[Clinical evaluation of lenampicillin in oral and maxillofacial infections].

Clinical efficacies of newly developed synthetic oral ampicillin prodrug lenampicillin (LAPC, KBT-1585) applied to 109 cases of oral infection were studied. There were 7 dropout cases. The results as determined on a point system are as follow: Remarkably effective, 26 cases; effective, 63 cases; and not effective, 13 cases, for an efficacy rate of 87.3%. When rated by the subjective judgement of the doctors in charge, these figures are as follow: remarkably effective, 21 cases, effective, 67 cases; slightly effective, 10 cases; and not effective, 4 cases. The rate of efficacy in this way being 86.3%. In either way, the results obtained were favorable. Among 102 cases in this study, pus was aspirated with sterile needle from obstructed abscesses in 65 cases, with the result that 161 strains of bacteria were isolated and identified. Most of infections were found mixed type by aerobic Gram-positive cocci and anaerobes. Especially, cases caused by alpha-Streptococcus were observed in 48 out of 55 mixed infective cases. LAPC's MIC distribution against the detected bacteria showed strong antibacterial effect as follows: against Gram-positive cocci, less than 0.39 micrograms/ml; against Gram-negative bacteria (excluding some insusceptible strains), less than 3.13 micrograms/ml. Thus, LAPC demonstrated a superiority when compared to CEX by 4 approximately 128-fold, and when compared to AMPC by about 2-fold. Adverse reactions among the 109 cases consisted of 6 cases of gastro-intestinal disorders including 3 cases of diarrhea. Recognized cases of abnormal laboratory findings were 3 cases out of 76 (3.9%), but none were serious.

Spontaneous calcified tongue lesions in DBA mice.

Large numbers of spontaneously occurring polypoid or slightly elevated lesions were observed in the tongue, mainly the dorsum linguae near the margo linguae, of DBA mice. Histologically the lesion consisted of granulation tissue with focal calcification, and involved superficially the tongue muscle. Often the calcareous deposits were encircled by multinuclear giant cells. The frequency of the calcified tongue lesion was high in lines of DBA/2 (DBA/2NCrj, DBA/2NJcl and DBA/2J), and DBA/1 (DBA/1Jcl and DBA/1J); the SM/J, BALB/c, C57BL/6 and C3H/He strains did not have the lesion. Among hybrid mice, CDF1, a hybrid of DBA/2 and BALB/c, a few had the lesions but no BDF1 mice, a hybrid of DBA/2 and C57BL/6, had any. The frequency was high in the hybrids of DBA/1 and SM/J. These results seem to indicate that the occurrence of the tongue calcified lesions was controlled by polygene.

Synthesis and antihypertensive activity of stereoisomers of 4-piperidyl-1,3-dihydro-2-oxo-2H-benzimidazoles. Enhanced potencies of (+) isomers.

To elucidate the relationship between the pharmacological activity and stereochemical structure, we resolved 1-[2-(3-,4,5-trimethoxyphenyl)-2-hydroxy-1-methylethyl]-4-(1,3-dihydro-2-oxo-2H -benzimidazol-1-yl)piperidine (1 and 2) and 1-[2-(3,4-dimethoxyphenyl)-2-hydroxy-1-methylethyl]-4-(1,3-dihydro-2-oxo-2H-benzimidazol-yl)piperidine (3), which produced hypotensive effects mainly through their alpha-blocking actions. Threo isomers 1 and 3 were resolved via diastereomeric carbamates. Erythro isomer 2 was obtained by an oxidation and reduction sequence from optically active 1. No significant difference was found between the pharmacological activities of the threo and erythro isomers of the corresponding compounds. However, a clear difference was found between the pharmacological activities of the optical isomers. Difference was most clearly shown in the hypotensive actions of normotensive rats and in alpha-adrenergic blocking activities of isolated rat vas deferens. In these actions, (+) isomers were always more potent than the corresponding (-) isomers.

A new antibiotic XK-90. II. The structure of XK-90.

The new antibiotic, XK-90, produced by Streptomyces sp. is active against Gram-positive and Gram-negative bacteria. The structure has been determined as N-acetyl-N'-(3-formyl-4-hydroxyphenyl)hydrazine (1) and is the second example of a naturally occurring antibiotic having the phenylhydrazine skeleton.
