Cardiodynamic effects of experimental right bundle branch block in canine hearts with normal and hypertrophied right ventricles.

Cardiodynamic effects of acute experimental right bundle branch block (RBBB) were studied in canine hearts: group A included 15 normal hearts; group B-1 had seven hearts with mild right ventricular hypertrophy (RVH), and group B-2 had 11 hearts with marked RVH. The main sequential changes following RBBB were marked prolongation of the Q upstroke interval of the right ventricle and striking shortening of right ventricular systolic time that affected right and left ventricular interaction, particularly in the hearts with RVH. Hemodynamic changes were: the right ventricular end-diastolic pressure was elevated markedly (4.4 +/- 2.2----9.8 +/- 2.6 mm Hg, p less than 0.001) in group B-2, moderately (p less than 0.01) in group B-1, and not at all in group A. The right ventricular positive peak dp/dt decreased remarkably (1036 +/- 151----827 +/- 152 mm Hg/sec, p less than 0.001) in group B-2 and negligibly in the other groups. A significant correlation existed between the percentage of decrease in right ventricular peak dp/dt and the QRS duration of RBBB in each group (p less than 0.01). The left ventricular peak negative dp/dt decreased distinctly (2570 +/- 326----+/- 2055 +/- 362 mm Hg/sec, p less than 0.01) in group B-2 and not at all in the other groups. The stroke volume showed 12% decrease in group B-2 (p less than 0.001), 8% decrease in group B-1 (NS), and no decrease in group A. In the presence of RVH, acute RBBB causes significant impairment of right and left ventricular function. The magnitude of the impairment invariably depends upon both the prior degree of RVH and the width of the QRS complex.

[Concentration of cefotiam in the human prostatic tissue and bladder wall].

Cefotiam (CTM) was administered in 31 patients before surgery and the concentration of CTM in serum, extirpated prostatic tissue and bladder wall was measured. Additional studies on pharmacokinetic parameters were carried out. The maximum level of serum concentration, following 1 hour intravenous infusion of 1 g, was 54.0 micrograms/ml after 1 hour and T1/2 was 1.16 hours. The maximum level of prostatic tissue was 19.5 micrograms/g and of bladder wall was 29.9 micrograms/g after 1.2 hours and T1/2 was 1.14 hours for both. An appropriate administration method for single use of CTM in the patient of UTI was discussed and 1 hour infusion of 1 g CTM was suggested.

Kay's mitral valve repair for mitral regurgitation due to ruptured chordae tendineae--clinical and hemodynamic observations.

Kay's mitral valve repair was performed in six consecutive patients with symptomatic mitral regurgitation due to ruptured chordae tendineae of the posterior leaflet. All patients including one with mild residual murmur showed a marked decrease in the heart size and significant clinical improvement. Postoperative hemodynamic studies, performed in four patients, showed restoration of normal or near-normal dynamics. The medium-term follow-up, 15 to 30 months after the operation, revealed continuing asymptomatic states in all. The function of the repaired valves was significantly better than that of the Hancock mitral bioprostheses. The advantages of this procedure compared with mitral valve replacement for the same condition were stressed.

Dose-dependent induction of immunologic enhancement and suppression after oral administration of antigen.

The effects of feeding various quantities of a particulate antigen, sheep red blood cells (SRBC), on plaque-forming cells (PFC) in the spleen were determined. Mice were given various numbers of SRBC orally daily for 14 days, then injected with SRBC intravenously. Splenic IgA PFC responses to SRBC were enhanced in the mice fed 5 X 10(8) SRBC and splenic IgG PFC responses to SRBC were depressed in the mice fed 5 X 10(9) SRBC. Adoptive transfer experiments showed that enhancement of splenic IgA PFC responses and suppression of splenic IgG PFC responses were induced by the T-cell rich fraction from Peyer's patches (PP) and the spleen in 5 X 10(8) SRBC- and 5 X 10(9) SRBC-fed mice, respectively. Kinetic studies revealed that IgA helper cells or IgG suppressor cells appeared in PP 2 days after oral administration and 4 days after it in the spleen.

A case of idiopathic hypertrophic obstructive cardiomyopathy causing severe right ventricular outflow tract obstruction in infancy.

An infant with idiopathic hypertrophic obstructive cardiomyopathy, presenting as severe right ventricular outflow obstruction, is described. The nonspecific clinical features and an unexpected angiocardiographic appearance made determining the correct diagnosis difficult. This infant was mistakenly diagnosed as having severe pulmonary stenosis. Attempted surgical correction was unsuccessful and at necropsy this diagnosis was confirmed. It is suggested that cross-sectional echocardiography may be diagnostic for infants with idiopathic hypertrophic obstructive cardiomyopathy.

Coenzyme Q10: the prophylactic effect on low cardiac output following cardiac valve replacement.

A randomized, prospective study of the effectiveness of preoperative administration of coenzyme Q10 on the prophylaxis of postoperative low cardiac output state was performed in 50 patients with acquired valvular diseases necessitating valve replacement. There were 25 patients in the treatment group and 25 in the control group. Patients in the treatment group received 30 to 60 mg of coenzyme Q10 orally for six days before operation. Preoperative clinical variables, operative procedures, total cardiopulmonary bypass time, and aortic cross-clamping time were similar for the two groups. Postoperatively, mild to severe low cardiac output state developed in 28 of 50 patients (56%) and necessitated the administration of considerable amounts of inotropic agent. The treatment group showed a significantly lower incidence of low cardiac output state during the recovery period than the control group (p less than 0.05). These results suggest that preoperative administration of coenzyme Q10 will increase the tolerance of human hearts to ischemia during aortic cross-clamping.

Effect of traditional herbal medicine on serum testosterone levels and its induction of regular ovulation in hyperandrogenic and oligomenorrheic women (author's transl).

Eight infertile hyperandrogenic and oligomenorrheic women were investigated for lowering serum testosterone levels and inducing regular ovulation by Shakuyaku-Kanzo-To (mixed extracts of Paeonae Radix and Glycyrrhizae Radix). Serum testosterone levels (50-160 ng/dl) were lowered to less than 50 ng/dl in 7 patients by Shakuyaku-Kanzo-To 5-10 g daily administered for 2-8 weeks. Six of 7 patients ovulated regularly and 2 of 6 patients conceived. Serum LH levels, which were shown to be above 30 mIU/ml in 7 patients were lowered to less than 30 mIU/ml in 5 patients by dexamethasone 1 mg daily administered for 6.5 days. All of these patients ovulated regularly by Shakuyaku-Kanzo-To. No side effects were observed. Thus, Shakuyaku-Kanzo-To is indicated for lowering serum testosterone levels and inducing regular ovulation and pregnancy in infertile hyperandrogenic patients, especially in the patients whose serum LH levels were shown to be lowered to less than 30 mIU/ml by dexamethasone.
