Traumatic luxation of the stapes.

A case of traumatic middle and inner ear damage due to involuntary piercing of the tympanic membrane with a knitting needle is reported. The incudo-stapedial joint was dislocated and the intact stapes was depressed into the vestibule. Exploration of the middle ear, followed by extraction of the stapes, closure of the oval window with a vein graft and interposition of a prosthesis between the incus and oval window, improved the condition but did not restore inner ear function. A second case, of unclear etiology, is also presented. The indications, timing and technique of surgical intervention in cases of traumatic luxation of the stapes are briefly discussed.

[Hereditary angioedema. A hereditary disorder in the synthesis of the complement system].

Hereditary angioedema is characterized by recurrent attacks of painless, non itching edema of the face and limbs and sometimes by abdominal symptoms. It is due to a deficiency of functional inhibitor of the first component of complement (C1 Inh). We present a case where a normal antigenic level of C1 Inh was found but no functional activity was present (B variant). A short review is given of the pathogenesis, heredity, diagnosis and treatment of the disease.

[Hundred cases of general anesthesia for laryngoscopy and/or bronchoscopy, in children under five years age (author's transl)].

The authors describe an anesthetic technique used for endoscopies (laryngo-tracheo-bronchoscopies) in 100 children under five years age. Three different apparatus for jet ventilation are used: manual injection, automatic injection and high frequency positive pressure ventilation. The first apparatus is home made. It allows control of insufflation of O2 by manual compression of a gun type injector. With the second one (Wolf injectomat), injection of O2 or O2/N2O is automatic. The aga bronchovent is used for high frequency positive ventilation with O2.

[Endoscopy of the upper respiratory tract].

Instruments for endoscopy of the upper airways can be divided in two main categories : rigid and flexible. Rigid endoscopes are of very good quality since the introduction of Hopkins (Storz) and Lumina (Wolf) optics. These exist in several lengths and diameters and with different visual angles. Apart from mere visual observations, small diagnostic and even therapeutic procedures can be carried out under endoscopic control. A flexible scope of small dimensions permits a complete visualisation of the entire nasopharyngolarynx, especially in children.

[Aspergillosis of the maxillary sinus].

A new case of aspergillosis of the maxillary sinus is presented. The symptoms were those of a chronic unilateral sinusitis, which did not respond to conventional treatment. The diagnosis was suspected at sinoscopy and subsequently confirmed by histo-pathological examination. A Caldwell-Luc operation was performed and proved curative. A review is made of the literature and the epidemiology, symptoms, diagnosis and treatment of the different forms of paranasal sinus aspergillosis are discussed.

The temporal bone as route of infection in recurrent meningitis.

Three successive episodes of recurrent meningitis in connection with an infection of the upper respiratory tract appeared to be related to a congenital idiopathic oval window CSF leak. An inner ear dysplasia was the underlying factor and accounted for the deafness and for the absence of signs of acute labyrinthis during these infectious episodes.

[Important aspects of brain stem audiometry (BERA) with regard to the diagnosis of acoustic nerve neuroma].

Starting with a case report, the interest of the BERA for the diagnosis of acoustic tumor is examined. Moreover, factors being able to complicate a clear interpretation of the result (transmissional and sensorineural dysfunctions) are discussed. The conclusion is drawn that the E. Coch. G. offers a factor of greater precision in the use of the BERA as an oto-neurological diagnostic tool.

[Cogan's syndrome].

Since Cogan described his first case in 1945, about 50 cases have been published in the world literature, A 23 year old patient who suffered from this disease led us to make a detailed study of previous publications. The cochleo-vestibular, the etiological and the ophtalmical aspects of the case were studied, and, also the differential diagnosis and the therapeutical possibilities.

Panendoscopy for second primaries in head and neck cancer.

Four hundred patients with biopsy-proven squamous cell carcinoma of the upper aerodigestive tract were studied for evidence of a synchronous second primary tumor. The protocol of study included in most cases direct laryngoscopy, metal tube bronchoscopy, and esophagoscopy. Chest x-ray studies were performed on all patients and many had barium swallow x-ray studies. Asymptomatic second head and neck primary tumors were found only in the vallecula and esophagus among this series of patients. Metachronous second primary tumors were presented as early as one month after the original endoscopy and were located in areas which had been carefully examined by endoscopy and appeared asymptomatic to the examining physician at the time of the original evaluation. Based on this series of patients, we recommend careful indirect laryngoscopy and oral examination, direct laryngoscopy and esophagoscopy as well as a barium swallow for patients with clinically suspicious mucosal lesion of the upper aerodigestive tract. In view of our failure to discover a single lung carcinoma which was not evident from chest x-ray studies, we recommend bronchoscopy, which adds some morbidity to the endoscopic procedure, not be included unless a specific clinical suspicion is entertained. However, because of the important treatment implication which follow the diagnosis of a new squamous cell carcinoma of the upper aerodigestive tract, we feel it is imperative that the entire esophagus be evaluated in the course of the pretreatment exploration of the patient. Although few esophageal tumors were discovered, their implications in terms of surgical resection and debilitation without much likelihood of long-term patient survival has encouraged us to continue the above described procedure.

Recombinant humane thyrotropin (rhtSH) a new aid in the diagnosis and treatment of thyroid carcinoma with radio-iodine.

By example of two cases a newly available tool, the recombinant humane thyroid stimulating hormone (rhTSH) is discussed in its use as an adjunct in diagnosing and treating differentiated thyroid carcinoma with radio-iodine. This product is an easier and safe alternative to the necessary rise of TSH induced by thyroid hormone withdrawal and concurrent hypothyroidism. Although discrepancies in favour of the classical approach have been demonstrated in some patients, the clinical relevancy of these must be weighted against the advantage of avoiding undesirable symptoms and possible tumour growth by the use of rhTSH. The role of rhTSH in the follow-up diagnosis and radioiodine treatment of thyroid carcinoma will undoubtedly increase in importance.

Phonosurgery in gender dysphoria.

A particular type of phonosurgery is voice change surgery in cases of gender dysphoria. This paper presents a review of the surgical techniques available for raising or lowering speaking fundamental frequency. A cricothyroid approximation to raise the vocal pitch is the most common procedure. This procedure results in an increase of about one octave immediately postoperatively and about half an octave after stabilization of the voice.

Orbital fractures.

In this paper the importance of the clinical, ophthalmological and radiological examination in the diagnosis of orbital fractures is stressed. Indications for operative treatment can be esthetic or functional, such as enophthalmos or diplopia. A plea is made for early diagnosis and treatment. An open approach is used for all displaced fractures in order to obtain an anatomical reduction. Currently, miniplates and screws are used more frequently for stabilization. Defects in the orbital floor should be covered preferably with autogenous bone or, if unavailable, with bovine cartilage or some synthetic material.
