Population and family studies of HLA in Japanese with cleft lip and cleft palate.

Population and family studies of HLA were performed in Japanese patients with cleft lip and/or cleft palate (CL/P). Frequency of HLA-Cw 7 was significantly increased in cleft lip (CL) patients (37.5%) and cleft palate (CP) patients (37.8%) but was not increased in cleft lip and palate (CLP) patients (17.5%), compared with control subjects (13.3%). However, the intensities of the associations were not great (relative risk = 4.0). The affected sib pairs method was studied in 13 families with CL- or CLP-affected sib pairs and 10 families with CP-affected sib pairs. However, in both groups of families the distributions of HLA haplotypes in affected sib pairs did not significantly differ from random Mendelian expectation. Thus, HLA-linked major genes (loci) which determine the development of CL/P were not found. These results seem indirectly to support the multifactorial theory of CL/P, but does not exclude other possible genetic mechanisms.

[Adenoid cystic carcinoma of the breast--a case report].

We report a rare case of adenoid cystic carcinoma (ACC) of the breast. The patient was a 60-year-old female with a lump of the left breast which had present for 20 years. Macroscopically, the resected left breast tumor was well defined, 7.5 X 7 X 5 cm in size, oval shaped, yellowish gray-white and solid, parts of the tumor showed cystic appearance. Microscopic study revealed a cribriform pattern and alveolar proliferation of uniform, small cells forming cysts, and duct-like spaces which contained mucous content. Each tumor nest was surrounded by hyaline stroma. Alcian blue-PAS staining showed that the cysts were alcian blue-positive; the ducts were PAS-positive. The tumor was diagnosed as ACC of the breast.

[Combination chemotherapy with mitomycin C, methotrexate, and vincristine (MMV) for metastatic breast cancer refractory to adriamycin].

Twenty-five patients with metastatic breast cancer who had failed with combination chemotherapies including adriamycin were treated with a combination of mitomycin C, methotrexate, and vincristine (MMV). MMC 5 mg/m2, methotrexate 18 mg/m2, vincristine 0.7 mg/m2 were given i.v. on days 1 and 8, and repeated every 21 days. Seven patients (28%) experienced a partial response: 7 maintained NC and 11 failed to respond (PD). Median duration of regression was 12 weeks. Median survival time in PR patients was one year and 3 months, contrary to 6 months in patients with NC or PD. This combination was well tolerated except thrombocytopenia in 5 patients.

[Evaluation of steroid hormone sensitivity of human breast cancer cells in culture (MCF-7) by a colony formation method].

The effects of steroid hormones on the colony forming activity of a human breast cancer cell line (MCF-7) were studied. Endrogeneous estrogens in the fetal calf sera used for culture were deprived using the dextran-coated charcoal, and only batches of E2-deprived serum were chosen with which there was a significant increase in the plating efficiency after E2 (estradiol-17 beta, 10(-8) M) treatment. Progesterone showed little effect on the growth of the cells. not being influenced by the addition of E2. The capacity of the cells to form colonies was stimulated by androgens (5 alpha-dihydrotestosterone, testosterone) only in the absence of E2. Dexamethasone had suppressive effects regardless presence of E2. Thus, MCF-7 cells showed variable responses to these steroid hormones, although the cells have specific receptors to the hormones. This colony formation method of MCF-7 cells may be a useful model system for the study of hormonal and non-hormonal anti-cancer agents.

Asymmetric ventricular hypertrophy in patients with essential hypertension.

We attempted to clarify the pathogenesis of asymmetric ventricular hypertrophy in hypertensive patients, especially regarding sympathetic nervous system and renin-angiotensin system. Subjects were divided in 3 groups by echocardiographic findings; 1) 15 patients with non-hypertrophy (NH), 2) 14 patients with symmetric hypertrophy (SH), and 3) 10 patients with asymmetric hypertrophy (ASH). Subjects with ASH showed following features. Age (53.7 +/- 1.6 yr) was older than NH (43.7 +/- 1.4 yr) but not different from SH (49.7 +/- 2.3 yr). Mean arterial pressure (119.0 +/- 3.9 mmHg) was higher than NH (107.5 +/- 1.4 mmHg) but not different from SH (122.4 +/- 2.8 mmHg). End-diastolic and end-systolic dimensions were smaller and ejection fraction was larger than those of NH and SH. Cardiac index (3.90 +/- 0.37 L/min/M2) was largest among 3 groups. UNE (19.5 +/- 2.5 micrograms/day) was lower than SH (31.2 +/- 2.5 micrograms/day). PRA (0.44 +/- 0.16 ng/ml/h) was lower than SH (1.53 +/- 0.20 ng/ml/h) and NH (1.62 +/- 0.28 ng/ml/h). Ejection fraction was correlated with UNE (r = 0.835) and PRA (r = 0.736). We suggest that the heart of hypertensives with ASH is in hyperdynamic state due to the hyperresponsiveness to sympathetic stimuli, although they have a decrease of sympathetic nervous activity, and the renin-volume axis may have no important role on the pathogenesis of ASH.

Steroid hormone receptors and clinical usefulness in human breast cancer.

For more precise prediction of response to endocrine therapy in patients with advanced breast cancer, we investigated the correlation between the presence of combination of estrogen (ER), progesterone (PgR) receptors, or combination of ER, PgR and androgen receptor (AR) and response to therapy. The patients with ER+, PgR+ tumors survived longer than those with ER+, PgR-, or ER-, PgR- tumors. A better response rate with longer survival time was obtained in patients with tumors that had three positive receptors or PgR+ and/or AR+ in addition to ER+ than in those with tumors of other combinations. According to the sequential assays of ER in tumors in several stages of breast cancer, it is likely that the positive rate of ER becomes lessened by the progression of cancer and by treatments. We stressed the significance of steroid hormone receptor assay in the mastectomy specimens for the prediction of response of therapy in case of the future recurrence of malignancy.
