[The application of polymerase chain reaction-single strain conformation polymorphism in the pedigree analysis of familial hypercholerolemia patients].

OBJECTIVE: To discuss the value of polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP) in the pedigree analysis of familial hypercholesterolemia(FH) patients. METHODS: For 4 patients with FH detected by PCR-SSCP and DNA sequence analysis (1 homozygote has point mutation in exon 7, 1 heterozygote in exon 14, and 2 heterozygotes in 3' part of exon 4 ), members of each pedigree, altogether 23 individuals, were analysed using PCR-SSCP. RESULTS: Every member of the 4 pedigrees was examined genetically. Besides the 4 probands, another 1 homozygote and 8 heterozygotes were found. CONCLUSION: The PCR-SSCP method can be used in the pedigree analysis of FH probands. The early diagnosis of siblings and relatives can help to provide genetic consultation and direction so as to pay attention to development of hypercholesterolemia.

A clinical study of yi zhi capsules in prevention of vascular dementia.

The raised score in the revised Hasegawa dementia scales (HDS) demonstrated that the effect of Yi Zhi Capsules (YZC) in treating loss of intellectual function after cerebrovascular diseases was significantly better than that of the western drug Piracetam [symbol: see text] (P < 0.01). The morbidity of vascular dementia was much lower in the treatment group than that of the control group (P < 0.05) after a one-year follow-up; and the blood lipid level and some indexes in rheological examinations were also improved significantly (P < 0.05, or < 0.01). The data indicate that YZC is a relatively good preparation for prevention of vascular dementia.

Pain as a mutual experience for patients, nurses and families: a perspective from Shanghai, China.

The author of this article visited China for the purpose of helping the faculty of the School of Nursing learn research skills by participating in research--a kind of learn by doing. Both investigators had conducted quantitative studies in the US--one with children and one with adults--that were adapted for use in China. Seeking to also include a qualitative study, the investigators explored several possible research areas. Because pain is a universal phenomena, it was chosen as the subject for the study using the qualitative methodology reported in the first part of this article.

Interleukin-12 induces gene expression in interleukin-2 stimulated human T lymphocytes.

IL-12 is a critical immunoregulatory cytokine that promotes cell-mediated immune responses by inducing the differentiation of Th1 cells. To better clarify the molecular basis of IL-12 action, we compared the gene expression in human T lymphocytes activated by IL-2 and IL-12. mRNAs from T lymphocytes activated by either IL-2 alone or IL-2 plus IL-12 were transcribed into cDNAs. A differential mRNA display was conducted. As a result, differential display of five cDNA fragments was obtained. Sequence analysis suggests that they had high homology with recorded genes as found by a computer search against GenBank. Two full genes of the five fragments were cloned, which activation-induced C-type lectin and glucose transporter-like protein. Interestingly, these proteins were expressed in the T cells stimulated by IL-2 and IL-12, but not in the T cells stimulated by IL-2 alone. These results suggest that C-type lectin and glucose transporter-like protein may play an important role in the T lymphocyte activation induced by IL-12.

[Relationship between interleukin-2, interleukin-2 receptor and the non-and hypo-responsiveness to hepatitis B vaccine].

OBJECTIVE: To study the relationship between interleukin-2 (IL-2), soluble interleukin-2 receptor (sIL-2R) and the non-and-hypo-responsiveness to hepatitis B vaccine. METHODS: Peripheral blood mononuclear cells (PBMC) isolated from 21 non-and -hypo-responders, 22 hyperresponders and 21 chronic HBsAg carriers were incubated in the presence of HBsAg and phytohaemagglutinin (PHA). The levels of IL-2 and sIL-2R in the supernatants of activated cells were measured by an enzyme-linked immunosorbent assay. RESULTS: The average level of IL-2 in the non-and-hypo-responders was significantly lower than that in the hyperresponders (t=8.80, P<0.001), but was comparable to that in chronic HBsAg carriers (q=0.06, P>0.5). Between the hyperresponders and the non-and-hypo-responders, the average sIL-2R levels showed no noticeable difference. CONCLUSION: The results suggest that low level of IL-2 may be one of the causes and mechanisms of non-and-hypo-responsiveness to hepatitis B vaccine.

Flow injection analysis of fluoride: optimization of experimental conditions and non-linear calibration using artificial neural networks.

This paper deals with the application of artificial neural networks (ANNs) to two common problems in spectroscopy: optimization of experimental conditions and non-linear calibration of the result, with particular reference to the determination of fluoride by flow injection analysis (FIA). The FIA system was based on the formation of a blue ternary complex between zirconium(IV), p-methyldibromoarsenazo and F- with the maximum absorption wavelength at 635 nm. First, optimization in terms of sensitivity and sampling rate was carried out by using jointly a central composite design and ANNs, and a neural network with a 3-7-1 structure was confirmed to be able to provide the maximum performance. Second, the relationship between the concentration of fluoride and its absorbance was modeled by ANNs. In this process, cross-validation and leave-k-out were used. The results showed that good prediction was attained in the 1-4-1 neural net. The trained networks proved to be very powerful in both applications. The proposed method was successfully applied to the determination of free fluoride in tea and toothpaste with recoveries between 96 and 101%.

[Effect of land management in winter crop season on seasonal variations of CH4 emissions from rice paddy soils].

A pot experiment in greenhouse was carried out to study seasonal variations of CH4 fluxes from rice paddy soils and the effect of land management in winter crop season on the seasonal variations. The results showed that four emission peaks occurred respectively 13 days after flooding and 7, 40, 91 days after rice transplanting, and CH4 emission amounts during the first 30 days after flooding accounted for as high as 67.5%, 35.5% and 33.3% of the total emission amounts during the observing period of 133 days for treatments with dry fallow but rice straw being applied just before flooding, alfalfa, and flooded fallow in winter crop season. However, for treatments with dry fallow but rice straw being applied before winter crop cultivating and winter wheat in winter crop season, CH4 emission amounts during the first 50 days after flooding just accounted for 0.27% and 6.74 of the total emission amounts during the observing period, and CH4 fluxes thereafter were also very small. Difference in the seasonal variation patterns of soil Eh due to land management in winter crop season was the main reason why seasonal variation patterns of CH4 fluxes from different treatments were different.

[Field epidemiological and experimental study on relationship between genetic factor and nonresponse or hyporesponse to hepatitis B vaccine].

OBJECTIVE: To evaluate the role of genetic factor in the development of nonresponder or hyporesponder to hepatitis B vaccine in Chinese people from field epidemiology and experimental study. METHODS: Six hundred and thirty-four susceptible healthy students were vaccinated with three doses of plasma-derived hepatitis B vaccine. Twenty-nine non- or hypo-responders (subjects) and 30 hyperresponders (controls) as well as their 80 and 79 first degree relatives were determined and recruited in the study. The first degree relatives of subjects and controls were then immunized with hepatitis B vaccine and followed up. On the basis of the above results, alleles of HLA-A, B, C, DR and DQ locus were further detected in the two groups. RESULTS: The non- and hypo-response rate in immunized students was 4.6% (29/634). After administered with hepatitis B vaccine, the first degree relatives of the subjects displayed a 17.9% frequency of nonresponsiveness, higher than in the controls. The phenotype frequency of HLA-DR7 and B54 was 52.2% and 21.7% in subjects, 9.1% and 0.0% higher than in controls, respectively. The extended haplotype B54-DR7 was more frequent in the former (17.0%) than in the latter (0.0%) (P < 0.01). CONCLUSION: These results demonstrated that the genetic factor was likely associated with the non- and hypo-response to hepatitis B vaccine in Chinese people.

Risk factors for development of diabetes mellitus in women with a history of gestational diabetes mellitus.

OBJECTIVE: To determine whether diabetes recurs in their later life when women have a history of gestational diabetes mellitus (GDM) or abnormal glucose tolerance test (impaired glucose tolerance, IGT). METHODS: Three groups of women were investigated at 5-10 years postpartum. GDM group (n = 45) had been diagnosed as having GDM in their previous pregnancy. IGT group (n = 31) had a history of abnormal glucose tolerance test during previous pregnancy. Normal control group (n = 39) was normal previous pregnant population. Their previous obstetric and medical histories were thoroughly reviewed. Fasting plasma glucose (FPG) and oral glucose (75 g) tolerance test (OGTT) were repeated in all women. RESULTS: Diabetes mellitus (DM) was diagnosed in 33.3% of patients in the GDM group, while in 9.7% in the IGT group and in 2.6% in the normal control group. Incidence of recurring DM in later life was significant higher in the GDM group (P = 0.017). When one or more blood glucose values exceeding WHO criteria for diagnosis of diabetes in their previous pregnancy, the incidence of DM in later life was 60% (3/5, including GDM in women having four abnormal OGTT values), 41.7% (5/12) in women having three, 25% (7/28) in women having two and 9.7% (3/31) in women having one. The women with DM, also with a history of GDM and abnormal OGTT in previous pregnancy, tends to have a high pregnant body mass index (BMI > 25 kg/m2). CONCLUSION: The women suffering from GDM during previous pregnancy have a high risk of recurrence DM. Two or more abnormal OGTT values during pregnancy, blood glucose level exceeding the maximal values at 1 and 2 hours after oral glucose loading and high pregnant BMI are concluded to be useful factors in predicting the recurring DM in their later life.

Intravesical immunotoxin as adjuvant therapy to prevent the recurrence of bladder cancer.

OBJECTIVE: To assess the intravesical application of immunotoxin as adjuvant therapy to prevent recurrence after tumor resection in bladder cancer patients. METHODS: An anti-human immunotoxin against bladder carcinoma, BDI-1-RT, was prepared and its in vitro targeting cytotoxicity estimated. The immunoreactivity of BDI-1-RT with human bladder cancer tissue of different grades and stages was detected by immunohistochemical analysis. After safety test, intravesical administration of BDI-1-RT was performed in 31 patients while mitomycin C (MMC) was used in 36 patients serving as a control group. The recurrence rates and side effects in both groups were recorded. In addition, the development of human anti-mouse antibodies (HAMA) was determined by ELISA, to assess the potential safety of this immunotoxin. RESULTS: In our study, BDI-1-RT had immunoreactivity with 81.6% of bladder transitional cell carcinomas. The immunoreactivity of BDI-1-RT correlated with tumor grade. High-grade carcinoma had stronger staining than low-grade (P < 0.05). There was no significant difference between the BDI-1-RT group (10%) and MMC group (19.3%) in recurrence rate (P > 0.05). Side effects, including systemic and local, were more frequent in the MMC group (11 of 36 patients versus 2 of 31, P < 0.05). HAMA was not detected in any of 7 patients. CONCLUSION: Immunotoxin may have considerable potential in the prophylaxis of bladder transition cell carcinoma.

[The value of CEA mRNA, CEA protein and cytology of peritoneal washings in the prediction of metastasis of gastric cancer].

OBJECTIVE: To ascertain the usefulness of CEA determination and cytology of peritoneal washings in the prediction of metastasis of gastric cancer to the peritoneum. METHODS: CEA was detected at mRNA level by RT-PCR and at protein level by radioimmunoassay (RIA). Cytologic examination was done by conventional method. Peritoneal washings were collected during operation from 43 patients with gastric carcinoma and 10 with benign lesions. RESULTS: The positive rate of RT-PCR was 56.3% and that of RIA was 38.1%. Both were more sensitive than that of cytologic examination (33.1%) of free cancer cells in the peritoneal washings. The level of CEA mRNA expression increased with the depth of invasion and stage of the disease. It also varied significantly in tumors different in the gross type and serosal involvement. CONCLUSION: To detect CEA in peritoneal washings by RT-PCR and RIA, and to examine them for tumor cells by cytotogic method are useful to predict subclinical metastasis to the peritoneam in patients with gastric carcinoma, but RT-PCR is most sensitive.

[Judgement of gross type and local invasion of advanced gastric carcinoma by spiral computed tomography].

OBJECTIVE: To evaluate the value of spiral computed tomography(sCT) in the judgement of gross type, invasion to serosa and adjacent organs of advanced gastric carcinoma. METHODS: The sCT results of 53 cases of advanced gastric carcinoma undergone gastrectomy were compared with surgical pathologic findings. RESULTS: The accuracy of judging gross type, location and size of gastric carcinoma was 92.5%, 88.7%, 81.1% respectively; 80.5% of serosal invasion was correctly judged by extraluminal fat sign, while 63.4% by serosal contour sign (P < 0.05). Different serosal types could be differentiated by CT features. The accuracy of judging invasion to pancreas, spleen and transverse colon was 87.8%, 87.8%, and 90.2%; and the specificity was 96.7%, 97.0% and 96.7%, respectively. CONCLUSION: Spiral computed tomography is of help in the identification of the gross type, invasion to serosa and adjacent organs of gastric cancer in its stage of progression.

[Clinical effect of Xiongshao capsule on preventing restenosis post-PTCA or/and stenting].

OBJECTIVE: To evaluate the effect of Xiongshao capsule (XSC) in preventing clinical and angiographic restenosis after coronary angioplasty or/and stenting. METHODS: Sixty-five coronary heart disease (CHD) patients with successful angioplasty or and stenting were randomly divided into the control group (37 cases, routine treatment) and the XSC group (28 cases, routine treatment plus XSC). The recurrent of angina, changing of blood stasis syndrome score (BSSS) and coronary angiography within 6 months after coronary angioplasty or/and stenting were observed. RESULTS: Recurrent angina occurred in 6 out of 28 patients in the XSC group, which was much better than that in the control group, 18 out of 37 patients (P < 0.05). Follow-up angiography was performed in 26 including 10 patients in the XSC group (restenosis was observed in 4 cases), and 16 cases in the control group (restenosis in 13 cases), there was significant difference between the XSC group and the control group (P < 0.05). For BSSS at follow-up, XSC group was much lower than that of the control group (P < 0.01). Logistic regress analysis showed the combination with XSC and the base-line BSSS were important affecting factors on the occurrence of restenosis (P < 0.05). The odds ratio of the occurrence of angiographic restenosis (BSSS > 25 to BSSS < or = 25) was 6.5. CONCLUSIONS: XSC could markedly reduce the occurrence of restenosis, recurrence of angina and improve the condition of blood stasis after coronary angioplasty or/and stenting. The severity of blood stasis syndrome was an important affecting factor on the occurrence of restenosis. BSSS > 25 can be served as one of the predicting factors of the occurrence of restenosis after coronary angioplasty or/and stenting.

[Distribution of interleukin-6 receptors (CD(126) and CD(w130)) on circulating lymphocytes in patients with lupus nephritis].

OBJECTIVE: To study the distribution of IL-6 receptors (CD(126) and CD(w130)) on lymphocytes in patients with lupus nephritis (LN). METHOD: The percentages of lymphocyte subsets were examined by direct two colour immunofluorescence on flow cytometry. RESULTS: (1) In active and inactive cases, the percentages of CD(+)(4) cells obviously decreased as compared with the normal controls (P < 0.01). Increased numbers of CD(+)(8) cells could be detected in LN patients (P < 0.05), especially in active cases (P < 0.01). Lymphocytes of active cases contained significantly greater percentage of CD(+)(23) cells than those of normal subjects (P < 0.05). On the contrary, the percentage of CD(+)(23) cells was statistically lower in the inactive cases than in normal subjects (P < 0.01). No significant difference in the percentages of CD(+)(126) and CD(+)(w130) cells was observed among the active, inactive and healthy groups (P > 0.05); (2) The proportion of CD(+)(4) CD(+)(126) and CD(+)(4) CD(+)(w130) cells in total lymphocytes was significantly lower in the active than in the inactive ones or normal subjects (P < 0.05; P < 0.01), whereas that of CD(+)(8) CD(+)(126) and CD(+)(8) CD(+)(w130) cells was not different among the active, inactive and normal subjects (P > 0.05). The proportion of CD(+)(23) CD(+)(126) cells and CD(+)(23) CD(+)(w130) cells augmented significantly in the active (P < 0.05), whereas it lessened in the inactive without statistical meaning as compared with the normal subjects (P > 0.05). (3) There was a positive correlation between the percentage of CD(+)(23) CD(+)(126) cells and that of CD(+)(23) CD(+)(w130) cells in total lymphocytes in the active group (r = 0.756, P < 0.05). However, no correlation between the percentage of CD(+)(23) CD(+)(126) cells and that of CD(+)(23) CD(+)(w130) cells in total lymphocytes was found in the inactive (r = 0.376, P > 0.05) or normal group (r = 0.256, P > 0.05); (4) The fraction of CD(+)(23) cells expressing CD(126) and CD(w130) was related to the SLE disease activity index (SLEDAI) and lymphopenia (P < 0.05, P < 0.005). But it was neither associated with the number of damaged organs and the degree of damage, not with that of ANA, anti-dsDNA antibody and complements (C(3) and C(4)) (P > 0.05). CONCLUSIONS: LN patients present not only B cell hyperactivity but also T cell abnormality. In active cases, there is abnormal distribution of IL-6 receptors on lymphocytes: reduced CD(126) and CD(w130) expression on CD(+)(4) cells and elevated CD(126) and CD(w130) expression on CD(+)(23) cells. Detecting the proportion of CD(+)(23) CD(+)(126) cells or CD(+)(23) CD(+)(w130) cells in total lymphocytes may serve as one of activity indexes for LN patients.
