[Screening for colorectal cancer based on our experience with early colorectal cancer and adenoma].

In order to establish method of screening for colorectal cancer, we studied clinicopathological features in 116 cases of adenoma, precancerous lesion, and in 80 cases of early colorectal cancer. The incidence of early cancer began to rise at the age of more than 40, reaching a peak in the sixth decade. The risk for men was higher than for women. In relation to the location of these lesions, 71% of the adenoma and 91% of the early cancer were in the left colon and rectum. As symptoms on admission, overt bleeding from the rectum appeared in 35% of the adenoma and in 56% of the early cancer patients, respectively. Therefore, we emphasize that it is extremely important to examine overt and occult bleeding as the first screening for colorectal cancer.

[The significance of the measurement of a serum gonadotropin-like substance in gastric cancer].

The serum concentration of hCG and CEA in 277 gastric cancers was extensively studied. 8.6% for hCG and 18% for CEA were regarded as positive. 9.4% of patients showed elevated serum level for both hCG and CEA, on the other hand 7% of the patients showed high hCG only. All of hCG positive patients had severe lymph nodal involvement. All cases of hCG positive and 85% of CEA positive cases were classified as stage 4. Macroscopically, 85% of hCG positive cases and 40% of CEA positive cases showed generalized type. Histological type was poorly differentiated adenocarcinoma in 72% of tumors with elevated serum hCG level, but in only 35% of tumors with elevated serum CEA level. The content of hCG subunits in serum and tumor showed different levels among patients. The hCG positive cases did not reveal endocrine symptom. The prognosis of these patients with elevated serum hCG and CEA level were poor. One year survival rates were 0% and 30%, respectively. These results showed that both hCG and CEA, especially when used in combination, was potentially useful marker for determination of staging, recurrence and prognosis in gastric cancer.

[The effect of continuous hyperthermic peritoneal perfusion using high-dose cis-diamminedichloroplatinum for peritoneal dissemination of gastric cancers].

A continuous hyperthermic peritoneal perfusion using high-dose cis-diamminedichloroplatinum (CDDP) and its antidote, sodium thiosulfate, was applied to eight patients with peritoneal metastasis from gastric cancers. Two of eight patients showed partial response and the rate of decrease of ascites was 38%. Renal failure and leukopenia were observed in one and two patients, respectively, but no severe or lethal dysfunction was observed and all were tolerated. These results showed that CDDP exerted anti-tumor effects on peritoneal metastasis from gastric cancers when used in high doses in combination with hyperthermia.

Perfusion preservation of cadaver rat pancreas: I. Morphological observation and biological function of the islets.

When using methods of perfusion to preserve the rat pancreas, we found that perfusion into the celiac axis was the most effective. The viability of the cadaver pancreas from decapitated rats preserved by perfusion into the celiac axis for 6 hours under various conditions of hypothermia, hyperbaria and oxygenation was investigated. The condition of perfusion affected the ratio of degenerative islets/normal islets in the pancreas. The ratio, under conditions of hypothermia and oxygenation was the lowest, while that under conditions of hyperbaria was the highest. Insulin-releasing activity of islets from 6-hour-perfusion-pancreas, under conditions of hypothermia and oxygenation was 86.5 per cent or more of that of the control. Stainings with fluorescent antibody and peroxidase antiperoxidase, revealed a large number of A and B cells in the islets of the pancreas, in cases of up to 6 hours of perfusion.

Perfusion preservation of cadaver rat pancreas: II. Culture after perfusion and successful transplantation.

We designed a new process for culturing pancreatic islets and applied this method to cultures of rat pancreatic islets which had degenerated during preservation by the perfusion-method for 6 hours under the condition of hypothermia and oxygenation. The objective was to determine the extent of the original function. Transplantation of these so-treated islets was also attempted. When pancreatic islets isolated from the pancreas after 6-hour-perfusion were cultured, morphological restoration was apparent within the first 3-4 days. Insulin contents of the culture media renewed every 3 days, ranged from 851 to 1,134 microU/ml/two islets during culture period of 21 days. In the glucose-loading test, insulin secretion of the islets was the same as that of islets in the control experiments. Transplantation of these islets into the portal vein of streptozotocin-induced diabetic rats resulted in a good recovery from the diabetic state.

Hepatic resection for primary liver malignancy.

The results of hepatic resection for patients with primary liver malignancy seen at our clinic during the past 21 years are reported. Of 92 patients, 57 had cirrhosis in addition to hepatocellular carcinoma, and 49 (53 percent) underwent hepatic resection of various degrees from partial resection to trisegmentectomy. Resectability rates of the liver were 52 percent in 77 patients with hepatocellular carcinoma, including 19 in whom the tumor was less than 5 cm in diameter, and 60 percent in 15 patients with other malignant tumors; operative mortality rates were 15 percent in the former and 0 percent in the latter. Cumulative survival rates of all patients who underwent hepatic resection, excluding death within one month, were 55 percent at one year, 29 at 3 and 5 years. In patients with hepatocellular carcinoma, survival rates of 15 those who had a curative resection of the tumor were 87 percent at one year and 47 percent at 3 or 5 years, there was a significant difference in survival curves between those with tumors less than 5 cm and more than 5 cm (p less than 0.05). On the other hand, survival rates of all patients with other malignant tumors were 78 percent at one year and 37 percent at 5 years. These results indicate the importance of performing hepatic resection for patients with small hepatocellular carcinoma associated with cirrhosis and those with other malignant tumors.

Clinicopathological studies of minimal thyroid and ordinary thyroid cancers.

Clinicopathological studies were carried out on 27 patients with minimal thyroid cancer and 56 with ordinary thyroid cancer at Kanazawa University, from April, 1979 to December, 1982. There was a significant difference in the rate of preoperative diagnosis between the minimal and the ordinary cancer groups. Subtotal thyroidectomy with modified neck dissection was usually performed in both groups. The histological types in minimal cancer group included 16 papillary carcinomas, 7 nonencapsulated sclerosing carcinomas, one follicular carcinoma, one anaplastic carcinoma with squamous cell metaplasia and two multiple mixed carcinomas. There was no significant difference in the rates of intrathyroidal and lymph node metastases between the two groups. While the metastatic lesions of sclerosing carcinoma were localized to the central cervical lymph nodes, the ordinary cancer in general and the papillary variant of minimal cancer metastasized not only to the central cervical lymph nodes but also to the ipsilateral and even to the contralateral jugular lymph nodes.

Amelioration of diabetes mellitus in partially depancreatized rats by poly(ADP-ribose) synthetase inhibitors. Evidence of islet B-cell regeneration.

Ninety percent depancreatized rats received daily intraperitoneal injection of 0.5 g/kg nicotinamide and 0.05 g/kg 3-aminobenzamide, potent inhibitors of islet poly(ADP-ribose) synthetase. One to three months after the partial pancreatectomy, urinary and plasma glucose levels in nicotinamide- and 3-aminobenzamide-treated rats were markedly lower than those in saline-treated control rats. Morphologic examination of the remaining pancreata revealed that islets in the poly(ADP-ribose) synthetase inhibitor-treated rats were markedly enlarged and consisted largely of B-cells. These results suggest that poly(ADP-ribose) synthetase inhibitors induce islet B-cell regeneration, thereby preventing or improving diabetes mellitus in partially depancreatized rats.

[The pathogenesis of diabetes produced by subtotal pancreatectomy, with special reference to the cell kinetics of the islet cells].

The pathogenesis of the diabetes produced by the subtotal pancreatectomy was investigated from the pancreatic endocrine function and the cell kinetics of the islet cells. The authors prepared the following two groups in the rats. The first group was 90% pancreatectomized rats and the second group was the 60% pancreatectomized rats. Most of the 90% pancreatectomized rats developed diabetes twelve weeks after operation. In these diabetic rats the insulin secretion was undetectable during glucose infusion. To the contrary, the relative increase of the glucagon secretion was found during arginine infusion in spite of the hyperglycemia. However, non of the 60% pancreatectomized rats developed diabetes during post-operative course up to 6 months. To investigate the cell kinetics of the islet cells, the 3H-thymidine autoradiography was prepared at several intervals after operation. The results of the thymidine labelling index showed two phase of regeneration. The first phase was observed on three to seven days after pancreatectomy and the second phase was on fourteen to twenty-eight days. After the second phase of regeneration, the degeneration of the B-cells was observed in the 90% pancreatectomized rats. On the ninetieth days after operation the atrophy of the islets and the disturbance of the arrangement of A, B and D cells were observed by the immuno-histochemical studies. Meanwhile, the 60% pancreatectomized rats did not show degeneration of the islets. These results show that the onset of diabetes after subtotal pancreatectomy was produced by degeneration of the B cells surpassing their regenerative capacity.

[Evaluation of preoperative administration of N1-(2-tetrahydrofuryl)-5-fluorouracil (FT-207) suppository in surgical adjuvant chemotherapy for large bowel cancer].

A prospective randomized study was performed for 72 patients with large bowel cancer, and 36 cases each of rectal cancer and colonic cancer, who had received curative resection respectively. The regimen consisted of two adjuvant chemotherapies: group A, postoperative administration of FT-207 suppository; group B; preoperative and postoperative administration of the same suppository. A follow-up study was then done. The results revealed the 5-year survival rate to be 65.1% for group A and 72.4% for group, B respectively. With rectal cancer, 5-year survival was 57.8% for group A and 70.5% for group B respectively. In colonic cancer the figures were 70.8% for group A and 75.0% for group B. Thus, there was no significant difference but a somewhat higher survival rate was observed in the preoperative plus postoperative administration group. Comparison of in the prognosis two groups classified according to the degree of nodal metastasis and invasion revealed good results in group B. The above-mentioned facts suggest that pre- and post-operative administration of FT-207 suppositories as adjuvant chemotherapy for colorectal cancer is superior to postoperative use alone.

[A case of gastric carcinoid associated with multiple early gastric cancers and atypical epithelium].

A 69-year-old man was admitted to our hospital complaining of the sensation of abdominal fullness and tarry stool. X-ray and endoscopic examination revealed a gastric tumor measuring 6.5 X 7.5 cm in the antrum along the greater curvature. Histologically, the tumor was diagnosed as carcinoid; five lesions of early gastric cancer and one lesion of atypical epithelium were also detected. Through 1982, 24 Japanese patients with gastric carcinoid associated with gastric cancer have been reported.

Intracystic carcinoma of the breast in an elderly man.

An eighty year old man with a mass in the left breast visited our department on October 27, 1980. Examination disclosed a smooth surfaced, round, elastic soft, 3 cm in diameter sized, non-tender mass underneath the left nipple. Mammogram showed a well defined oval opacity and echogram showed a cystic lesion. The mass was punctured and dark brown fluid was aspirated, but there were no malignant cells. The cyst was extirpated and a rice grain sized mass was found in the cyst wall. Histologically, it was diagnosed as non-infiltrating medullary tubular carcinoma. On November 27, 1980, simple mastectomy revealed no histological evidence of a residual cancer.

Radical mastectomy with intrapleural en bloc resection of internal mammary lymph node by sternal splitting.

An operative method of extended radical mastectomy involving intrapleural en bloc resection of the internal mammary lymph nodes by sternal split was proposed. The operation enables complete resection of axillary and internal mammary lymph nodes. Seventy patients with primary breast cancer underwent the extended operation and two patients with recurrence in the internal mammary lymphnode chain following standard radical mastectomy underwent resection of internal mammary lymphnodes by sternal split, with no fatalities and no increase in postoperative disabilities. Metastases to the internal mammary lymphnode chain were histopathologically found in 14 of 70 patients with primary breast cancer and in two with recurrence in the internal mammary lymphnode chain, following standard radical mastectomy; The location of the metastatic internal mammary lymphnodes was from just below the subclavicular vein to the third intercostal space along the internal mammary vessels. Cancer cells were seen not only in the lymphnodes, but also in lymphatics of areolar tissue near the node or in lymphatics between the parietal pleura and endothoracic fascia in patients with primary breast cancer. And cancer invasion to parietal pleura was seen in patients with recurrence in the internal mammary lymphnode chain following standard radical mastectomy. All these findings indicate the rationality of our extended procedures.
