Follow-up studies in pattern VECP in demyelinating diseases in children.

Demyelinating disease in children is known to be relatively rare. We report three cases defined as Devic's disease, acute disseminated encephalomyelitis, and multiple sclerosis. Follow-up studies of pattern appearance-disappearance visual evoked cortical potentials (VECPs) and pattern reversal steady state VECPs were performed. Results showed delays of appearance-disappearance VECPs that improved to shorter latencies with the improvement of visual acuity; however, recovery of VECPs was found to be behind the recovery of subjective vision. On the other hand, steady-state VECPs showed no normal waveforms even when vision recovered. Recovery of steady-state VECPs was found to be far behind that of transient VECPs and vision.

Effects of branched chain enriched amino acid solutions on septic rats.

An experimental study was performed to determine the effects of a BCAA enriched amino acid solution on sepsis in rats. After sepsis had been induced in male Wistar rats by caecal ligation and puncture the animals were divided into four groups according to the amino acid solutions infused: Group B-I, a solution containing no amino acid; Group B-II, a commercially available solution containing 21.1% branched chain amino acids (BCAA); Group B-III, a solution containing 38.7% BCAA and Group B-IV, that containing BCAA only. The control group of the animals received simple laparotomy and infusion without amino acids. Infusion was continued for 96 hours after the operation and all the solutions had the same caloric and nitrogen value. Group B-I had a plasma aminogram peculiar to sepsis; a significant rise in the concentration of the total 17 amino acids (TAA), BCAA and aromatic amino acid (AAA) together with a decrease in the BCAA/AAA ratio. Furthermore, the arterio-venous difference (AV-D) of AAA and BCAA showed a large negative value. In the three groups infused with amino acids, only Group B-III showed a significant improvement in the survival rate and nitrogen balance compared with Group B-I. Moreover, not only did the concentration of TAA and AAA significantly decline, but also the BCAA/AAA ratio increased. Therefore, the abnormal aminogram in sepsis tended to return to the control level. In Group B-IV despite the significant decrease of the peripheral TAA, no improvement in plasma aminogram, nitrogen balance or survival rate were shown. In Group B-II, in addition to the persisting plasma aminogram changes and peripheral efflux of amino acids, no improvement of survival rate were noted. Therefore, it was concluded that an amino acid composition richer in BCAA and lower in both AAA and SCAA is beneficial in the treatment of sepsis.

[HCG-producing intrahepatic cholangiocarcinoma].

The serum level of human chorionic gonadotropin (hCG) in a 60-year-old man with intrahepatic cholangiocarcinoma was found to be abnormal (44.6 mIU/ml) at the diagnosis. The patient underwent right hemihepatectomy, and the high serum hCG returned to normal 28 days postoperatively. Six months after the operation, the cholangiocarcinoma recurred in the remnant liver and the hCG level was again elevated to 268 mIU/ml. The patient died of liver failure eight months after the operation. Immunohistochemical studies of the resected tumor showed hCG-positive cells in the cancer nest. In this case, it is (believed) that the intrahepatic cholangiocarcinoma produced hCG and secreted it to the serum.

[Huge Virchow's metastasis in gastric cancer made resectable by hyperthermochemoradiotherapy].

A 75-year-old woman who had undergone gastrectomy for gastric cancer at the age of 61 developed a huge left supraclavicular tumor (15 X 10 X 10 cm) suspected of being Virchow's node. She was treated with hyperthermochemoradiotherapy (HCT-therapy). After 10 sessions of radiofrequency hyperthermia by a Thermotron RF8, irradiation with 52.8 Gy of 60Co and injection of 8 mg of MMC, the tumor mass decreased by 77% on CT. The histologically resected specimen revealed coagulation necrosis in almost all areas. This case, which was considered HCR-therapy-effective, was estimated as Grade 3 according to The General Rules for the Gastric Cancer Study (The 11th edition.)

[A case of advanced esophageal cancer resected curatively by preoperative chemotherapy with cisplatin].

A case of highly advanced esophageal cancer with bilateral multiple cervical lymph-node metastasis is reported. The largest size of lymph node was 5.0 cm in diameter, and intramural metastasis to the cardia of the stomach and provable invasion to the aorta were also seen. After treatment, with 100 mg/m2 cisplatin on two occasions these lesions were resected curatively. This preoperative chemotherapy would be expected to increase the resectability rate and possibly the survival rate.

[Carcinoma of the biliary tract].

Six problems were pointed out in General Rules for Surgical Studies on Cancer of Biliary Tract; division of extrahepatic bile duct and papilla of Vater, definition of carcinoma of the bile duct, gallbladder and papilla of Vater, classification of the lymphnodes, macroscopic classification of cancer of the biliary tract, tumor size and staging, pathological rules. Furthermore, in 15 cases of the gallbladder carcinoma and 21 cases of the bile duct carcinoma, the correlation between the prognosis and the histological findings of cancer invasion to the wall or of lymphnode metastasis was evaluated.

[Multiple rectal carcinoids--with special reference to the histogenesis of these lesions].

A 61 year-old man was diagnosed prior to surgery as having multiple carcinoids of the rectum. Postoperative histological examination revealed 52 carcinoids, 1-6 mm in diameter, invading the submucosal layer, and innumerable endocrine cell micronests, less than 0.1 mm in diameter, were present in the rectum. A careful examination revealed several argyrophil cell micronests "budding from" the basis of the glands. Most of the tumor cells showed a positive reaction for Grimelius stain and some immunoreactivities for GLI, PP, PYY, insulin and NSE by the PAP method. Moreover, PYY cell hyperplasia was detected in the rectal mucosa, markedly in the areas surrounding the carcinoids. The authors suggest that (initiative) progenitor cells may develop into these lesions due to unknown factors.

[A study of malignancy in gastro-intestinal carcinoids with special reference to the association of the prognosis and peptide hormone or oncofetal protein producing pattern].

It is difficult to differentiate malignant carcinoid tumors from benign ones early in the course of the illness. We investigated retrospectively a series of thirty-nine gastro-intestinal carcinoids not only for primary tumor anatomic site, macroscopic type, tumor size, histological depth, vessel invasion and mitosis, but also for peptide, ectopic hormone and oncofetal proteins by immunohistochemistry. Carcinoids primary in the appendix or rectum rarely metastasize, but those primary in the stomach or colon grow rapidly. Those greater than 2 cm in diameter and Borrmann's type had a poor prognosis. Carcinoids with deep penetration, vessel invasion or mitosis easily metastasize to lymph nodes or to the liver. Peptide hormone producing carcinoids were present in 21 of 39 (54%) tumors, but there were no correlation between the prognosis and peptide hormone producing patterns. Carcinoids producing carcinoembryonic antigen (CEA) were demonstrated in 11 of 37 (29%) tumors. These tumors were greater in size, more deeply penetrating and had more remarkable vessel invasion and mitosis than CEA non-producing ones. These results show that CEA producing carcinoids grow rapidly and easily metastasize. We concluded that these findings in carcinoid tissue appeared to be a reliable markers for metastasis and poor prognosis.

[Distribution of 1-hexycarbamoyl-5-fluorouracil in rats and human patients, and clinical results in patients with colorectal cancer].

The distribution of 1-hexylcarbamoyl-5-fluorouracil (HCFU), one of the "masked-type" derivatives of 5-fluorouracil (5-FU), was studied following oral administration in rats and human patients with colorectal cancer. In rats, the highest level of 5-FU from HCFU was shown in the stomach and kidney, followed by the large intestine. After oral administration of HCFU for seven days, the level of 5-FU in tumors from patients was higher than that in tumors from patients given single administration. The clinical response of HCFU administered orally was then evaluated in patients with colorectal cancer. Out of 25 evaluable patients the overall response rate including a CR and six PRs was 28.0%. Overall survival rates for patients were 75.0% in one year, 32.0% in two years, and 13.6% in three years. From these results, it was suggested that HCFU is a useful anticancer agent for colorectal cancer.

Management of pancreatic pseudocysts in 42 patients with inflammatory or traumatic cysts.

In attempts to determine and appropriate treatment for pancreatic pseudocysts, forty-two cases were reviewed. Twenty-three were inflammatory and nineteen were traumatic. Elevation of serum amylase levels and white blood cell count occurred more frequently in patients with traumatic cysts than in those with inflammatory ones. Filling of the pseudocysts occurred in eleven of twelve patients in whom endoscopic retrograde pancreatography (ERP) had been performed. Spontaneous regression of the cysts occurred more frequently in those with traumatic cysts (42 per cent) than in those with inflammatory cysts (26 per cent). Excision of the cysts were performed in seven of twenty-three patients with inflammatory cysts; external drainage in five with inflammatory cysts and in one with traumatic cysts; and internal drainage in five with inflammatory cysts and ten with traumatic cysts. There was a recurrence of the cysts in two patients, one due to multiple stenosis of the pancreatic duct located to the right of the resected cysts, and the other was caused by an anastomotic stenosis of the cystogastrostomy. One persistent pancreatic fistula following external drainage was treated by fistulogastrostomy. We recommend the evaluation of the condition of pancreatic duct by ERP for individualizing pancreatic pseudocysts.

[Gonadotropin and alkaline phosphatase-producing gastric carcinoma with widespread metastasis to the bones].

A case of advanced gastric carcinoma in a 47-year-old man with widespread metastasis of generalized bone and associated with high serum levels of human chorionic gonadotropin (hCG) and alkaline phosphatase (ALP) is reported. Macroscopically, the primary tumor was Borrmann type IV. Microscopically, it showed poorly differentiated adenocarcinoma. Bone metastasis, however, showed well-differentiated adenocarcinoma. These lesions did not manifest conspicuous trophoblastic differentiation. Immunohistochemical studies revealed that hCG-positive cells were observed sporadically in the primary tumor and lymph node metastasis, but remarkably in the bone metastasis. Cancer cells in the lymph nodes were positively stained by modified Burstone's ALP staining. The serum levels of hCG and ALP fluctuated with the clinical course. Therefore, the authors concluded that this gastric carcinoma produced hCG and ALP.

[A case of gastric cancer successfully treated with hyperthermo-chemo-radiotherapy].

A 56-year-old woman was diagnosed as having gastric cancer with lymph node metastasis and invasion to the pancreas head and was admitted to our hospital in September, 1984. She was treated with hyperthermo-chemo-radiotherapy, using radiofrequency-induced hyperthermia (8 MHz) with a daily dose of fractionations at 42-43 degrees C for 60 minutes, 5 fractions per week of irradiation at 180 cGy and systemic chemotherapy. After the total doses of hyperthermia and radiation had reached 11.8 hours and 2500 cGy, the primary tumor and lymph node metastasis measured by X-ray examination were reduced in size. On October 8th, laparotomy was done, and total gastrectomy combined with total pancreatectomy, left hepatic lobectomy and right hemicolectomy was performed. Histologically, almost all of the cancer cells in the primary tumor and lymph node metastasis had disappeared. These results show that this multimodal therapy is effective for local control of gastric cancer, when used preoperatively.

[Intra-arterial continuous infusion in patients with advanced and recurrent cancer of the digestive system].

Intra-arterial continuous infusion of antineoplastic agents was carried out for 90 patients with cancer of the digestive system at our clinic during the past 16 years. These patients had inoperable, advanced or recurrent tumors in various organs: stomach, colorectum, liver, pancreas and others. 5-fluorouracil, mitomycin C, adriamycin and others were infused, singly or in combination, into the aorta (81 patients), or iliac (3) and hepatic (6) artery over periods varying from 2 to 197 days. Partial response was revealed in 41% of the overall patients, no change in 28% and progressive disease in 31%. There were significant differences in anti-tumor effects and survival curves between terminal and non-terminal patients, or between patients with and without long-term infusion (p less than 0.05-0.005). These results suggest that the performance status of patients and the infusion period both influence the anti-tumor or long-survival effect obtained by this therapy.

[Necrotomy of brain stem glioma and local administration of anticancer agents].

We studied 14 cases with brain stem glioma. Five cases in 7 malignant gliomas showed large central necrosis in CT scans. The central necrosis neighboring the surface of the pons or the fourth ventricle was removed with CO2-LASER and cavitron ultrasonic aspirator (necrotomy) and a few pellets were given in the residual tumor in four cases. Three cases showed remarkable improvement of clinical course. A case came back to work. Other two children resulted in high Karnofsky rating. A case did not improve and died 10 months after the surgery. CT scans showed the decompression of the fourth ventricle due to decrease in volume of central necrosis. The preoperative enhanced lesion was decreased in degree of contrast enhancement. The necrotomy of brain stem glioma and local administration of anti-tumor-agents in residual tumor showed excellent results in some cases. We should scrutinize CT scans and NMR in cases with brain stem glioma.

[Surgical results and problems of pancreatic cancer].

Twenty-nine surgically resected specimens of pancreatic head cancer were examined from a clinicopathological stand point. Serial sections 3 to 5 mm in thickness were cut from each specimen to demonstrate the precise histopathological extent of cancer. Tumor size histologically determined was then divided into several groups as follows: t1 (less than 2 cm in diameter), t2a (2.1-3.0 cm), t2b (3.1-4.0 cm), t3 (4.1-6.0 cm) and t4 (greater than 6.1 cm). The number of cases in each group was 3 in t1, 10 in t2a, 9 in t2b, 6 in t3 and 1 in t4. Tumor size was not always correlated with prognosis and it was necessary to consider other important influential factors, namely, invasion to the serosa of the pancreas (s factor), retroperitoneal infiltration beyond the pancreas (rp factor), and regional lymph node metastasis (n factor). All patients with a positive s factor died less than 2 years after surgery, while the survival rate of s factor-negative patients was 41% at 2 years and 8% at 5 years. Survival rate of patients with positive n factor was 13% at 2 years and 6% at 5 years, and that of n factor-negative patients was 71% at 2 years and 33% at 5 years. Survival rate of patients with positive rp factor was 21% at 2 years, while that of rp factor-negative patients was 75% at 2 years and 50% at five years. Positive rp factor patients comprised 100% of t1, 80% of t2a, 89% of t2b and 83% of t3. This suggested that rp factor was the most important factor even in cases of t1. Furthermore, the frequency with which retroperitoneal surgical margins of specimens were infiltrated ranged from 64% of patients with extended retroperitoneal dissection by the translateral retroperitoneal approach, to 100% of those without such procedure. These results indicate that retroperitoneal radical dissection is basically necessary for pancreatic cancer and that a newer approach combined with some other modality also needs to be done.
