Measurement of Gly m Bd 30K, a major soybean allergen, in soybean products by a sandwich enzyme-linked immunosorbent assay.

By a sandwich enzyme-linked immunosorbent assay, a soybean major allergen, Gly m Bd 30K, in soybean products was measured. The allergen occurred at high concentrations in soy milk, tofu, kori-dofu, and yuba, but its content in kinako was small. No allergen was found in fermented foods such as miso, shoyu, and natto. The allergen was clearly shown to occur in meat balls, beef croquettes, and fried chicken that contained soybean protein isolate.

Immunochemical crossreactivity between globulins from buckwheat and indigo seeds.

Immunochemical relationships between salt-soluble proteins (albumins plus globulins) from buckwheat and indigo seeds were shown by immunoblot and immunodiffusion analyses using rabbit antisera raised against buckwheat globulins. These antigenic crossreactivities were roughly consistent with their polypeptide components judged by two-dimensional electrophoresis and SDS-PAGE analyses.

Alpha-subunit of beta-conglycinin, an allergenic protein recognized by IgE antibodies of soybean-sensitive patients with atopic dermatitis.

One of the major allergenic proteins in the soybean 7S-globulin fraction, which was recognized by sera of about 25% soybean-sensitive patients with atopic dermatitis, was identified as alpha-subunit of beta-conglycinin. The IgE antibodies recognizing the alpha-subunit did not show a cross-reactivity against both alpha'- and beta-subunits known to be highly homologous to alpha-subunit, and also against other allergenic components identified in soybeans [Ogawa et al., J. Nutr. Sci. Vitaminol., 35, 555-565 (1993)]. The IgE-binding site(s) was estimated to be located on the peptide 232-283 of alpha-subunit.

I-123 metaiodobenzylguanidine cardiac scintigraphy in patients with an implanted permanent pacemaker.

T1 scintigraphic abnormalities have been reported in patients with an implanted permanent pacemaker, but little is known about the MIBG scintigraphic findings in such patients. This study was performed to assess the MIBG scintigraphic findings in patients with an implanted permanent pacemaker, and to test the hypothesis that imaging characteristics of MIBG scintigraphy differ according to its mode. Twelve patients (4 men and 8 women, mean age: 72.4 +/- 9.5 years), who had undergone the implantation of a permanent pacemaker for bradyarrhythmias, underwent MIBG scintigraphy. The patients were divided into VVI pacemaker and DDD pacemaker groups. The tomograms were divided into nine segments and the MIBG defect in each segment scored on a scale ranging from 0 (normal uptake) to 3 (no uptake). Total MIBG defect scores were generated by summing the scores for the nine segments in each patient. MIBG scintigraphic abnormalities were found in ten of the twelve patients. The six patients with the VVI pacemaker manifested MIBG scintigraphic abnormalities. These MIBG scintigraphic abnormalities were observed in all segments, particularly in the posterior segments. The mean total defect score of the VVI group was higher than that of the DDD group (14.8 +/- 9.8 vs 3.0 +/- 3.5, respectively p < 0.05). Therefore, we conclude that despite several limitations of the study, MIBG scintigraphic abnormalities occur in patients with implanted permanent pacemakers, and that such abnormalities are more prominent with the VVI than DDD pacemaker.

Results of carinal resection for bronchogenic carcinoma.

Fifteen patients underwent carinal resection for lung cancer during the years of 1957 through 1993. All were males with ages ranging from 40 to 81 (mean 60.1) years old. Cell Types of the cancer were squamous cell carcinoma in 12 cases, adenocarcinoma in 2 and small cell carcinoma in one. In 14 of 15 cases there was cancerous invasion into the tracheal carina (T4) and one had no involvement of the carina (T3). Four patients had no lymph node metastasis (N0), 2 had hilar node metastasis (N1), 8 had ipsilateral mediastinal node metastasis (N2) and one had contralateral mediastinal node metastasis (N3). Operative procedures for carino-plasty were sleeve pneumonectomy in 3 cases, pneumonectomy with wedge resection of the carina in 3, sleeve resection of the carina followed by reconstruction of the carina (Montage type) in 8, and carinal resection with reconstruction of the one-stoma type in one. Two patients died within one month due to pneumonia or renal failure (13.3%). Postoperative complications occurred in 14 cases and bronchorrhea (n = 8), anastomotic stenosis (n = 5) and pneumonia (n = 4) were frequent. Excluding 2 operative deaths, 8 cases died from 2 to 37 months after operation (mean 13.1 months) and 5 are alive from 10 to 24 months without recurrences. Patients with squamous cell carcinoma with N0 or N1 had better prognosis.

Regional lung function following upper sleeve lobectomy for bronchogenic carcinoma.

Regional lung function of the operated side following upper sleeve lobectomy (n = 8) and simple upper lobectomy (n = 7) for lung cancer was evaluated. Regional ventilation was studied with Krypton 81m and regional pulmonary blood flow was studied with Technetium 99m. Measurements were taken from 12 to 24 months after operation. The ventilation rate of the operated side following right upper sleeve lobectomy (n = 5) was 42.9 +/- 6.7% and the perfusion rate was 37.4 +/- 4.6%. The regional ventilation rate of the operated side after simple right upper lobectomy (n = 3) was 45.9 +/- 10.5% and the perfusion rate was 46.2 +/- 5.2%. For the patients with left upper sleeve lobectomy (n = 3), these ratios were 29.9 +/- 11.1% and 19.2 +/- 3.6%, respectively. For the patients with simple left upper lobectomy (n = 4), these ratios were 27.3 +/- 4.5% and 22.3 +/- 3.3%. There were no statistical differences between the group with upper sleeve lobectomy and that with simple upper lobectomy. Regional function improved gradually with time. In conclusion, the regional lung function of the operated side following sleeve lobectomy was well preserved and showed no difference when compared with the regional function after simple lobectomy for lung cancer.

Virological and serological characterization of asymptomatic blood donors positive for anti-hepatitis C virus antibody.

To study the virological and serological characteristics of asymptomatic hepatitis C virus (HCV) carriers, 165 blood donors positive for antibody against HCV proteins by the second generation assay, were analyzed for their clinical backgrounds, serological reactivity against antigens derived from HCV by recombinant immunoblot assay, and the amount and genotype of HCV by the polymerase chain reaction. Compared with blood donors having abnormal levels of alanine aminotransferase (ALT), sera from the donors with normal levels of ALT reacted less frequently against NS4 antigens (anti-5-1-1: 34.4% vs. 54.5%, P = 0.0609; anti-c100-3: 34.4% vs. 56.1%, P < 0.05). Also the positivity for antibodies against these antigens were more frequent in sera from donors with genotype 1b HCV-RNA than other genotypes (anti-5-1-1: 61.0% vs. 23.5%, P < 0.01; anti-c 100-3: 61.0% vs. 26.5%, P < 0.01). The prevalence of each genotype in blood donors with normal ALT levels was different from that in patients with advanced liver disease (P < 0.05), genotype 1b being less and genotype 2a being more frequent. The number of HCV-RNA copies/0.5 ml in donors with normal ALT was 10(7.9 +/- 1.0) (n = 27) and that in patients with chronic liver disease was 10(7.4 +/- 0.8) (n = 116), the difference being statistically significant (P < 0.05). In conclusion, the results of this study suggest that asymptomatic blood donors carrying HCV have the serological and virological characteristics different from the patients with advanced liver disease.

[Clinical significance of diffuse vasoconstriction of coronary arteries--a study using hyperventilation thallium-201 myocardial imaging].

Hyperventilation Thallium-201 imaging was evaluated for the examination of the existence of ischemia in the cases of diffuse vasoconstriction under the ergonovine maleate provocative test for coronary artery. Transient myocardial perfusion defect (PD) was demonstrated in 14 patients with ergonovine induced vasospasm (group S), and 13 of these patients also demonstrated redistribution (RD) (92.4%). In 14 patients with diffuse vasoconstriction (group D), nine demonstrated PD, and all of them revealed RD. On the other hand, only one of ten (10%) patients demonstrated PD and RD in a group of patients without spasm or diffuse vasoconstriction (group N). In addition, the left ventricular myocardium was divided into nine segments on a SPECT image, and the mean minimum washout rate (WOR) of each segment was evaluated. These values were compared with the percent change of the lung/heart ratio between early and delayed images (delta L/H%). Both the mean minimum WOR mean and delta L/H% of group D were significantly smaller than that of group N (p < 0.001), and only approximated to group S. Thus, the possibility of myocardial ischemia of diffuse vasoconstrictive coronary artery is implicated and such patients are supposed to be treated medically as vasospastic angina.

[Laboratory findings in patients with Yusho: 26 year follow-up study].

To evaluate chronic effect of PCBs on laboratory findings, peripheral blood cells and biochemical parameters were studied in 74 patients with Yusho in 1994. Serum level of triglyceride was elevated in 21 cases (28.4%) of these patients. Serum triglyceride levels showed statistically significant correlation with body mass index (Quetelet Index) and blood PCB concentration. We conclude that hypertriglyceridemia in patients with Yusho is not frequent and it may be associated with both obesity and blood PCB concentration.

[Monitoring for oral anticoagulant therapy].

Warfarin has been widely used for an oral anticoagulant therapy against thrombotic diseases. For the monitoring of its anticoagulant intensities, prothrombin time (PT) ratio and percentage of thrombotest (TT) are commonly used in Japan. Recently, International Normalized Ratio (INR) was recommended by ICSH/ICTH. Practicality and usefulness of INR and its combined use of thrombin-antithrombin III complex (TAT) for the monitoring of oral anticoagulation therapy were evaluated among patients of ischemic heart disease with or without interventions, and of cardiomyopathies and valvular diseases. Difference in thromboplastin sensitivities have been shown to cause errors in PT elongation and in the evaluation of anticoagulant activity, so that the monitoring only by PT ratio is considered to be irrelevant, and that INR is recommended to be used. INR was comparable to the levels of TT. Majority of the patients, whose TAT levels were kept normal, were controlled below the proposed therapeutic ranges of INR. With the combination of INR and TAT monitoring, anticoagulant effect of warfarin could be achieved safer in lower dose than the levels that might cause bleeding accidents.

[Dose-volume histogram analysis of patients with hepatocellular carcinoma regarding changes in liver function after proton therapy].

Seventy-five patients with hepatocellular carcinoma were treated with proton beams from 1983-1993 at the Proton Medical Research Center, University of Tsukuba. For the purpose of confirming the feasibility of proton therapy for hepatocellular carcinoma, we investigated the influence of proton therapy on liver function and also tried to evaluate the possibility of optimization using dose-volume histogram (DVH) analysis. The results indicated that proton therapy did not cause clinically symptomatic damage in liver function and the only notable change after proton therapy was the transient increase of transminase. DVH analysis showed that this transient increase of transaminase was well correlated to the normal tissue complication probability (NTCP). These results indicate that localized high dose radiation using proton beams is feasible for the treatment of liver cancers and optimization of this treatment may be possible using DVH analysis.

Oophorectomy predisposes to degenerative spondylolisthesis.

We performed a case-control study on the influence of oophorectomy on the development of degenerative spondylolisthesis, including a clinical review and determination of serum oestradiol levels. We also compared the radiological appearance of the lumbar spine at L4/5 in patients with and without spondylolisthesis and with and without oophorectomy. Oophorectomy was a risk factor for degenerative spondylolisthesis with an odds ratio of 7.5 (95% confidence interval, 1.6 to 46). The incidence of degenerative spondylolisthesis in 69 oophorectomised patients was about three times higher than in 69 non-oophorectomised matched control subjects. There was also a difference in spinal variation between oophorectomised and non-oophorectomised patients with spondylolisthesis. A high incidence of sagittal-plane orientation of the L4/5 facet and an increase in pedicle-facet angle were seen in both groups and are typical radiological features of this disease. An increase in lumbosacral angle and in disc-space narrowing was seen only in the non-oophorectomised patients with this condition. Our results suggest that the abrupt decrease in oestradiol level caused by oophorectomy may be a predisposing factor in degenerative spondylolisthesis at L4/5.

[Comparison of exercise-stress single photon emission computed tomography with thallium-201 reinjection and postextrasystolic potentiation to assess myocardial viability in patients with myocardial infarction].

Myocardial viability was examined in 20 patients with myocardial infarction 24 +/- 3 (mean +/- standard deviation) days after onset. Postextrasystolic potentiation (PESP) was measured by left ventriculography and thallium-201 (Tl) single photon emission computed tomography (SPECT) redistribution was assessed after stress loading and reinjection of Tl. The results were compared with the improvement of left ventricular segmental wall motion after coronary revascularization which was performed successfully in all patients. The uptake of Tl was impaired initially in 41 segments, classified into three groups: normal group, 13 segments with smooth reversion to normal; new fill-in group, 15 segments with a new fill-in after reinjection of Tl; no fill-in group, 13 segments with no fill-in after reinjection. When evaluated by the PESP before coronary revascularization, segmental wall motion was significantly augmented in all segments in the normal group and new fill-in group (p < 0.001), but not in the no fill-in group. After coronary revascularization, left ventriculography revealed that segmental wall motion was significantly improved in normal group and new fill-in group, but not in the no fill-in group except for mild improvement in five segments. The sensitivity and specificity of PESP (90% and 93%) and Tl SPECT (80% and 100%) for improvement of segmental wall motion after the coronary revascularization were very high, with the accuracy of either method attaining 87%. This result indicates that both Tl SPECT and PESP are useful for detecting viable myocardium in patients with recent myocardial infarction.

Cytokinin induces a rapid decrease in the levels of mRNAs for catalase, 3-hydroxy-3-methylglutaryl CoA reductase, lectin and other unidentified proteins in etiolated cotyledons of cucumber.

Large-scale differential hybridization was performed to examine rapid changes in gene expression caused by a phytohormone, cytokinin, in etiolated cotyledons of cucumber (Cucumis sativus L.). We isolated 86 cDNA clones for mRNAs whose levels decreased within 2 h of the start of treatment with N6-benzyladenine (BA). Partial nucleotide sequences showed that some of the cDNAs were homologous to those for catalase, 3-hydroxy-3-methylglutaryl CoA reductase (HMGR) and a lectin. This is the first report that the levels of the mRNAs for those proteins are regulated by a cytokinin in darkness. Together with previous results [Teramoto et al. (1993) Physiol. Plant. 87: 584, (1994) Planta 193: 573, (1995) Planta 196: 387], the present study suggests that the cytokinin act to lower the levels of mRNAs transcribed from various genes in etiolated cotyledons.

Back muscle injury after posterior lumbar spine surgery. Part 1: Histologic and histochemical analyses in rats.

STUDY DESIGN: Back muscle injury caused by retractor application during posterior spine surgery in rats was examined histologically and histochemically according to the postoperative time with reference to the retraction time-pressure relationship. OBJECTIVES: The results were correlated to provide the risk factors for back muscle injury during posterior spine surgery. SUMMARY OF BACKGROUND DATA: Back muscles were examined histologically and histochemically after application of quantitative pressure and retraction time. No previous study has assessed this relationship. METHODS: Five groups were studied: Group 1, 1-hour low-pressure load group; Group 2, 1-hour high-pressure load group; Group 3, 3-hour low-pressure load group; Group 4, 3-hour high-pressure load group; and sham group. In each group, the multifidus muscle was evaluated 3 hours, 48 hours, 1 week, 3 weeks, and 6 weeks after surgery. RESULTS: In all groups except the sham group, degeneration of the muscle and neuromuscular junction was found at a very early postoperative time, but regeneration began at 1 week, and recovery was attained by 6 weeks. The extent of muscle fiber necrosis and the severity of degeneration of the neuromuscular junctions showed a parallelism with the magnitude of the pressure load and retraction time. As the duration and pressure load increased, the time required for regeneration also increased. The fiber type grouping in group 3 and 4 was consistent with the severity of degeneration of neuromuscular junctions. CONCLUSIONS: The muscular degeneration and the regeneration was largely dependent on the retraction pressure-time product. These results suggest that denervation muscle injuries are likely secondary responses to muscle retraction injury in any case of posterior spine surgery.

Back muscle injury after posterior lumbar spine surgery. Part 2: Histologic and histochemical analyses in humans.

STUDY DESIGN: The histologic and histochemical changes in back muscle were studied in virgin surgery patients with lumbar spine disorders and in patients who underwent repeat posterior lumbar surgery. OBJECTIVES: The results were correlated to provide the evidences of histologic changes of back muscle after posterior lumbar surgery. SUMMARY OF BACKGROUND DATA: Back muscles were examined histologically and histochemically after posterior lumbar surgery. No previous study has assessed these changes. METHODS: Back muscles were obtained before and after retraction from 18 virgin surgery cases with lumbar spine disorders. In four patients, the retraction pressure was monitored and the retraction pressure-time products ([P][T]) were calculated. In 21 repeat lumbar surgery cases, muscle samples were obtained before muscle retraction. Samples were evaluated by histologic and histochemical methods. RESULTS: Abnormal findings were slight in virgin surgery cases. Early back muscle injury tended to depend on operation time and [P][T] products. Late back muscle injury in reoperated patients was marked. Various types of neurogenic changes were observed more than 10 months after the first operation. CONCLUSIONS: Histologic damages of back muscle due to previous surgical intervention were long-lasting. To avoid permanent muscle injury, the retraction time and pressure should be shortened or the pressure on the back muscle should be monitored during posterior surgery.
