[Typing of enteric adenoviruses in feces of infants with diarrhea].

OBJECTIVE: To study the infection condition of adenovirus types in infants with diarrhea in Shenzhen. METHODS: 114 fecal samples from diarrhea infants were tested by PCR. The amplified products were analyzed by Taq I and Rsa I endonucleases. RESULTS: The positive rate of adenovirus DNA in fecal samples was 14% (16/114). The amplified products typing procedure started with Taq I endonuclease to identify the type of adenovirus, 11 cleaved by Taq I yielded two fragments of 191 bp and 110 bp, the positive rate of enteric adenovirus was 9.65% (11/114). Followed by Rsa I cleavage, two amplified products yielded 256 bp and 45 bp fragments (Ad40), the positive rate of Ad40 was 1. 75% (2/114), nine amplified products yielded 211 bp and 90 bp fragments (Ad4l), the positive rate of Ad41 was 7.89%(9/114). CONCLUSIONS: The positive rates of the adenovirus types 40 and 41 were 9.65% in feces of infants with diarrhea, and that of the other types of adenovirus was 4.35%.

[Viral etiology study of acute conjunctivitis].

OBJECTIVE: To study the viral etiology of acute conjunctivitis broke out in Shenzhen in 1997. METHODS: Isolated the virus from eye and pharynx swabs of acute conjunctivitis in acute stage, and detected virus nucleic acid by polymerase chain reaction (PCR). DNA of viral strains were subjected to restriction enzyme analysis using several endonucleases. Also, neutralizing antibody of paired sera were detected. RESULTS: The positive virus isolation rates of 14 pharynx and eye swabs were 71.43% and 64.29%, respectively; and that of the virus DNA by PCR were 78.57% and 71.43%, respectively. All of the strains isolated were adenovirus type 3. Three paired viral strains chosen and subjected to restriction enzyme analysis proved belonging to Ad3a2 gene type. The neutralization test of paired sera of 24 cases showed that the antibody titers had 4 fold rising or above. Neutralization test of isolated viruses with paired sera of itself was carried out, the level of neutralizing antibody in convalescent stage was above 4 times as compared with that of acute stage. CONCLUSIONS: The results showed that the outbreak of acute conjunctivitis occurred in 1997 in Shenzhen was caused by adenovirus type 3.

[The molecular epidemiology of transfusion transmitted virus infection in Shanxi Province].

OBJECTIVE: To study the molecular epidemiology of transfusion transmitted virus (TTV) infection and its distribution in different populations in Shanxi Province and the gene types of the isolates. METHODS: TTV DNA in serum samples was detected by polymerase chain reaction. Partial gene of the strains from different populations were cloned and sequenced, and their gene types were analyzed. RESULTS: Of 24 cases with chronic hepatitis B, 31 cases with non A-G hepatitis, 53 cases of professional blood donors, 112 cases of healthy lying -in women, the positive rates of TTV DNA were 41.66%, 29.03%, 58.49% and 8.93%, respectively. TTV DNA in serum samples of 18 cases of 2 - 3 years old healthy children were negative and also were negative in umbilical cord blood of 10 cases of TTV DNA positive healthy lying-in woman. Sequence analysis of 4 isolated strains of Shanxi Province showed that the nucleotide identity among them were more than 90% and they all belonged to the same gene type (la) CONCLUSIONS: This study proved that TTV infection existed in Shanxi Province, TTV might be an important pathogen for non-A to G hepatitis. TTV often coinfected with hepatitis B virus and there were high rates of TTV infection in professional blood donors and in patients with chronic liver disease. Successful treatment of invasive burn wound infection with sepsis in patients with major burns.

OBJECTIVE: To investigate the clinical characteristics of invasive burn wound infection with sepsis in patients with major burns and to summarize the successful experiences in the treatment of such patients. METHODS: Eight patients with major burns, complicated by invasive burn would infection and sepsis were consecutively admitted to our hospital from September 1997 to October 1998. Among them, 6 patients developed multiple organ dysfunction syndrome (MODS) and 2 developed septic shock. The plasma concentrations of IL-6, IL-8, TNF alpha and lypopolysaccharide (LPS) were assayed before and after surgical intervention, as well as when the patient's vital signs became stable. RESULTS: The patients' conditions usually deteriorated abruptly when extensive invasive burn wound infection emerged. While multi-microbial infection was usually found, Pseudomonas aeruginosa was the predominant bacteria isolated from the subeschar tissue. The plasma concentrations of IL-6, IL-8, TNF alpha and LPS before surgical intervention were significantly higher than those after surgical intervention (P < 0.05). The lowest levels of the inflammatory mediators were observed when the patients' conditions became stable, and the values were significantly lower than those before surgical intervention (P < 0.001). CONCLUSION: Since the main cause of burn wound sepsis is the presence of a large area of infected burn wound, they should be excised and covered as early as possible. LPS and pro-inflammatory mediators play an important role in the pathogenesis of burn sepsis. Although favorable results should be attributed to comprehensive treatment, we believe that early, aggressive and thorough surgical excision of infected burn wounds, followed by sound and complete coverage of the area, play a crucial role.

[Establishment of MRP-overexpression subline of bladder carcinoma and its MDR phenotype].

OBJECTIVE: To investigate MRP expression and multidrug resistance(MDR) phenotype of a bladder carcinoma subline transfected with the full length MRP cDNA. METHODS: After transfection, a stable MRP-overexpressed subline named EJ/MRP was established. Gene expression of MRP and mdr1 were detected by using RT-PCR and immunohistochemistry methods. Drug sensitivity testing of the EJ/MRP cells to 11 kinds of anti-cancer agents was examined. RESULTS: Compared with EJ/Vect which was mock transfected, MRP mRNA level of EJ/MRP increased 14.3 fold and MRP expression was mainly located in cytosol and plasma membrane. The relative resistance (RR) to VP-16, vincristine increased more than 10 fold, and that to doxorubicin, hydroxycamptothecin, thiotepa, mitomycin and cyclophosphamide increased 3 to 10 fold. CONCLUSION: Bladder carcinoma with stably over-expressed MRP presents typical MDR phenotype but without mdr1/P-gp expression. It provides a good model and positive control for the study of MRP-mediated MDR.

[Noxious stimulation-induced morphological changes of nitric oxide synthase-positive neurons in the tongue of rats].

OBJECTIVE: To explore noxious stimulation-induced morphological changes of nitric oxide synthase (NOS) positive neurons in the tongue. METHODS: The histochemical method of reduced nicotinamide adenine dinucleotide phosphate(NADPH)-diaphorase activity was used. RESULTS: NOS positive neuron bodies in the tongue were obviously enlarged, the staining intensity increased and nerve fibers became thicker after injury. At the body and tip of the tongue, near the body cells of NOS positive neurons deeply stained fine granular particles were seen, and structures of plum blossom form were seen along the route of the nerve fiber extending all the way to the terminal. NOS positive nerve fibers in the striated muscles and around the gland in the tongue root became thicker, and bead structures were seen in some of the fibers. CONCLUSION: NOS positive neurons of the tongue, when noxiously stimulated, are inflicted with pathological changes, which suggest that nitric oxide might play a role in the transmission and control of the message related to pathological lesions.

[The role of MHC molecules on OSCC cells in the induction of autologous cytotoxic T lymphocytes].

OBJECTIVE: To investigate the role of major histocompatibility complex(MHC) molecules in the induction of cytotoxic T lymphocyte(CTL) from oral squamous cell carcinoma. METHODS: 1. The treated autologous tumor cells served as stimulating cells, on which the MHC mAb was used to block the HLA-ABC and/or HLA-DR antigens. The specific cytotoxicity of CTL induced by stimulating cells and the level of released TNF-alpha were then detected. 2. By blocking the HLA-ABC and/or HLA-DR antigens on target cells, the inhibition effect of MHC mAb on CTL cytotoxicity induced by autologous stimulating cells (not treated with mAb) was measured. RESULTS: 1. The cytotoxicity induced by stimulating cells in experimental groups was significantly lower than that of the control group. The successive order was: the DR antigen positive stimulating cell group > ABC antigen positive stimulating cell group > both ABC and DR antigen negative stimulating cell group. The specific TNF-alpha released by CTL was completely blocked by anti-ABC mAb. 2. The average inhibition rate by mAb on cytotoxicity of CTL was: 52%, 27% and 72% by anti-ABC mAb, anti-DR mAb, and anti-ABC mAb plus anti-DR mAb respectively. CONCLUSION: Both MHC-class I and class II antigens on OSCC cells play an important role in inducing CTL.

Effect of extensive excision of burn wound with invasive infection on hypermetabolism in burn patients with sepsis.

OBJECTIVE: To evaluate the effect of extensive excision of invasive burn wound infection on hypermetabolic response in burn patients with sepsis. METHODS: Eight patients with major burn, complicated by invasive burn wound infection and sepsis, were consecutively admitted to our hospital from September 1997 to October 1998. Resting energy expenditures (REEs) were monitored by means of cardiorespiratory diagnostic system (Medical Graphics Corporation, USA) at patients' bedside. Plasma concentration of IL-6, IL-8, TNF-alpha, and LPS were assayed before and after surgical intervention and at the time when the patients' vital signs became stable. Correlation analysis between REEs and IL-6, IL-8, TNF-alpha, and LPS were made, respectively. RESULTS: A total of 8 treated patients survived. Values of REE before surgical intervention were significantly higher than those after surgical intervention (P < 0.01), and when patients' vital signs became stable the values were significantly lower than those after surgical intervention (P < 0.01). The plasma concentrations of IL-6, IL-8, TNF-alpha, and LPS after excision of invasive burn wound infection were significantly lower than those before surgical intervention (P < 0.05). The lowest levels of these inflammatory mediators were observed when the conditions of patients became stable, and the values were significantly lower than those before surgical intervention (P < 0.001). There was a significant positive correlation between REE level and respective values of plasma IL-6, IL-8, TNF-alpha, and LPS (P < 0.01). CONCLUSIONS: It seemed that the extensive excision of invasively infected burn wound in patients with major burn should be performed as early as possible to reduce an increased release of inflammatory mediators and to control the hypermetabolic response during sepsis.

[Prevention and treatment of MODS in severely burned patients: an institute based retrospective study].

OBJECTIVE: To determine if the preventive and treatment measures developed from basic and clinical research work have any effects on the incidence and outcome of MODS in severely burned patients. METHODS: 657 extensively burned patients (burn size exceeding 30% TBSA) admitted to the clinical department from January 1970 through October 1999 were studied. The morbidity and mortality rates were compared in patient groups hospitalized before and after January 1990 when major preventive and treatment measures began to be extensively executed. These measures consisted of prompt, rapid and adequate fluid resuscitation, restoration of bowel blood supply with anisodamine (654 - 2), use of oxygen free radical scavengers (e.g. mannitol, vitamin C and E), extensive escharectomy during shock stage, early tube feeding, support of organ function, etc. RESULTS: Before 1990 when very few preventive and treatment measures were applied, the incidence of MODS, 17.3% (64 out of 369), and the death rate 87.5% (56 out of 64). After 1990, with the application of various preventive measures and treatment, the morbidity and mortality rates were reduced to 6.9% (20/288) and 40.0% (8/20), respectively. CONCLUSION: Our results suggest that measures which mitigate or eliminate the predisposing and causative factors of MODS may be salutary in the prevention and treatment of MODS after extensive burn injury.

[Anisodamine in prevention and treatment of sepsis of severely burned patients].

OBJECTIVE: To observe the preventive effect of anisodamine on possible sepsis of patients with major burns and the effect of anisodamine on patients with sepsis. METHODS: Forty-two patients with extensive burn admitted to our burn institute from April 1998 to November 1999 were divided randomly into two groups: treatment group (T group) and control group (C group). In the T group, all 20 patients received fluid resuscitation regimen with anisodamine, and in the C group, 22 patients received the regimen with no anisodamine. A tonometry catheter was positioned in the stomach, connecting with the automatic gas analysis machine (Datex-Engstrom Corporation, Dutch) for determining gastric intramucosal pH (pHi). The plasma concentrations of diamine oxidase (DAO) and endotoxin were measured. Correlation analysis between pHi, DAO and endotoxin were made respectively during early stage of postburn. All the parameters in 7 patients with sepsis before and after administration of anisodamine were compared with those in 6 patients with sepsis without use of anisodamine. RESULTS: The incidence of sepsis in the T group was lower (20.0%) than that in C group (40.9%). The gastric pHi value in the early period of postburn was significantly higher in the T group than in the C group (P < 0.05). Concurrently, the plasma concentrations of DAO and endotoxin were significantly lower in the T group than in the C group (P < 0.05 or 0.01). A significant negative correlation was seen between the gastric pHi and respective values of DAO, endotoxin (P < 0.05 or 0.01). There were a decrease in gastric pHi, and an increase in plasma DAO and endotoxin level in patients with septic episode; however all the parameters after administration of anisodamine were improved compared with those in septic patients without use of anisodamine. CONCLUSIONS: Intestinal ischemic injury plays an important role in provoking sepsis during early postburn period. Anisodamine is effective in restoring intestinal circulation both in the shock phase and after the development of sepsis.

[Clinical use of acellular allogenic dermis or acellular porcine dermis with split-thickness autologous skin graft in 119 cases].

OBJECTIVE: To observe the effect of acellular allogenic dermis or acellular porcine dermis together with split-thickness autologous skin graft on coverage of deep burn wound and the wound of scar excision. METHODS: Acellular allogenic dermis or acellular porcine dermis produced by our unit, with split-thickness autologous skin graft, were used in repairing various wounds in 119 cases. The take rate of various wounds was compared, and the take rate of wounds in which autologous skin from different sites was used to cover either acellular allogenic dermis or acellular porcine dermis was also compared. Histological examination and follow-up were made in some cases. RESULTS: After tangential excision, eschar excision and scar excision, the wounds were covered with either allogenic acellular dermis or porcine acellular dermis and autologous split-thickness skin. The take rate was found to be (93.4 +/- 3.4)%, (92.1 +/- 4.6)%, (94.5 +/- 3.5)%, respectively. There was no a significant difference in take rate (P > 0.05). No significant difference in take rate between the transplantation of allogenic acellular dermis with autologous split-thickness skin and transplantation of porcine acellular dermis with autologous split-thickness skin was found (P > 0.05). When autologous split-thickness skin harvested from the trunk or extremities was used, the take rate was (93.1 +/- 4.8)%, (89.0 +/- 6.2)%, respectively, whereas the take rate was only about 70% - 80% when skin harvested from the scalp was used. The take rate was only about 40% when acellular allogenic dermis and autologous micro-skin graft were used to cover the wound. Histological examination 19 months after the transplantation revealed normal structure. Collagenous fibers were orderly arranged. There was no apparent hyperplasia of collagenous fibers. Hair follicles, sweat glands and other skin appendages were not found in the healed area. Grossly, no obvious pigmentation was seen, the surface was smooth with slight wound contraction, and the consistency was flexible. CONCLUSION: Acellular allogenic split-thickness dermis or acellular porcine split-thickness dermis with autologous split-thickness dermis for coverage of deep burn wound and the wound of scar excision is an ideal material.

[The study of RDS gene mutation and clinical phenotype in a family with primary retinitis pigmentosa].

OBJECTIVE: To investigate retinal degeneration slow (RDS) gene mutation in a Chinese family with primary retinitis pigmentosa (RP) and the association of the mutation with clinical phenotypes and to explore the pathogenesis of RP. METHODS: Blood DNA from 2 patients in the same family with RP and 2 normal persons was analyzed by molecular genetic methods. RDS gene mutation was screened out by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analysis. The mutant RDS gene fragment was cloned, then sequenced with an automatic DNA sequencer using a dideoxy chain termination protocol. The phenotype of the patients with the gene mutation were examined and determined by clinical ophthalmologic examinations. RESULTS: The PCR-RFLP analysis of the RDS gene in 2 patients with RP revealed codon 216 mutation of RDS gene. The mutation was heterozygous, and not found in 2 normal persons as controls. The alteration in the DNA sequence was identified as a heterozygous transversional change of C to T at the second nucleotide in codon 216 of RDS gene, resulting in the amino acid replacement of proline residue with leucine residue (Pro216Leu). The ocular finding of the patients with Pro216Leu mutation of RDS gene included severe visual loss and diffuse distribution of pigmentary changes with macular degeneration. CONCLUSIONS: The Pro216Leu mutation of RDS gene is found in Chinese patients with RP. The gene mutation is associated with the ocular phenotype, diffuse RP with macular degeneration.

[Digital photography and blurred image processing of retinal nerve fiber layer in glaucoma].

OBJECTIVE: Red-free photographs are effective in detecting retinal nerve fiber layer (RNFL). The reasons why it is not used by most clinics are difficulty in laborious developing and blurred image evaluating. To account for getting real-time image and blurred image evaluating, the digital photography and computerized image processing are used. METHODS: RNFL photographs were taken by Nikon Fundus Camera connected with a high resolution digital camera (Kodak) and a green filter. As usual it is necessary to see the texture of RNFL in evaluating its image, but blurred images due to media opacities greatly lose their resolution. The way we used no focus on seeing the texture of RNFL, but on evaluating the distribution pattern of RNFL. RNFL images have more white reflex in temporal superior, temporal inferior, nasal superior and nasal inferior sections around optic disc according to the thickness of RNFL. Threshold segmentation was used, we shift a threshold bar to the position until the nasal or temporal section around optic disc is black with the software Photoshop. RESULTS: In the normal RNFL image, the white reflex was of butterfly pattern, i.e., the temporal superior and temporal inferior sections around optic disc were white. In cases with glaucoma, the characteristic of early damage of optic nerve was the loss of RNFL at the temporal superior and temporal inferior part of the retina, thus the normal butterfly pattern was changed, the temporal superior or/and temporal inferior sections were black. With this method it is more effective to evaluate RNFL. However, in cases with retinal pigment epithelium faint in color and in cases with choroidal defect, it is still difficult to evaluate the distribution of RNFL. CONCLUSIONS: It is an easy and effective way to detect glaucomatous damage with the digital photography and RNFL pattern evaluation.

[The measurement of normal values of exophthalmos, interpupillary distance and interorbital distance of children and adolescence in Xiamen and the rule of their development].

OBJECTIVE: To investigate the developing rhythm of children and adolescent exophthalmos, interorbital distance (IOD) and interpupillary distance (IPD) and their mutual relationships. The study will provide biometry data for formulating juvenile criteria of correcting spectacles. METHODS: Random sampling was performed among children and adolescence aged 5 - 17 years in Xiamen. Adopting epidemic survey, we measured the biometry values of IOD, IPD and exophthalmos with a sliding gauge and exophthalmometer. RESULTS: The average value of exophthalmos was (14.48 +/- 1.71) mm. For the 5 - 8 years old children, the value of exophthalmos trended to increase with the increase of age, meanwhile the value of exophthalmos of adolescence aged 9 - 17 years old tended to be stabilized. The average value of IOD was (95.55 +/- 5.32) mm and the average value of IPD was (56.99 +/- 3.93) mm. Both the values of the IOD and IPD of the male were higher than that of the female. The results of the IOD and IDP distance increased with the increase of age and it trended to be periodic. There was positive correlation between the development of IOD and that of IPD, and it could be described by a linear equation. CONCLUSIONS: There is only one rush-increase stage of ocular development which is before the 8 years old in children. The rule of its development is in accordance with the growing and developing mode of neurological system.

[Homogeneity study on the Streptococcus suis isolated from human and swine].

OBJECTIVE: To identify S. suis and to evaluate the homogeneity of isolates of S. suis from human and swine. METHODS: Culture, morphology, API biochemical tests and serum coagulate tests were used. All the 7 strains of streptococcus were confirmed to be S. suis serotype 2. Two strains and 1 strain were isolated from blood and cerebrospinal fluid (CSF) of patients, respectively, while the other 4 strains were obtained from the infected swine or their corpses. Seven strains of S. suis serotype 2 and reference strain SS2 were analyzed by randomly amplified polymorphic DNA analysis with six primers and Rep PCR with four primers and thalli fatty acid profile analysis. Cluster and principal compound analysis of results were performed with RAPD, Phylip and Treeview software. RESULTS: By analysis on RAPD patterns, a close relationship among 7 strains of S. suis serotype 2 and reference strain SS2 was discovered. Human-born strains and swine-born strains exhibit similar RAPD patterns. There was a same clonal relationship between strains originated from the blood and CFS of the patients. These results were confirmed by thalli fatty acid profile to have genotypic and phenotypic identity. CONCLUSION: Homogeneity exists among the 7 strains and reference strain SS2.

[Studies on human streptococcal infectious syndrome caused by infected pigs].

OBJECTIVES: To demonstrate the clinical manifestations of S. suis infectious syndrome; to study the characteristics of causative organism and its source. METHODS: 25 cases of unknown causes of food poisoning were identified in the central area of Jiangsu province, China in July 1998. Biological specimens was collected in some patients, and the causative organism was isolated using a number of different culture medium. Isolated organisms were identified by serum antibody tests, API biochemical tests, drug sensitivity tests and animal model experiments. The human born strains and the pig born strains were compared by RAPD technique. Epidemiological methods were applied to trace the source of causative organisms. RESULTS: Cases were clinically categorized into two types: Streptococcal toxic shock syndrome (STSS) and streptococcal meningitis syndrome (SMS). Three strains of streptococci were isolated respectively from the blood and cerebrospinal fluid of 6 cases. All these 6 strains were confirmed to be S. suis type 2 by culture characteristics, morphology, biochemical characteristics, serum antibody tests and fatty acid profile analysis. Animal model experiment showed that these strains were sensitive to rabbits and pigs, but not to rats. RAPD fingerprint test revealed an identity between human born and pig born strains. CONCLUSIONS: The aetiological agent of these human cases was S. suis type 2. Human cases were contracted by direct contact with the infected pigs or the corpse of infected pigs.

[Seven years' summary report on the treatment of acute promyelocytic leukemia with arsenic trioxide--an analysis of 242 cases].

OBJECTIVE: To observe the effectiveness of arsenic trioxide (As(2)O(3)) in the treatment of acute promyelocytic leukemia (APL). METHODS: Data of clinical observation, seven year follow-up survey were analysed with American SAS system software, concentration of arsenic in cerebrospinal fluid (CSF) was determined by atomic absorbing spectrum. RESULTS: The complete remission (CR) and effectiveness rates were 87.9% and 94.4% for the previously untreated group; 60.0% and 65.0% for the relapsed in As(2)O(3) maintenance treatment group; 69.5% and 84.8% for the relapsed in other than As(2)O(3) maintenance treatment group; and 48.7% and 94.1% for the refractory group, respectively. In the previously untreated group, the relapse rate at 3 years after CR was 26.7%. The probability of survival in 136 follow-up survey patients was 92.0% and 76.7% at 5 years and 7 years, respectively. There were mild side effects. Central nervous system leukemia (CNSL) occurred in 138 out of 412 cases in 6 years, including some refractory leukemia, extramedullary residual leukemia and extramedullary relapse. Prophylactic intrathecal injection could reduce the incidence rate of CNSL. Determination of arsenic concentration indicated that it was difficult for arsenic to penetrate the blood-brain barrier. CONCLUSION: The CR rate and long-term survival are high and the relapse rate is low in APL patients treated with As(2)O(3). There is no cross-resistance between ATRA and As(2)O(3) and other chemotherapeutic agents. As(2)O(3)-resistance occurs scarcely. It can also be used in maintenance treatment.
