A bacteriological study of craniocerebral missile wounds from Vietnam.

Bacteriological studies were performed on 45 craniocerebral missile wounds incurred in Vietnam within 2 to 4 hours of occurrence. All missiles had penetrated into the brain. Aerobic and anaerobic cultures were taken of the skin wound, brain, and indriven bone fragments. Forty-four of the skin wounds were contaminated, predominantly with staphylococcus. Only five brain wounds showed bacterial contamination 2 to 4 hours after wounding, indicating that many missile tracks within the brain are initially sterile. Of the patients who had early debridement, 45% had contaminated bone within the brain; possibly up to 75% of all indriven bone chips were sterile. The authors draw the following conclusions. Complete brain debridement with removal of all indriven bone is ideal. Accessible retained bone should be removed by reoperation. Multiple reoperations for an inaccessible retained fragment are inadvisable, however, as fatalities or severe neurologic residua may result. An individual indriven bone chip has a small likelihood of bacterial contamination provided initial debridement was done early. This knowledge may justify an expectant policy in certain individuals harboring an inaccessible retained bone fragment. The retained fragment would be removed only if untoward difficulties develop.

Anti-Sdx: a "new" auto-agglutinin related to the Sda blood group.

Two examples of a "new" IgM saline-agglutinating auto-antibody are described. The antibodies bind complement, have the ability to cause in vivo hemolysis, and are most active at room temperature at a pH of about 6.5. Despite tests on more than 5,000 people, no nonreactive cell sample has been found. The reactive antigen is not denatured by neuraminidase, papain, or ficin, and is present on i adult red blood cells. The antibodies appear to be slightly inhibited by human saliva and milk, and more convincingly inhibited by urine from Sd(a+) persons. They are not inhibited by urine from Sd(a-) persons, but are strongly inhibited by guinea pig urine. The serologic characteristics indicate a relationship to the Sda blood group and the auto-antibody has been named antiSdx. Sdx antigen is present on red blood cells from some higher primates and is absent from rabbit, rhesus monkey, dog and sheep cells.

Gonorrhea in the homosexual man: frequency of infection by culture site.

The aims of this study were determine the frequencies of infection with Neisseria gonorrhoeae at various sites in homosexual men who were attending clinics for treatment of sexually transmitted diseases in central Scotland and to appraise the diagnostic tests used. Specimens for culture were taken from the urethra, pharynx, and anorectum of every homosexual man in the study. When the first cultures of pharyngeal and rectal specimens were negative, these cultures were repeated twice at weekly intervals. The urethra was infected in 169 (60.8%), the anorectum in 114 (41.0%), and the pharynx in 23 (8.3%) of 278 patients who had gonorrhea. By reliance on only one set of tests, eight (7.0%) of 114 patients who had rectal gonorrhoea and six (26.1%) of 23 patients with pharyngeal infection would have been missed. The results indicate the importance of obtaining specimens for culture from all sites that might possibly be infected, regardless of the symptoms.
