[Clinico-pathological features and possible pathogenesis of rifampicin-induced acute renal failure].

OBJECTIVE: To explore the clinico-pathological features and possible pathogenesis of rifampicin-associated acute renal failure (ARF). METHODS: The clinico-pathological features of 4 patients with Rifampicin-associated ARF were analyzed. Rifampicin-dependent antibody in the patient's serum was detected with Coomb's test. RESULTS: 4 cases with rifampicin-associated ARF was distinguished from 202 in-patients with ARF in 5 years(1.98%). Most of them had infection before the occurrence of ARF. Clinical features were characterized by fever, fatigue and gastro-intestinal symptoms after taking rifampicin (especially when it was reused). Oliguria or anuria developed suddenly. Laboratory data showed hemolytic anemia, thrombocytopenia and decline of renal function. Some also had impaired liver function. Renal biopsy revealed acute tubular necrosis in 3 cases and acute interstitial nephritis in 1 case. Serum rifampicin-dependent antibody was positive in all the cases. CONCLUSION: Rifampicin-associated ARF is not rare clinically, especially in those rifampicin re-exposure patients. Renal function should be closely monitored in susceptible patients. Detection of serum rifampicin-dependent antibody may be helpful to distinguish this group of ARF patients.

[Observations of properties of the L-form of M. tuberculosis induced by the antituberculosis drugs].

OBJECTIVES: To investigate the mechanism of generation of L-form of M. tuberculosis and its significance on the development, diagnosis and treatment of tuberculosis. METHODS: M. tuberculosis was inoculated into the non-high osmotic medium with rifampin, isoniazid or ethambutol and then the L-form was observed by microscopy daily. The cultures were filtrated to get the pure cultures of stable L-form by subculture with the non-high osmotic medium and characteristics of morphology, growth, susceptibilities to the antibacterial drugs and the special gene of M. tuberculosis were observed when the pure subcultures of the L-form were isolated. RESULTS: L-form of M. tuberculosis was induced by the concentrations of routine inhibition test of rifampin, isoniazid or ethambutol. The L-form would not be susceptible to the above mentioned antituberculosis drugs but susceptible to streptomycin, erythromycin, ofloxacin, norfloxacin and others. The morphologies of L-form were irregular or spherical with single, paired or chain form, and growth under the bottom of the medium but not movement or adhere to the glass. The L-form was negative by acid-fast stain and negative or positive by Gram stain. The gene of L-form reacted with the PCR kit for the M. tuberculosis and showed the same band. CONCLUSIONS: M. tuberculosis could be killed by rifampin, isoniazid or ethambutol but also could be induced to become the L-form by these antituberculosis drugs, and it is one of the important factor that affecting the effect of treatment of the tuberculosis. The cell wall deficient variants of M. tuberculosis could be determined by the PCR of M. tuberculosis. It is recommended that the L-forms should be noticed during the treatment with the antituberculosis drugs and combination treatment with antituberculous drugs to which the L-forms were susceptible, is also very important.

[Pharmacokinetics of adriamycin by bronchial artery infusion for non-small cell bronchogenic carcinoma].

OBJECTIVE: This pharmacokinetic study was conducted to determine the plasma concentration of adriamycin after bronchial artery infusion (BAI), and to compare it with the conventional chemotherapy. METHODS: Twenty-four healthy rabbits were divided into 4 groups with 6 animals in each. Comparison of the parameters was investigated in pharmacokinetics after ear-lobe venous injection of ADM (Group 1) and in combination with DDP or/and MMC (Groups 2, 3, 4). Intracardial blood samples were obtained according to the fixed time and quantity. Dose for each drug was: ADM 2 mg/kg, DDP 4 mg/kg, and MMC 2 mg/kg. Seventeen patients suffering from primary non-small cell bronchogenic carcinoma, the central type, confirmed by pathology and cytology comprised the clinical group, all of whom showing single blood supply by angiography, was given bronchial artery infusion. According to the scheduled time and quantity, we sampled blood species via indwelling catheter in the right median cubital vein after the bronchial artery infusion. Dose of the each drug was: ADM 40 mg/m2, DDP 80 mg/m2, and MMC 14 mg/m2. Plasma concentration of ADM was determined by fluorophotometry. Since the cessation of chemotherapy, blood samples were taken from the thoracic aorta via indwelling catheter at 0, 1, 3, 5, 10, 15 and 20 minutes; venous blood samples were taken from the right cubital vein through the indwelling catheter at 0, 5, 10, 15, 20 minutes, and also at 0.5, 1, 2, 4, 8, 12, 24 and 48 hours altogether at 20 time-spots with 2 ml per sample. RESULTS: The calibration curve was linear from 10 ng/ml to 1,000 ng/ml with r = 0.9983. The relationship standard difference (RSD) within a day was lowered to 2%, and the RSD between days was lowered to 3%. The formula of concentration-time curve for ADM was C microgram/ml = 129.44e(-13.36t) + 0.25e(-0.072t) in animal Group 1, and C microgram/ml = 370.93e(-23.43t) + 0.04e(-0.09t) in Group 4 animal. The formula in the clinical group was C microgram/ml(artery) = 448.61e(-66.62t) + 16.35e(-8.13t)(artery) and C microgram/ml(vein) = 15.69e(-21.66t) + 0.07e(-0.038t)(vein). CONCLUSION: Compared with the conventional chemotherapy in pharmacokinetics, bronchial artery infusion can be used as a treatment for non-small cell bronchogenic carcinoma with more effectiveness and less side effects.

[Combined high dose chemotherapy with autologous stem cell transplantation and total body radiotherapy for 32 malignant lymphoma patients].

OBJECTIVE: To evaluate the effect of high dose chemotherapy with or without total body irradiation (TBI) supported by autologous hematopoietic stem cell transplantation (AHSCT) for malignant lymphoma. METHODS: From June 1991 to April 2000, 32 malignant lymphoma (ML) patients were treated with AHSCT. Their ages ranged from 19 to 52 years (median 37). There were 23 non-Hodgkin's lymphoma (NHL) and 9 Hodgkin's lymphom (HD). Twelve patients received autologous bone marrow transplantation (ABMT) and 20 patients received autologous peripheral blood stem cell transplantation (APBSCT). In the latter, the mobilization plan was carboplatin and etoposide (CE) or CHOP (CTX, ADM, VCR, PDN) plus G/GM-CSF 10 micrograms/kg/d in high dose chemotherapy. BEAM (BCNU, Vp-16, AraC, MEL) or MEL 140 mg/m2 + Vp-16 200 mg + total body irradiation (TBI) 8 Gy. RESULTS: Up to May 2000, hematopoietic reconstitution was reestabished in 100% (32/32) patients after AHSCT, with median follow-up of 1,020 days (40-3,240). The 1- and 2-year disease-free survivals (DFS) were 78.1%(25/32) and 46.9% (15/32) with the longest survival of 8 years. Even though eight patients have developed relapse, there is no transplantation-related death. CONCLUSION: High dose chemotherapy and radiotherapy supported by autologous hematopoietic stem cell transplantation (AHSCT) for high risk, relapsed or sensitive ML, being highly efficient, should be the first indication. It is better than the standard conventional treatment. The hematopoietic reconstitution of autologous peripheral blood stem cell transplantation (APBSCT) is quicker than autologous bone marrow transplantation (ABMT). No significant difference between chemotherapy alone and chemotherapy plus total body irradiation is observed.

[Experimental study of motor nerve innervation of uvala muscle].

OBJECTIVE: To investigate the motor nerve innervation of uvala muscle. METHODS: Horseridish peroxidase (HRP) retrograde tracing technique was used in conjunction with selective intracranial severing of cranial nerve rootlets in 8 rabbies. RESULTS: Following HRP injection into uvala muscles, labeled motoneurons were located in the rostral section of nucleus ambiguus. No labeled motoneurons were found in the facial nucleus. Labled motoneurons were ipslaterally abolished after the cranial rootlets of accessory nerve on one side were intracranially severed. CONCLUSIONS: Uvala muscles are innervated by the motoneurons in the nucleus ambiguus. The axons of those motoneurons come out of the cranium through the cranial rootlet of the accessory nerve, then join the vagus, distributed via the pharyngeal branches of vagus.

[Poison effect of plant granules on Oncomelania hupensis].

Plant granules (phi 2-3 mm) made of starch, gelatin and medicine plant powder were used to kill Oncomelania hupensis. The results showed that after eating the plant granules, the mortality of the snail was 92.5%-100% within 24 hrs. The poison effect of the plant granules was much better than that of others, e.g., they could kill snails 12-24 h earlier than Niclosamidum (1 x 10(-3) g.L-1). In a field experiment, the snail mortality was up to 61.5% by using plant granules.

Different expression of estrogen receptors alpha and beta in human myometrium and leiomyoma during the proliferative phase of the menstrual cycle and after GnRHa treatment.

Uterine leiomyomas (uterine fibroids) are sex-steroid dependent benign tumors. Limited knowledge is available regarding the role of estrogen and their receptors in the regulation of fibroids in premenopausal women, and in their shrinkage after treatment with a gonadotropin-releasing hormone analogue (GnRHa). The expression of the two subtypes of the estrogen receptor (ER), ER alpha and ER beta, was studied in leiomyoma and homologous myometrium from women in the proliferative phase of the menstrual cycle and from women treated with GnRHa. The mRNA levels of ER alpha and ER beta were monitored by solution hybridization and in situ hybridization, and receptor proteins were detected and localized by immunohistochemistry. Both ER alpha and ER beta were present in the leiomyomas and homologous myometrium. The ER alpha mRNA level in the leiomyomas was higher than in the surrounding myometrium. The ER beta mRNA level was lower than that of ER alpha in both groups. ER beta immunoreactivity was lower in leiomyomas when compared with the myometrium after GnRHa treatment, while ER alpha was higher in the leiomyomas. The present results imply that the increased ratio of ER alpha/ER beta observed in the fibroids after GnRHa treatment could reflect or be the cause of the shrinkage of the leiomyoma, which is the clinical outcome of this treatment.

Response of cancer cells to molecular interruption of the CK2 signal.

Protein kinase CK2 is one of the key cellular signals for cell survival, growth, and proliferation. It is has been observed to be elevated in various cancers that have been examined. Various observations suggest that moderate dysregulation of CK2 may profoundly influence the cell response. We have examined the effects of interfering with the CK2 signal in various cancer cell lines by employing antisense oligodeoxynucleotides (ODN) against the alpha and beta subunits of CK2. Our results demonstrate that antisense CK2-alpha and antisense CK2-beta ODNs markedly influence cell viability of these cancer cells in a dose and time-dependent manner. Antisense CK2-alpha was slightly more effective than antisense CK2-beta in most of the cells tested. The efficacy of the antisense ODN seemed to vary with the cell type; however, in all cases potent induction of apoptosis was observed. Significantly, the effects of the antisense ODN on the CK2 activity in the nuclear matrix were relatively small compared to the much stronger induction of apoptosis in cells. This suggests that modest downregulation of CK2 can evoke a much greater apoptotic response in cancer cells.

Consequences of CK2 signaling to the nuclear matrix.

Protein kinase CK2 is recognized as one of the key cellular signals for cell growth and proliferation. Its nuclear targeting appears to be critical to its role in these functions. In the nucleus, nuclear matrix (NM) which plays a major role in growth-related activities is a primary locus for CK2 signaling. A variety of growth stimuli evoke a rapid translocation of the CK2 to the NM whereas removal of these factors has the opposite effect. These studies, employing various experimental models of cell growth (involving different growth-stimulatory factors), have suggested that rapid shuttling of CK2 to the NM is a key feature of early growth control. By contrast, removal of growth-stimulatory factors leading to the loss of cell viability is associated with early loss of CK2 from the NM (and chromatin). This indicates that absence of CK2 from the nuclear compartment is contributory to induction of cell death via apoptosis, implying a protective role for CK2 against cell death. Here, we review the evidence that suggests that CK2 signaling in the NM is not only involved in cell growth but also in cell survival.

[A study on expression of CD44V6 and PCNA in squamous cell carcinomas].

OBJECTIVE: To investigate the expression of cluster of differentiation 44 variant 6 (CD44V6) and proliferating cell nuclear antigen (PCNA) in squamous cell carcinomas (SCC) in eyes. METHOD: Streptavidinbiotin peroxidase complex (SABC) immunohistochemical method was used to study the expression of CD44V6 and PCNA in 35 cases with squamous cell carcinoma in eyes, 20 cases with papilloma, 11 cases with normal eyelid tissue. RESULTS: The CD44V6 positive rate was 62.9% (22/35) in squamous cell carcinomas in eyes, 15.0% (3/20) in papillomas, none in 11 cases with normal eyelid tissue. The positive expression rate of CD44V6 in the malignant group was significantly higher than that in the benign and control group, and the positive expression rate of CD44V6 in cases with metastasis was significantly higher than that without metastasis. PCNA labeling index (PI) in the CD44V6 positive expression of tumors was significantly higher than that in the CD44V6 negative expression of tumors (t = 20.21, P < 0.01). CONCLUSIONS: The over-expression of CD44V6 is correlated with the progress and metastasis of SCC. CD44V6 protein expression is correlated positively with PCNA expression, and the evaluation of both CD44V6 and PCNA is useful for predicting the prognosis.

[A study on the epidemiology of Helicobacter pyroli infection among immigrants in Three Gorges area].

OBJECTIVE: To investigate the epidemiologic features of Helicobacter pyroli (Hp) sero-prevalence rates among immigrants in Three Gorges area. METHODS: Enzyme labelled immuno-sorbant-assay was used to determine the Hp prevalence rate. A case-control study including 1 600 immigrants as cases and 1 600 non-immigrants as controls was carried out to explore the features of Hp infection in Three Gorges area. RESULTS: The prevalence rate of Hp in immigrant area was 67.50% higher than 53.19% that of Hp in non-immigrant (chi(2) = 13.40, P < 0.01). Rate of Hp infection was highest in infants than in children, increasing with age to clearly show a positive correlation between age and Hp prevalence rate (r = 0.62, P < 0.01). However, there was no marked sex difference in Hp infection rates in the immigrant area. Same results were also presented in the non-immigrant area. Hp prevalence rates were different which related to the stages of flooding. It is 89.17% in the area of first stage, while in second and third stages of flooding, the rates were 76.40% and 62.20% respectively. CONCLUSION: Preliminary result indicated that Hp infection rate of immigrant area was higher than that of non-immigrant area. It appeared that living condition, environment, education condition might possibly have played important role in Hp prevalence. It is more important to prevent from infected children. Further study with respect to Hp prevalence rate was necessary when immigration is completed and the standard of living improved.

[Study on the risk factors of psoriasis].

OBJECTIVE: To investigate psoriasis patients with family history and non-family history on their age and season of onset, disease severity and predisposing factors. METHODS: Two hundred and twenty psoriasis patients with family history and 547 psoriasis patients without, together with 647 normal controls have been investigated through questionnaires. EPI info 6.0 method was applied for data analysis. RESULTS: (1) In 220 psoriasis patients with family histories, morbidity rates of the first and second-degree relatives were 16.98% and 2.7% respectively. (2) Both mean age and age of onset were younger in females than that in males with family histories (t = 1.715 0, P < 0.05). The mean age of onset was younger in males with family histories than those without. However, there was no significant difference found in females (t = 0.739 2, P > 0.05). The mean age of onset seemed to be younger in females with family histories than that without, but with no significant statistical difference. (3) There were no significant differences in the age and season of onset, disease severity, and predisposing factors between groups with family histories and without. However, environment factors seemed to influence not only psoriasis patients with family histories but also those without, which showed significant difference (chi(2) = 0.23, P < 0.05). CONCLUSIONS: There was no significant difference in the following aspects as season of onset, disease severity and predisposing factors between the ones with family history and those without, except the age of onset and age of average. The results showed that genetic factor might play an important role in psoriasis patients with family history. It also demonstrated that psoriasis is a multifactor hereditary disease.

[Experimental research of laser-induced collaterals in retinal vein occlusion].

OBJECTIVE: To evaluate the feasibility and safety of establishing chorioretinal venous anastomosis in rabbit eyes with a model of branch retinal vein occlusion METHODS: By using laser photocoagulation, a chorioretinal venous anastomosis was created in rabbit eyes in which a branch retinal vein occlusion had previously been established photochemically. A similar attempt to create an anastomosis was made in the control eyes in which no branch vein occlusion was present. RESULTS: In the 21 eyes with the model of branch retinal vein occlusion, a chorioretinal venous anastomosis formed in 4 eyes within 3 to 5 weeks. In the control eyes, an anastomosis in one eye presented by 5 weeks, during follow-up of 2 to 5 months. The hemorrhage in choroid, retina and vitreous associated with laser photocoagulation were absorbed within 3 weeks, and no other severe complications were found. CONCLUSIONS: Chorioretinal venous anastomosis can be created by using laser photocoagulation. This technique is relatively safe. However, to improve the rate of successfully creating an anastomosis, more research work needs to be performed.
