Lymphocyte response to staphylococcal enterotoxin B in patients with annular erythema associated with Sjögren syndrome.

Lymphocyte response to staphylococcal enterotoxin B (SEB) was analysed in 11 cases with Sjögren syndrome (SjS) who developed annular erythema during the course. Increased lymphocyte response against SEB was observed on day 5 in those patients who developed annular erythema at the time of examination. A similar reaction pattern was observed when Con A and PWM, but not protein A, were used as the mitogen. No significant lymphocyte response was observed in the patients in the inactive stage without annular erythema or in normal controls. Furthermore, peripheral blood lymphocytes express LFA-1 after SEB stimulation. These results suggest that lymphocytes from SjS patients react to various types of antigenic or mitogenic stimulation and that they express cell adhesion molecules, especially in patients with active annular erythema.

Annular erythema. A comparative study of Sjögren syndrome with subacute cutaneous lupus erythematosus.

Annular erythema developed in 22 patients with Sjögren syndrome. Clinically, the annular erythema was subdivided into three forms: Sweet disease-like annular erythema with an elevated border (14 cases); subacute cutaneous lupus erythematosus (SCLE)-like marginally scaled erythema (5 cases); and papular erythema (3 cases). Histopathologically, features commonly seen in annular erythema are deep perivascular and/or periappendageal infiltration of the lymphocytes with an admixture of neutrophils or plasma cells and less frequent epidermal changes suggestive of cutaneous lupus erythematosus. Immunoglobulin or complement deposition along the dermoepidermal junction of lesional skin was observed in 8 of 18 cases, and most of the dermal infiltrates consisted of CD4(+), 4B4(+) cells. The appearance of anti-SS-A(Ro) (100%) and anti-SS-B(La) (77%) was significantly higher in patients with annular erythema. These results suggest that patients with Sjögren syndrome might have a distinct annular erythematous lesion that is both clinically and histopathologically different from SCLE, although close immunologic abnormalities exist in these two diseases.

[A case of systemic lupus erythematosus with the central nervous system manifestations (CNS-lupus) mimicking herpes simplex encephalitis (HSE)].

A 21-year-old male with SLE developed seizure, loss of consciousness and focal signs referable to involvement of the front-temporal brain regions. MRI (magnetic response imaging) image revealed high signal areas in the temporal lobes. By these findings, herpes simplex encephalitis (HSE) was suspected at first. But neither isolation of herpes simplex virus nor HSV specific IgM by ELISA was detected. Acyclovir administration by intravenous infusion was'nt effective but corticosteroid pulse therapy was effective. The level of anticardiolipin antibody was very high. Finally, the diagnosis of CNS-lupus with HSE-like characteristics was made in this case.

Clinical manifestations in anticardiolipin antibody-positive patients with progressive systemic sclerosis.

Anticardiolipin antibody-positive patients with progressive systemic sclerosis were analyzed. Elevated anticardiolipin antibody titers were observed in 13 of 40 cases (33%). Anticardiolipin antibody titer was significantly higher in patients with progressive systemic sclerosis type 1 than in those with type 2 or type 3. Occurrence of anticardiolipin antibody was significantly more frequent in patients positive for anti-nRNP antibodies, rheumatoid factor, or thrombocytopenia. In contrast, patients with proximal scleroderma, scarring, or esophageal hypomotility were positive for anticardiolipin antibody less frequently. These results suggest that anticardiolipin antibody might be closely associated with lupuslike clinical manifestations in a subset of progressive systemic sclerosis or definite progressive systemic sclerosis with thrombocytopenia, rheumatoid factor, or anti-nRNP antibodies.

Production of interleukin 1 alpha by a trichilemmoma cell line.

In order to obtain a good source of human epidermal cell derived thymocyte activating factor (ETAF) for biological study, several cell lines of epidermal origin were cultured. One of the trichilemmoma cell lines, KTL-1, was found to produce ETAF satisfactorily in its culture supernatant. The culture supernatant was collected and analyzed to determine the nature of the ETAF activity. The ETAF activity was completely inhibited by anti-IL 1 alpha antibody, but not by anti-IL 1 beta antibody. Messenger RNA expression by KTL-1 cells was analyzed by using cDNA of IL 1 alpha as a probe. Messenger RNA was not detected in KTL-1 cells by either dot blot analysis or northern blot analysis. However, it was expressed weakly in the cytoplasm by in situ hybridization technique. Messenger RNA expression was easily detected in KTL-1 cells stimulated by lipopolysaccharide (LPS). Messenger RNA expression reached a peak at 3 hours after stimulation of LPS. A trichilemmoma cell line, KTL-1, can be a good source of human ETAF for biological study.

A case of Sneddon's syndrome with positive ANA and anti-cardiolipin antibodies: primary anti-phospholipid syndrome?

A 22-year-old woman developed ulcerative lesions on the lower extremities which usually exacerbated during the summer. Histological analysis revealed a micro-thrombotic lesion in the deep dermis without inflammatory cell infiltration or fibrinoid degeneration of blood vessels. Magnetic resonance imaging revealed multiple cerebral infarctions. Abnormal laboratory findings included an elevated anti-cardiolipin antibody titer and positive speckled pattern ANA (x80), but without other manifestations or signs of SLE. FACS analysis revealed that the patient's serum reacted with ethanol fixed endothelial cells in addition to keratinocytes and peripheral blood neutrophils. This case was thought to be livedo reticularis and cerebral thrombotic lesions (Sneddon's syndrome) associated with atrophie blanche or livedo(id) vasculitis and may be one clinical subset of primary anti-phospholipid syndrome.

ICAM-1 and LFA-1 expressions in the lesional skin of annular erythema associated with Sjögren syndrome.

ICAM-1 and LFA-1 expression was studied in the lesional skin of ten cases of annular erythema associated with Sjögren syndrome. Most of the infiltrating mononuclear cells around blood vessels expressed LFA-1 in addition to its strong expression on vascular endothelial cells and focal expression on the epidermal basal cell layer in 3 cases. ICAM-1 expression on vascular endothelial cells was similar to LFA-1, although relatively focal and weak expression was observed on mononuclear cells. ICAM-1 expression on keratinocytes was focal and limited to the basal cell layer in annular erythema. These findings suggest that strong expression of ICAM-1 on endothelial cells but not keratinocytes and LFA-1 on mononuclear cells might play some role in the induction of skin lesions in annular erythema associated with Sjögren syndrome.

Poikiloderma-like lesions on the neck in atopic dermatitis: a histopathological study.

Reticulate pigmentation with or without skin atrophy, depigmentation and telangiectasia is frequently encountered on the neck of severe cases of adult type atopic dermatitis. These skin changes were graded clinically into 3 stages. Based on histological features, hyperplasia of the sebaceous gland, dilated tortuous capillaries, and mild degeneration of elastic fibers were noted in stage I lesions. Lesions of both stages II and III contained increased melanin in the basal cell layer with incontinence of pigment, remarkable destruction and degeneration of elastic fibers, proliferated and dilated capillaries, and deposition of mucinous substances. The numbers of mast cells in papillary dermis were significantly increased in late stage I and stage II lesions. Poikiloderma-like lesions on the neck could be attributable to chronic inflammation and delay of wound healing process, possibly caused by long-standing topical corticosteroid therapy.

Induction of eczematous skin reaction in experimentally induced hyperplastic skin of Balb/C mice by monoclonal anti-DNP IgE antibody: possible implications for skin lesion formation in atopic dermatitis.

Biphasic skin reaction with peak response at 1 and 24 h with prominent mast cell degranulation was induced by intravenous application of a monoclonal anti-DNP IgE antibody and subsequent skin test. This reaction was hapten-specific and mast-cell-dependent because no reaction was observed when oxasolone was used as an elicitation antigen or skin test was elicited in genetically mast-cell-deficient mice (W/Wvv). A partial spongiotic reaction and mononuclear cell infiltration into the epidermis were observed in mice with hyperplastic epidermis induced by topical retinoic acid. Cotransfer of DNFB-sensitized lymph node cells with anti-DNP IgE antibodies failed to enhance the skin test reaction in unsensitized mice. These results suggest that, to some degree, IgE antibody may play some role in the development of eczematous skin lesions in the rodent system without the involvement of cellular hypersensitivity.

[Six cases of Sjögren's syndrome with lichen mucosae--studies on the histopathology of salivary gland and clinical manifestation of sicca features in the patients with lichen mucosae].

A case of a 54-year old man with lichen mucosae and Sjögren's syndrome was reported. Clinical and histopathological analysis was made on the presence of sicca features in 6 patients with lichen mucosae. All patients complained either xerophthalmia or xerostomia and 5 out of 6 cases showed specific lymphocytic foci seen in Sjögren's syndrome in addition to lichenoid tissue reaction in the specimen obtained from buccal mucosae. Objective keratoconjunctivitis sicca was demonstrated in 3 cases. Immunologically, abnormal laboratory findings were observed in 5 cases (4 cases with a positive ANA and 3 cases with a positive anti microsome antibody). These findings suggest that lichen mucosae might develop in a close association with Sjögren's syndrome and relationship between these two disease was discussed in this article.

[Livedo with cerebrovascular thrombosis--correlation between clinical features, anti-cardiolopin antibodies, and cerebral microinfarction].

Twenty one patients with livedo racemosa (LRa) and periarteritis nodosa cutanea were screened for the presence of anti-cardiolipin antibody (aCL) by ELISA. 11 out of 21 revealed positive aCL whose clinical features could be divided into 3 groups: 4 patients with livedo with ulceration, 2 with pyoderma gangrenosum-like lesions, and 5 with extensive LRa. #1. The summer ulceration of leg of 4 patients were surrounded by several tiny petechiae where hyaline microthrombi of blood capillaries in the upper dermis were demonstrated. All had multiple cerebral microinfarctions on magnetic resonance imaging (MRI). #2. Pyoderma gangrenosum-like lesions of 2 patients consisted of painful punched-out ulcers on livedoid lesions where intravascular endothelial hyperplasia in a small artery was demonstrated. Both had multiple cerebral microinfarctions. #3. Another clinical manifestation of the patients with positive aCL was extensive form of LRa of 5 patients. All had positive ANA, however none of them exhibited cerebral microinfarctions on MRI. #4. The remaining 10 patients with negative aCL showed no relationship with such clinical and laboratory manifestations. These results indicate that there is a significant correlations between summer ulceration with petechiae, capillary microthrombi of the skin, positive aCL and cerebral microinfarctions on MRI.

[Studies on the influence of composite resin filling material (Command Ultra fine) on the dental pulp].

Studies on the influence of composite resin filling material on the dental pulp of human 1st premolar are reported in this thesis. An other experiment was carried out on rabbit with the left 1st molar in their lower jaw as subject and the right 1st molar in the lower jaw as a control. In both experiments with human and rabbit, routine operations were performed to prepare the cavities and filled with composite resin material. The human teeth were extracted 21 days afterwards and the rabbit teeth were extracted 7 days afterwards. Edema was found in the dental pulp of both human and rabbit teeth by pathologic examination and was accompanied by hemangiectasis and lymphaniectasis. There was also vacuolar degeneration in the odontoblasts. Some of these cells were reticulated and the of number of odontoblasts was fewer than in the control. Hemangiectasis was even more serious in the rabbit's teeth than those of human.

Study of estimation of color recognition on the dentist. On the ability of subjects to discriminate color in terms of hue, value and chroma.

The tooth crown color space model was manufactured and the improvement of the color representation and communication on the dental clinic have been trying. The study of color discrimination by dentist and student viewing with the use of tooth crown color chips reported here in basic knowledge on the color sensitivity of human eye. The test card were 55 x 90 mm in size and the color chip, 5 x 10 mm. The paired color chips were placed on the test card to have a 2 mm distance between them. Color difference of two tooth crown color chips on the test cards was 3.5 (delta E*ab), the combination of hue, value and chroma was constant of all cards. The subjects were 22 people, 6 dentists and 16 dental school students with normal color sensibility. In proportion of method of comparison for surface color (JIS Z 8723), the subject observed the test card which were different of hue, value and chroma on the their naked-eye, were investigated to evaluate their judgement of the test cards. The following results were obtained. The standard of judgment for the color discrimination was made firstly in terms of hue, secondly in terms of value, and thirdly in terms of chroma.

Primary hepatic lymphoma presenting as intravascular lymphomatosis.

A case of primary hepatic lymphoma presenting as intravascular lymphomatosis was diagnosed in a 69-year-old woman who presented with symptoms related to the digestive tract. These symptoms were followed by neurologic disorders, including disturbance of sensation and mentality, and pathologic reflexes suggestive of a broad intracerebral lesion. Computed tomography of the brain showed an ill-defined, low-density area in the bilateral parietotemporal lobes. Autopsy revealed a well-circumscribed hepatic tumor, 6 cm in diameter, and scattered hemorrhagic foci in the cerebral cortexes. Microscopically, the hepatic tumor was a diffuse lymphoma, large cell, of the B-cell type. The small vessels of the brain were found to be packed with lymphoma cells, having caused the florid neurologic manifestations.

[The induction of differentiation of myeloid antigen positive acute lymphoblastic leukemia by phorbol ester (TPA) or GM-CSF].

In 8 cases of myeloid antigen positive (My+) ALL (L2), morphology and phenotype of their blasts and the differentiation-inducing effect of each of 12-o-tetradecanoyl phorbol-13-acetate (TPA) or recombinant GM-CSF (rGM-CSF) on the blasts were analysed. Five cases expressed common ALL phenotype such as CD19+, CD20-(-)+, CD10+, DR+ and c-mu-(-)+, and also My antigens such as CD11c+, CD11b+, CD14+ and CD13+ (B+My+ALL). One case expressed T-ALL phenotype such as CD5+, CD2+, CD3+, CD8+, CD4+ and CD10+ and also My antigen such as CD14 and CD13 (T+My+ALL). These six cases were diagnosed as biphenotypic ALL. The other 2 cases expressed mainly My antigen such as CD11c+, CD11b+, CD14+, CD13+, CD33+ and DR+, and one of the two expressed also CD2 (B-T-(-)+My+ALL). TPA induced the marked morphological change to monocyte-macrophage like cells and the increase of CD11+ cells in both of B-T-(-)+My+ALL cases. rGM-CSF transformed the blasts to large cells or cells with azurephilic granules in B+My+ALL cases.
