[HLA antigens in diffuse toxic goiter].

The paper is concerned with some data of investigation of the association of the HLA-system with the development of diffuse toxic goiter (DTG). An attempt was made to predict clinical variants of a course of DTG using HLA-antigens. Comparative analysis of the distribution of HLA-antigens was performed in 50 patients with various degrees of DTG severity, different therapeutic methods (operative and conservative) and outcomes of disease (postoperative hypothyrosis, DTG recurrence and euthyroid condition). The data obtained confirmed a high informative value of HLA-markers not only for predicting clinical variants of a DTG course, therapeutic methods but also for outcomes of therapy.

[The effect of acute ACTH1-24 loading on the steroid hormone and prolactin content of the blood serum in women with hyperprolactinemia and hyperandrogenism].

Adrenocortical and ovarian function in women with hyperprolactinemia and hyperandrogenism was evaluated using the functional ACTH1-24-test (with syncorpin--25 U/ml), a radioimmunoassay was used to estimate the levels of II-OCS, androgens, progestins, estrogens and prolactin in the blood serum of 29 women with primary prolactinemic hypogonadism (HG), in 15--with hyperandrogenic ovarian dysfunction, in 27--with the hypothalamic syndrome of neuro-endocrine type, and in 13 healthy women. Marked interference of a clinical picture was found in patients with disturbed menstrual and reproductive function with relation to the presence of galactorrhea and hirsutism. All the examinees retained glucocorticoid function. Patients with hyperprolactinemia demonstrated an increase in the basal concentration of the ovarian androgen androstenedione in the absence of its response to ACTH. The level of 17-oxyprogesterone in them was decreased. A paradoxical response of prolactin to syncorpin was observed in patients with microprolactinemia, the syndrome of the "empty" Turkish saddle and functional type of HG.

[Use of the microcomputer in teaching clinical endocrinology].

The paper is concerned with the results of the use of the Electronika NMS 01900.1 microcomputer manufactured in the USSR for teaching clinical endocrinology. Owing to its small dimensions, simple maintenance and sufficiently vast computing potentialities it can be widely used for teaching. A program with broad functional possibilities was prepared permitting students to objectivize their knowledge in clinical endocrinology. Fast automated test control of knowledge raises the efficacy of teaching many times.

[Magnetic resonance tomography of the adrenals in the diagnosis of total and partial hypercorticism].

The paper is devoted to the use of a new method of nuclear magnetic resonance tomography (MR tomography) in the diagnosis of diseases of the adrenal glands developing with the syndrome of total and partial hypercorticism. A MR tomograph with resistive magnet (0.234 T) was employed. Altogether 25 patients were examined (21 with Itsenko-Cushing disease of various degrees of severity, 2 patients with Itsenko-Cushing syndrome and 2 patients with aldosteronoma). MR tomography was shown to possess a high informative value permitting the assessment of a type of adrenal changes without irradiation of a patient. In combination with routine methods of examination (hormonal investigation, retropneumoperitoneum, x-ray computerized tomography) MR tomography makes it possible to improve considerably the diagnosis of Itsenko-Cushing disease and to specify tumor sizes and tumor growth type.

[The use of nuclear magnetic resonance in the diagnosis of hypophyseal adenomas (preliminary report)].

The paper is concerned with the use of a new tomographic method based on the principle of nuclear magnetic resonance (NMR-tomography) in the diagnosis of adenohypophyseal diseases. A NMR-tomograph with a resistive magnet (0.234 T) was employed. Altogether 15 patients with the most common types of hypothalamo-hypophyseal diseases including Itsenko-Cushing disease (6) of various degrees of severity, hyperprolactinemic hypogonadism (8), and one patient with acromegaly were investigated. NMR-tomography was shown to possess a high informative value permitting assessment of the presence of adenohypophyseal pathology without irradiation of a patient. NMR-tomography combined with routine methods of examination of patients with hypothalamohypophyseal pathology (roentgenography, computerized tomography, hormonal investigation) made it possible to improve considerably early diagnosis of pituitary adenomas and to specify the volume and type of tumor growth.

[Computer-automated calculation of allowed human body weight].

A body weight (BW) classification has been proposed. It has been used in a PC program which compared BW of the patient with the current international criteria for "optimal" and "normal" BW depending on the sex, age, height, complexion, and estimated the level of obesity. The program has been tested on a group of students. The studies demonstrated that most men and women had average BW. Excessive BW was found in 29% women and 5% men. Besides, fat women are psychologically predisposed to have excessive BW, and thin ones, insufficient BW. The complexion of the patient significantly influences the allowed BW.

[Effect of hyperbaric oxygenation on residual insulin secretion in patients with diabetes mellitus type 1].

The paper is concerned with the results of a study of the C-peptide concentration in blood serum of 45 patients with type I diabetes mellitus prior to and during therapy with hyperbaric oxygenation (HBO). Each patient was given 10-12 HBO sessions. A HBO therapeutic course was shown to stimulate residual insulin secretion in type I diabetes mellitus with a period of disease not more than 3 years, a daily dose of insulin not more than 60 units, in the absence of excess body mass over 10%, and retinopathy over stage I. Prognostic criteria make it possible to take into account clinical factors in the course of diabetes mellitus preventing the normalization or stimulation of residual basal insulin secretion which can be used for the determination of indications for HBO therapy aimed at the stimulation of insulin secretion.

[Automated design of a diet for a patient with diabetes mellitus and obesity].

A program of automated calculation of an individual diet for patients with diabetes mellitus and obesity has been worked out for the Elektronika HMC-80 PC. During a 15-20 min. interactive dialogue with the PC the caloric value and balance of a diet of 160 food items are assessed. Indices like sex, age, height, body mass, physical activity and diet are taken into account in dietary calculation. The testing of the program under clinical conditions has shown its multifunctional nature: it permits the detection of disturbed quantitative (caloric value) and qualitative (the ratio of proteins, fats and carbohydrates) indices of a diet, errors of an inpatient dietetic service and helps to form habits of nutritional culture in patients and medical staff.
