Crystal structure of human mitochondrial NAD(P)(+)-dependent malic enzyme: a new class of oxidative decarboxylases.

Background: Malic enzymes catalyze the oxidative decarboxylation of malate to pyruvate and CO(2) with the concomitant reduction of NAD(P)(+) to NAD(P)H. They are widely distributed in nature and have important biological functions. Human mitochondrial NAD(P)(+)-dependent malic enzyme (mNAD-ME) may have a crucial role in the metabolism of glutamine for energy production in rapidly dividing cells and tumors. Moreover, this isoform is unique among malic enzymes in that it is a cooperative enzyme, and its activity is controlled allosterically. Results: The crystal structure of human mNAD-ME has been determined at 2.5 Å resolution by the selenomethionyl multiwavelength anomalous diffraction method and refined to 2.1 Å resolution. The structure of the monomer can be divided into four domains; the active site of the enzyme is located in a deep cleft at the interface between three of the domains. Three acidic residues (Glu255, Asp256 and Asp279) were identified as ligands for the divalent cation that is required for catalysis by malic enzymes. Conclusions: The structure reveals that malic enzymes belong to a new class of oxidative decarboxylases. The tetramer of the enzyme appears to be a dimer of dimers. The active site of each monomer is located far from the tetramer interface. The structure also shows the binding of a second NAD(+) molecule in a pocket 35 Å away from the active site. The natural ligand for this second binding site may be ATP, an allosteric inhibitor of the enzyme.

Treatment of squamous cell carcinoma of the tongue using arterial embolism with cisplatin-loaded albumin microspheres: a microstructural and ultrastructural investigation.

OBJECTIVE: To study the mechanism of cisplatin-loaded albumin microsphere infusion through the lingual artery to treat carcinoma of the tongue and the in vivo process of microspheres. METHODS: Lingual arterial infusion was performed in seven patients with advanced carcinoma of the tongue using cisplatin-loaded albumin microspheres 56.3 microns in diameter. The specimens were taken immediately after infusion and at 1, 2, 3, 4, 5, and 6 weeks after infusion. They were examined by microscopy and transmission electron microscopy. RESULTS: Histologic and ultrastructural changes after infusion included programmed cell death, ischemic necrosis, and degeneration. Almost all carcinoma cells were killed by the fourth week. Microspheres were gradually hydrolyzed in the body and vanished completely within 6 weeks. CONCLUSION: The mechanism of arterial embolism with cisplatin-loaded albumin microspheres to treat squamous cell carcinoma of the tongue involves the combined effects of cytotoxicity and ischemic necrosis of the tumor after microembolism.

BUBR1 phosphorylation is regulated during mitotic checkpoint activation.

Eukaryotic cells have evolved a mechanism that delays the progression of mitosis until condensed chromosomes are properly positioned on the mitotic spindle. To understand the molecular basis of such monitoring mechanism in human cells, we have been studying genes that regulate the mitotic checkpoint. Our early studies have led to the cloning of a full-length cDNA encoding MAD3-like protein (also termed BUBR1/MAD3/SSK1). Dot blot analyses show that BUBR1 mRNA is expressed in tissues with a high mitotic index but not in differentiated tissues. Western blot analyses show that in asynchronous cells, BUBR1 protein primarily exhibits a molecular mass of 120 kDa, and its expression is detected in most cell lines examined. In addition, BUBR1 is present during various stages of the cell cycle. As cells enter later S and G2, BUBR1 levels are increased significantly. Nocodazole-arrested mitotic cells obtained by mechanical shake-off contain BUBR1 antigen with a slower mobility on denaturing SDS gels. Phosphatase treatment restores the slowly migrating band to the interphase state, indicating that the slow mobility of the BUBR1 antigen is attributable to phosphorylation. Furthermore, purified recombinant His6-BUBR1 is capable of autophosphorylation. Our studies indicate that BUBR1 phosphorylation status is regulated during spindle disruption. Considering its strong homology to BUB1 protein kinase, BUBR1 may also play an important role in mitotic checkpoint control by phosphorylation of a critical cellular component(s) of the mitotic checkpoint pathway.

CD4+ T cell response to factor VIII in hemophilia A, acquired hemophilia, and healthy subjects.

These studies have begun to clarify the complex cellular mechanisms involved in the immune response to factor VIII. Although vigorous sensitization of CD4+ cells occurs in healthy subjects, the absence of clinically significant levels of inhibitor antibodies is likely related to the prompt down-regulation of the immune response. It may also be possible that the specific epitope repertoire recognized by CD4+ cells plays a role in the outcome of the immune response to factor VIII. Further characterization and comparison of the CD4+ repertoire in healthy subjects with that of hemophilia patients with and without inhibitors will help clarify which mechanism explains the absence of productive inhibitor synthesis in certain individuals. Also, it might identify CD4+ epitopes recognized by T helper cells that are essential for inhibitor synthesis. Additional studies to further characterize the role of Th1 and Th2 cells in the immune response to factor VIII may also be needed for the design of novel therapeutic strategies aimed at preventing inhibitor development.

[Comparison of internal fixation with mini-titanium plate and wire osteosynthesis in midface fractures].

OBJECTIVE: To compare clinical results of the internal fixation with wire and the internal fixation with mini-titanium plate in midface fractures. METHODS: By the coronal approach and supplementary incisions, forty patients with midface fractures were treated with wire osteosynthesis or rigid internal fixation (RIF) with mini-titanium plate. RESULTS: A good stability can not he obtained with wire osteosynthesis in the treatment of midface fractures. The supporting bony structure of midface can be reconstructed with mini-titanium plate RIF, which results in a sufficient three-dimensional stability of the fractured segments and an ideal facial configuration Mini-titanium plate RIF is more convenient and reliable than wire osteosynthesis in the difficult operation fields of the infraorbital margin and zygomaticomaxillary suture. CONCLUSION: Mini-titanium plate RIF is the best method for treatment of midface fractures.

Diagnostic role of antibodies to glutamic acid decarboxylase in latent autoimmune diabetes mellitus in adults.

OBJECTIVE: To investigate the diagnostic role of antibodies to glutamic acid decarboxylase (GAD65-Ab) in latent autoimmune diabetes of adults (LADA) and the frequency of GAD-Ab in Chinese patients initially diagnosed as non-insulin-dependent diabetes mellitus (NIDDM). METHODS: Forty-five control subjects and 195 consecutive inpatients initially classified as NIDDM with > or = 35 years of age at onset and nonketotic history for > 6 months after diagnosis, were recruited. In vitro transcripted and translated recombinant human 35S-GAD65 was used in radioligand assay of GAD-Ab. RESULTS: The overall prevalence of GAD65-Ab was 14.8% (29/195) in NIDDM patients and 2.2% (1/45) in control subjects, respectively. Of the 29 GAD65-Ab positive patients, 17 (58.6%) were insulin-deficient while 12 (41.4%) were non-insulin-deficient. The prevalence of GAD65-Ab in NIDDM group with age of < 40 years at diabetes onset, ketotic history, body mass index (BMI) < 21 kg/m2, were significantly higher than that of corresponding control diabetic subgroups (2.5, 4.1 and 3.2 times, respectively). The sex, duration, symptoms of polyphagia, polydipsia, polyuria and weight loss at onset of the disease were not related to the prevalence of GAD65-Ab positivity. CONCLUSIONS: In China, patients initially diagnosed as NIDDM may in many cases suffer from LADA. Testing by GAD65-Ab may be of assistance to identifying LADA at the earliest stage of disease.

[Characteristics of human platelet fibrinogen receptors regulated by anti-CD9 monoclonal antibodies].

OBJECTIVE: To investigate the regulatory effect of two anti-CD9 monoclonal antibodies (McAbs), HI117 and SJ9A4, on human platelet fibrinogen receptors, and explore their potential mechanisms. METHODS: By using 125I-labeled human fibrinogen (Fg), the Fg specifically bound to human platelets induced by HI117 and SJ9A4 was measured, and the changes of anti-CD9 McAbs-exposed Fg receptor in the absence of Fg were also observed. RESULTS: HI117 (10 micrograms/ml) and SJ9A4 (20 micrograms/ml) induced markedly the specific binding of Fg to human platelets, suggesting that the two McAbs evoked obvious exposure of Fg receptors. But in the absence of Fg, the McAbs-exposed Fg receptors gradually lost their capacity to bind Fg and closed. Further study indicated that HI117 and SJ9A4-induced Fg binding was reduced by pretreatment of platelets with sphingosine, aspirin, apyrase, and/or PGI2. CONCLUSION: The anti-CD9 McAbs, HI117 and SJ9A4, can reversibly expose platelet Fg receptors, probably via three signaling pathways, i.e. thromboxane, secreted ADP and cAMP, protein kinase C (PKC) activation presumably being the common passage for the signaling.

[Study on conformational changes of GP IIb/IIIa complex during platelet activation].

OBJECTIVE: To determine whether a conformational change in GPIIb/IIIa occurs during platelet activation. METHODS: Epitopes on GPIIb/IIIa complex were labeled with McAbs to GPIIb (SZ-22) and GPIIIa (SZ-21) conjugated to either a donor fluorescein (FITC-labeled) or an acceptor (TR-labeled) chromophore. The FRET between platelet-bound FITC and TR was measured and calculated by flow cytometry during platelet activation induced by some agonists. RESULTS: In unstimulated platelets, a small but consistent change of FRET (5.5%) was detected, regardless of which antibody served as FRET donor. Platelet activation resulted in an obvious increase in FRET, suggesting a significant change in the separation or orientation of the epitopes within GPIIb/IIIa. This kind of changes could also occur on removing the extracellular calcium, and was independent of receptor occupancy. CONCLUSION: The increase in FRET can reflect the rearrangement of the chromophore-labeled McAbs bound to GPIIb/IIIa during platelet activation, and this kind of structural rearrangement subunits of GPIIb/IIIa complex subunits can induce fibrinogen receptor expression.

[Reconstruction of posterior cruciate ligament using bone-patellar tendon-bone autograft].

OBJECTIVE: To introduce the surgical procedures of posterior cruciate ligament (PCL) reconstruction using bone-patellar tendon-bone autograft and to evaluate the clinical effect of treatment of posterior instability. METHODS: Twenty cases of chronic instability of knee secondary to rupture of PCL underwent reconstruction of PCL using free autograft of bone-patellar tendon-bone by isometric tibial and femoral attachment through anterior approach of knee. Larson knee function score, patient subjective evaluation and physical testing were used to evaluate the effect of this procedure. RESULTS: In this series, the average point of Larson score was 62 preoperatively. By the period of 6 - 58 month follow-up, the average point was 92. Subjectively 18 cases showed satisfactory results. Lachmann test showed negative results in all cases. The range of motion was 115 degrees on the average. CONCLUSIONS: This method can provide a strong and stable PCL replacement. But the surgical procedure is more difficult than ACL reconstruction. Some factors for a successful operation include evaluation of instability, preparation of graft material, internal fixation of graft, isometric attachment, reconstruction of other structures of knee, and bearing tension of graft.

[Multiple inhibitory mechanisms of wild-type p16 gene transfer into human bladder cells by retroviral vector].

OBJECTIVE: To investigate the inhibitory mechanisms of wild-type p16 gene transfer into human bladder cancer cells with or without endogenous p16 gene expression. METHODS: p16 recombinant retrovirus vector was constructed and transfected in human bladder cancer cell EJ and 253J. Northern blot analysis and immunocytochemistry were used to detect the expression of exogenous p16 gene. Flow cytometry analysis was used to detect tumor cell cycle. Electronic microscope and in situ labelling apoptotic DNA fragment detection (TUNEL) was used to detect tumor cell apoptosis after gene transfection. Tumorigenicity was observed in nude mice. RESULTS: The growth rate of the transfected EJ cells was significantly retarded, the majority of transfected EJ cells were arrested on G(0) + G(1) phases of cell cycle, tumorigenicity of the transfected cells was reduced on nude mice. H-ras gene expression in transfected EJ cells reduced as compared with non-transfection. Apoptotic cells were observed in transgenic 253J cells. CONCLUSIONS: Malignant proliferation of bladder cancer cells can be inhibited by exogenous p16 gene transfection via different molecular pathway.

[Clinical protocols for the treatment of severe acute pancreatitis].

OBJECTIVE: To set up principles for the treatment of severe acute pancreatitis (SAP). METHODS: The therapeutic principles and results of SAP in three periods were analyzed. Period I (1980 - 1990) was characterized by early surgical intervention supplemented with integrated traditional and western medical therapy, period II (1991 - 1993) by integrated traditional and western medical therapy supplemented with surgery for early complications, and period III (1994 - 1997) by integrated traditional and western medical therapy supplemented with surgery for local infectious complications only. RESULTS: Because of the improvement of intensive care, the operation rates kept declining in the three periods. They were 77.59%, 54.55% and 29.55% with a mortality rate of 40.52%, 17.17% and 11.36%, respectively. CONCLUSIONS: Integrated traditional and western medicine has been effective in most SAP patients, and surgery is indicated only for late complications such as necrotizing infection. Early surgery should be restricted.

[Effects of occupational exposure to formamidines on cardiovascular functions].

OBJECTIVE: To understand the possible effects of occupational exposure to formamidines on human cardiovascular function, this study was carried out. METHODS: Sixteen farmers spraying chlordimeform, 14 packers packaging chlordimeform and 23 packers packaging mono-formamidine were followed-up pre- and post-exposure. Their urinary excretion of formamidine or its metabolite, as well as air concentrations of formamidine at their work places and their skin contamination with it were measured to estimate the exposure level. Furthermore, 24-hour urinary level of vanillinmandelic acid (VMA) was analyzed among mono-formamidine packers. RESULTS: Their whole body skin contaminated with 3.240 and 2.142 g/cm(2) of chlordimeform in the sprayers and packers, respectively. Their hand skin contaminated with mono-formamidine at 6.59 g/cm(2) in the packers. It indicated that the major route of exposure to formamidine was skin contamination. Urinary levels of formamidines or their metabolites increased significantly after exposure, reaching 6.194 and 3.378 micromol/L for the sprayers and packers exposed to chlordimeform, respectively, and 2.760 - 3.427 micromol/L for mono-formamidine in the packers. Their heart rates slowed down, P-R and Q-T intervals prolonged and blood pressure reduced after exposure, as compared with those before exposure. Consistency of the results in several studies demonstrated that formamidines could induce changes in the indices for cardiovascular functions under the relatively low exposure levels at present. Decrease of urinary VMA from 0.068 micromol/L pre-exposure to 0.040 - 0.055 micromol/L post-exposure suggested that catecholamine could play a role in these effects. CONCLUSION: Formamidines has certain effects on human cardiovascular function. It is suggested that attention be paid to the changes in cardiovascular functions of those exposed in their health surveillance.

[A case-control study on Guillain-Barre syndrome in children of North China].

OBJECTIVE: This study aimed at exploring the risk factors for Guillain-Barre syndrome (GBS). METHODS: A case-control study design was used with 51 cases of GBS, diagnosed based on their symptoms, signs and electrophysiological examinations and exclusion of poliomyelitis and other acute flaccid paralysis, and 51 controls matched on age, sex and resident village. Serum IgG antibodies specific for Campylobacter jejuni were determined for all the subjects by enzyme-linked immunosorbent assay (ELISA) with a preparation of surface antigen of C. jejuni C(1) strain isolated from the patients and prevalent in north China. Each case and control were interviewed with his/her parents or guardians by a trained interviewer using an ad hoc questionnaire, including his/her demographic information, socioeconomic status, onset of the illness, and potential risk factors in their environment and personal hygiene. Data were analyzed with SAS software release 6.04 in a microcomputer. RESULTS: GBS was associated with a few factors, such as residential areas (45 of the 51 cases living in the rural areas, accounting for 88.2% of the total), polio and hepatitis B vaccine immunization before onset of the illness (with ORs of 7.27 and 3.14, respectively), no hand washing after defecation and before meals (with an OR of 6.15) and getting cold and going to the river or lake site before onset of the illness (with ORs of 13.75 and 12.20, respectively). Infection with Campylobacter jejuni associated strongly with the illness (with an OR of 9.5, P < 0.001). Thirty-five of the 51 cases had precursor symptoms before onset of the illness (68.6%). CONCLUSION: It suggests that occurrence of GBS may correlate to infection with Campylobacter jejuni and poor personal hygiene in children.

[The mechanism of p16 gene inactivation in human bladder transitional cell carcinoma].

OBJECTIVE: To figure out the mechanism of p16 gene inactivation in human bladder transitional cell carcinoma (BTCC). METHODS: p16 gene was analyzed for genetic alterations by Southern blot analysis, PCR-SSCP and DNA methylation analysis in 52 cases of transitional cell carcinoma of bladder. RESULTS: p16 gene deletion was found in 11 of 52 (21.15%) BTCCs, only one tumor showed point mutation on p16 exon 2, nineteen cases of BTCCs (36.53%) showed p16 gene 5' CpG island methylation. CONCLUSION: There was a statistically significant association between p16 gene deletion and early stage, well differentiated tumors. Additionally, a statistically significant correlation was also noticed between p16 gene 5' CpG island methylation and tumors in late stage with poor cell differentiation.
