Thyroid function and circulating antithyroid antibodies in myasthenia gravis.

Evaluation of thyroid function in 104 patients with myasthenia gravis by T3, T4, TBG, and TSH radioimmunoassays and the TRH-stimulation test in 47 patients disclosed thyrotoxicosis in 5.7%, preclinical hyperthyroidism probably due to autonomously functioning thyroid tissue in about 10% of patients stimulated with TRH, hypothyroidism in 1.9%, and preclinical hypothyroidism in 3.4%. Eighty-four percent were euthyroid. Antithyroid antibody activity was detected by hemagglutination tests. Twelve patients had antithyroglobulin antibodies (Tab), and 28 had antimicrosomal antibodies (Mab). Among the euthyroid myasthenic patients, 7 were Tab-positive and 20 were Mab-positive. Euthyroid antibody-positive patients had a significantly higher TSH response in the TRH stimulation test and may be at high risk for hypothyroidism.

[Normal range and significance of serum ferritin; variations with age and sex (author's transl)].

Iron is necessary for every of organic life. The depot forms of the iron are Haemosiderin and Ferritin. Comparisons of the serum ferritin with the body iron contents measured through quantitative phlebotomy, intestinal iron absorption and bone marrow puncture showed extremely significant correlations. Since there are often lower serum iron levels to be found in elderly persons and the hypoferric anemia is the most important cause of it, the question of the valency for the serum ferritin analysis arises. Two groups of young and old persons were examined. Their age ranged between 20 and 30 as well as 60 and 90 years. It showed an increase of the serum ferritin level depending on the age. There are also sexual distinctions. The added immunoglobulin determination in older patients does not lead to a better interpretation of the investigated serum ferritin value. The analysis of the serum ferritin is of great importance in young patients as well as for some illnesses, but becomes problematic in old patients because of the increasing number of disturbing factors.

[T3, T4, TSH and circulating antithyroglobulin antibodies in myasthenia gravis].

Myasthenia gravis (MG) is a rare neuromuscular disease which is known to be associated with hyperthyroidism at high frequency (3-10%). These investigations, however, are basing on studies of the basal metabolic rate, protein bound iodine (PBI) and the thyroidal radioiodine uptake. In our study (104 MG patients, 104 control subjects) we wanted to evaluate thyroid function on the basis of T3, T4, TBG and TSH radioimmunoassays and in addition we looked for the presence of circulating antithyroglobulin antibodies (Tab) and antimicrosomal antibodies (Mab). The following thyroidal states were disclosed: Thyrotoxicosis (5.8%), hypothyroidism (1.9%), preclinical hypothyroidism (3.8%) and euthyroidism (88.5%). Twenty-eight myasthenic patients were Mab-positive and twelve in addition Tab-positive. Comparing the twenty euthyroid antibody-positive MG-patients with euthyroid antibody-negatives no statistically significant differences were found in thyroid function parameters. It is discussed that part of the thyroid antibody-positive patients may have a symptomless autoimmune thyroiditis which is finally resulting in hypothyroidism via a preclinical hypothyroid status.

Classification of organic brain syndromes by cluster analysis.

Cluster analysis was carried out on a sample of 92 patients with behavior disorders caused by degenerative, vascular, (alcohol) toxic, and other diseases of the brain. Rating variables of the AMDP system concerning mental state, social behavior, need for special care, sleep disorders, autonomic, physical, and neurologic symptoms were used in the absence of severe degrees of disordered consciousness such as stupor, coma, delirium tremens, and gross cerebral lesions. Results suggested the existence of four major groups of global cognitive impairment combined with neurasthenia and irritability in the first, hypochondriasis and depression in the second, withdrawal symptoms in the third, and severe disorientation in the fourth. At the seven-group level the groups were further distinguished according to severe withdrawal, amnestic syndrome, and dementia by various social and illness behaviors, sleep-wakefulness pattern, hypo- or hyperactivity, additional physical, and neurologic symptoms. Other minor types of organic brain syndromes were identified as individual cases by hallucinations or other circumscribed cognitive, psychomotor, affect, motivation, personality, and/or behavior disorder, symptomatic manic, or schizophreniform psychosis. The findings lent support to old classifications and new ones of organic mental syndromes (DSM-III).

Thyroid function in multiple sclerosis.

Thyroid function was studied in 112 MS patients and 175 controls on the basis of T3, T4, T.B.G. and T.S.H. radioimmunoassays. In 33 MS patients with T.R.H. stimulation test was employed additionally. The MS patients had significantly increased T4 levels whereas T3 and T.S.H. were found to be significantly lower. No differences were found in the T.R.H.- stimulated T.S.H. response. The changes in thyroid function parameters were similar in different courses of MS and did not correlate with the degree of clinical disability. The results suggest that in MS the T4/T3 conversion might be reduced as seen in acute or chronic diseases. X

Brain oxidative metabolism and blood flow in alcoholic syndromes.

42 alcoholic patients were grouped together according to the symptom-sign clusters derived from original classification variables of AMDP-rating scale system: (1) mild withdrawal, (2) severe withdrawal, (3) Korsakoff's syndrome, and (4) the clinical syndrome of dementia. Groups determined by clusteranalysis were subjected to a discriminant function analysis. Total cerebral blood flow (CBF) and the cerebral metabolic rates of oxygen, carbon dioxide, glucose and lactate were investigated. Patients with mild withdrawal tended to display increased oxygen uptakes reflecting 'pseudonormal' mean values with a wide range of the single data but predominantly elevated over the normal range. In mild withdrawal lactate production showed a tendency to increasing derangement of metabolism. There was much evidence of disturbance of brain oxidative metabolism with progressively decreasing cerebral metabolic rates of glucose, lactate, oxygen, and CBF from mild to severe withdrawal, Korsakoff's syndrome and dementia. Progression of both clinical phenomenology and cerebral dysfunction and/or damage culminates in simplified (reduced, "burnt-out") neuronal functional levels in advanced cases.

Gender differences and age of onset in the catatonic subtypes of schizophrenia.

In a family study involving 139 probands with chronic DSM-III-R schizophrenia, catatonic type, 83 probands met the criteria for periodic catatonia and 56 probands those for systematic catatonia according to Leonhard. In the systematic catatonias, we found a low morbidity risk of 4.6% in first-degree relatives, an early age at first hospitalization and a high prevalence of affected males. In the light of our recent report of an association between maternal gestational infection and systematic schizophrenia, male fetuses exposed to midgestational infection seem to be particularly at risk of developing systematic catatonia. Periodic catatonia with a family morbidity risk of 26.9% affected both genders with equal frequency and showed no age-at-onset differences between the genders. We found a moderate inverse relationship between early-onset probands and an increased risk in relatives of 24.1% compared to 17.8% in late-onset probands. Our findings substantiate the hypothesis that periodic catatonia is a clinically homogenous disorder with a major gene effect and an age at onset which is to a large extent genetically determined.

Worst-Fechner biconcave minus power phakic iris-claw lens.

OBJECTIVE: Evaluation of long-term results of correction of high myopia by implantation of a biconcave Worst-Fechner minus power iris-claw lens into phakic eyes. METHODS: One hundred and twenty-seven eyes of 70 patients were implanted with the phakic IOL between November 1986 and November 1991. The mean preoperative spherical equivalent refraction was -14.29 +/- 5.77 D (range -5.00 to -31.75 D). The total population decreased from 127 to 68 eyes (53.5%) at the 8-year examination. By this time, 26 of the 70 patients (37%) had been lost to follow-up for unknown reasons. Corneal endothelial density was estimated by comparing the image gained by non-contact specular microscopy with a graduated reticule (Karickhoff method). RESULTS: At the 6-month examination, mean deviation of achieved from calculated refractive correction was +0.52 +/- 1.46 D (range -2.25 to +6.50 D). Seventy-seven eyes (62.1%) deviated 1.00 D or less, and 15 eyes (12.1%) deviated by more than 2.00 D from the calculated correction. A refractive outcome of +/- 1.00 D was attempted in 68 eyes (54% of the 124 eyes) and was achieved in 51 eyes (75%). Mean spectacle-corrected visual acuity preoperatively was 0.54 +/- 0.27 (range 1.20 to 0.05). At the 6-month examination it was 0.73 +/- 0.3, and by the 8-year examination had decreased to 0.65 +/- 0.26. Statistical analysis of corneal endothelial cell density revealed a significant correlation of endothelial cell loss with age (> or = 45 years), anterior chamber depth (< or = 3.4 mm), and IOL power (> or = -11.00 D). Seventeen eyes (13.4%) had a decrease in endothelial cell density and projection to 8-year follow-up resulted in a decrease in 27% of eyes. Four eyes needed a penetrating keratoplasty. CONCLUSION: Implantation of a biconcave Worst-Fechner lens into a phakic eye effectively corrected high myopia with a stable refractive outcome, but 13.4% of implanted eyes suffered significant progressive reduction of endothelial cell density. This style lens is no longer in clinical use.

Brain tumor CT attenuation coefficients: semiquantitative analysis of histograms.

This paper reports on work in progress on semiquantitative curve analyses of histograms of brain tumors. Separation of statistical groups of attenuation values obtained by computer calculation is done separately from scanning, using histogram printouts as the data input for a programmable calculator. This method is discussed together with its results in 50 cases of malignant gliomas. The detection of hidden tissue portions and the more accurate evaluation of partial enhancement effects have been the investigators' main concerns to the present time; however, this method may allow more specific diagnosis of malignancy and changes in tumor characteristics than visual assessment alone. This has not been proven by studies that have evaluated large numbers of cases, but seems to be worth pursuing as a new approach.
