Diagnostic problems in chronic vitreous inflammation.

We reviewed 188 patients with chronic vitreous inflammation initially diagnosed as intermediate uveitis, who were seen at the Ghent Eye Clinic during the period 1978 to 1988. We were able to establish a more specific diagnosis in 55 patients. The different approaches that contributed to the diagnosis were analyzed, (comprehensive history-taking and clinical examination, laboratory tests and special investigations, vitrectomy, and following the evolution of the disease). No etiology was found in 133 patients (70.7%), subsequently diagnosed as idiopathic intermediate uveitis. The major characteristics of the disease were analyzed.

Persistent hyperplastic primary vitreous.

Case report of a pars plana surgery for persistent hyperplastic primary vitreous (PHPV) with a clear lens. This situation enabled us to show the important features of PHPV: the initial small but clear lens, the fibrovascular membrane giving rise to the leukokoria, the elongated processi ciliares visible in the pupil and a patent A. hyaloidea, feeding the retrolental fibrovascular membrane. The surgery includes removal of the lens, the fibrovascular membrane and the endocoagulation of the feeding vessel in order to interrupt the natural cause of PHPV leading to blindness. After the surgery the baby is adapted with a soft contact lens and an occlusion treatment is started.

The value of medical imaging techniques in the diagnosis of extra-ocular extension of malign melanoma of the choroid: a case report.

The major goal in the evaluation of posterior uveal tumours is to determine whether an intra-ocular malignancy is present and whether intervention is necessary. Small uveal pigmented tumours can safely be managed by observation until growth is documented. However, extensive evaluation for each suspicious lesion is necessary in order not to miss any signs of malignancy. A case is presented of a 41 year old male patient with orbital pain as initial symptom of a choroidal melanoma. Fluorescein-angiography revealed signs of double circulation. Abnormal choroidal vessels inside the tumour showed up clearly on indocyanine-green angiography. By ultrasonography we were able to detect scleral perforation and extra-scleral extension. Computed tomography confirmed the extra-scleral extension. As there were no signs of metastasis, an orbital exenteration with sparing of the eyelids was performed. Histopathology is compared with the results of clinical investigation.

Intranasal caffeine and amphetamine causing stroke.

A 37 year-old man with an ischaemic stroke after the nasal use of amphetamine and caffeine is reported. Transient arterial hypertension due to these agents may have been the mediator of the stroke. Mitral annular calcification was the only other abnormality found, and was thought not to play an important role in this patient. There was no evidence of a primary or secondary hypercoagulable state. Stroke due to nasal use of these agents appears not to have been previously reported (Medline literature search 1983-1993).

Diuretics in the treatment of hypertension.

With 30 years of experience, it is possible to claim that diuretics are well tolerated antihypertensive drugs producing a significant decrease in systolic and diastolic blood pressure as good as obtained with beta-blockers, converting enzyme inhibitors, calcium antagonists and centrally acting drugs. Recently diuretics have been criticised because of a substantial number of important side-effects, and are disregarded by some authors as first-line agents for the treatment of mild hypertension. Diuretics seem however safe in the majority of hypertensive patients who do not present special problems. Using low doses, which are equally effective, most toxic properties can probably be avoided. Combination therapy, especially with ACE inhibitors, has not only additional effects, but lowers also the potential adverse effects, caused by diuretics. We are convinced that diuretics are still important as first-line agents for the treatment of arterial hypertension in the general population.

Visceral leishmaniasis in an adult with HIV infection.

We present the first case of visceral leishmaniasis (VL) in a Spanish patient with HIV infection living in Belgium. After four weeks of stibogluconate and zidovudine treatment, the initially low CD4 count improved, and the splenomegaly regressed. VL is becoming frequently reported in association with HIV infection, especially in countries where leishmaniasis is endemic. The apparent effect of VL on the CD4 count may cause problems in the staging of HIV infections.

Bacteriological study of 92 cases of proven infectious endophthalmitis treated with pars plana vitrectomy.

92 cases of proven infectious endophthalmitis underwent pars plana vitrectomy first to identify the responsible agent and directing the antibiotherapy, second to remove the abscess and third to make room for intravitreal injection of antibiotics. A wide spectrum of organisms was isolated (16 different agents). Prognosis of endophthalmitis depends on (1) the responsible agent and (2) the reason for developing the endophthalmitis. The prognosis after lens implantation was rather favorable, whereas after vitrectomy the results were disastrous. Good results were obtained if the infecting agent was Staphylococcus epidermidis. Results after Streptococcus, Staphylococcus aureus, Bacillus or Pneumococcus were bad. Of the mycotic cases, only half of the Candida cases were cured. Other mycotic infections had a worse outcome.

[Retinal detachment following blunt trauma].

In the first part of this study a description of the different types of ocular trauma is given. This is followed by the pathogeny of retinal detachment after blunt trauma. Then the specific characteristics of the tears and other signs of non-perforation traumatic retinal detachment are discussed. Finally a description is given of the operative technique used in those cases.

Malignant melanomas of the choroid and ruthenium applicators.

Fourty patients with a malignant melanoma of the choroid were treated with ruthenium applicators. Depending on the height of the tumour, the patients were divided into 3 groups. In the first group with a tumour height superior to 6 mm the results were poor as well from a functional point of view as considering tumour regression. In a second group with a tumour height between 5 and 6 mm the results were favourable concerning tumour regression, but only in 50% of the cases did the eye remain functional. The results in the third group with a tumour height between 2 and 5 mm were satisfactory. Tumour regression was obtained in 96% of the eyes; 86% of the eyes remained functional. Tumour related death (7.5%) was not correlated with initial height of the tumour at the time of diagnosis.

HLA-B27 positive idiopathic acute anterior uveitis: a unique manifestation of subclinical gut inflammation.

Ileocolonoscopy was performed on 2 HLA-B27 positive patients with idiopathic acute anterior uveitis neither of whom had articular or intestinal complaints. Microscopic examination of the biopsy specimens revealed the presence of chronic inflammatory lesions in the ileum. Since these lesions have also been described in other forms of spondyloarthropathy, we postulate the presence of gut inflammation was related to the uveitis.

High field resolution magnetic resonance imaging of malignant choroidal melanoma.

High resolution surface receiver coil nuclear magnetic resonance imaging (MRI) of the eye and paraorbital areas was performed in a patient with a malignant choroidal melanoma before and after local radiotherapy using ruthenium application. The resulting improvement in signal-to-noise ratio allows for a reduction in the imaging voxel size to 0.31 X 0.31 X 3 mm3 which provided useful morphological information with respect to delineation of tumoral mass. In the same MRI session using a standard head coil imaging method, T1 and T2 relaxation time values were calculated in regions of interest of 3 X 3 mm by means of transversal scan images of orbital and cranial tissues performed in scan times of 7 to 9 minutes. The procedure presented here combines in one single session high resolution imaging performance, previously unobservable in MRI and in other noninvasive imaging techniques, with fast and reliable measurement of T1 and T2 relaxation times within small regions of interest. MRI, a non ionizing technique, offers within reasonable acquisition times not only a good multiplanar description of the site, size and volume of normal and pathologic orbital structures, but, in addition, the prospect of soft tissue differentiation and of accurately documenting the response to and side effects of the therapy of ocular tumors.

Actinobacillus (formerly Pasteurella) ureae meningitis and bacteraemia: report of a case and review of the literature.

A 26-year-old man addicted to alcohol was admitted to hospital with headache and rhinorrhoea. Investigation revealed Pasteurella ureae meningitis and bacteraemia. A course of intravenous cefotaxime and penicillin G, followed by surgical correction of a nasocranial fistula, led to full recovery. Fourteen cases of serious extrarespiratory infections due to P. ureae are briefly reviewed.

Treatment of endophthalmitis by pars plana vitrectomy.

Between 1976 and 1985 81 cases of endophthalmitis have been treated by pars plana vitrectomy and intravitreous injection of antibiotics or antimycotics. The functional results and the etiology of the endophthalmitis are discussed, as well as the reasons to perform a vitrectomy in endophthalmitis, and a possible explanation for the relatively high incidence of pseudophakic endophthalmitis is given.

Paraneoplastic Sjögren's syndrome.

A patient with primary Sjögren's syndrome preceding an oat cell carcinoma of the lung, is presented. Arguments to support a possible relationship between these two disorders are discussed.
