[The formulation of the current management of patients with atypical hyperplasia (AH)].

The authors study is the receptor status of 16 patients with AH. The amount of ER and PR in patients with AH is compared to the amount of the same receptors in patients with highly differentiated adenocarcinoma. A progestin treatment is proposed for AH together with dynamic evaluation of the amount of ER and PR. In patients ER, PR, even if they have not complete their reproduction, hysterectomy is proposed. The cases ER+, PR+ are actively followed up. Every patients should undergo complex investigation and evaluation in which together with the receptor status extremely important is the routine biopsy.

[Changes in the fertility indices among professional drivers].

With the purpose of studying the supposed risk for the male inseminative ability, the fertile indices of 80 professional drivers (main group) and 159 representatives of professions accepted as not having professional harms (control group) have been examined. The results show significantly higher percentage part for the drivers with disturbed inseminative ability (66.25%) compared to the males of the control group (35.85%)-(P < 0.001). The date for the frequency of pyospermia are analogical: 28.75% of the drivers versus 16.98% of the control group (P < 0.05). Statistically main differences in the percentage distribution among the smokers, sufferers from chronic prostate or varicocele, carriers of pathogenic microorganisms in the sperm are not noticed, but in all these cases there is an unfavourable tendency for the drivers. The representatives of this widely spread profession among men have to be warned of the risk for possible changes in the fertility.

[Asymptomatic bacteriospermia in men in infertile families].

Bacteriological research of sperm has been done in 160 asymptomatic men from infertile families. A tendency of greater frequency of pathogenetic microorganisms is seen in persons with infringed inseminated capability, but the difference with the control group is not significant. The increased quantity of leucocytes in the sperm of asymptomatic men is not compulsory connected with pathological bacteriological find. The limited diagnostic value of routine microbiological research of sperm in men without symptoms of genital infection has been discussed.

[The social adaptation of older primiparae].

A social-psychological study was carried out on 96 pregnant women divided into three groups: I (control)-20-25 year-old, II-26-34 year-old and III (experimental)-over 35 year-old, the so called elder primiparous women. Giessen test was performed. It was established a positive social resonance in a correspondence with better contact possibilities and a good social potency in the control group. The third group has normal role interrelations in slightly increased intraphysical control and positive social resonance leading to some obedience and depression which explains the embarrassment for the establishment of stable relation and marriage. It is drown a conclusion that elder primiparous women are high risk group from psychological and social-psychological point of view.

[Abdominal delivery in the practice of the Obstetrical Clinic of the Medical University of Pleven (1976-1990)].

The authors have made a profound analysis of the abdominal parturition for a period of time including 15 years. The results in the newborn babies were investigated in parallel. From 3145 SC in neonatological period, 57 babies died. The tendency of widening of the indications for abdominal operative parturition for prematurely born children must be conformed to the conditions of the Center of Perinatology for intensive cares and treatment.

[Sperm quality in smokers and nonsmokers among infertile families].

In 169 men from infertile couples were investigated the following sperm parameters--concentration, motility, morphology, in order to find the possible influence of smoking on sperm quality. The percentage of men with deviations from normal sperm motility is significantly higher among smokers (35.65%) in comparison with the non-smokers group (20.04%), p < 0.05. We found a higher percentage of men with changes in sperm morphology among smokers (40.00%) in comparison with non smokers. As for the concentration of spermatozoa, no significant difference was found between the two groups. We came to the conclusion that there exists a trend of increase of the percentage of men with deviations in motility and morphology with the increase of the cigarettes smoked per day.

[Primiparae--the role of age on the duration and disorders in the progression of labor].

The authors have made a study of age influence on the delivery duration and the breaches of delivery progress by women giving birth for the first time. In 6 groups 742 first birth giving women are investigated. The authors have found out a correlation between the age growth, particularly after 25 years, and the total delivery duration increase. Using the Friedman's partogram, the delivery activity have been analyzed. Significance of delivery progress breaches are found out mostly during the active phase-protracted active phase and secondary protracted active phase. There wasn't found out correlations between the age and growth and the latent phase protraction, and the arrest disorders.

[Hysterectomy in obstetrical practice (1979-1990)].

Caesarean hysterectomy and hysterectomy after vaginal delivery from 1979 to 1990, the indications, the complications and the mother's outcome are retrospective investigated. By 41010 births the hysterectomy rate is 0.068%, caesarean hysterectomy-15 (0.056%), and hysterectomy after vaginal delivery-12 (0.030%). The common est caesarean hysterectomy indications are infected uterus, haemorrhage, myoma, carcinoma colli uteri (80%). Haemorrhage and rupture uteri (75%) are the general hysterectomy indications after vaginal birth. A low complication rate is found out, 3 women are deceased (11.11%) in the group of hysterectomy after vaginal delivery. The authors have underlined benefits of the preferred recently organ keeping approach.

[The tumor markers CEA, AFP and CA125 in patients with endometrial carcinoma].

The plasma concentration of CEA, AFP and CA125 in 54 women with endometrial carcinoma and 12 with atypical hyperplasias, were investigated. In 20 of the patients the tumor markers were studied again 11 months after the surgical treatment. Elevation of CEA, AFP and CA125 levels was found in the third, and especially in the forth phase of the illness. Essential similarities between the histological differentiation and the levels of the tumor markers were not observed. The supposition was that the study of the CEA, AFP and CA125 is not a reliable criterion for the early diagnosis of the endometrial carcinoma. They have a prognostic significance only in patients of the second clinical phase after a surgical treatment. It is recommended two of the markers to be used, mostly CEA and CA125 and the study to be performed several times.

PROFORCE--a new, high-biologic-value protein product for athletes.

The protein product PROFORCE has been developed from a base of egg, milk and cereal using linear optimization. Its balanced composition has been specially designed to meet the physiological needs of athletes practicing endurance-demanding sports. A medicobiologic evaluation using a routine technique was performed using three concentration levels of protein feeding. This enhanced the range of the investigated parameters. The following results were obtained: Protein Efficiency Ratio (PER) = 2.64 +/- 0.22 (p < 0.001); Net Protein Ratio (NPR) = 3.27 +/- 0.22 (p < 0.001). Comparison with the control group shows that the protein product is of high quality. Albumin, cholesterol, triglycerides and total protein were determined for the three protein-feeding concentrations. The most favorable influence on the protein parameters and cholesterol levels were observed when feeding experimental rats on the nine-percent protein concentration. Amino acid content of the product was compared to a number of proteins of proven high quality. The comparison suggests that the product is suitable for athletes practicing endurance-demanding sports. The conclusion is that PROFORCE has a wide range of applications which would include use by athletes practicing aerobic sports.

[Episiotomy repair by the method of W. B. Shute].

For about 1 year 106 episiotomy are reapproximated using figure-of-eight sutures by W. B. Shute and 623 by using conventional techniques. Early (4-6 days) and lately (6 months and more) results are compared in these 2 groups. Authors have found out benefits of Shute's technique and recommend to use it as a routine procedure in current obstetric practice.

[The effect of E2 and progesterone on the receptor status in endometrial carcinoma].

The authors study the plasma levels of E2 and progesterone in 62 women with EC. Parallelly the ER and PR in the same number of cases have been studied. The values of E2 in the plasma of ER+ are much lower than those of E2 in ER-. The quantity of E2 and progesterone influence the values of ER and PR in cancers of women in pre and postmenopausal age. In positive ER and PR patients the level of E2 and progesterone is lower in comparison of ER- and PR-.

[Extrauterine pregnancy--the clinical experience of using a new differential-diagnostic immunological test].

The authors report results of immunologic test, that has been used to help the decision in some difficult for differential diagnosis cases. Two immunologic parameters were investigated--the amount of circulating immune complexes (CIC) and the lymphocytic nuclear coefficient (LNC). The results are put on a specially designed nomogram. The combined test used in the Department of "Obstetrics and Gynaecology"--Pleven, gives excellent results. The advantages of the test are discussed and a clinical case is presented as an illustration.

[Drug therapy of nocturnal enuresis].

Bed wetting is a disorder the origin of which different authors find in different etiopathogenetic mechanisms. Depending on the etiologic concept different therapeutic methods have been used. Treating bed wetting, authors claiming that the basic cause is a disorder in the integration of central vegetative functions and sleep, most frequently administer anticholinergic agents, making use of their marginal central anticholinergic effect and their impact on sleep phases. For children who wet in bed, having an urodynamic disorder and the disordered function of the pelvic floor musculature, effective drugs would be those with peripheral anticholinergic effects as well as pelvic floor musculature reeducation exercises. In conformity with the current understanding of the bed wetting etiology due to disordered Adiuretin excretion circadian rhythm, the applied drug is synthetic Adiuretin. It has been pointed out that bed wetting, being a heterogeneous group of disorders, should be etiologically classified by additional diagnostic methods, and that the treatment should include aimed etiological therapeutical methods.

A deficient function of the antioxidative system of the organism as an aetiopathogenetic factor in psoriasis.

The main idea of a new hypothesis about the aetiopathogenesis of psoriasis concerns the significance of interrelation between pro- and antioxidants, vitamin A-dependent keratin synthesis and proliferation of keratinocytes. On the basis of postulated deregulation in the antioxidative system of the organism, we explain the predisposition, the initiation and pathogenesis of lesions, their persistence and resolution and the operation mechanisms of antipsoriatics.

[The role of cardiotocographic study in older primiparae].

The authors aimed to study the significance of cardiotocographic examination (ECTE) for determination of fetal risk in elderly primiparas (EP). Data on NST, one the one hand, and the state of the newborn and perinatal outcome on the other, were, compared for its realization. 207 primiparas were investigated, 73, of whom were EP (35 years). The fetal reactivity and variability of fetal heart frequency (FHF) were evaluated during the performed NST. Semiquantitative evaluation of FHF was also made, in order to improve the quality of NST. FHF score was examined as well. The following inferences were made as a result of the examination: 1. Lacking reactivity of the fetus, variability 5 min beats per min, FHF score--6 points and even curve of NST as a signal for fetal distress were found mainly in EP. 2. These cardiotocographic criteria were encountered in EP with accompanying and worsening the pregnancy diseases and should pray a leading role in determining obstetric management. 3. The performance of ECTE is compulsory in EP.
