[Pathogenesis of endogenous endotoxemia in chronic liver disease--with special reference to the experimental fatty liver in germfree animals].

The effect of intestinal bacterial flora and endotoxin on fatty liver with germfree (GF) and conventional (CV) rat in the 12th and 24th week was investigated after giving fatty diet which was added 1% cholesterol-0.5% cholic acid to the basic diet. Results are as follows. Serum biochemistry Serum GOT, GPT, ALP and cholesterol values increased after giving the fatty diet in both groups. Limulus Gelation Test In CV group, endotoxin was detected in 2 of 10 cases in portal blood and was completely absent in arterial blood. After the fatty diet, endotoxin increased gradually both in portal and arterial blood. Cyclic AMP values on glucagon challenge (P/B ratio) In both groups, the levels of the P/B ratio maintained low values compared with control. In CV group, the values were lower in endotoxin positive cases than negative ones. Hepatic carbohydrate metabolism Abnormal hepatic F6P , glucose, FDP and PEP values were observed in CV group and reduction of the levels of hepatic F6P , G6P and glucose values were remarkable in GF group. Hepatic G6P in CV group and FDP in GF group remained unchanged. Impairment of F6P and G6P in CV group, was significant in endotoxin positive cases than in negative ones.

A new instrument for closed irrigation-suction treatment.

Orthopedic surgeons are often faced with the surgical complication of fulminating deep wound infection and bone and joint infection after complicated fractures. Two hundred and seventy patients were treated for bone and joint infection by closed irrigation-suction from 1970 to 1982. Obstruction of the drainage tube, solution leakage, and post-operative bleeding were the knotty problems. Our new instrument for irrigation-suction treatment resolved these problems. The instruments consists of a suction pump, a channel changer, and a double tube system.

Lymph node removal for advanced gastric cancer with special reference to peritoneal metastasis.

The 5-year survival rate of gastric cancer patients was evaluated in relation to cancer infiltration to the gastric serosa, lymph node metastasis and lymph node removal, the pattern of cancer recurrence, and the presence of free cancer cells in the peritoneal cavity. The 5-year survival of patients with cancer infiltration to the gastric serosa was not improved even by more extensive lymph node removal (R3 removal) than secondary lymph node removal (R2 removal). In these patients, viable free cancer cells were detected in the peritoneal cavity. In patients with cancer recurrence, peritoneal metastasis was often present within 2 years of curative surgery. Based on our present results, we suggest that in advanced gastric cancer with marked serosal invasion, selective removal of more distant lymph nodes than secondary lymph nodes according to the location of the primary cancer lesion should be carried out in addition to R2 removal and that this selective removal is superior to uniform R3 removal.

[Metastasis of gastric cancer to the spleen--a case report].

Metastasis to the spleen is often found in cases with malignant hematologic disorders. To our knowledge, however, few survivors with primary gastric cancer and splenic metastasis have been reported. A 64-year-old woman underwent curative total gastrectomy combined with pancreaticosplenectomy for gastric cancer, Borrmann III type, in the upper body of the stomach and cardia. Postoperative histologic examination revealed that the poorly differentiated adenocarcinoma had metastasized to the spleen. The low incidence of splenic metastasis from gastric cancer may be a reflection of special antineoplastic traits of the spleen related to phagocytic qualities, or to humoral factors which are as yet unidentified.

[Studies on the appropriated interval of mass screening for cervical cancer].

In order to study the appropriate interval for cervical cancer screening, we investigated mainly the screening history of 1,086 cervical squamous cancer cases detected by mass screening. 1) The cancer detection rate (CDR) for the 2-year successively screened group who were class I in the first screening was 0.051% and all cases were carcinoma in situ (CIS). In the 2-year interval screened group, detection rates for CIS, microinvasive cancer and stage 1b cancer were 0.053%, 0.035% and 0.018%. But there is no significant difference in CDR between the 2-year successively screened group and 2-year interval screened group (p less than 0.05). 2) In the 2-year successively screened group who were class II in the first screening, the CIS and microinvasive cancer detection rates were 0.044% and 0.022%. There is no significant difference in CDR between class I group and class II group. In the group who were class I in the first screening, we detected most cases in the stage 0 and a few cases in the stage Ia and Ib by mass screening at 2-year intervals. If the detection of a few cases in stage Ib is permitted, we can enforce mass screening for cervical squamous cancer at 2-year interval and it is considered that class II group can be screened at same interval as class I group.

Carcinoembryonic antigen in the bile in patients with pancreatic and biliary cancer. Correlation with cytology and percutaneous transhepatic cholangiography.

The level of carcinoembryonic antigen (CEA) in the bile of 17 patients with benign pancreatic and biliary diseases and 50 patients with pancreatic and biliary cancer were determined by enzyme immunoassay. The bile specimens were obtained at the time of percutaneous transhepatic cholangiography. The bile was centrifuged and the supernatant was used for CEA measurement, while the cell pellet was examined cytologically. High CEA values in the bile were significantly more frequent in patients with pancreatic and biliary cancer than in those with benign pancreatic or biliary diseases; increased CEA concentrations in the bile were observed in 76.0% and 60.0%, respectively, of all the patients and of the patients with localized cancer. The location of the cancer had no influence on the bile CEA level, but the CEA levels in the bile tended to be high when the tumor had distant metastases, or when the biliary tract was completely obstructed. Although patients with pancreatic and biliary cancer had a high CEA value in the bile significantly more frequently, bile CEA measurement is not sufficient to distinguish an individual patient with pancreatic or biliary cancer from those with other disease, since the overlap between the values is too great, and therefore, additional examinations are required. Correct diagnosis of malignancy were made by cytological examination alone, by bile CEA assay alone and visual examination of dye in the biliary tract alone in 72.0, 76.0, and 88.0% respectively, of the patients examined, while the combination of these methods raised the diagnostic rate to 100%.

Endoscopic diagnosis of early gastric cancer by the endoscopic Congo red-methylene blue test.

The endoscopic Congo red test combined with dyeing with methylene blue was performed in 85 patients with early gastric cancer (94 lesions). Results revealed that gastric cancer bleached the Congo red and methylene blue sprayed over their surface and this appeared in sharp contrast to the red-colored mucosa of unaffected areas. Grossly, polypoid and flat types, and histologically differentiated adenocarcinomas bleached the dyes more frequently and more intensely than depressed and undifferentiated adenocarcinomas. Thus the spread of cancerous growth could be judged rather accurately and so the target area could be reached by biopsy in cases where there were few if any visual signs of abnormality.

Perchloric acid treatment of human blood for quantitative endotoxin assay using synthetic chromogenic substrate for horseshoe crab clotting enzyme.

A new reliable human blood treatment was established for quantitative endotoxin assay using synthetic chromogenic substrate [Boc-Leu-Gly-Arg-rho-nitroanilide]. Addition of perchloric acid in a final concentration of 1.25% to platelet-rich plasma or serum in a 2:1 volume ratio completely eliminated nonspecific amidase activities as well as inhibitors. By this method, the recovery of added endotoxin was nearly 100%, and was almost independent of sample dilutions and anticoagulants.

Comparison of 5- and 10-year survival rates in operated gastric cancer patients. Assessment of the 5-year survival rate as a valid indicator of postoperative curability.

To evaluate whether it is appropriate to estimate the postoperative curability of gastric cancer based on the 5-year survival rate, we compared 5- and 10-year survival rates in 477 primary gastric cancer patients who had undergone gastrectomy during the 8-year period from 1960-1967. In patients who had died more than 5 years after surgery, the cause of death was investigated. The 10-year survival rate of 315 curatively operated patients was 59.1%, 2.9% lower than the 5-year survival rate. Of 176 patients who survived more than 5 years postoperatively, only 9 (5.1%) died of cancer recurrence, suggesting that the 5-year survival rate adequately reflects the curative success of gastric cancer surgery. The postoperative curability of gastric cancer is usually estimated by the 5-year survival rate. However, even in patients who survived for more than 5 years postoperatively, cancer recurrence has been observed. Therefore, we deemed it useful to assess whether it is appropriate to estimate the postoperative curability of gastric cancer based on the 5-year survival rate. From this point of view, we compared the 5- and 10-year survival rates of operated primary gastric patients and investigated the cause of death in patients who died more than 5 years postoperatively. Furthermore, in patients with cancer recurrence more than 5 years postoperatively, the pathologic findings on the cancer in the originally resected specimen were re-evaluated.

Immunohistochemical localization of estrogen receptors in human mammary carcinoma using antibodies to the receptor protein.

An immunofluorescent method has been developed for localizing estrogen receptors (ER) in frozen sections of human breast cancer biopsies which utilizes the biochemically well-characterized antibodies to the cytoplasmic ER of human breast cancer produced in rabbits. Frozen sections of sixteen breast cancer biopsies, in which the receptor content has been quantified using dextran-coated charcoal assay (DCC technique), were used to study the localization of ER. Two aspects of ER detection, namely "estrogen-binding" and ability to react with the homologous antibodies, have been analyzed and the results compared with those of DCC technique. Fluorescent labelled estrogen method (F.E2) of Pertschuk was utilized to detect the distribution of estrogen binding proteins and the indirect immunofluorescent (IM-AR) method using anti-receptor antibodies to assess the antigenic sites of the receptor protein. In addition to the comparison of the results of three techniques, DCC, IM-AR and F.E2, for ER detection, a procedure for processing the frozen sections, which enables the study of estrogen binding characteristic as well as the antigenic reactivity of the receptor molecules, is described.

Healing and recurrence of duodenal ulcers in relation to the acid-secreting area.

The relation of the gastric acid-secreting area to the healing and recurrence of duodenal ulcers was examined in 67 patients with duodenal ulcers. Studies were made by the endoscopic Congo red test developed at this institute. Results indicate a close correlation between the healing and recurrence of duodenal ulcers, and the extent of the acid-secreting area. In general, when the acid-secreting area was large, the ulcers did not heal satisfactorily within 4 weeks after initiation of medical treatment and frequently recurred. In contrast, when the acid-secreting area was small, the ulcers tended to heal rapidly and not to recur.

Tissue-specific distribution of DNA-binding nuclear proteins.

The DNA-binding capacity of nuclear proteins of mouse cells was examined by the protein-blotting method. Under conditions in which the lac repressor specifically binds to the lac operator, the DNA-binding nuclear proteins from different tissues showed a tissue-specific distribution, suggesting that the species and amounts of nuclear proteins with DNA binding activity differ in different tissues. When cloned eukaryotic genes were used for binding, eukaryotic DNA showed stronger binding than prokaryotic DNA. Competition experiments suggested that many nuclear proteins have different DNA binding properties from that of the prokaryotic repressor.

IgD antibody to Mycoplasma pneumoniae in human sera as measured by the RAST.

Anti-human delta chain antibody was prepared and specific IgD antibodies to Mycoplasma pneumoniae (MAC strain) antigens were studied by means of the RAST. M. pneumoniae antigens were chromatographically separated by Sephadex G-25 and Sephadex G-200 gel filtration. Specific IgD antibody titers were significantly higher in patients with primary atypical pneumonia due to M. pneumoniae infection than those of healthy and disease controls. The specific IgD antibodies increased at an earlier stage of the disease and decreased rapidly. Further studies are needed to clarify the role of the IgD antibodies in regulating antibody production during infection.

In vivo and in vitro studies on steroid metabolism in a case of primary aldosteronism with multiple lesions of adenoma and nodular hyperplasia.

In order to systematically analyze the regulation and metabolism of steroid hormones in a case of primary aldosteronism with multiple lesions, including adenoma and nodular hyperplasia of the left adrenal gland, the amounts of 9 steroids (progesterone (P), 11-deoxycorticosterone (DOC), corticosterone (B), 18-hydroxycorticosterone (18-OH-B), aldosterone (Aldo), 17 alpha-hydroxyprogesterone (17-OH-P), 11-deoxycortisol (S), cortisol (F) and dehydroepiandrosterone sulfate (DHEAS)) contained in the plasma and in the adrenal tissues were measured. The patient (a 39-year-old female) was admitted to our hospital because of hypokalemia and hypertension. A diagnosis of primary aldosteronism was made on the basis of a complete evaluation, and an adenoma (1.8 x 1.2 cm), a nodular hyperplasia (0.5 x 0.5 cm), a microadenoma and a cortical nodule were found on the left adrenal gland. In vivo studies revealed that the plasma level of Aldo was high, but those of the other steroid hormones were within the normal range. After ACTH infusion, the plasma levels of the 9 steroid hormones increased by 2 to 17 times the base levels. In particular, the responses of DOC and B were markedly high. In vitro studies on P, DOC, B, Aldo and F content in the adenoma (A), the nodular hyperplasia (A'), the adjacent adrenal tissue (C) and the right normal adrenal tissue (D) revealed that, except for F, they were highest in A, followed by A', D and C in that order. In incubation studies with ACTH using A and C, it was found that the levels of 8 steroid hormones with the exception of DHEAS were high in A than in C. In particular, the response of B in A was markedly increased. These findings suggest that aldosteronoma produces 8 steroid hormones under conditions of excess ACTH, while at physiological levels of ACTH, it produces only Aldo in excess.
