[Studies on antigenicity, phototoxicity and other specific toxicities of hydrocortisone 17-butyrate 21-propionate (HBP) (author's transl)].

The present experiments were undertaken to determine the antigenicity and other toxicities of HBP; such as phototoxicity, photosensitivity, ulcerogenicity, adrogenic-myotropic, estrogenic and progestational activities, and mutagenicity. No antibody formation and delayed type skin reaction of HBP were seen in rabbits. Active systemic anaphylaxis was not observed in guinea pigs challenged by HBP. In the phototoxicity and photosensitivity test, 0.1% HBP ointment, 0.1% HBP cream and 10% HBP acetone solution did not show any skin reaction with or without irradiation of ultraviolet light. Repeated subcutaneous administration of HBP irritated the gastric and intestinal mucosa dose dependently in rats as hydrocortisone 17-butyrate and betamethasone 17-valerate (BV). HBP had neither androgenic-myotropic nor estrogenic activity, but antiestrogenic activity was observed. The progestational activity of HBP in immature rabbit pretreated with estrone was less potent than BV. In the mutagenicity test os HBP investigated by the reverse mutation according to the method by Ames, no significant increase in the number of revertants was observed in the presence or absence of S9 mixture.

[Variation of growth hormone and prolactin levels in umbilical vein (author's transl)].

Umbilical venous blood, obtained immediately after the delivery of 71 non-complicated pregnants between 38 and 42 weeks of gestation, was investigated for the variation in growth hormone and prolactin levels in the serum. Growth hormone and prolactin levels remained almost constant during the observed period of the gestation. Further, it was shown that while growth hormone levels did not significantly vary through the 24-hour period, prolactin levels significantly did. In prolactin levels, the maximum value at 2 approximately 4AM was significantly about 2 times higher than the minimum value at 6 approximately 8 and 8 approximately 10AM. The levels did not vary through 2 approximately 4 and 10 approximately 12 PM period. These suggest that circadian periodicity exists in the fetal prolactin secretion, but not in the growth hormone, and in addition, the periodicity is not affected by labor and delivery.

In vitro estimation of cell kinetic parameters in human urinary bladder cancers.

The DNA synthetic time (T4) and the labeling index (LI) or urinary bladder carcinomas from 10 patients were measured by using the 3H-thymidine labeling technique in vitro under hyperbaric oxygen, and the potential doubling time (Tpot) was determined. The LI and Tpot were enormously variable depending on the histologic type, and ranged from 3.7 to 18.0% and from 48 to 136 hr, respectively. In contrast, Ts values fell in a rather narrow range. On comparing these results with the histologic patterns it was found that papillary carcinoma had longer Tpot than nonpapillary carcinoma and histologic grades were not well correlated with the LI's However, patients with higher LI and/or shorter Tpot showed poor prognosis. The practical significance of these kinetic parameters is discussed

Predisposing factors of renal dysfunction following total correction of tetralogy of Fallot in the adult.

A retrospective study of 21 adult patients with tetralogy of Fallot (TOF) was undertaken to determine the predisposing risk factors of renal dysfunction following total correction of the disease. Five of the 21 patients exhibited moderate-to-severe postoperative azotemia and an additional five exhibited mild azotemia. Significant risk factors for postoperative renal dysfunction found in routine preoperative examinations were as follows: arterial oxygen saturation of less than 90%, cardiothoracic ratio (CTR) of greater than 50%, mean electrical axis of the QRS complexes of greater than +120 degrees, S wave in Lead V6 of greater than 7 mm, R/S voltage ratio in Lead V6 of less than 2, and negative T wave in Lead V6. In addition, preoperative cardiac catheterization data showed that the patients exhibiting postoperative azotemia had more severe pulmonary stenosis, a smaller pulmonary-to-systemic flow ratio (Qp/Qs), and a larger left ventricular cavity than nonazotemic patients. The incidence of postoperative low cardiac output state (LOS) was significantly higher in the azotemic patients. These findings suggest that a combination of the severe form of TOF and a large left ventricle increase susceptibility to LOS and postoperative renal dysfunction. The cause and the clinical significance of the large left ventricular cavity in adults with TOF are discussed.

Immunogenicity of Bacteroides isolated from mice: relationship between immunogenicity and cell wall antigens.

Three different strains of Bacteroides were isolated from feces and cecal contents of mice. The immunogenicity of the strains was determined by measuring the serum agglutinin titers after intraperitoneal antigen injection. There were marked differences in quantity and quality of produced antibodies among the three strains. One strain (2-2) induced low antibody titers in both the primary and secondary responses, and a significant 2-mercaptoethanol (2-ME)-resistant antibody production occurred. Another strain (Y) induced low antibody titers in the primary response and high titers in the secondary response, but 2-ME-resistant antibody production did not occur. The third strain (2-4) induced very high antibody titers in both the responses, and a large amount of 2-ME-resistant antibody production occurred. Further, heat-ethanol-treated strain Y induced only immunoglobulin M antibody, but periodate-treated strain Y induced no antibody. Heat-ethanol- or periodate-treated strain 2-4 induced immunoglobulin M or G antibody, respectively. These observations suggest that the surface antigens of the two strains are distinctly different: the antigen of strain Y would be mostly O-antigen, whereas those of strain 2-4 would be O-antigen and protein moieties.

Hepatitis B surface antigen and chronic hepatitis in infants born to asymptomatic carrier mothers.

Twenty-three infants born to asymptomatic hepatitis B surface antigen (HBSAg) carrier mothers were followed up to determine the vertical transmission of hepatitis B virus. Four infants became positive for HBSAg within four months after birth. Three showed hepatic dysfunction; liver biopsy specimens demonstrated mild chronic hepatitis in one and persistent hepatitis in another. In six infants followed up for more than six months after birth, antibody to HBSAg (anti-HBS) without antigenemia could be detected. Antigenemia of cord blood strongly suggested that hepatitis B viral infection of the infant would occur. HBSAg was also demonstrated in breast milk by radioimmunoassay in three cases. These findings indicate that vertical transmission of hepatitis B virus occurs in many infants born to asymptomatic HBSAg carrier mothers, and that some of them become persistent carriers or develop chronic hepatitis without showing any signs suggesting hepatic involvement.

Ventricular septal defect: selection of patients and timing for surgery.

Eighty-seven patients with isolated ventricular septal defect (VSD) operated on in the first 5 years of life, that had a mean interval of 10 months between diagnostic catheterization and surgery, were studied retrospectively. The size of the VSD was correlated with diagnostic catheterization data and comparative analysis of clinicl findings (ECG and CTR) was made between the time of diagnostic catheterization and just prior to surgery. The study showed that 42 per cent of patients with the ratio of systolic pulmonary artery pressure to systolic systemic artery pressure (Pp/Ps) 0.5 to 0.8 had a marked reduction in size of VSD in a rather short interval of 10 months. In patients with Pp/Ps greater than or equal to 0.8, the incidence of decrease in VSD size was close to nil. The highest incidence of decrease in VSD size was seen in patients with high flow and low resistance. ECG and CTR were very useful diagnostic tools to follow the course of the VSD size. The patients with Pp/Ps 0.5 to 0.8 should be observed for an interval of 10 to 12 months, expecting spontaneous decrease in VSD size. If clinical parameters (ECG, CTR) fail to improve in that interval, then the patients should have surgery. The patients with Pp/Ps greater than or equal to 0.8 should be operated on without delay.

The deleterious effects of surgically induced complete right bundle branch block on long-term follow-up results of closure of ventricular septal defect.

The effects of complete right bundle branch block (CRBBB) on the early postoperative course and long-term follow-up results of closure of ventricle septal defect (VSD) were investigated. (1) Postoperative change in cardiothoracic ratio (CTR); In patients whose ratio of total pulmonary resistance to total systemic resistance (Rp/Rs) was larger than or equal to 0.33, CTR of the group with CRBBB did not decrease significantly (p greater than 0.1) but CTR of the group without CRBBB was improved markedly (p less than 0.01). However, in patients whose Rp/Rs was less than 0.33, there was no significant difference in CTR reduction between the two groups. (2) Postoperative change in Rp/Rs in patients with preoperative Rp/Rs larger than or equal to 0.33: In the group with CRBBB the Rp/Rs decreased from a preoperative average of 36 to 27 percent; in the group without CRBBB, it decreased from 39 to 21 percent. There were significant differences in both the postoperative value of RP/Rs and the degree of postoperative decrease in Rp/Rs between the two groups (both p less than 0.05). (3) Change of pulse rate in the early postoperative period: In patients with Rp/Rs larger than or equal to 0.33, the postoperative increase in pulse rate of the group with CRBBB was significantly higher than that of the group without CRBBB (p less than 0.05). These results suggest that CRBBB has a strong deleterious effect on right ventricular function. The possibility that CRBBB has an ill effect on left ventricular function in patients with Rp/Rs larger than or equal to 0.33 is discussed.

A comparative study on the humoral immune responses in germ-free and conventional mice.

The plaque-forming cell (PFC) responses to sheep red blood cell (SRBC) dinitrophenyl-lysine-Ficoll (DNP-lys-Ficoll), and dinitrophenylated bovine serum albumin (DNP-BSA) have been studied in both germ-free and conventionally reared ICR mice. In germ-free mice, the IgG response to SRBC and the IgM and IgG responses to DNP-BSA were lower than in conventional mice, but no difference was observed in the IgM response to SRBC or the IgM and IgG responses to DNP-lys-Ficoll. Further, the number of 0-bearing cells in the spleen was smaller, and the mitogenic response of spleen cells to PHA was lower in germ-free mice than in conventional mice. These observations suggest that T cells of germ-free mice remain functionally immature.
