Pars plana vitrectomy in aphakic and pseudophakic retinal detachment.

Eighty-two consecutive aphakic or pseudophakic retinal detachments were treated by buckling and vitrectomy procedures. Functional results (86.6%) and the incidence of proliferative vitreoretinopathy (11%) were comparable with reports of cases treated by the classic method.

Aphakia↗

[Etiology of endophthalmitis].

Sixty p. cent of endophthalmitis were seen after recent ocular surgery. Another 14% were seen after surgery performed some years before. Non perforating trauma were responsible for 9.5%, and another 9.5% were seen complicating old ocular lesions by chemical, mechanical or infectious agents. Only in 7% the etiology was endogenous in one case general sepsis by meningococci, in another case, mycotic infection after prolonged systemic cortisone treatment, and one case in which endophthalmitis remained unexplained. Most cases complicating recent ocular surgery were seen after cataract extraction.

Eye Injuries↗

Treatment of endophthalmitis with and without pars plana vitrectomy.

Two series of patients with endophthalmitis were compared. In the group treated with antibiotics or fungistatics, only 1 eye (4.3%) could be salvaged, whereas in the group with pars plana vitrectomy and antibiotics or fungistatics, 7 eyes (33%) had adequate visual acuity afterwards.

Pars plana vitrectomy in aphakic retinal detachments.

Thirty-three cases of aphakic retinal detachment have been treated by pars plana vitrectomy and retinopexy. This procedure allowed us to lower the incidence of postoperative massive periretinal proliferation from 15,4%, in a series that was performed by a classical method, to 6% in the pars plana treated group.

Aphakia↗

Retinal phototrauma during intra-ocular lens-implantation.

The use of the operating microscope has become almost a necessity in the surgery of the anterior segment of the eye. However the coaxial light of the operating microscope is potentially hazardous for the retina, especially during intra-ocular lens implantation. The risk of producing a photic retinopathy increases if the duration of the surgical procedure is prolonged once the pseudophakos is in the eye and if high levels of illumination are used. We encountered our first case in a patient, whose intervention was filmed and where thus an increased illumination was needed. This complication is however also found in cases where no extra illumination was needed. As the clinical signs are usually very mild, the retinal lesions related to over-exposure to light are easily overlooked.

Scopolamine intoxications.

Pure scopolamine intoxications are extremely rare. We treated a series of severe intoxications exclusively caused by scopolamine and due to the intentional mixing of pure scopolamine into drinks. The clinical course and therapy are reported. On the basis of our experience and a survey of literature, we found physostigmine to be an excellent antidote. The symptomatology of the patients confirms that scopolamine has a dose-related stimulant effect on the central nervous system.

Five years pars plana vitrectomy.

Between November 1976 and October 1981, 516 pars plana vitrectomies have been done with the vitreous stripper of Klöti. The indications and the results are discussed.
