Treatment of acute myocardial infarction with cardiogenic shock using left ventricular assist device.

We have treated ten cardiogenic shock patients after acute myocardial infarction (AMI) with a left ventricular assist device (LVAD). These patients were later divided into three groups: the first group with ventricular septal perforation, the second with aorto-coronary bypass grafting (ACBG) before LVAD implantation and the third group without ACBG. LVAD maintained the systemic circulation in each group, and cardiac function recovered enough to remove LVAD in 70% of the total patients. Two of three patients in the first group were discharged from hospital. Two weaned cases in the second group died of multiple organ failure and one was discharged, and hemorrhagic necrosis was seen in the bypassed area of the myocardium. One patient of the third group could not be weaned from LVAD because of respiratory failure though his heart function began to recover. Another case in the third group underwent bypass grafting after removal of LVAD. However ACBG surgery should be done very carefully because a patient in shock is occasionally intolerant to major surgery. In all groups, the major cause of death was multiple organ failure which was probably caused by the prolonged low output condition prior to LVAD application. In the light of this experience, it appears that LVAD should be applied before irreversible damage occurs to major organs, including the heart itself. To ensure the timely application of LVAD, some way must be found to introduce systematic application of LVAD into the normal course of AMI treatment.

Determination of infarct size of acute myocardial infarction in dogs by magnetic resonance imaging and gadolinium-DTPA: comparison with indium-111 antimyosin imaging.

Acute myocardial infarctions were produced in nine dogs by ligation of the left anterior descending coronary artery. Twenty-four hours after ligation, 0.5 mM/kg of gadolinium (Gd)-DTPA was injected intravenously, followed by cardiectomy 30 min later. Indium-111 antimyosin was administered intravenously 6 hr before cardiectomy to compare the infarct size with Gd-DTPA contrast enhancement. Areas of Gd-DTPA contrast enhancement were closely correlated with those of indium-111 antimyosin uptake (r = .86), although the former showed slightly greater than the latter. Partial and complete enhancements were observed in three and six dogs, respectively. In the T1 and T2 maps, T1 relaxation times of the infarcted area showed greater T1 shortening compared with normal myocardium, whereas T2 relaxation times were not different between infarcted and normal myocardium. Thus, Gd-DTPA showed significant contrast enhancement of the infarcted area because of greater T1 shortening and the extent of Gd-DTPA contrast enhancement expressed the infarct size precisely.

[Evaluation of total cavopulmonary shunt operation from late results].

Total cavopulmonary shunt was performed in 6 patients (pts) with single ventricle and common atrioventricular valve (CAVV) with left isomerism between 1978 and 1987. Four patients survived including 1 with CAVV-replacement and follow-up period ranged from 1.5 to 10 years. The first pt tolerated a pregnancy and re-replacement of CAVV. All 4 pts showed significant improvements in symptom. The arterial oxygen saturation was around 90% early postoperatively, but 2 pts developed pulmonary arteriovenous fistula with decreased arterial oxygen saturation. This operation can be indicated in left isomerism with low operative risk, and also may be applied in those with CAVV regurgitation although the risk becomes high. PAVF is a matter of concern in the late follow-up.

Continuous measurement of oxygen delivery and oxygen consumption in awake live animals.

For continuous measurement of both SO2 and [Hb] of whole blood, a hybrid type optical sensor was developed, tested in vitro and applied to the continuous measurement of arterial and mixed venous SO2 and [Hb] of awake live animals whose circulation was entirely supported by a pair of artificial hearts. The purpose of using artificial hearts was to control the cardiac function precisely and to obtain independent responses of the peripheral circulatory system. Both sensors in the arterial and venous circuits functioned satisfactorily for a duration of 40 days; no blood clots were observed around the sensor and the mixed venous SO2 varied sensitively in response to changes in the respiratory status, pump output, [Hb], and oxygen consumption, thus showing that it could be used as an indicator to evaluate the adequacy of oxygen delivery to peripheral tissues.

Vincristine-resistant human cancer KB cell line and increased expression of multidrug-resistance gene.

A multidrug-resistant clone of human cancer KB cells was isolated by stepwise selection on exposure to increasing doses of vincristine. The final clone, VJ-300, obtained after ethylmethane sulfonate mutagenesis showed 400-fold higher resistance to vincristine than did KB cells. Cellular accumulation of vincristine in VJ-300 was decreased to less than one-tenth of that in KB. The cells were also cross-resistant to daunomycin, adriamycin, actinomycin D, colchicine and VP-16. During continuous culturing in the absence of any drug for several months, a different colchicine-resistant and multidrug-resistant clone, KB-C1, reverted almost completely to drug sensitivity, whereas drug resistance in VJ-300 was stably maintained. Amplification of the multidrug-resistance-1 (mdr-1) gene was more than 20-fold in KB-C1, but less than 2-fold in VJ-300. mdr-1 mRNA was, however, expressed in VJ-300 at a rate comparable to KB-C1. Acquisition of high multidrug resistance in VJ-300 might be correlated with both activated transcription of mdr-1 gene and amplification.

Effect of drive mode of left ventricular assist device on the left ventricular mechanics.

Pneumatically driven left ventricular assist devices (LVADs) were acutely implanted between the left atria and the descending aortas of dogs, and were driven in five pumping modes: electrocardiogram synchronous modes with the duty factors of 1:1, 2:1, and 4:1, and asynchronous modes with the pulse rates of 60 and 80 beats/min (bpm). The ventricular diameter and myocardial segment length were measured by an ultrasonic displacement meter and implantable miniature sensors. Bulk mechanical work of the left ventricle and regional mechanical work of the myocardium were calculated from these dimensions and the left ventricular pressure. LVAD reduced the bulk mechanical work of the left ventricle by 30-50% and the regional work by 30-60%. The mean aortic pressure and the total flow (= aortic flow + pump bypass flow) were highest in the 1:1 synchronous pumping mode, which indicates that this mode is most effective to maintain the systemic circulation and coronary blood flow. Asynchronous pumping and synchronous pumping with 2:1 duty factor were most useful to reduce the mechanical work of the left ventricle.

Use of a paracorporeal pneumatic ventricular assist device for postoperative cardiogenic shock in two children with complex cardiac lesions.

Pneumatic ventricular assist device (VAD) was utilized for cardiogenic shock after intracardiac operation in two children with complex cardiac anomalies based with single ventricle. In the first case (a 10-year-old), after a modified Fontan operation, VAD was placed between the functional left atrium and ascending aorta, serving as a "artificial single ventricle" with neither pumping chamber nor artificial support in the right side of the heart. The systemic circulation was maintained by keeping relatively high central venous pressure. In another child (a 3-year-old) who underwent repair of incompetent atrioventricular valve leaving intracardiac lesions, VAD was placed between the common atrium and ascending aorta, serving as a pump for both pulmonary and systemic circulation with regulation of pulmonary blood flow through an aortopulmonary Gore-Tex shunt. The circulatory assist with VAD was utilized for 5 and 6 days, respectively. Although weaning from the device was not feasible in both patients because of the pulmonary dysfunction, these experience showed the possible use of VAD for cardiogenic shock after surgery in patients with complex cardiac anomalies.

Changes in distribution and staining reactivity of PAS-positive material in the mouse epididymal duct after efferent duct ligation.

The distribution of spermatozoa and that as well as the staining reaction of PAS-positive material in the epididymal duct were histologically observed in 60 day-old mice and mice 12 h, 24 h, 2 days, and 7 days after efferent duct ligation at 60 days of age. We divided the mouse epididymal duct into five segments; Segments I, II, and III making up the head of the epididymis, Segment IV the body, and Segment V the tail. The cytoplasm of the principal cells in Segment II was PAS-positive. The lumen of the epididymal duct contained spermatozoa and PAS-positive material in the distal portion of Segment I and other segments distal to Segment I. The lumen in Segments IV and V was distended with abundant spermatozoa and PAS-positive material. After efferent duct ligation, spermatozoa disappeared from the lumen in Segments I, II, and III within 2 days, whereas they took 5 days to pass from Segment IV. As spermatozoa disappeared, the luminal material increased in intensity of the PAS-reaction in the distal portion of Segment I and initial portion of Segment II and Segment IV. Segment I became similar in appearance to Segment II 7 days after the ligation. The findings suggest that the PAS-positive material is produced in the distal portion of Segment I and Segment II and accumulates, in the absence of spermatozoa, at the production site and Segment IV, a storage site of spermatozoa; furthermore, Segment I has the capacity of exerting a Segment II-like function, which becomes distinct after blocking the entrance of the testicular fluid.

[Testing performance of the improved pinhole collimator and its application to small animals].

Testing performance of the improved pinhole collimator of 1.5 mm aperture attached to the gamma camera was carried out with three kinds of radionuclides, 99mTc, 67Ga and 131I, using a hand-made phantom. The results obtained in this experiment were compared to those obtained with a human pinhole collimator of 4 mm aperture. Those were nearly understood by taking account of the effective aperture of a pinhole collimator for each effective photon energy. And all scintigrams obtained from rats with various scintigraphy showed high resolution images.
