[Thoracic peridural opiate-analgesia].

In patients with cancer pain caused by tumours in the upper thorax, shoulder or neck area the thoracic approach for peridural opiate analgesia is indicated. The thoracic puncture technique is more difficult and the possible complications are more serious than puncture in the lumbar area; it therefore belongs, as pain therapy generally, in the hands of an anaesthesiologist experienced in regional anaesthesia. In selecting the puncture level appropriate to the site of pain, it is necessary to take account of the fact that the vertebral interspace does not correspond with the spinal segment. Only thus can an optimal analgetic effect with a minimal dose b assured.

Catalytic utilization of eIF-2 and mRNA binding proteins are limiting in lysates from vesicular stomatitis virus infected L cells.

Infection of mouse L cells by vesicular stomatitis virus results in the inhibition of cellular protein synthesis. Lysates prepared from these infected cells are impaired in their ability to translate endogenous or exogenous cellular and viral mRNAs. The ability of initiation factors from rabbit reticulocytes to stimulate protein synthesis in these lysates was examined. Preparations of eukaryotic initiation factor 2 (eIF-2) and the guanine nucleotide exchange factor (GEF) stimulated protein synthesis strongly in L cell lysates from infected cells but only slightly in lysates from mock-infected cells. Maximal stimulation was obtained when a fraction containing eukaryotic initiation factors 4B (eIF-4B) and 4F (eIF-4F) was also present. In lysates from infected cells, these initiation factors increased endogenous cellular mRNA translation on the average 2-fold. In contrast, endogenous viral mRNA translation was increased to a much greater extent: the M protein was stimulated 8-fold, NS 5-fold, N 2.5-fold, and G 12-fold. When fractions containing eIF-4B, eIF-4F, or eIF-4A were added to these lysates in the presence of eIF-2, all three stimulated translation. Fractions containing rabbit reticulocyte initiation factors eIF-3 and eIF-6 had no effect on translation in either lysate. The results suggest that lysates from infected L cells are defective in the catalytic utilization of eIF-2 and deficient in mRNA binding protein activity.

Differences in protein phosphorylation in vivo and in vitro between wild type and dunce mutant strains of Drosophila melanogaster.

The protein phosphorylation patterns of wild type and dunce mutant strains of Drosophila melanogaster, as detected by sodium dodecylsulfate-gel electrophoresis and autoradiography, have been compared. After labelling in vivo with 32Pi or in vitro in homogenates with [gamma-32P]ATP, radioactive bands at and above apparent polypeptide mol. wt approximately 110,000 were more pronounced in dunce fly heads than in wild type heads. When labelling in vitro, in dunceM11 there appeared a radioactive band at apparent mol. wt approximately equal to 53,000 that was faintly visible in the wild strain. The same band could be intensified in both strains by adding cyclic AMP to the homogenate or by performing homogenization in the presence of theophylline. The data suggest that the mol. wt approximately equal to 53,000 protein is a substrate for cyclic AMP-dependent protein kinase.

Localization and frequency of degenerative changes in the knee joint: evaluation of 200 necropsies.

A study was performed of 400 knee joints of 200 cadavers. Degenerative changes of the femoropatellar joint affecting the retropatellar articular surface were more frequent and more severe, while in the tibiofemoral joint changes were more frequent and more severe on the femoral articular surface. Owing to the relationship of the two joints, changes of the femoropatellar joint occur more frequently and are usually more severe than those of the tibiofemoral joint. These findings imply that diffuse osteoarthritis of the knee starts mostly in the femoropatellar joint. It is emphasized that early diagnosis and management of degenerative changes in the femoropatellar joint will facilitate the prevention of knee joint panarthrosis.

[Abnormal renal angiograms caused by injection of contrast medium].

The anomalies caused by the injection of contrast medium were studied in animal experiments with dogs on 53 kidneys, and on clinical material, via the renal angiograms of 130 patients. In these trials vascular spasms were seen or, alternatively, filling defects in the parenchyma. Both were using the classical tri-iodated contrast medium iodamide and the monoacid dimeric contrast medium with low osmotic pressure (Hexabrix); in several cases, histological changes (for example, necroses of the tubular epithelium) were seen. Anomalous angiograms were also seen in the human experiments, especially in case of incorporation of large quantities of contrast medium and selective use of a thick catheter. Anomalies were more likely to occur in kidneys with diseased parenchyma.

Cyclic nucleotide phosphodiesterases in larval brain of wild type and dunce mutant strains of Drosophila melanogaster: isoenzyme pattern and activation by Ca2+/calmodulin.

Like adult heads and whole flies, larval brains of wild type Drosophila melanogaster contain two major soluble cyclic nucleotide phosphodiesterases, forms I and II. Larval brains of the learning-defective mutant strain, dunceM11, contain only the form I enzyme. In both wild type and dunce strains the form I enzyme is activated by Ca2+/calmodulin. A time-dependent loss of this Ca2+ activation was observed.

Identification of two new promoters probably involved in the transcription of a ribosomal RNA gene of Escherichia coli.

The DNA sequence in the region preceding the rrnB gene of Escherichia coli was determined up to the 1821st nucleotide upstream from the beginning of the sequence coding for mature 16 S rRNA. In vitro transcription experiments indicated the presence of two new promoters in this region, located more than 1 kb upstream from the known P1 and P2 promoters of rrnB. Previous electron microscopic studies demonstrated that these sites bind RNA-polymerase very strongly. In vitro transcription, starting at these sites reads through the entire region into the rrnB gene without termination. A similar uninterrupted transcription into rrnB in vivo can be demonstrated by S1-mapping, and by fusing the DNA containing the new promoters (but not P1 and P2) to the lacZ gene. Thus it seems likely that these promoters (P3 and P4) belong functionally to the rrnB gene and play some role in its regulation of expression.

Effect of experimental patellar luxation on the knee joint in the rat.

In 16 male Wistar albino rats the patella was fixed to the lateral aspect of the lateral condylus of the femur. Animals with luxated knee joint and controls were sacrificed 7-9 months after surgery. Operated knee joints were processed for histology and subjected to morphometric analysis in comparison to the controls.- It was found that (1) the artificial disturbance of the femoro-patellar junction resulted in a severe destruction of articular cartilages of the tibio-femoral and femoro-patellar junction; (2) the destructions progressed with time; (3) there was no substantial difference between the changes of the medial and lateral articular surfaces of the femur and tibia; (4) changes comprised focal cartilage necrosis, widening of cartilaginous area under the tide-mark, proliferation of subchondral bone and formation of osteophytes; (5) the fact that a change of the statics of femoro-patellar junction leads to panarthrosis, moreover that articular capsule, menisci, epi-, meta-, and diaphyseal bone areas are also involved, indicate the complexity of the question. The findings indicated that changes of the femoro-patellar junction play a role in the degenerative processes of the knee joint. It is concluded that early recognition and treatment of femoro-patellar alterations may help in the prevention of knee panarthrosis.

Effect of dithiocarbamate-type chemicals on the nervous system of rats.

Neurotoxic effect of some dithiocarbamate- and thiuram-disulphide-type chemicals as well as CS2 were examined on rat brain and behaviour. A significant reduction of the orientation hypermotility and at the same time a depression of the subcortical EEG activity were observed. The biochemical assay showed a decreased re-formation of norepinephrine. It is suggested that the inhibition of DBH enzyme may be involved in the delayed psychosedative effect, but not in the short-term electrophysiological changes.

Absolute and relative refractory periods of the substrates for lateral hypothalamic and ventral midbrain self-stimulation.

The pulse-pair paradigm was used to behaviorally assess the absolute and relative refractory periods of neurons subserving brain-stimulation reward. The amplitude of the second pulse was either equal to, 41% greater than, or 73% greater than the amplitude of the first pulse. In the equal amplitude condition, recovery from refractoriness began as early as 0.4 msec and did not asymptote until as late as 3.5 msec. A 41% increase in the intensity shortened the time course of recovery in five out of six cases. In only one of these five cases did a 73% increase in the intensity of the second pulse produce further changes in time course. Neither increase in the amplitude of the second pulse affected the time course of recovery in one subject. The absolute refractory periods of the directly stimulated reward-relevant neurons appear to be less than 1.5 msec and as short as 0.4 msec; some of these neurons have relative refractory periods that range between 1.0 and 3.5 msec.

[On the use of physostigmine in anesthesia and intensive care].

Physostigmine is an antidote in cases of overdosage with intravenous anaesthetics such as dehydrobenzperidol, benzodiazepines and ketamine. It is also useful in antagonizing drug induced comas of different origin particularly in cases of acute intoxication with tricyclic antidepressants and alcohol. We discuss our therapeutic observations in anaesthesiological and intensive care patients. We have not seen any side effects after physostigmine administration, but the possibility of undesired parasympathomimetic activity demands correct indication, well calculated dosage and continuous surveillance of the patient.

Elemental concentrations in human erythrocytes and blood plasma following radiotherapeutic irradiation.

Elemental concentrations in whole blood samples from irradiated mice were found [1] to change uncorrelated with the depression of red blood cell count. The aim of the present work was to clear up whether or not similar effects appear in the case of human radiotherapeutic irradiations. Concentration ratios were taken for erythrocyte and plasma fractions with the proton induced X-ray emission method for the elements P, K, Ca, Fe, Zn in the case of patients undergoing postoperative gamma irradiation following mastectomy. None of the concentration ratios were found to be influenced under the present conditions of irradiation.
