[Arterial infusion of anticancer agent and lipiodol using a totally implanted drug delivery system].

We attempted arterial infusion of anticancer agent using a totally implanted drug delivery system in 52 patients who had inoperable liver cancer or the scheduled adjuvant chemotherapy after hepatic resection. The response rate of the cases using lipiodol was 38%, while that of the cases using only ADM and MMC was 0%. We studied changes in serum concentration of ADM and MMC. The results indicated that using 60% Urographin to make ADM, MMC-lipiodol emulsion was effective for targetting and control release.

High serum levels of DUPAN2 antigen and CA19-9 in pancreatic cancer: correlation with immunocytochemical localization of antigens in cancer cells.

We determined the concentration of serum DUPAN2 and CA19-9 in 43 pancreatic cancer patients by enzyme immunoassay, and compared the staining patterns of the antigens in the tissues of pancreatic cancer in order to clarify the mechanism of the elevation of serum DUPAN2 and CA19-9 levels in the patients. In 26 patients (60%), the serum DUPAN2 concentration was higher than 300 U/ml. This positive rate was not so high as that of CA19-9. However, seven out of the twelve CA19-9-negative patients were DUPAN2-positive. Using a combined assay of serum DUPAN2 and CA19-9, the diagnostic sensitivity for pancreatic cancer was 88% (38/43). Immunocytochemically, both DUPAN2 and CA19-9 were restricted to the apical surface and/or supranuclear cytoplasm in the normal pancreatic duct epithelia. In cancerous glands, however, the two were found over the entire surface and cytoplasm of the cancer cells--losing the polar distribution pattern of the antigen--and in the surrounding stroma adjacent to the cancer cells. DUPAN2 was detected in 47 (89%) out of 53 adenocarcinomas of the pancreas, and CA19-9 in 44 cases (83%). Cases with high serum antigen levels tended to display high proportions of stromal staining in the cancer tissues. These findings suggest that shedding of the antigen into the stroma adjacent to the malignant glands is one of the major mechanisms in the elevation of high serum DUPAN2 and CA19-9 levels.

[The present status and future of liver transplantation].

Since the introduction of cyclosporine the survival rate of orthotopic liver transplantation (OLT) has been dramatically improved (74% and 64% at one and 5 years reported from Pittsburgh). Additional contributing factors for this improvement are refinement of surgical techniques including using biopump for veno-veno bypass, introduction of OKT3 monoclonal antibody to treat acute allograft rejection, and adoption of new hepatic preservation fluid named as University of Wisconsin solution. There are, however, several continued threats of primary graft dysfunction, hepatic arterial thrombosis, early acute refractory rejection, and so on. Retransplantation is the only hope for these patients, the possibility of which is not negligible. The financial problem is also very large burden to OLT and the cost ranges from $135,000 to $238,000, which is twice of heart transplant and six times of kidney transplant. In our country the biggest barrier to starting OLT is social and medical agreement of brain death which seems hopefully coming close to resolution through persistent public discussion. Much more vigorous efforts should be made for the establishment of more efficient organ procurement system including donor card registration.

[Prospective adjuvant chemotherapy of mitomycin C plus HCFU for colorectal cancer (2d report)--6-year survival after surgery: Tokai Cooperative Study Group of Surgical Adjuvant Chemotherapy].

A prospective randomized controlled trial of adjuvant chemotherapy for colorectal cancer was performed in forty-one institutions in Tokai district. In this study, effect of postoperative administration of mitomycin C (MMC) and followed by HCFU for more than 3 months (group B) was on the survival time compared with MMC alone (group A). Of 173 patients subjected to curative resection (group A = 80, group B = 93) for colorectal cancer, 148 patients including 69 cases of group A and 79 cases of group B were eligible for survival study. At six years after surgery, Kaplan-Meire's life table showed that the survival of group B was superior to that of group A (generalized Wilcoxon test: p = 0.0446). The difference of survival curves was more distinct (generalized Wilcoxon test: p = 0.0226) for patients of advanced stage those to have lymph node metastasis time and serosal inversion.

[Pharmacokinetic and clinical studies of ceftizoxime in obstetrical and gynecological field (2)].

Pharmacokinetic and clinical evaluations of ceftizoxime (CZX) in 3 obstetrical and gynecological clinics have substantiated the therapeutic usefulness of this drug in the relevant specialty. The results are summarized below. 1. The peak serum CZX concentration after drip infusion of 2 g given over 60 minutes was 115.3 micrograms/ml. The peak CZX concentration in the pelvic dead space exudate was 34.10 micrograms/ml which was attained 2.02 hours after beginning infusion. Half-lives of CZX in the serum and in the pelvic dead space exudate were 1.64 hours and 3.65 hours, respectively. 2. The passage of infused CZX to the umbilical cord serum was satisfactorily rapid, as evidenced by figures reaching 10 micrograms/ml or higher at 1 hour and 15 minutes and still as high as 3 micrograms/ml or higher at 6 hours after administration. 3. The passage of CZX to the milk of mothers receiving this drug was low, and no untoward effect on the infant was likely. 4. CZX would effectively prevent infections due to premature ruptures of membrane.

[Basal encephalocele--a report of two cases and consideration of its pathogenetic classification].

The authors have experienced two cases of basal encephalocele without intra and extra cranial anomalies. We have reviewed previous reported cases and studied, from the view point of developmental pathology, reasons why these cases did not have associated anomalies. In this paper we will report our two cases of basal encephalocele and propose a new classification for it, based upon the developmental pathology of the basal part of the skull and associated anomalies. Case 1. A 32 year old male complained of continuous rhinorrhea for 8 months. Coronal CT scan demonstrated a defect of the right sphenoid and ethmoid bone, and a protrusion of isodensity mass, which was enhanced by administration of metrizamide in the subarachnoid space. During the operation, the protruded mass was removed, and the defects of bone and dura mater were repaired. Postoperative course was uneventful. Case 2. A 33 year old male was referred from the department of otology to the department of neurosurgery, because of rhinorrhea and the presence of brain tissue at the time of nasal operation 3 months before. Coronal CT scan revealed a defect of the left ethmoid bone and a protrusion of isodensity mass in it. RI cisternography clearly demonstrated the leakage of CSF through the left nasal cavity. During the operation, the protruded mass was removed and the bone and dura mater defects were repaired. Postoperative course was uneventful. The common findings of both cases are that they were diagnosed by accident in adulthood cases of rhinorrhea, and midsagittal basal bone structure was preserved without extra and intra cranial anomalies.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fundamental and clinical evaluation of imipenem/cilastatin sodium in the perinatal period].

To evaluate the efficacy of imipenem/cilastatin sodium (IPM/CS) in the field of obstetrics and gynecology, fundamental (measurement of IPM/CS concentrations in mothers' milk) and clinical studies were performed. Concentrations of IPM/CS in mothers' milk were measured every 1 hour up to 6 hours after a 30-minute drip infusion of 500 mg/500 mg of IPM/CS. IPM/CS was distributed similarly to other cephalosporins. In the clinical study, a 500 mg/500 mg dose of IPM/CS was administered to 5 patients with puerperal intrauterine infections and to 3 patients with urinary tract infections by a 30-minute drip infusion b.i.d. or t.i.d. Good responses were observed in all 8 patients, though the infections were mild or moderate. From these results, IPM/CS appeared to be a useful drug for the treatment of perinatal infections.

Excess release of substance P from the spinal cord of mice during morphine withdrawal and involvement of the enhancement of presynaptic Ca2+ entry.

Possible presynaptic mechanisms involved in the naloxone-precipitated withdrawal jumping in morphine-dependent mice were studied. The withdrawal jumping precipitated by intrathecally (i.t.) administered naloxone (10 nmol) was abolished by the i.t. pretreatment with a substance P antagonist, [D-Pro4,D-Trp7,9,10]-substance P4-11 (10 nmol). The high KCl-evoked 45Ca2+ entry into synaptosomes and the release of immunoreactive substance P from slices were significantly enhanced by 10(-4) M naloxone in the morphine pelleted spinal cord, but not in the vehicle pelleted ones. These findings suggest that naloxone activates the depolarization-evoked Ca2+ entry into the nerve terminals, enhances the release of substance P from the spinal cord and the induction of withdrawal jumping in morphine-dependent mice then follows.

Presynaptic opioid kappa-receptor and regulation of the release of Met-enkephalin in the rat brainstem.

Opioid kappa-agonists had much more potent inhibitory effects on the high K+-evoked Met-enkephalin release from rat brain slices than did the mu- or delta-agonists. The opioid kappa- antagonist, MR2266 enhanced the evoked release of Met-enkephalin to a greater extent than did mu- or delta-antagonists in vitro and had a potent analgesia in mice in vivo. These findings suggest that the release of Met-enkephalin may be regulated in vitro and in vivo, mainly by presynaptic kappa-receptor-mediated mechanisms.
