[Comparative studies on activities of antimicrobial agents against causative organisms isolated from urinary tract infections (1989). II. Background of patients].

Clinical background was investigated on 1,197 bacterial strains isolated from patients with urinary tract infections (UTIs) in 10 hospitals during the period from June, 1989 to May, 1990. 1. Distribution of sex and age A majority of female patients with UTIs accounted for a twenties and thirties. The proportion of male patients with UTI has increased with age, accounting for majority in patients 70 years or older. 2. Distribution of infection types and age Most cases among twenties or thirties were uncomplicated UTIs. Fifties and older cases were most frequent in complicated UTIs. 3. Distribution of isolated bacteria and age Escherichia coli was most frequent on the whole, followed by Enterococcus faecalis, Pseudomonas aeruginosa, Staphylococcus spp. and Klebsiella spp.. E. coli had declined with age, and on the other hand, E. faecalis and P. aeruginosa had increased with age. 4. Administration of antibiotics and pathogens isolated from UTIs. In uncomplicated UTIs, pathogens, after administration of antibiotics, isolated from patients have obviously decreased from 439 to 4. Before an administration of antibiotics, E. coli was mainly detected, but after an administration of antibiotics, E. coli has decreased into two strains. After administration of antibiotics, pathogens of complicated UTIs, without indwelling catheter, have decreased from 324 to 19: E. coli has decreased from 86 to 2, but E. faecalis, Staphylococcus spp., Enterobacter spp. and P. aeruginosa have increased. Pathogens of complicated UTIs with indwelling catheter, have decreased from 156 to 14 after administration of antibiotics, and mainly isolated pathogens were E. faecalis and P. aeruginosa.

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from urinary tract infections (1989). III. Secular changes in susceptibility].

Susceptibilities of Citrobacter spp., Enterobacter spp., Escherichia coli, Klebsiella spp., Proteus mirabilis, Pseudomonas aeruginosa, Serratia spp. isolated from patients with urinary tract infections (UTIs) in 10 hospitals during June 1989 to May 1990 to various antimicrobial agents were compared with those in the same period of previous years according to a classification, uncomplicated UTIs, complicated UTIs without indwelling catheter, and complicated UTIs with indwelling catheter. As for Citrobacter spp., P. mirabilis and Serratia spp., which were detected very few in 1989, their susceptibilities were not observed an obvious change. As for Enterobacter spp., the susceptible strains to flomoxef, cefixime, cefuzonam and ceftazidime increased in complicated UTIs with indwelling catheter. The susceptibilities of E. coli to penicillins increased slightly in complicated UTIs with indwelling catheter. Against Klebsiella spp., a good activity of minocycline or cephems was found. The susceptibilities of P. aeruginosa to ciprofloxacin and new quinolones increased in uncomplicated UTIs. These data should be considered in clinical treatment of various urinary tract infections.

[Clinical evaluation of endoscopic variceal ligation (EVL) using an intensive ligation method].

Endoscopic variceal ligation using an intensive ligation method, or intensive EVL, as we call it, was performed in 17 cases of esophageal varices, from August 1993 to November 1994. Intensive EVL involves making as many ligations as possible from just above the esophago-gastric junction at every session for the complete eradication of varices (F0 and negative Red color sign). Our success rate for the complete eradication of varices was 82.4% (14 out of 17 cases) with the mean number of sessions at 2.2 +/- 0.5 times. The total number of rubber bands used was 20.2 +/- 8.4, with 14.1 +/- 6.0 being used in the initial session alone. The rate of recurrence in complete eradicated cases was 50% (7 out of 14 cases) and the period to recurrence was 6.3 +/- 0.7 months. Although EVL using the intensive ligation method is useful for complete eradication of varices, a high incidence of recurrence in the short term is a concern. We conclude therefore that it is difficult to achieve long term complete eradication of varices with EVL alone, and that additional therapy is needed for long term control.

[A case of primary mediastinal germ cell tumor, successfully treated with chemotherapy and curative resection].

A 20-year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray Laboratory data revealed a high serum alpha-fetoprotein (AFP) and LDH level. Percutaneous needle biopsy of the tumor suggested primary mediastinal germ cell tumor. Curative resection was performed after three courses of combination chemotherapy (cis-platinum, VP-16, bleomycin and adriamycin). A post-operative histological examination of the mass revealed total necrosis. Post-operative course was uneventful, and he has been free of recurrence for the last six months.

Role of cytoskeleton and acidification of endocytic compartment in asialoglycoprotein metabolism in isolated rat hepatocyte couplets.

The process of receptor-mediated endocytosis is common to a variety of species and cell types. One of the best characterized receptor-ligand systems is the hepatocyte receptor for asialoglycoproteins. We investigated the morphological features of the uptake and intracellular transport of gold-conjugated asialofetuin in isolated rat hepatocyte couplets. We assessed the effects of colchicine, lumicolchicine, cytochalasin B, and chloroquine on the uptake and intracellular transport of asialoglycoproteins. Isolated rat hepatocyte couplets were incubated with gold-conjugated asialofetuin, and transmission electron micrographs of these cells were analyzed to determine the density and distribution of gold particles in the peripheral and pericanalicular areas. Results were analyzed morphometrically. Colchicine significantly inhibited the uptake and intracellular transport of asialoglycoproteins, but did not affect membrane fusion of endocytic compartments in the peripheral area. Lumicolchicine and cytochalasin B had minimal effects on these processes. Chloroquine inhibited the uptake of asialoglycoproteins, but did not affect the intracellular transport of asialoglycoproteins. Results suggest that the microtubule is essential for intracellular movement of endocytosed asialoglycoproteins and receptor recycling, and that endocytic structures in the peripheral regions can fuse in the absence of intact microtubules. We also found that uptake and intracellular transport of asialoglycoproteins were independent of the microfilaments, and the pH gradient in endocytic compartments was important in receptor-mediated endocytosis of asialoglycoproteins.

[Gynecologic endoscopic surgery].

Recently, endoscopic operation represents a major innovation in various fields of surgery. In the gynecologic fields, hysterectomy, salpingo-oophorectomy, ovarian cystectomy, myomectomy and even lymphadenectomy are performed under the assistance of laparoscopy and hysteroscopy at present. Based on our clinical experiences of endoscopy, we have started endoscopic operation since 1992. The endoscopic operation in our department was reviewed in regard to methods and clinical results.

[Study on human eye movements induced by prolonged eye closure and lid opening--report 2. Closing and opening of eyelids].

The eye movement during prolonged eye closure and the following eye opening has not been studied precisely. In the present study we evaluated the eye movement during prolonged eye closure and the following eye opening using a magnetic search oil method. In a bright room, the exact moment of recognizing a visual target during eye opening could not be determined. Therefore, other conditions were used to elucidate the precise relationship between the presence and lack of visual input during eye opening. Eye movement during eye opening was accomplished in three phases. The first phase had rapid movements like visually guided saccades toward the imaged target. The second phase had slow movements and drift toward the imaged target, and the final eye position was reached within 3 degrees from the target from 5 different gaze directions. The third phase was final visually evoked corrective saccades and the subjects found the target. We discuss the origin of these eye movements.

[Scleroderma and porphyria cutanea tarda].

Examination was made of sclerodermoid changes in porphyria cutanea tarda (PCT) and PCT in association with true scleroderma. Although sclerodermoid change in PCT is rare in Japan, it is important in the diagnosis of PCT when there are few other clinical signs characteristic of PCT. In the present case, slight improvement in cutaneous sclerosis was noted following treatment for PCT, but pulmonary changes somewhat intensified. The reason for the association of PCT with progressive systemic sclerosis (PSS) remains obscure. Elevated uroporphyrin due to PCT may possibly increase cutaneous sclerosis in PSS. A diagnosis of PCT should be considered for patients presenting scleroderma, since it is difficult to distinguish sclerodermoid changes from true scleroderma.

[Experimental porphyria].

There are many chemicals that affect porphyrin metabolism. Certain of these can be used to induce experimental porphyria in animal. Human porphyrias are relatively uncommon, thus preventing their adequate examination in human patients. But using experimental animals, the pathogenesis, biochemical pathways and treatment of these diseases may be studied. In this paper, we conducted our own investigations on griseofulvin induced protoporphyria, and also reviewed experimental porphyrias induced by hexachlorobenzen and polychlorinated biphenyls.

[In vitro analysis for cellular toxicity of polychlorinated biphenyls(PCBs) and 2, 3, 4, 7, 8-pentachlorodibenzofuran (PCDF) on HeLa cell proliferation(III)--the effect of cytoactivator and antilipemic agents on cell toxicity].

We investigated the cell toxicity of polychlorinated biphenyls (PCBs) and 2, 3, 4, 7, 8-pentachlorodibenzofuran (PCDF) as indicators of the optical density (280nm) which is total protein in HeLa cells. Furthermore, the reductive action of cytoactivator and antilipemic agents on the PCBs and PCDF toxicity were evaluated. The quantity of total cellular protein increased to 20% with the addition of sodium dextran sulfate (2.5%) at the presence of PCBs, and 25% in the case of PCDF. However, the slope of the curve of cell proliferation of HeLa cells at the presence of PCBs or PCDF became to overlap with a control group at the presence of any other drugs except for sodium dextran sulfate. These results mean that PCBs and PCDF cell toxicity were suppressed a little by sodium dextran sulfate, but the case of other cytoactivator and antilipemic agents did not.

[Polychlorinated biphenyls (PCBs) and polychlorinated quaterphenyls (PCQs) concentrations in skin surface lipids and blood of patients with Yusho].

The relationship between Polychlorinated Biphenyls (PCBs), and Polychlorinated Quaterphenyls (PCQs) concentrations in the skin surface lipids and blood was investigated in twenty two patients with PCB poisoning (Yusho) and 9 control subjects were analyzed. The collection of skin surface lipids was performed by the method of cotton pad with 70% ethyl alcohol. The alkaline decomposition method described as "the official standard analytical methods for the isolation of PCBs and PCQs fractions" was used. In the blood of control group, the mean value of PCBs concentration was 1.8 ng/g, and that of PCQs concentration was very low and undetectable by our analytical method. On the other hand, the PCBs concentration in the Yusho group was three times higher than those in the control group. The mean value of PCQs concentration was 1.90 ng/g in Yusho group, while it was not detectable in the control group. In the skin surface lipids of patients with Yusho, the mean value of PCBs concentration was 668.6 ng/g, but was 256.7 ng/g in the control group. The mean value of PCQs concentration in the skin surface lipids of Yusho patients was 35.4 ng/g, although it was not detected in the control group. These values of the skin surface lipids in 1993 were decreased as compared with those in 1992. PCBs and PCQs levels in the skin surface lipids were higher than those in the blood. These findings suggests that cutaneous sebaceous system is one of the excretory systems of polyhalogenated chemicals, such as PCBs or PCQs, when these chemicals are precipitated in human or mammalian.

[Effects of polychlorinated biphenyls (PCBs) on porphyrin metabolism--experimental porphyria induced by large single doses of PCBs].

The effects of large single doses of PCBs and iron overload on porphyrin metabolism were investigated in male C57BL/6 mice. A single intraperitoneal dose of Kanechlor-500 (10 mg/mouse) caused urinary uroporphyrin to increase three days after injection. This increase was transient without porphyrin accumulation in the liver. Iron overload (12.5 mg/mouse) showed a tendency to prolong the action of PCBs.

[Correlation analysis between patlak plot and DAR using 11C-methionine PET].

Seven patients were studied with positron emission tomography (PET) using 11C-methionine. All had non-Hodgkin's lymphoma but one, who had Hodgkin's disease. By assuming a simple three-compartment model for dynamic data analysis, we used the graphic method proposed by Patlak et al., where Ki (K2K3/(K1+K3)) was compared with DAR (differential absorption ratio). Trichloroacetate was added to each plasma sample, and the activity of the acid-soluble fraction was counted to eliminate the influence of the protein-bound fraction of injected activity. Patlak data plots, using this acid-soluble fraction as an input factor, showed good linearity over the time of data collection. All tumors showed clear 11C-methionine accumulation. Tumor activity reached almost a plateau within 10 min and was kept at the same level for at least 30 min. Ki and DAR were 0.070 +/- 0.035 and 4.64 +/- 1.85 (mean +/- SD), respectively. There was no apparent correlation between histological type and Ki or DAR, while the correlation efficient between Ki and DAR was 0.875. Our conclusion is that DAR will give almost the same result as dynamic data in the analysis of methionine metabolism using PET.

[Appropriateness and limitations of bone mineral measurements by DXA (dual energy x-ray absorptiometry) in the elderly--comparison with x-ray findings].

A group of 674 (266 males and 410 females) elderly living in a rural community of Nangai Village, Akita Prefecture, were subjects of bone mineral measurements in the lumbar spine and three areas of proximal femur (femoral neck; FN, trochanter; TR and Ward's triangle; WD). Measurement was by dual energy x-ray absorptiometry (DXA) set in a mobile van during mass health examination. The purpose of this study was to verify the appropriateness and imitations in the bone mineral measurements by DXA in elderly who have other aging related abnormal calcifications such as osteophytosis in the lumbar spine and calcification of the abdominal aorta, all of which may have an influence on the 'true' value of bone mineral density (BMD) particularly in the lumbar region. The results were as follows: 1) Subjects who were not capable of being measured by DXA tended to be older and reported experiencing pain and who scored low in TMIG index of competence compared to measurable subjects. 2) BMD of 2nd-4th lumbar spine with antero-posterior projection (AP) did not show simple age-declines that are seen in younger generations. In contrast, BMDs of proximal femur show linear aging declines. 3) Analysis of the association between BMDs and osteophytosis by spondylosis deformans in the lumbar spine and calcification of the abdominal aorta in front of the lumbar spine showed that AP-BMD had a strong correlation with the grade of both spinal osteophytosis and aortic calcification. On the other hand, BMDs of proximal femur showed no significant associations with these abnormal calcifications. 4) In this context, in order to evaluate the 'true' BMD in the elderly, BMDs in the proximal femur are a more appropriate indicator than AP-BMD which may be easily contaminated by other aging-related calcification in and around the lumbar region.

Isoforms of PML-retinoic acid receptor alpha fused transcripts affect neither clinical features of acute promyelocytic leukemia nor prognosis after treatment with all-trans retinoic acid. The Leukemia Study Group of the Ministry of Health and Welfare (Kohseisho).

All-trans retinoic acid (ATRA) has been used as a potent differentiation drug for acute promyelocytic leukemia (APL). Although the mechanism of its effectiveness upon APL remains unclear, the PML-retinoic acid receptor alpha (RARA) chimeric protein produced by t(15;17) is assumed to underlie the sensitivity of APL cells to ATRA. There are two major isoforms of PML-RARA transcripts; short (S) and long (L), according to the breakpoints in the PML gene. We therefore compared the clinical variables, the response to ATRA and the prognosis between 28 patients with type S and 68 patients with type L. Patients were treated in multi-institutional trials with ATRA, and chemotherapy was combined when peripheral blasts and leukocyte counts increased during the therapy. The clinical features at diagnosis were similar between the two molecular subtypes, and there was no significant difference in remission induction rates; 86% for the type S group and 90% for the type L group. There was no statistical difference in overall survival and CR duration as well as disease-free survival (DFS). In newly diagnosed patients, predicted 2-year DFS was 66% for the type S group and 67% for the type L group. In refractory or relapsed patients, it was 19 and 23%, respectively. These data indicated that isoforms of PML-RARA fused transcripts affect neither the clinical features of APL nor the prognosis after treatment with ATRA.
