Generalization and simplification of the Moore-Goodman test for significant of alignment homologies.

A test given by Moore and Goodman (1977) that checks the significance of a homology between protein sequences is generalized to any type of distance measure and to any classification of codon pairs or amino acids according to this measure. A normal distribution is shown to approximate satisfactorily the probability distribution underlying this test for the examples chosen, and supposedly for any distance measure. The test accordingly reduces to a simple standard procedure.

[Cleido-cranial dysplasia--two observations].

The radiological changes of cleido-cranial dysplasia are described on the basis of two cases. Special attention is paid to the dental and facio-maxillary aspects. The main features are: (1) Supernumerary teeth, persistence of the first dentition, retention of the second dentition. (2) Open fontanelles and sutures and intercalary bones. (3) Hypo- or aplasia of the clavicles. (4) Defects in the vertebral neural arches and (5) dehiscence of the symphysis due to absent or delayed ossification of the pubis and ischium.

[Anti-reflux plasty for oesophageal varices (author's transl)].

An operation to prevent reflux was carried out in 36 patients with oesophageal varices; its aim was to prevent ulceration of the oesophageal mucosa due to gastric or duodenal secretions. The anti-reflux plasty consists of a distal gastrectomy with orthograde, isoperistaltic interposition of 20 cm. of jejunum between the gastric remnant and the duodenum, and fixation of the fundus and fornix. During the first three months, and even somewhat later, there was a little passage of contrast from the interposed jejunum into the stomach and duodenum. The reflux from the duodenum was, however, very small and only reached the vicinity of the stoma so that a duodeno-gastric reflux could be practically excluded. The fundus-fornix fixation resulted in absence of radiologically demonstrable gastro-oesophageal reflux. The interposed jejunal loop took over in part the function of the stomach, regulated gastric emptying and acted as a valve during upper gastro-intestinal transport.

[Radiation dose and risk in gastrointestinal roentgendiagnostic of the child (author's transl)].

In gastrointestinal roentgendiagnostic of the child the exposure of the skin and the testis depends on body height and body weight. The dose grows with the age. The squaredose product (Rcm2) and the energy absorbed by the body (integraldose, rdkg) increases comparably. On the other hand the quotient integraldosis/body weight is similar for the different age-groups. The radiation risk (the probability to die as consequence of a roentgenray induced disease) is approximately 1:100,000 to 1:10,000,000.

[Recurrency in primary duodenal carcinoma (author's transl)].

18 months after the operation (duodenectomy and partial pancreatectomy) a tumor recurrency is observed in a 64 years old patient with primary adenocarcinoma of the duodenum. The roentgen diagnostic shows a partial destruction of the upper jejunal loops combined with a free passage.

Adenocarcinoma↗

[Duodenogastric reflux in roentgenexaminations (author's transl)].

Duodenogastric reflux (DGR) favours gastritis, ulcer and carcinoma of the stomach, as well as bleeding from oesophagealvaricosis. DGR is seen regulary in roentgenexaminations of the operated stomach--with exception of selectiv proximal vagotomie--, and in approximately 30-50% in roentgenexaminations of the non operated stomach. Quantification of the DGR is possible by intraduodenal application of Barium or Jodcontaining substances via transpyloric tube, endoscope or intracholedochal T-tube, and cinematographical and electronic registration of the roentgenexamination.

[Peritoneo-venous ascite drainage using the Le Veen shunt].

Ascites drainage by using the Le Veen shunt should be attempted after other treatments have failed. A silicon catheter connects the intraperitoneal space with the vena cava superior via a jugular vein. According to our own observations, misleading may occur during the insertion of the venous part of the catheter into the vena jugularis externa. Later, a thrombosis may occur. Intra- or postoperative roentgen controls for corrections or replacements seem advisable.

[Roentgen examination of the operated small bowel (author's transl)].

After operations on the small bowel roentgenology shall help in the diagnosis of complications (suture insufficiency, peritonitis, hemorrhage, ileus), shall analyse the modified function and shall identify reappearing tumors or Crohn's disease. Watersoluble jodated substances are preferable during the first days. Later barium suspensions permit to obtain pictures of good quality. Middle and lower segments of the small bowl are best analysed with duodenal intubation. Pneumocolon gives often the best results in controlling ileocolic anastomosis.

[Annular pancreas (author's transl].

Eight cases of annular pancreas treated at the University Hospital of Hamburg between 1964 to 1977 are presented. In adults, the disease follows the pattern of chronic duodenal obstruction being often asymptomatic unless complications such as peptic ulceration, acute pancreatitis or bile duct obstruction occur. Preoperative diagnosis is only possible by using endoscopic retrograde pancreatography. Attention is drawn to the frequent co-existence of other malformations of the gastro-intestinal tract. Treatment should consist of relief of any obstruction to the duodenum by a bypass gastrojejunostomy is recommeded in those cases having associated peptic ulcer. The division or resection of the annulus bears the risk of fistula formation. A brief review of the theories of the development of annular pancreas is given.

[Small-angle X-ray-scattering investigation and structural-model study of the fatty-acid synthetase from pig liver (author's transl)].

The structure of the fatty acid synthetase from pig liver was studied on models based upon structural and functional properties selected from pertinent results available from numerous investigations carried out with fatty acid synthetases from this and other sources. When comparing small-angle X-ray-scattering curves calculated with these models and curves obtained from small-angle X-ray-scattering experiments carried out with the pig-liver enzyme, we tried to select a model which would lead to an acceptable correlation between the calculated and the experimental curves and at the same time fulfil the known structural and functional requirements. The comparison of the curves was started with a model of low complexity. The observed discrepancy, together with arguments from the structural and the functional properties, helped decide which is the next most reasonable model to be considered. This procedure was repeated for five models of increasing complexity. In the model which led to the best fit the multienzyme complex is composed of two halves in an assymetric conformation including hollow spaces. This highly anisotropic model would imply that the two halves change their conformation each time a synthetic cycle is completed and that the growing fatty acid is handed over from one half to the other.
