A 60-year-old woman was diagnosed as having liver metastasis from gastric cancer 14 months after total gastrectomy and total pancreatectomy. The liver tumor was so huge and the complication, diabetes mellitus, was so severe that she was palliatively treated by hyperthermo-chemo-radiotherapy (HCR therapy) with 8-MHz capacitive heating system. Because hyperthermia for deep seated tumor is very difficult, irradiation (10 MV X-ray, 36 Gy) and systemic chemotherapy (CDDP, MMC) were combinedly used. After 10 session of hyperthermia, the tumor showed a remarkable regression in size, followed by S8 subsegmentectomy of the liver. Histologically, cancer cells were still viable in the midst of fibrosis around coagulation necrosis, while normal liver cells remained intact. Multidisciplinary HCR therapy is quite a useful modality for liver tumors and may serve to expand the indication for surgical operation.
To evaluate the effect of UFT on nuclear DNA distribution of carcinoma, we measured the DNA content of biopsy specimens and resected specimens obtained from 18 patients by microfluorometry. The tumors consisted of 7 gastric and 11 colorectal carcinomas. Five patients who received administration of 400mg of UFT (400mg of tegafur) were classified as the chemotherapy group and another 13 patients were classified as the control group. 1) DNA indices of biopsy specimens and resected specimens of the same patients showed almost the same values both in the control group (r = 0.81) and in the chemotherapy group (r = 0.89) (p less than 0.05). 2) S and S + G2M fractions of resected specimens showed almost the same values as compared with those of corresponding biopsy specimen in the control group. However, in the chemotherapy group S and S + G2M fractions of resected specimens showed a tendency to be higher than those of corresponding biopsy specimen (0.05 less than p less than 0.1). This study indicates that the biopsy specimens are sufficient for DNA analysis and that UFT-induced DNA change may be increase of S and S + G2M fraction.
The influence of endocrine therapy on the proliferation of estrogen receptor (ER) positive cells and ER negative cells of human breast cancer (MCF-7) serially transplanted into nude mice was analyzed by tumor growth, dextran-coated charcoal (DCC) method, ER-immunocytochemical assay (ER-ICA) and ER-immunocytochemically stained 3H-thymidine autoradiography. In the tamoxifen (TAM) group and the medroxyprogesterone acetate (MPA) group, tumor growth was inhibited, but it was promoted in the 17 beta-estradiol dipropionate (E2) group. The ER level by the DCC method significantly decreased in the TMA, the MPA and the E2 groups. The ER-ICA showed that the ER positive cells rate in the TAM and the MPA group decreased, but it increased in E2 group. However, the ER-immunocytochemically stained 3H-thymidine autoradiography showed that not only the labelling index of ER-positive cells but also that of ER negative cells significantly decreased in the TAM and the MPA groups, but significantly increased in the E2 groups. Therefore it was concluded that endocrine therapy affected the proliferation of both ER positive cells and ER negative cells of ER positive breast cancer.

Clinical significance of preoperative serum NCC-ST 439 level was studied in 119 cases of colorectal cancer (90 of primary, 29 cases of recurrent). The positive rates for serum NCC-ST 439 were 28.9% in primary and 65.5% in recurrent cases. The false positive rates for benign disease were 5.6%. These rates were low when compared with those for other tumor markers. The positive rates for serum NCC-ST 439 exhibited a strong correlation with wall invasion, lymph node and liver metastases. A combination assay of NCC-ST 439, CEA and CA 19-9 produced a high positive rates as 43.3% in primary and 86.2% in recurrent cases. These results demonstrate that measurement of serum NCC-ST 439 may be useful for cancer staging and improves the diagnostic rate in combination with CEA and CA 19-9.
Flow cytometric cell cycle analysis using a monoclonal antibody to Bromodeoxyuridine was performed on 117 gastric cancers. Dissociated cells were stained with indirect immunostaining for BrdU (FITC-BrdU) and propidium iodide. Bivariate BrdU/DNA distribution were obtained using EPICS-C flow cytometry. Tumor ploidy was classified as follows, D1: diploidy, D2: diploidy + aneuploidy, A1: single aneuploidy, A2: multiple aneuploidies. The ploidies of noncancerous gastric mucosa were all diploidy and that of S-phase fraction(SPF) were ranged from 0.0% to 1.2%. In 117 gastric cancers, aneuploidy was observed in 80 cases, D2: 38, A1: 15, A2: 27. SPF was higher in aneuploidy (14.5 +/- 5.1%) than diploidy (6.1 +/- 5.1%). Significant differences were observed between that of D1 (6.1 +/- 2.5) and D2 (13.6 +/- 6.2), A1 (12.5 +/- 4.5), A2 (16.0 +/- 3.2), and A1 and A2 (p less than 0.01). The patients with aneuploid tumors had poor prognosis than diploid tumors (p less than 0.05). In concerned with DNA ploidy pattern, the patients with A2 had most poor prognosis than the other (p less than 0.05). Furthermore, the patients with SPF over 10% had poor prognosis than that of SPF below 10%. These results indicated that DNA ploidy pattern and SPF may possibly be useful prognostic markers for gastric cancers.
Twenty-five cases of gastrointestinal leiomyosarcomas were subjected to clinico-pathological studies in an attempt to correlate the prognosis with tumor size, mitosis, cellularity and DNA ploidy pattern. Leiomyosarcomas greater than 5cm in diameter had poorer prognosis. Those with mitotic index larger than 3.0/mm2, cellularity larger than 3.0/0.0004mm2, DNA aneuploidy had poorer prognosis. By multiple regression analysis, mitotic index was useful for the prediction of tumor recurrence in earlier postoperative period, but cellularity, tumor size were useful for the prediction of recurrence in later postoperative period. As for the type of tumor recurrence, hematogenous metastasis was observed in 7 cases, peritoneal dissemination in 2 and local recurrence in 4. Four cases with local recurrence had all hematogenous metastasis. Two cases of gastric leiomyosarcomas developing local recurrence were greater than 10cm in diameter and gastric local resection was done for them. One case was diagnosed benign leiomyoma in the rectum histologically, but after trans-anal tumor resection local recurrence and metastasis to the lung occurred. We must pay attention to the surgical margin and the surface of tumor dissection during of the tumor, especially in larger tumors. Aggressive surgical resection is efficient for the treatment of recurrent tumors.

The pancreas is located in the retroperitoneal space, and its anatomical position is very important in devising a rational surgical approach to pancreatic cancer. In cancer of the pancreas head, cancer cells could invade the portal vein and perineural space of the celiac plexus, and metastasize to regional lymph nodes around the celiac axis. For these reasons, we have performed on extensive operation for cancer of the pancreas head, in which a pancreaticoduodenectomy was performed with extensive resection of the regional lymph nodes around the celiac axis, resection of the celiac plexus and segmental resection of the portal vein. As a result, seven out of 31 resected cases survived more than 5 years after the operation. On the other hand, local recurrence was still found at autopsy in 11 of 12 patients who underwent the extensive operation and died of the recurrent disease. Therefore, further removal of adjacent tissues behind the pancreas and extensive dissection of the regional lymph nodes around the celiac axis seem important for improving the survival of patients with cancer of the pancreas head. Postoperatively, skillful management is also required for severe intestinal malabsorption and diabetic state following the operation.

An immunohistochemical study revealed that NCC-ST-439 positive tumor were detected in 33% of the 244 gastric carcinomas. The incidence of NCC-ST-439 immunoactivity in well differentiated adenocarcinomas (papillary and tubular adenocarcinoma) was significantly higher than in poorly differentiated adenocarcinomas (including signet-ring cell and mucinous adenocarcinoma). Moreover, NCC-ST-439 immunoreactive tumors showed more frequent vessel invasion and higher DNA content than non-reactive tumors. The recurrence rates of NCC-ST-439 immunoreactive tumors in the liver and lymph nodes were significantly higher than those of non-reactive tumors. Patients with NCC-ST-439 immunoreactive carcinoma had much worse prognosis than those with NCC-ST-439 non-reactive carcinoma. These results suggest that NCC-ST-439 antigen produced by tumor cells plays an important role in the invasive growth and vessel invasion and also serves as a biological marker of malignancy in patients with gastric cancer.
