A conventional limulus test is not specific to endotoxin because of the presence in amebocyte lysate of a (1----3)-beta-D-glucan-sensitive factor. By fractionating coagulation enzymes in the lysate and recombining only those factors involved in endotoxin-induced coagulation, we have developed a new test specific to endotoxin. The recombined enzymes reacted only with endotoxin, and not with fungal polysaccharides. Conventional amebocyte lysate, on the other hand, reacted with both of them. A good linearity was obtained with this method between endotoxin concentration and absorbance with a sensitivity of 1 pg/ml of Escherichia coli 0111:B4 endotoxin. The regression lines for different types of endotoxins were parallel to one another. For the correct diagnosis of endotoxemia, this new test has a definite advantage over the one using whole amebocyte lysate.

Percutaneous aspiration biopsy of the pancreas using a heparinized 22-gauge fine needle was performed under ultrasonic guidance in five patients with benign pancreatic diseases and in 18 patients with pancreatic cancer. Using a heparinized needle and syringe, it was possible to make good smears containing abundant tumor cells and to obtain small tissue specimens. Using egg albumin as binding material, a new cell-block technic was developed to conveniently obtain histologic specimens. In this way, a correct diagnosis was made cytologically in all 23 patients suspected of having a pancreatic malignancy. Histologic specimens were obtained in 22 (95.6%) our of 23 patients. A correct diagnosis was established histologically in all patients from whom histologic materials were obtained. This procedure thus has proved a very reliable method for diagnosing pancreatic cancer.
This study explores the relationship between extracellular calcium (Cao) and Na+-K+ regulation as it particularly pertains to the activity of the Na+ pump in cultured vascular smooth muscle cells (VSMCs) originating from Sprague-Dawley rats. As compared with cells incubated in media containing 0.5, 2.0, or 4.0 mM calcium, when the Na pump is active, VSMCs incubated in a Ca-deficient medium show a marked increase in intracellular sodium and no significant change in intracellular potassium. Associated with the rise in intracellular sodium there is an augmented activity of the Na pump. When the Na pump is inhibited, VSMCs incubated in either high-Ca medium (Cao = 4.0 mM) or Ca-deficient medium manifest a greater decline in intracellular potassium than cells incubated in media containing 0.5 or 2.0 mM calcium. Furthermore, when the Na pump is inhibited, VSMCs incubated in a Ca-deficient medium exhibit higher intracellular sodium levels in comparison with their counterparts incubated in media containing calcium. Flux experiments indicate that the aforementioned changes reflect increased membrane permeabilities to Na+ and K+. It is concluded that by regulating the permeability of the VSMC membrane, Cao plays an important role in the intracellular Na+-K+ homeostasis and that its effect on the Na pump is mediated via perturbations in the intracellular Na+ and K+ concentrations.
An epizootic of dermatitis with erosion, ulcer and crust broke out in an experimental colony of JCL-ICR mouse over a period from December 1975 to June 1976. The disease was detected in 592 of a total of 1831 mice of 3-24 months old, especially in males of 7-24 months old (517/821). At the beginning of December 1975, only a few males of 12 months old were found to have the lesion on the back skin, and thereafter the dermatitis prevailed gradually among the mice. Histopathologic examinations showed the loss of the epidermis, necrosis and/or collapse of the corium, accumulation of serous exudate with neutrophilic cell infiltration and a few cocci scattered on the surface. In chronic cases, fibrous granulation tissues with neutrophilic cell infiltration were formed in the corium. Staphylococcus aureus was isolated in pure culture from the skin lesions in all of the mice examined. Skin disease similar to that of the field case was reproduced in mice inoculated subcutaneously with 10(7) viable organisms of the fresh isolate. By giving chlortetracycline in drinking water for 7 days, treatment of the affected mice was efficacious in mild cases, but not in severe cases.
The characteristics of 167 isolates of S. aureus from 106 mice suffering dermatitis were examined. All 167 isolates coagulated both rabbit and human plasmas and 161 of them also coagulated bovine plasma. All the isolates produced heat-stable and heat-labile DNase, phosphatase and yellow pigment, reduced nitrate, hydrolysed egg yolk, Tween 80, and hippurate, and grew on crystal violet agar in colonies of the negative type C and on medium with 10% NaCl. The majority of them produced fibrinolysin, protease and acetoin. Fifty-three percent were gelatinase positive. In hemolysis tests, 25, 57 and 45 isolates showed alpha-, beta-, alpha beta-hemolysis, respectively. Forty isolates did not produce hemolysins in the rabbit and sheep blood agar. All of 75 isolates tested produced acid from fructose, galactose, glucose, glycerol and mannose, but did not from arabinose, dextrin, inulin, raffinose, salicin, sorbitol and xylose. Most of these isolates produced acid from lactose, mannitol, sucrose and trehalose. All of the 75 isolates were highly sensitive to penicillin, methylphenylisoxazolyl penicillin, erythromycin, spiramycin, lincomycin, chloramphenicol, tetracycline, kanamycin, gentamicin and cephaloridine, but were resistant to sulfisoxazole. With phages of human set, all 167 isolates were typable at 100 X RTD. All but one of the typable isolates belonged to mixed lytic groups. These were I + III (35 isolates), I + M (1), I + III + M (124) and I + II + III + M (6), with long phage patterns. When the 167 isolates were biotyped as described by Hájek and Marsálek [7, 8], 5 belonged to biotype A, 1 to biotype B and 60 to biotype C.(ABSTRACT TRUNCATED AT 250 WORDS)
The accuracy of diagnosis of hepatic malignancy by percutaneous aspiration biopsy was compared using heparinized and nonheparinized 22-gauge needles. When a heparinized instrument was used, it was able to accurately diagnose malignancy and cytologic types of exfoliative cells. Heparinization also made it possible to recover small tissue fragments and make ultrathin sections for histologic examination. In this way, material suitable for cytologic and histologic examination was obtained from 100% and 95%, respectively, of 59 patients suspected of having hepatic malignancy. A correct diagnosis was made cytologically in 92.5% of patients with hepatic malignancy and in 89.5% of those from whom histologic material was obtained. From a combination of histologic and cytologic results, the overall diagnostic rate for hepatic malignancy was increased to 95%. This procedure proved to be a reliable method for diagnosis of hepatic malignancy.
Melittin effect on transport of Na+ and Rb+(K+ analog) was examined in cultured skin fibroblasts originating from the Spontaneously Hypertensive, Wistar Kyoto and Wistar rats. Melittin increased both Na+ (22Na+) uptake and 86Rb+ efflux as well as the activity of the Na+-pump (ouabain sensitive 86Rb+ uptake) in all three preparations. The effect of the toxin was maximal at a dose of 160-240ng/10(5) cells/ml. At this dose, fibroblasts of the Spontaneously Hypertensive rat demonstrated the greatest response to melittin with respect to the increase in Na+ and Rb+ fluxes and increase in the intracellular Na+ concentrations. It is concluded that melittin can be utilized as a probe to delineate subtle differences in the cellular regulation of Na+ and K+ in the Spontaneously Hypertensive rat as compared with its normotensive controls.

A multiple regression method of defining "reasonable cost" for Medicaid reimbursement of nursing home patient care was implemented in Washington State in July 1978. A patient-mix index, the Katz ADL (Activities of Daily Living), was included in the regression equation as one of the independent variables to allow systematically for individual patient characteristics and needs in the rate-setting method. From the six measures investigated, the Katz ADL index was selected as the best predictor of patient care per patient-day cost. The way a "reasonable cost interval" of patient care per patient-day was established for each of 277 providers is described. Evidence is presented that this statistical rate-setting method contained efficiency incentives. The potential usefulness of such methods in obtaining management information at low cost should be given more consideration in efforts to reduce administrative costs of governmental health care programs, thus allocating scarce resources to patients on an equitable basis.
