Solid-phase enzyme immunoassay for immunoglobulin M antibodies to cytomegalovirus.

A sensitive, solid-phase enzyme immunoassay for the detection of immunoglobulin M antibodies to cytomegalovirus is described. The results of the enzyme immunoassay correlated well with those obtained by an indirect immunofluorescence method. Horseradish peroxidase proved to be a more sensitive label than alkaline phosphatase. Nonspecific reactions, occurring with commercially available cytomegalovirus antigens, could be avoided by using a nuclear antigen prepared from sonically disrupted nuclei of cytomegalovirus-infected cells.

Simple mathematical deductions in the seroepidemiology of viral infections. I. Herpesvirus group (herpesvirus hominis, varicella-zoster virus, cytomegalovris, Epstein-Barr-Virus).

Large samples of nonselected persons collected in South-West Germnay were investigated for the prevalence of serum antibodies to the human Herpesviruses HSV, VZV, CMV and EBV. According to "catalytic models", which compare the infection spread to simple chemical reactions of molecules, a mathematical approximation of the serum surveys was performed. Through a new deduction of the exponential function y = k (l - e-r+) a simple way was found to estimate the annual attack rates in percent of the susceptible, seronegative people. While it was possible to represent the prevalence of serum antibodies to VZV by a continuous curve, a biphasic curve to the antibody prevalence rates in the epidemiology of the other Herpesviruses proved to be more adequate indicating changes in hormonal balance and social behaviour. The use of the epidemiologic parameters k and r for the characterization of the test sensitivity was examined for CMV. By evaluation of the mean antibody titres to CMV, HSV, and EBV throughout different age groups, information about the reactivation of the Herpesvirus diseases could be obtained.

Simple mathematical deductions in the seroepidemiology of viral infections. II. (Para) myxoviruses (measles, mumps, influenza B), rubella, enteroviruses (polio, coxsackie B), adenoviruses, and mycoplasma pneumoniae.

Large samples of nonselected persons collected in South-West Germany were investigated for the prevalence of serum antibodies to Poliovirus 1-3 and Coxsackievirus B 1-5 (neutralisation test), to Measles, Rubella, and Mumps (hemagglutination inhibition test), Mumps, Influenza B, Adenovirus, and Mycoplasma pneumoniae (complement-fixation test). According to "catalytic models", which compare the infection spread to simple chemical reactions of molecules as self-limiting procedures, a mathematical approximation of the serum surveys was performed. By the use of only two parameters it became possible to calculate the annual attack rates (without regard of age group arrangement and test sensibility), to construct "true" epidemic curves, and to estimate the persistence of humoral immunity in the population investigated for NT and HIT antibodies.

Neutralizing serum IgM antibodies in infections with Herpes simplex virus hominis.

Forty-two patients with herpes simplex virus infections were tested for neutralizing serum IgM antibodies. A technique in which serum antibody fluorescein staining was combined with sucrose gradient centrifugation facilitated the isolation of the serum IgM fraction for the use in neutralization tests. In nearly all cases with primary infection, especially those presenting heavy clinical signs (encephalitis/meningitis) the IgM tests were positive. In one case we could detect the IgM antibodies for 11 weeks after the onset of the illness, in another case in cerebrospinal fluid samples for 6 weeks. In localized herpes infections, which were mostly due to reactivations, serum IgM antibodies could only rarely be demonstrated. Among the serologic tests used in this study (NT, CFT, IFT, ACIFT), only the CFT beside the NT (with certain reservations) can be applied for subtyping HSV serum antibodies.

[Use of nekal BX for pretreatment of clinical material before culture to diagnose tuberculosis (author's transl)].

Recently, also detergents have been proposed for use in the pretreatment of clinical material for cultural demonstration of Mycobacterium tuberculosis. One of such substances is the anionic detergent. Neckal BX (diisobutyl naphthaline sulphonate) which in combination with 0.5% sodium hydroxide, is capable of homogenization and decontamination of the material within 16-18 h of action. At the same time, the mycobacteria are accumulated in the sediment by precipitation in the presence of barium, calcium, and phosphate ions. The results obtained by the new method were compared with those obtained by the sulphuric acid method, in a paralles study of samples of sputum, gastric juice, and faeces and a second one of sputum samples only. By using two different formulas for reagents (Table 1), the mycobacterial isolation rate was shown to be dependent upon the concentration of the precipitant. By the following criteria, the Nekal method was superior over pretreatment with sulphuric acid: 1. The reduction of the number of contaminated was obvious in the cases of sputum and gastric juice samples and significant for sputum samples. (Tables 2-7). 2. Using Nekal BX, 31 out of 616 sputum samples were found to be positive; using sulphuric acid, their number was only 22. This difference was found to be statistically significant. The additional yield came primarily from material containing only few mycobacteria and samples which could not be assessed because of contamination present after pretreatment with sulphuric acid (Table 8). The average period which passed until reading of the cultures was approximately the same: 4.2 weeks for sulphuric acid and 4.4 weeks for Nekal (Table 9). When applying the new method, the material admits of mechanical shaking and need not be centrifuged. No strict control of the period of action is required. Taking into account these operational advantages, the Nekal method is considered particulary suitable for laboratories receiving high numbers of samples.

Envelope and nucleocapsid antigens of Cytomegalovirus (CMV).

A method for the purification and concentration of Cytomegalovirus nucleocapsids and particles complete with envelope is described. Antibody to either nucleocapsid or envelope antigens can be detected in human sera. In patients with CMV disease the antibodies to the envelope antigen could not be detected before the 3rd week after the beginning of disease whereas antibodies to the nucleocapsid were detected earlier.

[Investigations on immunity against poliomyelitis (author's transl)].

Six virus laboratories from various parts of the German Federal Republic and West Berlin undertook investigations into polio-myelitis immunity in 1972. A total of 267 persons aged up to 20 years were investigated for neutralizing antibodies against the three types of polio virus. The study showed that from the fourth year of life approximately 70% of the persons investigated had antibodies against all three types of polio virus. In some younger children the equivalent values were considerably lower. After the fourth year of life there were only slight differences in the humoral immunity against the three polio virus types. There was no evidence that the immunity level had deteriorated up to the 20th year of life and thus there is no indication of the necessity for a repeat immunisation at a certain time. The investigation confirmed that three oral administrations of trivalent vaccine, as are now generally recommended as basic immunization in the German Federal Republic, result in a conversion rate of over 90% against each of the three virus types.

Rapid method to detect rubella immunoglobulin M and immunoglobulin A antibodies.

Immunoglobulin (Ig) G was removed from serum specimens by precipitation with gamma chain-specific anti-human IgG of rabbit origin. The remaining rubella virus-specific IgM (and IgA) antibodies were then detected by the rubella hemagglutination-inhibition test. This procedure has proven to be as reliable as estimations carried out with IgM fractions separated on a sucrose density gradient.

[Quantitative and virus specific serum IgM detection in newborns (author's transl)].

Serum IgM concentration was determined in umbilical cord blood samples from 1000 newborns. 34 cases presenting elevated IgM values (greater than or equal to 30 mg%) were tested for virus specific IgM antibodies. We could demonstrate such virus specific IgM antibodies against Cytomegalovirus (4), against Rubella, Influenza A and Influenza B (2 each) and against Coxsackie virus (1). The quantitation of IgM in blood samples of child bearing women showed no elevated values compared to those of non-pregnant women of similar age groups.
