[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1982). III. Secular changes in susceptibility].

In vitro susceptibilities have been investigated against several species isolated from patients with simple and complicated urinary tract infections (UTI) during 1980-1982. Antimicrobial activities of the third generation cephems against E. coli isolated from patients with complicated UTI were found to decrease slightly in 1982. And those against Klebsiella spp. isolated from patients with simple and complicated UTI were also found to decrease similarly. Against P. mirabilis, all the drugs tested have showed relatively potent activities and slight changes in the susceptibility. The marked decrease of susceptibility against Citrobacter spp. isolated from UTI have been found even in the third generation cephems. Especially, Citrobacter spp. exhibited a greater degree of resistant to CZX and CPZ. Strains of P. aeruginosa were on the whole susceptible to the drugs tested, CFS, GM, TOB and AMK, inhibiting 50-80% of the strains tested at 1.56 micrograms/ml. CTX, CZX and CMX seemed most effective against S. marcescens among the third generation cephems, inhibiting 50-90% of the strains tested at 3.13 micrograms/ml.

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1982). II. Background of patients].

The frequency of the causative strains in urinary tract infections was investigated from the point of view of the patient's background such as age, sex administration of antibiotics and so on. Especially, the frequency of the causative strains isolated from patients within 3 days after the administration of antibiotics has markedly decreased. However, the frequency of the resistant strains such as P. aeruginosa, Enterobacter spp., Citrobacter spp. and Serratia spp. has increased after 8 days' administration of antibiotics.

[Study of immunosuppressive acidic protein (IAP) in patients with renal cell cancer].

We evaluated immunosuppressive acidic protein (IAP) as a tumor marker in renal cell cancer. The study was comprised of patients with renal cell cancer (6 stage A, 3 stage C, 11 stage D, 7 recurrences). The IAP positive rate of renal cell cancer was 84%, the mean was 937.5 +/- 442.1 ug/ml. The positive rate is statistically higher than in other malignant and benign urological diseases. The IAP positive rate tended to increase with cancer stage. IAP may represent a tumor marker in renal cell cancer.

One-stage total cystectomy and ileal loop diversion in patients over eighty years' old with bladder carcinoma. Pre- and postoperative functional reserve of various organs.

During the period from 1976 to 1981, 364 patients with bladder carcinoma were seen at the Keio University Hospital. Extensive preoperative investigation of pulmonary, cardiovascular, and renal function was obtained in all patients. Of the 12 patients studied, 9 underwent a one-stage total cystectomy and ileal loop diversion and the remaining 3 a two-stage procedure. Of the 9 patients, decreased FEV 1.0 per cent by spirometry was noted in 5, ECG abnormality such as bundle branch blocks in 8, and diminished creatinine clearance ranging from 28 to 68 ml/min were observed in all 9. Major postoperative complications included pyelonephritis in 2 patients, pneumonia in 1, pelvic abscess in 2, renal insufficiency in 3, and paralytic ileus in 2. There was no immediate postoperative death. In these elderly patients, functional reserve of the lung, heart, and kidney is less than optimal and is further decreased by major surgical procedures. Therefore, total cystectomy in the elderly patients is justifiable only in a selected group of patients, when functional status of the vital organ is thoroughly worked up and prophylactic and therapeutic measures are instituted promptly if indicated.

[A case of advanced testicular seminoma--chemotherapy and serum marker].

A 52 year-old male with advanced testicular seminoma (stage II N4) underwent right radical orchiectomy, followed by chemotherapy with vinblastine, bleomycin and cis-platinum. This resulted a partial response and he was well for 24 months after the operation. Serial monitoring of serum HCG and HCG-beta has been done, using three kinds of double antibody radioimmunoassays. A substance with immunological similarity to free HCG-beta was detected in sera and tumor extract. The serum level of this substance was reduced in parallel with the cytoreduction of metastasis by chemotherapy. A rational therapeutic approach to advanced seminoma and its serum marker are discussed briefly.

[Comparison of antibacterial potencies of oral and parenteral antibiotic preparations against Escherichia coli, Klebsiella, Citrobacter, and Proteus isolated from urinary tract infections (3: 1981) 1. Susceptibility distribution].

In vitro activities of antibacterial agents against E. coli, Klebsiella, Citrobacter and Proteus which were isolated from patients with urinary tract infections at 8 hospitals in Japan, were investigated by dilution broth method using MIC 2000 (Dynatec) during July to October in 1981. The summarized results are as follows: Among oral antibacterial agents, MPC and PPA have showed potent antibacterial activities against E. coli and Klebsiella. In vitro activities of oral antibacterial agents against Proteus and Citrobacter showed not so potent. Among the first and second generation's parenteral antibacterial agents, CTM has showed potent antibacterial activities against E. coli and Klebsiella. Among the third generation's parenteral antibacterial agents, CMX, CTX and CZX have showed potent antibacterial activities against E. coli, Klebsiella, Proteus and Citrobacter.

Since 1979 the antibacterial activity of antibiotics against E. coli, Klebsiella, Citrobacter and Proteus isolated from patients with urinary tract infections has been investigated. The serious transition of susceptibilities of E. coli and Klebsiella could not be recognized in these antibiotics (MPC, ABPC, NA, PPA, CEX, CEZ, CTM, CMZ and CFX). However, a few resistant organisms against the third generation's antibiotics (CTX, CMX, CZX, LMOX and CPZ) have already been appeared, we have to observe these results, continuously.

[Comparison of antibacterial potencies of oral and parenteral antibiotic preparations against Escherichia coli, Klebsiella, Citrobacter, and Proteus isolated from urinary tract infections (3. 1981). 3. Patient backgrounds and bacterial sensitivities].

Routine susceptibilities testing of microorganisms isolated from patients are of restricted usefulness in the treatment of patients because of the delay in obtaining results. Thus, the empiric chemotherapy based on the susceptibility of microorganisms isolated from patients is necessary for the majority of patients with simple and complicated urinary tract infections. In this study the relation between changing susceptibility and background factors such as age, a sex distinction, antibiotics, areas in Japan, simple and complicated UTI and so on, has been investigated.

[Angiography of primary retroperitoneal tumor--case report and proposal of a flow chart].

The development of ultrasonography and CT scanning has facilitated the diagnosis of primary retroperitoneal tumors. However, the tumor resection rate remains low. Factors such as tumor histology and stage of the disease play a role. Angiographic study of the tumor vessels is important in determining the procedures to the used in operative treatment. We present 3 primary retroperitoneal tumors and one malignant adrenal tumor discuss the importance of angiographic evaluation in the resection of recurrent tumors. A flow chart for surgically treated primary retroperitoneal tumor is proposed.

[Traumatic cerebral aneurysm: 94 cases from the literature and 5 cases observed by the authors].

Traumatic intracranial aneurysm is rare lesion which is complicated severe head injury. In spite of this rarity, this aneurysm is well known by the fact of high probability of rupture, and high mortality rate. Ninety-four cases of traumatic aneurysms, including 5 cases of our own are discussed. In 94 cases, 83 cases (88.3%) are located at the middle cerebral arteries and/or the anterior cerebral arteries. There are only 2 cases (2.1%) located at the posterior fossa. All of these 94 aneurysms are situated at the peripheral arteries and/or cortical branches, in the distance of Willis' arterial ring. There are 8 cases which lead to be cured spontaneously without any surgical treatment. It was reported by some authors that the traumatic intracranial aneurysms expecting spontaneous healing had some characteristics by the cerebral angiography, such as irregular shapes, and unclear connections from parent arteries. However, these characteristics are sometimes common in the traumatic intracranial aneurysms. Therefore, it is actually difficult to differentiate spontaneous healing aneurysms from deteriorating ones. As for the prognosis of these 79 cases which are able to be examined, the mortality rate of non-surgical group is 4 times higher compared with that of the surgical group, that is the former 40%, and the latter 11%, respectively. According to these facts, authors concluded that the principle therapy of traumatic intracranial aneurysm is surgical treatment.

Periodic urine cytology surveillance of bladder tumor incidence in dyestuff workers.

The validity of urine cytology surveillance of occupational bladder tumor incidence was evaluated by reviewing the clinical history of nine bladder tumor cases found in dyestuff plant workers. A bladder tumor surveillance system has been organized for workers (179) active and 65 retired) who had been exposed to benzidine or beta-naphthylamine in the plant. The system consists of two-stage tests: periodic urine cytology as the first-stage screening test, followed by, in suspicious cases, urological examinations as second-stage tests. The urine cytology surveillance was found to be useful for two reasons: first, it detected tumors in five out of six cases surveyed by athe system, and second, four cases screened by cytology had tumors curable by transurethral operation, while other cases underwent cystectomy. Calculated average latent periods from the first and last exposure was 26.4 years (SD = 6.0) and 14.0 years (SD = 6.2), respectively. This time period indicates that the surveillance system should continue to be used for the exposed group even after workers have ceased being exposed.

Multimodal treatment of advanced adult Wilms tumor.

We report 2 cases of advanced adult Wilms tumor that were treated with surgery, radiation and chemotherapy. The first patient had relapse of a Wilms tumor in the liver 2 years after nephrectomy. Combination chemotherapy, consisting of actinomycin D and vincristine, radiation therapy and final resection of the liver metastasis were successful and the patient has been free of disease for 4 years. The second patient had undergone transcatheter embolization of the renal artery elsewhere with the tentative diagnosis of an inoperable renal cell carcinoma metastatic to both lungs. A left renal tumor, weighing 4,500 gm., and a tumor thrombus in the vena cava extending to the right atrium were removed, and histologically diagnosed as a Wilms tumor. Subsequent chemotherapy and radiotherapy resulted in complete disappearance of the lung metastases. We conclude that multimodal treatment, namely a well timed combination of surgery, chemotherapy and radiotherapy, could potentially eradicate the disease even at an advanced stage.
