[Indications for surgery in ruptures of ankle joint ligaments (author's transl)].

X-ray films of the forced extreme joint position are required for diagnosis of lateral ligamentous injuries of the ankle joint. These x-ray films must be taken in forced extreme joint position in two planes (a.p. and lateral planes) by means of a special holding device simulating the mechanism of trauma by supination, inversion and adduction of the ankle joint by plantar flexion of the foot. If there is a 5 degree difference in talar tilt angle or a 5 mm difference in talar subluxation, an operation is indicated to prevent post-traumatic weak ankle and early arthrosis following chronic instability.

[Thoracic sonography with the rapid B picture (author's transl)].

The orienting investigation should be a compulsory component of every internist echographic status with the rapid B picture. Besides its diagnostic aspects, thoracic echography with the rapid B picture in the region of the pericardium is also of therapeutic importance for the reliable specific ultrasonic pericardial puncture for the relief a tamponading pericardial effusion. All radiological shadows which are not completely clear must be subjected to echographic assessment before proceeding to more expensive diagnostic measures. This permits the sonographic differentiation between a collection of fluid and a solid structure as far as the shadows are confined to the wall.

[Leading symptom: esophageal varices hemorrhage in a young man].

Indicating sign: Esophageal hemorrhages in adolescence. Congenital hepatic fibrosis and infantile polycystic disease of the kidneys. A 17-year-old patient with hypersplenism and signs of portal hypertension came to clinical treatment after an esophageal hemorrhage. Rare syndrome of congenital hepatic fibrosis with portal hypertension and splenomegaly, with infantile polycystic disease of the kidneys and aneurysms of the splenic artery was recognizable by means of ultrasound, laparoscopy and angiography. Therapeutically splenorenal shunt and splenectomy were carried out.

Flashing tomosynthesis--a new tomographic method.

A new tomographic method called tomosynthesis and its first clinical results are presented. The method is based on classical tomography. All information necessary for the tomography of an object is obtained in one procedure without moving the X-ray tube, the film, or the object. Thus the investigation requires only a few seconds.

Skin test and lymphocyte stimulation in delayed hypersensitivity against Staphylococcus aureus antigens. Development of hypersensitivity.

The development of delayed hypersensitivity against staphylococcal antigens in guinea pigs was observed from day 3 to day 50 after sensitization with staphylococcal homogenate in Freund's incomplete (FIA) and Freund's complete adjuvant (FCA). Skin test reactivity, stimulation of lymph node lymphocytes, and peripheral blood lymphocytes and the titre of precipitating antibodies were followed during this period. Maximal skin test reactivity as well as maximal lymphocyte responsiveness occurred at day 21 after sensitization in FCA-sensitized guinea pigs. In FIA-sensitized animals highest skin reactivity was observed at day 14 and maximal lymphocyte stimulation 35 days after sensitization. Precipitating antibodies reached a plateau at day 20 in plasma of animals sensitized with FCA and at day 35 in FIA-sensitized animals.

Skin test and lymphocyte stimulation in delayed hypersensitivity against staphylococcal antigens. Relation to bacterial cell fractions.

Staphylococcal homogenate was fractionated into cell walls (CW), cell membranes (CM) (insoluble part left after removal of the cell wall fraction) and a soluble fraction. The capacity of these fractions to evoke delayed skin reactions and to stimulate lymph node and peripheral blood lymphocytes from guinea pigs sensitized with staphylococcal homogenate in Freund's complete adjuvant was tested 21 days after sensitization. Highest skin reactivity was observed with the cell wall fraction. In the lymphocyte stimulation test similar results were obtained with all three fractions. With peripheral blood lymphocytes higher stimulation indices were observed than with lymph node lymphocytes.

Inhibitory effect of 8-methoxysporalen plus UVA (PUVA) on the systemic induction of contact sensitivity to dinitrochlorobenzene (DNCB) in guinea pigs.

To investigate the influence of 8-MOP and UVA on the induction of cell-mediated immunity, guinea pigs, sensitized with a single injection of dinitrofluorobenzene (DNFB) in Freund's complete adjuvant (FCA) in all footpads and the nuchal skin, were treated with 8-MOP, UVA, or 8-MOP + UVA on days 0, 2, and 5 and tested with dinitrochlorobenzene (DNCB) on day 14 after sensitization. Two control groups, exposed in a covered condition to the PUVA 4000 lamp, to observe the heat effect, showed a slightly enhanced contact sensitivity in comparison to the only sensitized control group. No altered reactivity was observed after irradiation with longwave UV light alone, whereas a statistically significant enhanced resp. reduced contact sensitivity was obtained after the treatment with 8-MOP alone and the combined action of 8-MOP + UVA, respectively.

Retinopathy in juvenile-onset diabetes of short duration.

To study objectively the epidemiology of retinopathy in juvenile-onset diabetes, we performed fundus photography and fluorescein angiography, using the Diabetic Retinopathy Study protocol, on 122 juvenile diabetics and 65 demographically similar non-diabetic subjects as the control group. Photographs and angiograms were masked as to subjects' identities and evaluated independently by five retinal subspecialists. There was no retinopathy in control subjects. In diabetics, prevalence of retinopathy increased with the duration of disease, being 0% after zero to four years, 27% for five to nine years, and 71% for more than ten years. Retinopathy also increased in prevalence with age with a sharp rise after age 15. There is indication that age and duration act independently. Details of our method for establishing the diagnosis of diabetic retinopathy are presented, together with the degree of observer variability in identifying early lesions.

Flow cytofluorometry, cytomorphology and histology in gastric carcinoma.

68 cases of gastric carcinoma and 31 tissue samples adjoining the carcinomas were investigated by impulse cytophotometry, cytomorphology and histology, respectively. Differentiated adenocarcinoma tended to aneuploidy, while undifferentiated carcinoma often showed euploidy. The adjoining mucosa was characterized by polymorphism of the mucosa superficial epithelial cells and the histograms showed slight abnormalities.

Coenzyme Q10 levels in rat heart of different age.

CoQ10 is intrinsic to human tissues and is a vitamin, according to the basic science of nutrition. It is known that CoQ10, plays a role in the respiratory process, moreover it seems to be involved in the mechanism of blood coagulation. As a vitamin a deficiency of CoQ10 may be related to some disease, that is, first of all, a disease is bioenergetics. It has been demonstrated that during aging, other than a diminution of EFA (and an increase of palmitoleic and eicosatrienoic fatty acids), a decrease of about the 20-30% in the heart CoQ10 levels occurs in the rat. Perhaps these modifications may be intrinsic to the "biochemical" changes of aging.

The influence of heparin on DNA-distribution patterns of gastric mucosa as obtained by flow cytometry. Preliminary report.

Preliminary results about the influence of Heparin to DNA-distribution patterns of gastric mucosa obtained by flow cytometry are reported. Changes in the configuration of nuclear chromatin, caused by Heparin, are connected with changes in the stainability; thus they are reflected by DNA-distribution patterns. With precaution, some special features in the 4C region of DNA-distribution patterns of gastric mucosa in severe chronic atrophic gastritis and carcinoma are supposed to be attributed to changes in the nuclear chromatin occurring in these diseases.
