to 35 and more years of age the so called aged primiparas (AP). The causes for operation are divided into ten basic groups. The indicated increased frequency with advancement of age is due mainly to: fetal distress, grave obstetric history, extragenital pathology, broad and combined indications. Their relative part in AP is respectively; 23.21%, 37.50%, 16.96%, 15.17% and 33.03%. A critical attitude is expressed to the unjustified increased frequency of the operation especially after 35 years of age (during 1987 up to 85.71%). The authors take a firm stand on their concept that the age of primiparas irrespective of years could not be a single cause for cesarean section.
Local and systemic effects of organ hypoxia detected by chemiluminescence and photochemiluminescence.
Perfusate, blood plasma and tissue homogenate were investigated by H2O2-stimulated chemiluminescence (CL) and photochemiluminescence (PCL) during experimental liver and kidney transplantation. The canine kidneys were perfused prior to reimplantation for 68 h at 4 degrees C. The increase of the H2O2 CL of the perfusate correlated positively with the LDH activity. After ischemic load of the liver, the antioxidative capacity (AC) of the tissue as determined by PCL decreases. After transplantation of the stored liver (1 h, 4 degrees C) an increased AC is observed in peripheral blood. This increase is lower after addition of ascorbate to the preservation solution. The results allow to conclude that both methods are suitable for the characterization of the organ status after hypoxic injury during conservation.
[Autonomic infantilism].
In the middle of the last century the term od infantilism was introduced in medical terminology denoting the retardation in physical and/or psychic development with the features characteristic of childhood and adolescent age. From that time on, numerous authors have described various forms of retardation in the development and psychic infantilism in various aspects has been put in the first place, such as: dysharmonious, organic, cerebrostenic, neuropathic, dysproportional, endocrine and others. The retardation in the development of the vegetative nervous system was defined as a special form of infantilism by I. Popov in 1976 who suggested the term "vegetative infantilism". Conception of the idea of vegetative infantilism has made the theoretic basis for the whole range of very heterogenous appearances in the developing age being manifested as the sequal of the retardation in the development of vegetative nervous system, such as: affective respiratory spasms, encopresis, urination in sleeping (enuresis nocturna), urination at an awakening stage (enuresis diurna), various disturbances in the function of the cardiovascular system, digestive tract and others.
[The effect of ascorbic acid in preservation solutions on the antioxidative potential of blood plasma during liver transplantation in miniature swine].
The antioxidative potential (AP) of blood plasma was defined for its O2-.-trapping capacity. The superoxide radicals were generated in a photochemical system and detected by luminol chemiluminescence. During the preparation phase of experimental liver transplantation on mini-pig the AP of peripheral blood increased twice. There is a 20-fold increase of AP one hour after the blood recirculation in transplant. An antioxidative defence of organ with ascorbic acid during the cold preservation period removed these increase. The possible mechanisms of the observed phenomena and the information significance of the AP measurement in blood plasma are discussed.
[Changes in the clinical picture of chorea minor].
The paper dealt with the change in clinical picture of chorea minor. It used to be a very frequent disease but now it is all the rarer appearance and it always occurs as a complication of rheumatic disease. Except for its rarer frequency, once it has occurred it is more often complicated by cardiac incidents, the course of disease is longer and more resistant to therapy and is accompanied by psychic changes without frequent relapses observed in its previous appearances. Contemporary immunologic knowledge of this field has detected the complex neuroimmunologic changes taking place in CNS. According to the authors' opinion, modification of the clinical picture is the sequel of the broad application of antibiotics in general but the prevalence of chorea in girls keeps remaining unclear. However, there is no specific attritude toward neuroimmunologic mechanism so far, because previous knowledge of this problem is lacking.
[Laterality in institutionalized patients with moderate, severe and very severe mental retardation and stereotyped behavior].
A total of 54 moderately (n = 13), more severely (n = 27) and severely (n = 14) mentally retarded persons living in institutions were dealt with in this paper. Qualities of their stereotyped actions were analysed and compared in respect to the used lateralization according to Cordić-Bojanin's modified scale and to Berg's scale for used lateralization. Results obtained pointed to the differences among stereotypes themselves in respect to the influence of social environment and the degree of retardation. Therefore, twisting of the body was regarded as the "mildest" form of retardation and passivity, while the motions of hands only were associated with the most severe ones. Used lateralization in the form of dextrality occurred in 62.96%, in the ambidextrality in 14.8% while sinistrality accounted for 22.2%. Right-handed persons most frequently showed stereotyped behaviour which was manifested by the forward-backward motion of the body, while left-handed persons showed stereotypes connected with hands. It was believed that the tonus which represented the termination of the voluntary action and the basis of the development of lateralization would be regarded as the connection between lateralization and stereotypes.
[The familial emotional whirlpool syndrome].
Good intentions and positive emotions of strong potentials predominantly directed toward children are singled out from the group of pathology of familial relationships. Nevertheless, they have negative effects just on children which are the objects of good intentions. By a more profound analysis of very complex emotional relationships a dominant personality--antecedent is detected which authoritatively imposes ethic and behavioural norms taking thus function both of parents and the next generation. Members of family react in various ways in dependence on their structure but they always show their emotional link. Opposition to the dominant person leads to the permanent emotional tension and conflicts which create a maelstrom in which newly come members are also emotionally engaged or rejected always in accordance with the attitudes of dominant person. The problem could be solved only by the therapeutic approach to the whole family. Positive attitudes toward children could be the factor for establishing cooperation with the therapist of the family.
[Reconstruction of the uterine wall in cesarean section by a single-stage double-layer suture--our experience].
The authors described their own experience in usage of the figure-of-eight suture in the uterine wall after caesarean section. The method was used in 42 patients for the period of 1984-1987. In addition the corners of the uterine cut were strengthened by P-like sutures. An arch-like suture, turned with its concave part upwards, was used on the lower uterine segment for opening of the uterus since 1986. Polyamide antimicrobic threads were used for reconstruction. Very good results were found during the early puerperium, which supported the efficiency of the applied method.
[The potentials of echographic diagnosis in pelvic inflammatory disease].
The study was a part of a complex investigation of adnexal inflammatory diseases as a medicosocial problem and its effect on health, working capacity and fertility of the women. Object of the programme was the examination of 529 women, admitted to the gynecological clinic of the Higher Medical Institute in the town of Pleven for the period of 1985-1986. The echographic examination was undertaken before the onset of treatment. Predominating majority of women with pelvic inflammatory pathology was referred to the risk groups, proposed by the authors: young age, intrauterine manipulations, users of intrauterine contraceptive devices, etc. The described diagnostic echographic results were some of the best in the literature respectively for the two clinical groups the noncoincidence between echographic and clinical diagnosis was 10.1 and 11.3%. It should be taken into consideration that the majority of hospitalized patients suffered from chronic adnexitis and diagnosis was established on the basis of multiaspect examination. The specialists, who performed ultrasound examination were well acquainted with concrete clinical and laboratory data.
[Changes in anti-oxidative homeostasis in hemodialysis patients].
Changes of the antioxidative homoeostasis in hemodialysis patients were registered by determination of the antioxidative capacity (AC). This capacity has been increased before dialysis and decreased by 50% during dialysis. After 30 minutes of dialysis the AC was much more decreased than creatinine and urea. The cause of this AC reduction is not only a loss of antioxidants into the dialysate, but possibly also an oxygen radical formation by C5a-activated granulocytes, sequestered in pulmonary capillaries during transient leucopenia.
