[Effect of anti-opioid peptide sera on the enhancement of electroacupuncture analgesia induced by neurotensin in PAG of rats].

With the use of potassium iontophoresis induced tail-flick for measuring the pain threshold, the effects of injecting neurotensin (NT), naloxone (NX), anti-metenkephalin serum (AMEKS), anti-beta-endorphin serum (AEPS) and anti-dynorphin A1-13 serum (ADYNS) into periaqueductal gray (PAG) on electroacupuncture (EA) analgesia in rats were investigated. NT administration enhanced EA analgesia remarkably. Pre-injection of NX, AMEKS and AEPS into PAG could significantly attenuate the enhancement of EA analgesia induced by NT, but not by administration of ADYNS. The results indicate that NT in PAG is responsible for the enhancement of EA analgesia. The effect of NT may be partly mediated by met-enkephalin and beta-endorphin.

[Coverage of the lower leg defects using the reversed island skin flap based on the nutrient vessels of the saphenous nerve].

OBJECTIVE: To report the application of the distally based neurocutaneous saphenous island flap for the coverage of soft tissue defects in the lower limb. METHODS: According to the size and site of the defect and the rotation point, the flap was designed, and it was based on the arterial axis associated with the saphenous nerve and the greater saphenous vein. RESULTS: Six neurocutaneous saphenous island flaps were used and all survived. Flap dimensions were as large as 6 cm x 8 cm. The results were satisfactory after 6 to 20 months' follow-up. CONCLUSION: The vascularity of the saphenous nerve is closely connected with the vascularity of the skin. The flap supplied by the accompanying vessels of the cutaneous nerve can be utilized with good results.

[Effects of acute hypoxia and hypoxic acclimatization on cardiac functions of rats].

OBJECTIVE: To study the effects of acute hypoxia and intermittent hypoxic acclimatization on cardiac systolic and diastolic functions in rats. METHOD: Cardiac functions were investigated after intermittent hypoxic acclimatizated (3000 m and 5000 m, 2 wk respectively, 4 h/d) and normoxic rats were exposed to hypoxia (8000 m) for 4 h. RESULT: The systolic functional parameters such as LVSP, +dp/dtmax, Vpm, Vmax and the diastolic functional parameter -dp/dtmax of acute hypoxic rats were reduced significantly as compared with normoxic control rats. After hypoxic acclimatization, the left ventricular functions of rats were increased significantly as compared with those of acute hypoxic rats. But they were still lower than those of normoxic control rats. CONCLUSION: Acute hypoxia can affect cardiac functions and hypoxic acclimatization can alleviate the hypoxic effects on cardiac functions.

Study of activity of promoter from mouse alpha 2(I) procollagen gene.

OBJECTIVE: To clarify the segment and sequence in mouse alpha 2(I) procollagen gene which are responsible for high transcriptional activity during fibrogenesis. METHODS: This study was focused on further fractional analysis of 2 kb-length mouse alpha 2(I) procollagen gene promoter activity. Six chimeric genes were constructed in which various lengths of sequences between 2000 bp upstream of the start of transcription of the mouse alpha 2 (I) procollagen gene and 54 bp downstream of this site were fused to chloramphenicol acetyltransferase (CAT) reporter gene. These recombinant plasmids were transfected transiently to collagen-producing cells (NIH/3T3) and non-collagen-producing cells (COS7) with liposomal transfection method. The activities of putative promoters were observed and compared by means of CAT measurement in the transfected cells. RESULTS: The highest and partial cell specific CAT expression was observed in construction driven by -780 to bp fragment. The construction containing sequence deleted the proximal 500 bp from the transcription start site and part of exon I, and resulted in the lowest CAT expression. CONCLUSIONS: Some essential elements might exist in the 500 bp fraction proximal to transcription start site and part of exon I in mouse alpha 2(I) procollagen gene. The high potential promoter sequence between -780 bp from the start of transcription site and bp from this site is of great significance in our following study of searching for specific DNA-binding proteins in activated collagen-producing cells.

The clinical and epidemiological significance of serum PreS1/anti-PreS1 in HBV infection.

OBJECTIVE: To investigate whether PreS1 antigen and/or anti-PreS1 antibody in serum might have some significance different from routine hepatitis B virus (HBV) markers in hepatitis B virus infection. METHODS: Forty-six acute hepatitis B patients and 377 chronic persistent hepatitis patients were examined to assess the clinical significance of serum PreS1/anti-PreS1. HBV family clustering analysis was carried out in a village in Shanghai suburb area, and case-control study on role of PreS1 in HBV spreading was undertaken in Tangshan, Hebei province. RESULTS: PreS1 in HBsAg positive sera from chronic persistent hepatitis patients correlated with HBV DNA (P < 0.05), while the presence of PreS2 in HBsAg positive sera was much more common than that of PreS1 and had no correlation with HBeAg or HBV DNA. Among 46 patients with acute hepatitis B, the appearance of anti-PreS1 antibody in serum correlated with the disappearing or decreasing of both PreS1 and ALT. In antiviral treatment, PreS1 antigen turned negative much earlier than PreS2 and HBsAg in acute hepatitis B patients' sera. Family clustering analysis suggested that people with PreS1 positive in serum were more infective than those with HBsAg positive alone (P < 0.01). The case-control study showed that chronic hepatitis B patients with PreS1 positive in serum played a much more important role in HBV spreading than those with PreS1 negative (P < 0.01) and asymptomatic HBsAg carriers (P < 0.01), and the odds ratios (OR) were 3.8 and 3.2, respectively. CONCLUSIONS: Serum PreS1 closely correlates with active virus replication, and PreS1/anti-PreS1 status indicates the outcome of acute hepatitis B. The serum PreS1/anti-PreS1 marker is of some significance in HBV epidemiology as well.

[Antimicrobial resistance in Streptococcus pneumoniae in Beijing and molecular typing of penicillin-resistant strains].

OBJECTIVE: To investigate antimicrobial resistance of Streptococcus pneumoniae in Beijing and molecular epidemiology of penicillin-resistant pneumococci strains. METHODS: The resistance to beta-lactam and non-beta-lactam antibiotics of 244 nasopharyngeal isolates of Streptococcus pneumoniae collected from September to November 1997 in 8 day-care centers in Beijing was studied by Etest and agar dilution method. Serotyping was done by "capsular swelling" technique. BOX-PCR technique was used to detect the DNA of resistant strains. RESULTS: 24.8% (244/985) of the children carried Streptococcus pneumoniae. The agreement between the MICs obtained by Etest and agar dilution for penicillin and ceftriaxone was > 93.7%. By Etest, 3/244 (1.3%) strains were resistant to penicillin(MICs, 3 micrograms/ml) and 32/244(13.1%) strains in termedinte (MICs, 0.094-0.25 microgram/ml). Penicillin-susceptible strains were also susceptible to the other 4 tested beta-lactams. In penicillin non-susceptible strains, 11.4% (4/35) isolates were resistant to the other beta-lactams. The resistant rates of erythromycin, tetracycline, chloramphenicol and trimethoprim-sulfamethoxazole (TMP-SMZ) were 74.0%, 87.6%, 47.8%, 63.3%, respectively. All of the strains were susceptible to vancomycin and rifampin. 119/244(48.7%) strains isolated were multiresistant to tetracycline, erythromycin and TMP/SMZ. Serotype 6A(33.6%), 19F(16.8%), 23F(15.1%), 15(11.2%), 6B(4.3%) were most common. 57.1% (20/35) penicillin non-susceptible strains were of serotype 23F and had similar or identical multiresistance and BOX patterns. CONCLUSION: In Beijing, the prevalence of penicillin-resistant pneumococci was obviously lower than that of neighboring countries, but resistant rate to non-beta-lactams was high. The epidemic multiresistant 23F clone found in day-care centers was different from the Spanish clone.

[The association between signal-regulatory protein alpha and hepatocellular carcinoma].

OBJECTIVE: To study the roles of signal-regulatory protein alpha (SIRP alpha) in the development of hepatocellular carcinoma (HCC). METHODS: 42 pairs of HCC and the surrounding noncancerous hepatic tissues were used. The expression of SIRP alpha in various tissues was determined by Northern blot analysis and immunohistochemistry assay, by which together with the pathologic characteristics of each sample, the role of SIRP alpha was investigated. RESULTS: The expression of SIRP alpha significantly decreased in 21 cases of samples (50.0%) to the 3.9 kb transcript and 27 cases of samples (64.3%) to the 2.5 kb transcript respectively. The SIRP alpha decreasing rate in large HCC(> 5 cm in diameter) was higher than in small HCC (< or = 5 cm in diameter), and in HCC with cancer embolus was higher than that of without cancer embolus. No association was found between the SIRP alpha expression and the patient's age, tumor grade, tumor capsule, serum AFP level, and HBV infection. Immunohistochemistry showed that all the normal hepatocytes, cholangioepithelial cells, Ito cells, regenerated hepatocytes, new generated cholangioepithelial cells and part of HCC cells were positive. The HCC tissues in which SIRP alpha was down-regulated showed positive particles becoming weaker and less, and some tumor cells and area were completely negative. CONCLUSION: The expression of SIRP alpha especially 2.5 kb transcript is down-regulated in HCC, which is closely related with the tumor growth and invasiveness. SIRP alpha may be an important negative regulator of HCC.

The significance of anti-endothelial cell antibodies in patients with lupus nephritis and immunoblotting analysis of the target components.

OBJECTIVE: To investigate the prevalence of anti-endothelial cell antibodies and its possible role in the pathogenesis in lupus nephritis. METHODS: Sera from 58 patients with lupus nephritis were studied. Cellular enzyme linked immunoabsaborbent assay was used to detect anti-endothelial cell antibodies, and immunoblotting technique was performed to determine its specific endothelial target antigens. RESULTS: 36.2% of the patients with lupus nephritis had elevated IgG isotype anti-endothelial cell antibodies. The association was revealed between the presence of anti-endothelial cell antibodies (AECA) and some laboratory and renal biopsy features, such as thrombocytopenia, higher serum IgG level, lower serum C3 level, higher erythrocyte sedimentation rate (ESR), presence of anti-nuclear antibody (ANA), anti-dsDNA antibodies and glomerular thrombosis. Anti-endothelial cell antibodies were consisted of a group of heterogeneous antibodies, and reacted with different endothelial cell antigens which ranged from 12-90 kD in molecular size in immunoblot. A correlation was demonstrated between heavy proteinuria (> 3.5 g/24 hr) and the existence of antibodies against both 27 and 29 kD endothelial cell antigens, an association between the presence of glomerular capillary thrombi and antibody against the 29 kD endothelial cell antigen was also found. CONCLUSIONS: Lupus nephritis patients with anti-endothelial cell antibodies showed a complex autoimmune disorder and clinical disease activity, and the results also provided an indirect evidence that anti-endothelial cell antibodies might contribute to the pathogenesis of lupus nephritis.

Effects of the configuration of a multi-epitope chimeric malaria DNA vaccine on its antigenicity to mice.

OBJECTIVE: Four B and Th cell epitopes were selected from conservative domain of Plasmodium falciparum antigens to construct two groups of chimeric malaria DNA vaccines with different configurations and their antigenicities were studied. METHODS: The partially synthesized oligonucleotide was annealed, PCR amplified and cloned into a mammalian cell expression vector. By using a pair of isocaudamers on the vector, different single copies of B epitopes were multiplied and were tenderly stringed into two groups of chimeric DNA vaccine with different configurations. BALB/c mice were immunized with these DNA plasmids by either intramuscular or intradermal injections. RESULTS: The antisera from the immunized mice tested by ELISA showed that only the configuration which had a single copy of universal T helper cell epitope, CS. T3, located at the C terminal of the multi-copy B cell epitopes induced a high antibody response. The T helper cell epitope at any other position of the peptide, or the double T helper cell epitopes configured with the B cell epitopes did not enhance antibody response, and some configurations even decreased the humoral response to a B cell epitope. CONCLUSION: This study demonstrated that both combination and configuration of the epitope may affect the antigenicity of a chimeric multiple antigen.

cDNA cloning and sequence analysis of hepatitis G virus genome isolated from a Chinese blood donor.

OBJECTIVE: To obtain full-length sequence of a Chinese hepatitis G virus (HGV) strain (HGVch) and investigate the genetic characteristic of HGVch and its identity to other isolates. METHODS: Reverse transcription (RT) and nested-PCR were used to screen HGV RNA positive serum and amplify cDNA fragments. A positive serum without known hepatitis virus markers was selected for isolating HGV RNA template. The HGV genome was divided into 12 overlapping fragments and directly cloned into pGEM-T vector. Sequences were determined by dideoxy terminus-end method of DNA sequencing and then analyzed by computer. RESULTS: The twelve fragments of HGVch cover 9213 nucleotides in length, containing a large open reading frame (ORF) encoding 2873 animo acids polyprotein that began with a methonine residue and ended at termination codon. HGVch is about 86.5%-89.5% identical to other known HGV isolates at the nucleotide level and about 93.9%-96.2% at the deduced animo acid level. CONCLUSION: HGV is a non-A-E hepatitis causal agent, proved to be related with posttransfusion hepatitis in all over the world. Chinese HGV isolate has very close relationship to other isolates from Africa, Europe, Japan, without significant difference across the entire genome. It is suggested that the sequences of HGV isolates are very conservative and the evolution is very slow.

Selection and determination of specific and protective antigens of Mycobacterium tuberculosis.

OBJECTIVE: To select and identify the specific and protective antigens of Mycobacterium tuberculosis (M. tuberculosis) for searching a new approach to diagnose and treat tuberculosis. METHODS: Extract and culture filtrates of M. tuberculosis were obtained by ultrasonic treatment and millipore membrane filtration, respectively. The protein samples were tested with monoclonal antibody (MAb) and patient sera. The proteins showing positive reaction were sequenced on Beckman/LF 3200/peptide amino acid sequencer. RESULTS: Proteins of M. tuberculosis with molecular weight of 31 ku and 30 ku showed positive results when reacted with anti-M. tuberculosis MAb and sera of tuberculosis patients, but not with normal mouse serum and healthy human sera. N-terminal sequences of the 31 ku and 30 ku antigen were Ala Glu Val Asp Trp Leu Val Phe Ala Val and Phe Ser Arg Pro Gly Leu Pro Val Glu Try respectively. CONCLUSION: 31 ku and 30 ku proteins of M. tuberculosis are immune protective proteins. They play important or dominant roles in determination of immunorecognization.

A study of platelet organelles and membrane glycoprotein in patients with Binswanger's disease.

OBJECTIVE: To discuss the possible role of platelets in Binswanger's disease by studying the changes of organelles and membrane glycoprotein in platelets. METHODS: The organelles (dense bodies, alpha granules, and mitochondria) within platelets were measured with transmission electron microscopy in 25 patients with Binswanger's disease and matched controls, while alpha granule membrane glycoprotein CD62 and lysosomal integral membrane glycoprotein CD63 on platelet membrane were analyzed by flow cytometry. RESULTS: The percentage of CD62-positive platelets was 3.6% +/- 2.1% and 5.7% +/- 2.4% in controls and in patients, respectively (P < 0.01). The percentage of CD63 expression platelets was 3.1% +/- 2.2% and 3.2% +/- 2.3% in controls and patients, respectively (P > 0.05). When compared with controls, platelets of patients contained fewer alpha granules (P < 0.01), and had no changes in dense bodies and mitochondria (P > 0.05). CD62 levels were different among patients with regarding to the degree of white matter low attenuation. CONCLUSION: There is selective degranuation of platelets in patients with Binswanger's disease and the abnormalities of platelet secretion may play a role in the pathophysiology of this illness.

[Effects of all-trans retinoic acid, arsenic trioxide and daunorubicin on tissue factor expression in NB4 cells].

OBJECTIVE: To investigate the effects of all-trans retinoic acid(ATRA), arsenic trioxide(As2O3) and daunorubicin(DNR) on tissue factor(TF) expression in acute promyelocytic leukemia (APL) cell line NB4 cells. METHODS: Procoagulant activity(PCA) of NB4 cells treated with 1 mumol/L ATRA, 1 mumol/L As2O3 or 0.2 microgram/ml DNR was detected using one-stage clotting assay, TF antigen by ELISA, and TF mRNA by RT-PCR. RESULTS: Both ATRA and As2O3 could down-regulate the TF antigen, its mRNA transcription and membrane PCA of NB4 cells with a time-dependent manner, while DNR was shown to increase these parameters. Moreover, by dideoxy sequencing of DNA fragment derived from PCR, it was found that there was a exon 5 deletion transcript of TF in APL cells. Its biological significance remained unknown. CONCLUSION: TF expression and PCA of APL cells may be down-regulated by ATRA and As2O3, therefore, As2O3 might also improve DIC-related hemorrhage of APL while inducing APL cells to apoptosis. The distinct regulation of TF and PCA on APL cells by As2O3 and DNR may at least partially contribute to their effects on APL coagulopathy through the influence on coagulant factors activation.

[Comparison of AutoSet with polysomnography in the diagnosis of sleep apnea syndrome].

OBJECTIVE: To evaluate the validation of the AutoSet system in the diagnosis of sleep apnea syndrome(SAS). METHODS: One hundred and twenty patients were studied simultaneously performed both with the AutoSet and the polysomnography(PSG). The apnea index(AI) and apneas + hypopneas index (AHI) were compared between the two methods by Bland and Altmen method. RESULTS: A significant correlation was found between the AHI assessed by the AutoSet and by the PSG (r = 0.92, P < 0.001). The respiratory irregularity index (RII) of the AutoSet was well correlated with the arousal index (Ai) from the PSG (r = 0.72, P < 0.001). The sensitivity and specificity of the AutoSet in diagnosing sleep apnea were 94% and 80% for AHI > or = 5/hour, 92% and 85% for AHI > or = 10/hour, 91% and 92% for AHI > or = 15/hour, 91% and 91% for AHI > or = 20/hour. CONCLUSIONS: The AutoSet system is a sensitive and valuable tool for identifying patients with sleep apnea and can make a diagnosis of sleep apnea for these patients with AHI > or = 30/hour, but has some limits especially for mild to moderate sleep apnea.

[Adrenal metastasis in primary lung cancer].

OBJECTIVE: To study the incidence, histologic types, clinical symptoms, response rates, and median survival time of adrenal metastasis in primary lung cancer. METHODS: The clinical data were retrospectively evaluated in 96 out of 6,976 patients with adrenal metastasis in primary lung cancer between January 1984 and January 1997. RESULTS: The incidence of adrenal metastasis was 1.38% (96/6,976) in 6,976 patients, 2.25% (37/1,643) in small cell lung cancer, 1.11% (59/5333) in non-small cell lung cancer. Abdominal pain and lumbago (excluding other reasons, for example, abdominal vertebrae and other abdominal organs were involved) occurred in 39% (37/96) in 96 patients. A overall response rate (CR + PR) was 43% in 54 patients who received chemotherapy (5 patients combined with irradiation) and could be evaluated. A median survival time was 7.17 (1-20) months in 96 patients. CONCLUSIONS: Primary lung cancer is easy to spread to adrenals. The incidence of adrenal metastasis is related to histologic types of primary lung cancer. Clinical symptoms include abdominal pain and lumbago. Surgical resection may be applied for a solitary adrenal metastasis after the primary tumor is removed. Chemotherapy is effective for patients with adrenal metastasis synchronous with other sites metastasis. The palliative radiation therapy can produce a high response rate in pain relief.

[A preliminary study on dental-manpower forecasting model of Miyun County in Beijing].

OBJECTIVE: To explore the dental-manpower forecasting model of Chinese rural region and provide references for Chinese dental-manpower researches. METHODS: Chose rural Miyun County in Beijing as a sample, according to the need-based and demand-weighted forecasting method, a protocol WHO-CH model and corresponding JWG-6-M package developed by authors were used to calculate the present and future need and demand of dental-manpower in Miyun County. Further predications were also calculated on the effects of four modeling parameters to the demand of dental manpower. RESULTS: The present need and demand of oral care personnel for Miyun were 114.5 and 29.1 respectively. At present, Miyun has 43 oral care providers who can satisfy the demand but not the need. The change of oral health demand had a major effect on the forecast of the manpower. CONCLUSION: Dental-manpower planning should consider the need as a prime factor but must be modified by the demand. It was suggested that corresponding factors of oral care personnel need to be discussed further.
