[The combined treatment with surgery and chemotherapy: primary approach to small cell lung carcinoma (SCLC)].

Forty cases of small cell lung carcinoma (SCLC) treated with surgical and chemotherapeutic combined therapy were reported. There were 28 males and 12 females in the group, most with history of more than 2 months, ranging in age of 27-66 years old. Cough, bloody sputum, low fever and chest distress are the main clinical manifestation. The small cell undifferentiated carcinoma was confirmed by fiberbronchoscope examination and pathology in all patients. Single lobectomy was performed in 20 cases, lobectomy of the upper and middle lobe in 9 cases, (sleeve resection of the lobarbronchus in 3 cases), and total pneumonectomy in 7 cases. Two cases were of stage I, 18 were of stage II, and 20 were of stage IIIa. Twelve patients received chemotherapy after operation, and 28 patients underwent the "chemotherapy-operation-chemotherapy" treatment model. Adriamycin (or CDDP), cytoxan, vincristin, and dexomathasone were used for the chemotherapy procedure. The 1, 3, 5-year survival rate of chemotherapy after operation and chemotherapy-operation-chemotherapy group were 70%, 45%, 30.5% and 54%, 30% and 22%, respectively. It is demonstrated that the long-term survival rate could be elevated in SCLC patients treated with chemotherapy after surgical operation, and the chance of operation also could be elevated by preoperative chemotherapy. The resection rate was 93% in the preoperative chemotherapy group.

[Determination of dust content in the lungs of 10 cases with stage I coal workers' pneumoconiosis (CWP)].

Dust deposition in the lung tissue of 10 cases with pathologically diagnosed stage I CWP was determined. The left lung tissue was digested by 37% hydrochloric acid, and its organic composition was removed by chloroform and 50% ethanol. Results showed total dust content averaged 8.64 grams per left lung, with a standard deviation of 5.54 grams. Dust content varied in different parts of the lung, measured the highest in lymph nodes, the upper lobe and pleura the next and the lowest in the lower lobe, with a very significant difference by ANOVA (F = 44.76, P < 0.001). Dust less than 5 microns in diameter accounted for 94.0% of the total content, and that equal to or more than 10 microns only 0.8%. A certain amount of fibrous dust could be found in the lung. Dust deposited in the lung accounted only for 0.64% of that exposed to.

[Laparoscopic resection of the adrenal tumor].

Laparoscopic resection of the adrenal tumor was carried out in 8 patients since January 1993. The age of the patients was 22 to 45 years, 6 with aldosteronism and 2 with Cushing's syndrome. The tumors were located in righ adrenal in 5 cases and left adrenal in 3. The tumor size was 1.0-3.6 cm. All laparoscopic procedures were performed under general edothacheal anesthesia. The operative time ranged from 90 minutes to 4 hours. Bleeding volume during the operation was 20-90 ml. We developed a reliable technique for adrenal tumor using a laparoscopic approach. The anatomy of adrenal glands, operative indications and complications are discussed. Laparoscopic resection of the adrenal tecmos is a valuable method.

[Unusual manifestation of cerebral cysticercosis].

We have surgically treated two patients with cerebral cysticercosis, which were pre-operatively unexpected due to unusual CT and MRI manifestations. A 24 year old chinese farmer with intermittent severe headache for 6 years was examined by CT and MRI. A huge cystic lesion with same density or intensities to CSF occupied retroclival posterior fossa to upper cervical spinal canal and displaced neural structures backwardly. By suboccipital craniectomy, watery clear cyst fluid of 44ml was aspirated and the shrunken cyst was pulled out with ease. It was unable to predict until an electron microscopic study, which revealed a larva with three layers of microvilli, vesicular tegmentum and infrategmental muscle bundles and vesicles. The findings were same to those of racemose cysticercus of human brain. Giant cysts of cysticercosis were infrequently present supratentorially, although infratentorial or basal cysticercosis were racemose and small. The present case may be the first case of a huge cyst of cysticercosis in posterior fossa. A 44 year old Japanese businessman with 15 years history of general convulsion underwent craniotomy due to a small, but growing cystic granulomatous mass in right inferior frontal gyrus. The CT scan examined at 3 years after his first general convulsion revealed a small round low density mass, 5mm in diameter, in right frontal lobe and its wall was weakly enhanced by contrast media. The follow-up CT scans and MRIs revealed that the enhancing cystic mass became thick and deformed, and perifocal edema came to evident and progressed stepwise by attacks of general convulsions.(ABSTRACT TRUNCATED AT 250 WORDS)

Determination of thiocyanate metabolite of sodium nitroprusside in serum by spectrophotometry.

A sensitive, reproducible and rapid method for the spectrophotometric determination of thiocyanate, the metabolite of sodium nitroprusside, was developed. Acetonitrile was used to remove protein in the serum. Uniform design technique was used in arranging the experiments and choosing the reaction conditions between thiocyanate and ferric nitrate reagent. The wavelength of maximum absorption was 456 nm, the calibration curve of SCN- was linear in the range of 1.68-13.4 micrograms/ml (r = 0.9999), the relative average recovery of thiocyanate was 94.0%. The day-to-day and within-day relative standard deviations were less than 4.0% (n = 8) and 3.2% (n = 10) respectively. The limit of determination was 0.3 microgram/ml. Drugs such as lidocaine, etc. presented no interference on the determination.

[Study on the pathogenic factors of pneumoconiosis in Beijing coal district].

Ferruginous bodies were first found in the lung tissues of coal miner autopsies in Beijing Coal mining. District irrespective of the presence or not pneumoconiosis, ferruginous bodies were found in the lung tissues as examined with two methods, tissue section and digestion and there was significance difference in their number. Two kinds of ferruginous bodies, one a "coal body", the other an "amphibole body" were distinguished when examined by EDXA. The fibrous particles in the dust extracted from the lung tissue are the physical basis of ferruginous bodies formation. After the lung dust has been subjected to X-ray diffraction analysis, there are also amphibole bodies in the lung tissue. There is no doubt that the presence of asbestos in the coal mine is one of the pathogenic factors of pneumoconiosis.

[CT findings in "fresh" cerebral paragonimiasis].

There are few reports on CT findings in "fresh" cerebral paragonimiasis. We have experienced four cases of "fresh" cerebral paragonimiasis examined by CT scan. Three patients were children aged 7, 9, and 14 years, and one was an adult aged 25 years. Three patients were examined by CT scan 2 to 6 months after the onset of high grade fever, convulsion and focal deficit signs, and a patient was examined one month after his progressive visual disturbance. The unique CT findings are multilocular cystic lesions in temporo-occipital or in temporo-parietal lobes with extensive brain edema. Two cases were also associated with "soap-bubble" calcifications. The cysts were more dense than CSF and enhanced by contrast media. The histopathological specimen showed that the eggs of paragonimus were in the abscess cavity, of which the wall was composed with highly vascular gliomesenchymal capsule and numerous cell infiltration. Three patients underwent craniotomy for removal of abscess and decompression. Bitionol were administered and all patients recovered well. We also discussed the differential diagnosis of cerebral parasitic granulomas.

[Morphology of the ferruginous bodies and characteristics of lesions in coal mine pneumoconiosis].

Ferruginous bodies were found in lung tissue collected from 105 coal miner autopsies in Beijing coal area. The incidence rate was 83.8%. Cores of the ferruginous bodies consist of black or transparent fibres partially or entirely coated with golden brown colored iron-protein. They were polymorphic in appearance, and were widely distributed in the lungs. Most of them located in the pulmonary alveoli, and some in the inflammatory or carcinomatous lesions. Besides the pneumoconiosis lesion, there were also abundant multinuclear foreign-body giant cells, and proliferation of type II alveolar cells in the alveoli. Regardless of contact history with coal dust, the average number of ferruginous bodies found in the lungs of workers with no pneumoconiosis was much lower than that with pneumoconiosis lesions (P less than 0.05). The number of ferruginous bodies tends to increase with the severity of the pneumoconiosis lesion. The authors consider that it is worth while to make a further study in elucidating the relationship between ferruginous body and the high incidence of pneumoconiosis in Beijing coal area.

[Quantitative determination of furosemide in serum and urine by high-performance liquid chromatography with electrochemical detector].

A high-performance liquid chromatographic method using electrochemical detection is described for the determination of furosemide in serum and urine. Serum proteins were precipitated with acetonitrile containing the internal standard and the clear supernatant was separated. Urine was diluted with water to 50 times. Furosemide was chromatographed on a reversed-phase column, 8MBC18 column, using 35% ethanol solution containing 5 mmol/L tetrabutylammonium phosphate (pH 7.50) as the mobile phase (1.0 ml/min) and detected at 0.90 volts. FD-Val-OH synthesized was used as internal standard in the furosemide assay. The retention times for furosemide and internal standard were 10 min and 15 min, respectively. The amount of furosemide was determined by measuring peak height ratio. The detection limit of furosemide was 16 and 9 ng per ml serum and urine respectively. The calibration curve was linear in the range from 0.25 ng/microliters to 5 ng/microliters (serum) and from 0.5 ng/microliters to 10 ng/microliters (urine). The recovery of furosemide was 100.5% (serum) and 100.6% (urine). Coefficient of variation was less than 4.6%.

[A pathohistological study on experimental brucellosis in inbred mice].

A pathohistological study was carried out in BALB/C and C57/BL inbred mice models infected with B. melitensis, B. abortus, and B. suis respectively. The results showed that a chronic pathological lesion was developed due to repeated infectious processes occurred in various organs at different stages of brucella infection. Significant pathological changes was observed in the reticuloendothelial system including the lymphocytes, spleen and liver etc. and less remarkable changes were seen in the lung, heart and kidney in sequence. Comparing the pathological changes induced by the three species of brucella infection, lesions developed in BALB/C was more remarkable and the pathological change induced by B. abortus infection was the most significant in both BALB/C and C57/BL mice.

Correlations of blood lipids and lipoproteins with coronary artery disease documented by angiography.

A study of the relationships between serum TC, TG and HDL.C and selective coronary arteriography was carried out on 117 patients who were divided into groups according to the extent of artery stenosis. Normal control group consisted of 153 healthy subjects. There were no statistical differences in TC, TG, HDL.C, LDL.C, TC/HDL.C, LDL.C/HDL.C and TC-HDL.C/HDL.C between normal control group and the group with normal coronary arteriogram. LDL.C, TC/HDL.C, LDL.C/HDL.C, TC HDL.C/HDL.C rose and HDL.C decreased as the degree of coronary artery stenosis and the extent of stenosis increased, besides the medium and severe stenosis group. Analyses based on the correlation coefficients indicate that 3 compound indexes (TC-HDL.C/HDL.C, TC-HDL.C, LDL.C/HDL.C) are better in assessing CAD than single index such as LDL.C, HDL.C and TC. The results of our study showed that the 3 compound indexes might be regarded as important risk factors for CAD.

Acute cerebrovascular disorder induced by allergic vasculitis. A case report.

A patient with vascular disease of the brain due to allergic vasculitis, which had been misdiagnosed for many years, was given anti-inflammatory, antituberculosis and antitumor drugs with no avail. He was admitted this time for an attack after eating food prepared with leftover oil used for frying hairtail. Considering the possibility of allergic vasculitis, we tried antianaphylactics and immunosuppression drugs including dexamethasone with satisfactory results. The final diagnosis was allergic granulomatous vasculitis.

Evaluation of the renal tubulo-toxic effect of gossypol by urinary protein analysis with SDS-polyacrylamide gel electrophoresis.

The present study assessed the urinary protein with SDS-PAGE technique and by using gentamicin as a positive control for a comparative study to evaluate the renal toxic effect of gossypol. Results indicated that gentamicin could induce proteinuria by alteration of glomerular permeability to cause over-filtration of high molecular weight proteins, damage the brush border (BB) membrane of proximal renal tubules and impair tubular reabsorption function. Gossypol could also induce proteinuria in certain cases of guinea pigs but not at all in gossypol-treated rats. However, an increase in low molecular weight protein bands (M.W. range 20-30 kd) in urinary electrophoregram of gossypol-treated rats were detectable.
