[Reducing the side effects of aggressive chemotherapy (cisplatin and epirubicin) with xenogenic peptides (factor AF2) in patients with hormone refractory metastatic prostate cancer. A prospective, randomized study].

The indication of a chemotherapy is advisable with patients who are suffering from a progressively metastasised, secondarily hormone refractory carcinoma of the prostate. In search of efficient chemotherapy protocols we combined cisplatin with epirubicin (PE scheme) in our clinic. Massive side effects of that aggressive chemotherapy scheme like gastro-intestinal trouble and myelotoxicity are the limiting factors of the scheme. With measures like reducing the dosage, delaying the next cycle, or breaking off the therapy the effective dosage can often not be achieved. The anti-emetics which are usually used today exclusively give anti-emetic protection. The additional administration of xenogenic peptides (Factor AF2) had additionally myeloprotective effect in former studies. In this study we examined whether, by additionally giving Factor AF2, the patients' subjective condition, and above all their hemogram, could be stabilised in order to achieve the effective dosage or dosage intensity. For that, the patients were prospectively randomised in two groups by means of a random selection board. The analysis of the data gained in the protocol showed that the additional administration of Factor AF2 improves the patients' subjective conditions significantly. Apart from that, we noticed a considerable reduction of the vomiting frequency. Concerning the objective measured parameters of the leukocytes, thrombocytes, erythrocytes, and the hemoglobin level, the significantly myeloprotective effect of Factor AF2 could be proved. Due to the fact that in the verum group there were considerably fewer cases of breaking off or delays of the treatment than in the control group, the effective dosage intensity could be achieved with a higher number of patients in that group.

[Xanthogranulomatous pyelonephritis with fatal outcome in a 2-month-old infant].

A 2-month-old infant died of xanthogranulomatous pyelonephritis. Preoperatively pyonephrosis was suspected, because the child presented with a number of inflammatory, septic symptoms. Nephrectomy was performed and histopathology showed the kidney to be affected by xanthogranulomatous pyelonephritis. Postoperatively the child developed persistent attacks of fever and bronchopneumonia that led to his death with signs of pulmonary insufficiency.

Monocyte/macrophage-directed antibodies Ki-M3 and Ki-M7 detect renal-cell carcinomas.

Monoclonal antibodies (MAbs) Ki-M3 and Ki-M7, which detect cells of the human monocyte/macrophage lineages, were tested using immunohistochemistry on a series of benign and malignant human tumors (n = 43). Positive staining was detected in all renal-cell carcinomas (n = 19 positive for both Ki-M3 and Ki-M7), a few cases of breast carcinoma (n = 6 for Ki-M3; n = 2 for Ki-M7), in 1 case each of prostatic carcinoma (Ki-M3) and prostatic adenoma (Ki-M3), and in 1 case of endometriosis (Ki-M3). In normal human kidney Ki-M3 and Ki-M7 reacted with proximal tubules and parts of the distal tubules and with the loop of Henle. The Ki-M3 and Ki-M7 MAbs proved useful in detecting renal-cell carcinomas and in distinguishing these tumors and their metastases from morphologically similar neoplasms: in addition, our study further hints that renal cell carcinomas are derived from tubular epithelial cells.

Diagnosis of urothelial tumor by ureterorenoscopy.

In the past, radiologically detected filling defects in the upper urinary tract were correctly interpretable only by means of cytological techniques. The introduction of ureterorenoscopy has provided a reliable method for the detection of tumors in the ureter and renal pelvis. In contrast to transurethral ureterolithotripsy, diagnostic ureterorenoscopy entails few complications. It thus fills a diagnostic gap and facilitates therapeutic planning. A report is presented on 15 patients with obscure hematuria, or unclear filling defects in the upper urinary tract. In 10 patients, the unequivocal diagnosis of urothelial carcinoma was endoscopically and bioptically established by ureterorenoscopy. Small nonradiopaque concrements were found in 3 cases. The origin of hemorrhage could not be determined in 2 patients.

[Disorders of wound healing using an infrapubic Kelâmi incision in comparison to scrotal incision].

The infrapubic incision technique developed by Kelâmi for andrological operations was examined with reference to the healing process. The collective studied comprised 200 patients treated 100 each with infrapubic and scrotal incisions. The comparative examination showed delayed wound healing in only 2% of the patients with an infrapubic incision, whereas 37% of those with a scrotal incision evidenced disturbed healing processes.

[Complications following surgery for female urinary incontinence].

The two competitive methods available for operative treatment of female stress incontinence are the suspension procedures according to Marshall-Marchetti and according to Burch. With respect to continence, equally favorable results are achieved using both dethods. One way of assessing their value is by measuring the complications involved. A report is presented on osteomyelitis of the pubic symphysis in two cases after the Marshall-Marchetti operation and hydronephrosis in one case after the Burch operation. In all three patients, the complications were accompanied by postoperative fever as an early symptom.

[Transurethral ureterolithotripsy in the treatment of ureteral calculi].

We report our experiences with transurethral ureterolithotripsy for treatment of ureteral calculi in 68 patients. 67% of the ureteral stones were extracted successfully. Failures and complications occur often by higher localisation of concrements. Only in 2 patients major complications occurred. Late complications (Ureter Manipulation Syndrome-UMS) were not observed in our patients. In cases with ureteral perforations or mucous membrane lesions ureteral stent must be used.

[Use of human fibrin glue in the closure of vesicovaginal fistulas].

This report describes animal experiments on the use of human fibrin adhesives to close vesicovaginal fistulas artificially created in 20 rabbits. In one group of 10 animals one of the usual closure techniques was supplemented by application of fibrin adhesive to the suture line, whereas in the control group of 10 animals the fistula was closed using only the same usual suture technique. The results were evaluated according to pathoanatomical and histological criteria. In the control group a vesicovaginal fistula recurred in 30% of the animals, whereas the test group showed 100% closure during the observation period.

[Urogenital bilharziasis in Germany].

Urogenital schistosomiasis was confirmed in four patients, three Egyptians and one from Guinea. They were admitted with typical complaints, some chronic some acute, and successfully treated with praziquantel (Biltrizide). Since a single oral dose is sufficient, this is a greatly simplified form of treatment, without significant side effects.

[Microsurgical splinted tubulovasostomy in rats].

Due to unsatisfactory results from conventional epididymovasostomies for treating male infertility because of the obstruction of the ejaculatory tract, a new method was tried by splinting the tubulovasostomy. This was first described by Ozdiler and Kelâmi. After 3-6 months, a total of 33% of the anastomoses were patent. With the surgeon's increased skill and practice of the operation, the patency of the anastomoses of the last group increased to 50% (15/30).

[Preoperative irradiation of hypernephroid renal carcinoma].

Two groups of patients with renal cell carcinoma were compared to test the effect of preoperative short-term-radiation: 30 patients with hypernephroma, were treated by the same method (nephrectomy and postoperative radiation) and a farther 30 patients were treated for one week with preoperative radiation (a total dose of 18 to 21 Gy), radical nephrectomy performed after a 3 to 4 day break and postoperative radiation. At the same time a clear increase was shown in the 3-year-survival period as well as that of the 5-year-period. Without preoperative radiotherapy 36.7% survived for 3 years and 33.4% for 5 years, with preoperative radiation 56.7% for 3 years and 50% for 5 years.

[Renal artery embolization by the Gianturco method].

Over a period of 2 years pre-operative embolization of the renal artery was carried out according to the method of Gianturco, which offers certain advantages over other methods of embolization, in 25 patients with renal tumours. The most important aims of pre-operative embolization of the renal artery were considered to be to prevent tumour cells from being flooded out and to be able to ligate the vena renalis first. Because of the clinical reaction to be expected the embolization should be performed immediately before the operation, which still allows the aims to be achieved. In our opinion embolization cannot replace pre-operative irradiation.

Examinations of polyvidone-iodine as an irrigation solution for spermatic duct occlusions.

In vitro experiments with human sperm as well as animal experiments were carried out to test the applicability of polyvidone-iodine as an irrigation solution for spermatic duct occlusions. The substance polyvidone-iodine was initially chosen on the basis of spermiograms because of its ability to immobilize spermatozoa, and then its spermicidal effect was clearly proven by means of eosin vital staining. The aim of the animal experiments was to examine the substance for possible histotoxic side effects and to ascertain the retention of residual sperm in the ampulla area of the vas deferens. Polyvidone-iodine fulfills all the required criteria as an irrigation solution: it is definitely spermicidal with no long-term histotoxic side effects; in addition, it acts as a disinfectant and is easily obtainable as well as inexpensive. Postoperative azoospermia is attained in animal experiments after four weeks at the most.

This is the first of a series of articles that will discuss some of the concepts associated with transcultural nursing. It suggests that nurse education fails in any significant way to deliver culturally sensitive care.

This article examines the problems of the multicultural approach in our multiracial society. It describes a need for nurses to be aware of the contribution of racism to inequalities in health care and to have insight into cultural and ethnic issues.

Health issues and the Greek Cypriot woman.

This article aims to help health care workers begin to understand the Greek Cypriot woman, her needs, problems and strengths. It will try to explore briefly how Greek Cypriot women perceive health and illness and will discuss factors that influence these perceptions. It will also examine some of the major health problems that Greek Cypriot women face. The article uses material from the author's on-going research into the health needs of the Greek Cypriot community living in North London.
