[Clinical use of central electronic monitoring system].

OBJECTIVE: To evaluate the effects of central electronic monitoring system (CEMS). METHODS: A total of 1,216 patients with > or = 37 weeks of gestation assigned as monitoring group, were performed central electronic monitoring during labor from Nov. 1997 to Mar. 1998. A total of 1,137 patients with same gestational age assigned as control group, were monitored by using intermittent auscultation during labor from Nov. 1996 to Mar. 1997. The rate of fetal distress, neonatal asphyxia, cesarean section, and using of forceps or vacuum extractor in the 2 groups were compared. RESULTS: Patients in the monitoring group had a higher fetal distress rate (12.8%) than that of the control group (9.8%, P < 0.05), but a significantly lower neonatal asphyxia rate (2.3%) than that of the control group (4.8%, P < 0.01). There were no significant differences in the overall cesarean rate, cesarean rate for fetal distress, overall use rate of forceps or vacuum extractor delivery, and vaginal operative delivery rate for suspected fetal distress between the two groups (P > 0.05). CONCLUSION: Quality for obstetric care was significantly improved by application of central electronic monitoring for labor management, it didn't increase the cesarean section rate and vaginal operative delivery rate, but decreased the neonatal asphyxia rate.

[Relationship between new TNM classification and the prognosis and biological behavior of gastric cancer].

OBJECTIVE: To determine the clinical value of 1997 TNM classification of gastric cancer. METHODS: 334 cases of gastric cancer with regional lymph node metastasis were used to determine the relationship between new TNM classification and the prognosis and biological behavior compared with old classification (1987). RESULTS: The 5-year survival rates of the old pN(1), pN(2) were 49.6% and 33.5% respectively; those of the new pN(1), pN(2), pN(3) were 48.5%, 24.1% and 5.6% respectively. There was a highly significant difference in survival in different new pN groups (P < 0.01). The new pN classification was correlated with the depth of cancer invasion, as well as the macroscopic type, growth pattern and serosal type (P < 0.01). Multivariate COX model analysis showed that the new pN classification was the most significant independent prognostic factor. CONCLUSIONS: The new TNM classification is not only an objective, simple and reproducible system, but also a significant prognostic index for gastric cancer superior to the old classification.

[Clinical and pathological characteristics of gastric stump cancer and recurrent gastric stump cancer].

OBJECTIVE: To study early diagnosis and rational treatment of gastric stump cancer and recurrent gastric stump cancer. METHODS: The data of 27 patients with gastric stump cancer and 23 patients with recurrent gastric stump cancer were analysed retrospectively during the past 20 years based on their clinical and pathological findings. RESULTS: The onset time was 10 years or more for gastric stump cancer and less than 10 years for recurrent gastric stump cancer after gastrectomy. Gastroscopy was better than double contrast radiography in diagnosis. Partial resection of gastric stump was performed for patients with early cancer at the anastomotic stoma of gastrojejunostomy. Total resection of gastric stump was made for other patients. Lymph node of the mesojejunum were dissected routinely because their metastasis rate was about 40.0% in patients with cancer of anastomotic stoma. The effect of surgical treatment was poor for patients with advanced cancer. CONCLUSIONS: Regular gastroscopy is useful in detecting gastric stump cancer in patients with gastric cancer after gastrectomy, or in those years after removal benign. Malignancy was more evident in recurrent gastric stump cancer than in primary gastric cancer. Prognosis is good for early stage cases and poor for late stage cases after surgical treatment.

[Efficacy and immune memory of plasma-derived hepatitis B vaccine 11 years after primary immunization].

OBJECTIVE: To evaluate the long-term efficacy of hepatitis B vaccine 10 years after primary immunization to provide scientific basis for the time of revaccination. METHODS: The study was strictly designed with randomization, double-blinding, and placebo-controlled method to observe the efficacy and immune memory 11 years following hepatitis B vaccination. RESULTS: Immunogenicity and protective rate of vaccine were still kept well 11 years after immunization with a protective rate against HBV infection of 73.5%. But, there was no significant difference in HBV infection rates between vaccine group and placebo-controlled group (7.89% vs. 13.25%, P > 0.1) nine to 11 years following immunization. There still existed immune memory 11 years after immunization, but it was significantly weaker than that within the first 10 years after immunization. CONCLUSION: The efficacy of the vaccine had begun to drop 11 years after immunization, which should be followed up further to reach a clear conclusion.

[Long-term expression of MHC-I gene in recipient mice].

OBJECTIVE: To explore the possibility of long-term expression of transferred MHC-I gene in recipient mice. METHODS: MHC-I gene of the donor mouse (C(57)BL/6) was transferred into the recipient (BALB/c) hematopoietic cells by retrovirus-mediated gene transfer technique, and the gene transferred cells were reinfused into the recipient mice. PCR, RT-PCR and FACS were used to assay the long-term expression of MHC-I gene in the recipient mice. RESULTS: The gene transferred mice hematopoietic cells and their G418-resistant CFU-GM were demonstrated to be able to express the MHC-I gene. The cells proliferation and differentiation had no change. The MHC-I gene could be expressed for a longer period in the recipient mice. CONCLUSION: This experiment makes ready for the further research on inducing transplantation tolerance through MHC-I gene transfer.

[Effect of xiongshao capsule on proliferation and apoptosis of vascular smooth muscle cells in rabbits observed by serum pharmacological method].

OBJECTIVE: To observe the effect of Xiongshao Capsule (XSC) on proliferation and apoptosis of aortic smooth muscle cells (SMC) of rabbits by serum pharmacological method. METHODS: SMC were isolated from thoracic aorta of the normal rabbits and passaged. The SMC proliferation model was established by endothelin (ET). The medicated serum was prepared after given the normal rabbits suspension of XSC through gastrogavage for 10 days. The proliferation and apoptosis of SMC were observed by methyl thiazolyl tetrazolium (MTT) colorimetry, flow cytometry and agarose gel electrophoresis. RESULTS: ET obviously promoted proliferation of SMC, and the serum medicated with XSC could lower the proportion of SMC in S + G2 phase and markedly inhibited the proliferation of SMC caused by ET in dose dependent manner. It also had certain effect on inducing apoptosis of SMC. CONCLUSION: XSC could markedly inhibit the proliferation of SMC caused by ET and induce apoptosis of SMC, which might be the mechanism of preventing coronary restenosis after interventional treatment.

[Infection of white spot baculo-like virus(WSBV) in Penaeus chinensis: evidence from electron microscopy and DNA hybridization].

Severe epizootic causing high mortalities in cultivated Penaeus chinensis has occurred every year in mainland China since 1993 with great economic loss. Naturally diseased P. chinensis sampled in Qingdao shrimp farming regions between 1993 and 1998 were examined by electron microscopy. A similar non-occluded nuclear bacilliform virus like the white spot baculo-like virus (WSBV) reported in Taiwan was mostly found in the gill, cuticular epidermis under the exoskeleton, stomach, lymphoid organ and other tissues of ectodermal and mesodermal origin from these diseased samples. The viron was enveloped with a mean size of (125 +/- 7.6) nm x (345 +/- 16) nm. Sometimes, a nipple-like appendage could be found protruding from one extremity of the viron. Envelopes were all lost after purification from cesium chloride gradient ultracentrifugation. The cylindrical nucleocapsid in negatively staining preparations was (80 +/- 13) nm x (380 +/- 24) nm with 13-16 stripes perpendicular to the longitudinal axis of the nucleocapsid. Based on its pathogenic and morphological characteristics, this virus should be related to WSBV. A pair of primers created from information of WSBV genome DNA SalI fragment produced a 355 bp band using the WSBV isolate from P. chinensis in mainland China, as the DNA template. The specific PCR product was cloned, sequenced and labeled with digoxigenin (DIG) DNA labeling kit (Boehringer Mannherm). The sequence of the probe is identical to that of WSBV. All of the sampled diseased shrimps between 1993 and 1998 reacted with the DIG-labeled probe by dot-blot hybridization and no hybridization was observed using DNA from health shrimp as templates. These results based on electron microscopy and DNA hybridization indicated that the shrimp epizootic occurred in mainland China was related to the WSBV infection previously occurred in Taiwan early 1992.

[Analysis of Amomun villosum species and some adulterants of zingiberaceae by RAPD].

Different Amomum villosum Lour. species and some their adulterants of Zingiberaceae were researched using RAPD. The PCR indicates favourable differentiation of the reaction. The Amomum villosum Lour. species have the similar DNA fingerprints while it is obviously differ from adulterants. There are 12.17 percent of primers which appear polymorphism. We can identify different species according to these dissimilarity. We also construct the tree of the molecular evolution through NJ software, the hereditary distance of the graph shows different relationships between Amomum villosum Lour. species and their adulterants. The result created by software is analogous the traditional methods.

[Study on inhibitory actions of san huang yi gan capsule (SHYGC) on HBeAg with seropharmacological method].

In this paper, the inhibitory actions of SHYGC on HBeAg in vitro were studied with the seropharmacological method. By Enzyme Linked Immuno Sorbent Assay (ELISA), it was found that the rabbit sera containing SHYGC have significant inhibitory effects on HBeAg, and they become stronger with the drug concentration in sera improved and the actions time prolonged, they decrease with the HBeAg concentration improved, and the effects of the sera containing hing-dose drugs equal those of Su Xiao Jing containing 250 micrograms/g effective chloric. The direct external inhibitory effects of SHYGC in original pharmaceutics are stronger than those of the sera containing drugs. It probably indicated that the active ingredients of SHYGC could not be digested and absorbed completely from gastrointestine, or were inactived by metabolism in vivo.

[mRNA expression of keratoepithelin in cornea after photorefractive keratectomy in rabbits].

OBJECTIVE: To investigate the roles of keratoepithelin(KE) during healing after photorefractive keratectomy (PRK). METHODS: 20 of 24 rabbits underwent bilateral 193 nm excimer laser photorefractive keratectomy to correct 10 diopters of myopia, and the other 4 rabbits as normal group. The mRNA levels of KE in corneas of the rabbits were determined by the in situ hybridization technique with a Digoxigenin labled probe. RESULTS: In normal group, there was no mRNA expression of KE in cornea. on 7, 14 days after operation, mRNA expression of KE was observed in epithelium and stroma of operation group, while on 28 days mRNA expression of KE was observed only in epithelium. CONCLUSION: KE was involved in the synthesis of extracellular matrix during healing after PRK and may play an important role in the development of haze by maintaining proper spacing among collagen fibrils.

[Two-dimensional surface Raman imaging of a roughened iron electrode in saline solution].

By using a confocal microprobe Raman system and proper surface roughening procedure for iron, the pit corrosion behavior of bare iron electrode in 3.4% NaCl (saline solution) solution has been investigated. The surface-enhanced Raman scattering (SERS) spectra of the corrosion products due to the pit corrosion at positive potential was obtained with high ratio of signal to noise. The application of SERS technique has been extended successfully to the study of iron corrosion. The two-dimensional surface Raman imaging acquired at 660 cm-1 was obtained also with the utility of surface Raman image technique. Results show that different iron oxides should coexist in the pit corrosion region and the distribution of the corrosion products was not uniform.

[Staining of complete denture: a preliminary clinical study]

OBJECTIVE:To study the relation between staining of complete denture and drinking tea, coffee and smoking, and to evaluate the result of denture cleaning agent on removing stain of complete denture.METHODS:A survey of 176 patients with complete dentures were carried out, which included the history of drinking tea, coffee and smoking, the use of denture-cleaning agent and the times of daily use. Statistical analysis was performed to determine the relationship between staining of denture and the above factors. RESULTS:The results showed (1)The longer the complete denture was weared, the more serious the staining. (2)Drinking tea and smoking were the main causes for denture staining. (3)Daily use of cleaning agent can effictively remove stain of denture. CONCLUSION:Change of life habit(drinking less tea and stop of smoking) and daily use of cleaning agent can reduce denture staining.

Tumor necrosis factor-a attenuates N-methyl-D-aspartate-mediated neurotoxicity in neonatal rat hippocampus.

Tumor necrosis factor-a TNFa. has been implicated in the pathophysiology of acute neonatal brain injury. We hypothesized that acute brain injury would induce TNFa expression and that exogenous TNFa would influence the severity of N-methyl-D-aspartate-induced tissue damage. We performed two complementary groups of experiments to evaluate the potential role s. of TNFa in a neonatal rodent model of excitotoxic injury, elicited by intracerebral injection of N-methyl-D-aspartate. We used immunohistochemistry and ELISA to evaluate N-methyl-D-aspartate-induced changes in TNFa expression, and we co-injected TNFa with N-methyl-D-aspartate, to evaluate the effect of this cytokine on the severity of tissue injury. Both intra-hippocampal and intra-striatal injection of N-methyl-D-aspartate 5 nmol. stimulated TNFa expression. Increased TNFa expression was detected 3-12 h after lesioning; TNFa was localized both in glial cells in the corpus callosum, and in cells with the morphology of interneurons in the ipsilateral hippocampus, striatum, cortex and thalamus. Intra-hippocampal or intra-striatal administration of TNFa 50 ng. alone did not elicit neuropathologic damage. In the hippocampus, when co-injected with N-methyl-D-aspartate 5 or 10 nmol., TNFa 50 ng. attenuated excitotoxic injury by 35%-57%, compared to controls co-injected with heat-treated TNFa. In contrast, in the striatum, co-injection of TNFa with N-methyl-D-aspartate had no effect on the severity of the ensuing damage. The data indicate that TNFa is rapidly produced in glial cells and neurons after an excitotoxic insult in the neonatal rat brain, and that administration of exogenous TNFa results in region-specific attenuation of excitotoxic damage. We speculate that endogenous TNFa may modulate the tissue response to excitotoxic injury in the developing brain.

Molecular basis of transient outward K+ current diversity in mouse ventricular myocytes.

1. Two kinetically and pharmacologically distinct transient outward K+ currents, referred to as Ito,f and Ito,s, have been distinguished in mouse left ventricular myocytes. Ito,f is present in all left ventricular apex cells and in most left ventricular septum cells, whereas Ito,s is identified exclusively in left ventricular septum cells. 2. Electrophysiological recordings from ventricular myocytes isolated from animals with a targeted deletion of the Kv1.4 gene (Kv1.4-/- mice) reveal that Ito,s is undetectable in cells isolated from the left ventricular septum (n = 26). Ito,f density in both apex and septum cells, in contrast, is not affected by deletion of Kv1.4. 3. Neither the 4-AP-sensitive, slowly inactivating K+ current, IK,slow, nor the steady-state non-inactivating K+ current, ISS, is affected in Kv1.4-/- mouse left ventricular cells. 4. In myocytes isolated from transgenic mice expressing a dominant negative Kv4.2 alpha subunit, Kv4.2W362F, Ito,f is eliminated in both left ventricular apex and septum cells. In addition, a slowly inactivating transient outward K+ current similar to Ito,s in wild-type septum cells is evident in myocytes isolated from left ventricular apex of Kv4.2W362F-expressing transgenics. The density of Ito,s in septum cells, however, is unaffected by Kv4.2W362F expression. 5. Western blots of fractionated mouse ventricular membrane proteins reveal a significant increase in Kv1.4 protein level in Kv4.2W362F-expressing transgenic mice. The protein levels of other Kv alpha subunits, Kv1.2 and Kv2.1, in contrast, are not affected by the expression of the Kv4.2W362F transgene. 6. The results presented here demonstrate that the molecular correlates of Ito,f and Ito,s in adult mouse ventricle are distinct. Kv1.4 underlies mouse ventricular septum Ito,s, whereas Kv alpha subunits of the Kv4 subfamily underlie mouse ventricular apex and septum Ito, f. The appearance of the slow transient outward K+ current in Kv4. 2W362F-expressing left ventricular apex cells with properties indistinguishable from Ito,s in wild-type cells is accompanied by an increase in Kv1.4 protein expression, suggesting that the upregulation of Kv1.4 underlies the observed electrical remodeling in Kv4.2W362F-expressing transgenics.

Optically pure (-)-4-[(N-allyl-3-methyl-4-piperidinyl)phenyl-amino]-N,N-diethylbenzami de displays selective binding and full agonist activity for the delta opioid receptor.

The optical isomers of 4-[(N-allyl-3-methyl-4-piperidinyl)phenylamino]-N,N-diethylbenzamide+ ++ (3) have been prepared and tested in both binding and functional assays. The data show that (-)-3 is responsible for the delta opioid activity demonstrated by the racemic material. This compound displays a binding affinity of 5.5 nM for the delta opioid receptor as well as a 470-fold delta versus mu selectivity. Importantly, (-)-3 is a full agonist at the delta receptor in comparison with SNC-80 (2). Taken together, the data suggest that (-)-3 behaves more like the prototypical delta agonists, BW373U86 or SNC-80, and less like the peptidomimetic compound SL-3111 (5).

Brain insulin receptors and spatial memory. Correlated changes in gene expression, tyrosine phosphorylation, and signaling molecules in the hippocampus of water maze trained rats.

Evidence accumulated from clinical and basic research has indirectly implicated the insulin receptor (IR) in brain cognitive functions, including learning and memory (Wickelgren, I. (1998) Science 280, 517-519). The present study investigates correlative changes in IR expression, phosphorylation, and associated signaling molecules in the rat hippocampus following water maze training. Although the distribution of IR protein matched that of IR mRNA in most forebrain regions, a dissociation of the IR mRNA and protein expression patterns was found in the cerebellar cortex. After training, IR mRNA in the CA1 and dentate gyrus of the hippocampus was up-regulated, and there was increased accumulation of IR protein in the hippocampal crude synaptic membrane fraction. In the CA1 pyramidal neurons, changes in the distribution pattern of IR in particular cellular compartments, such as the nucleus and dendritic regions, was observed only in trained animals. Although IR showed a low level of in vivo tyrosine phosphorylation, an insulin-stimulated increase of in vitro Tyr phosphorylation of IR was detected in trained animals, suggesting that learning may induce IR functional changes, such as enhanced receptor sensitivity. Furthermore, a training-induced co-immunoprecipitation of IR with Shc-66 was detected, along with changes in in vivo Tyr phosphorylation of Shc and mitogen-activated protein kinase, as well as accumulation of Shc-66, Shc-52, and Grb-2 in hippocampal synaptic membrane fractions following training. These findings suggest that IR may participate in memory processing through activation of its receptor Tyr kinase activity, and they suggest possible engagement of Shc/Grb-2/Ras/mitogen-activated protein kinase cascades.
