Immunolocalization of prealbumin as a marker for carcinoid tumours.

Using a PAP immunoperoxidase technique, we have demonstrated the plasma protein prealbumin in 92 (87 per cent) out of a series of 106 carcinoid tumours from various sites. The frequency of detection was greater than with the conventional stains; 83 (78 per cent) of the cases were positive for one or more of these stains. The Grimelius technique was by far the most effective of the conventional stains; it was positive in 81 (76 per cent) cases. We were unable to detect prealbumin in a variety of carcinomas. Although prealbumin identification did not appear to have any advantage over the routine stains in midgut-derived carcinoids, it was much more effective in the diagnosis of bronchial (foregut) and rectal (hindgut) carcinoids. We believe that prealbumin staining offers a cheap, reliable and valuable method for the detection of carcinoid tumours in routine histopathology.

Chronic granulomatous cheilitis and its relationship to Crohn's disease.

Investigation of a 15-year-old white male patient with chronic granulomatous cheilitis revealed asymptomatic Crohn's disease of the lower gastrointestinal tract. This is reported to support the suggestion that chronic granulomatous cheilitis and Crohn's disease may be intimately related. Tests for nutritional status are shown to be important in the investigation and management of patients with this condition, being the first indication of any gastrointestinal pathology.

[Functional role of several groups of neurons in oculomotor nerve nuclei (experimental study)].

The localization and functional significance of some groups of neurons in the main nuclei of the cat aculomotor nerve were studied with the aid of the method of retrograde degeneration. The topography of the oculomotor nerve nuclei in some representatives of the order of rodents, hare-like and predatory animals was specified. The conducted study demonstrated that the cell groups in the main nuclei of the oculomotor nerve were situated rostro-caudally in the following order: for the superior straight, internal straight and inferior straight muscles. In the middle and caudal sections of the nuclei there is a semidecussation.

[The autonomic division of oculomotor nerve nuclei in mammals and during human development].

Development of the vegetative component of the oculomotor nerve nuclei was studied in the main mammalia orders and in human ontogenesis. In phylogenesis parasympathetic nuclei of Jakubovitch--Edinger--Westphal and those of Perlia are formed later than somatic nuclei of the oculomotor nerve. The animals studied have, together with well developed somatic components of the oculomotor nerve nuclei, gomologues of parasympathetic nuclei, which are rather well developed in carnivores and monkeys. In human prenatal ontogenesis, somatic nuclei are formed first and then parasympathetic ones. Both parts of the oculomotor nerve nuclei are fully developed in adult man.

Treatment of portwine stains using the pulsed dye laser.

Five years of clinical experience of the treatment of portwine stains using the flashlamp-pumped dye laser is presented. The dye laser, when turned to a wavelength of 577 nanometres with a short pulsewidth of the order of 340 microseconds, may be used to target selectively the dilated vasculature constituting the lesion. Patients with ages ranging from 5-45 years were treated under general anaesthetic using a computer controlled scanning system developed by the authors. Several repeat treatments were found to be necessary. Results are presented ranging from total eradication of the lesion to marginal lightening only. No scarring of the treated sites was evident.

A pilot study with long term follow up of hyperbaric oxygen pretreatment in patients with locally advanced breast cancer undergoing neo-adjuvant chemotherapy.

Work in an animal cancer model suggests that pretreatment with hyperbaric oxygen can improve tumor vascularity rendering chemotherapy more effective. Accordingly 32 subjects with locally advanced breast carcinoma (>5cm diameter) entered into a randomized clinical trial where a course was administered of six intravenous pulses of cyclophosphamide 1000mg/m2 i.v., doxorubicin 50mg/m2 i.v. and vincristine 1.5mg/m2 i.v. In the case group this was preceded by ten, once daily, sessions of hyperbaric oxygen therapy (HBO2) administered either at 2.4 or 2.0 atmospheres absolute. Eleven out of 15 subjects tolerated a full course of HBO2 and chemotherapy. All 17 control subjects tolerated a full course of chemotherapy. Tumor extravascular extracellular or edema fluid was reduced after HBO2 but there was no reduction in tumor cell volume and no indication of increased vascularity on MRI. Clinical and pathological responses to chemotherapy were the same in both groups and there was no evidence of neovascularisation. Five year survival in those who tolerated the trial regime was 73% and did not differ between the groups. This mortality was cancer related.

The prevalence and prognostic significance of trophoblastic proteins in testicular teratoma.

Testicular neoplasms incorporating trophoblastic tumour can contain HCG, SPI and PP5 and have a bad prognosis. These placental proteins also may be localized immunocytochemically in other germ cell tumours in isolated cells which morphologically resemble those in the syncytial layer of trophoblastic tumour, but the prognostic significance of these syncytial giant cells is not established. In sections from 89 malignant teratoma intermediate tumours of pathological stage P1 syncytial giant cells containing HCG were observed in 66 per cent, PP5 in 47 per cent and SPI in 46 per cent. No correlation was observed between the presence of cells staining for each protein and survival at 4 years. Immunocytochemical localization of these proteins in this group of tumours is not a necessary part of a diagnostic service.
