Aberrant left gastric vein directly draining into left portal venous system. A case report.

A 66-year-old man with early gastric cancer and liver cirrhosis was diagnosed by preoperative angiography as having an aberrant left gastric vein communicating directly with the left lateral portal vein system. This communication was confirmed during operation for the gastric cancer. Our report is the first of an aberrant left gastric vein showing direct communication with the left portal vein system.

[Serial MRI findings in neuro-Behçet disease].

Behçet disease is a systemic disorder characterised by the triad of recurrent aphthous ulcers of the mouth, genital ulcers and uveitis. Neurological involvement is estimated at 10-25% in Behçet disease (neuro-Behçet). These include diplopia, pseudobulbar palsy, cranial nerve palsies, cerebeller ataxia, and cerebral and spinal sensory and motor disturbances. A case of neuro-Behçet disease is reported. A 51-year-old man was admitted with TIA attack. He had been suffering from recurrent oral and genital ulcers for several months before admission. Neurological examination on admission revealed poor mental activity, left facial nerve palsy and left hemiparesis. Lumbar puncture showed CSF pleocytosis. CT and MRI revealed multiple lesions in the cerebral hemisphere and the brain stem. CT showed spotty high density areas with perifocal low density areas in the frontal, temporal and parietal lobe which were enhanced with contrast materials. T1 weighted image of MRI revealed iso intensity areas with perifocal low intensity areas which were enhanced with Gd-DTPA in the frontal, temporal and parietal lobes. T2 weighted image revealed low intensity areas with perifocal high intensity areas in the same regions shown in the T1 weighted image. Moreover ring-like enhanced lesions with Gd-DTPA were revealed in the brain stem and corona radiata in the T1 weighted image. After high dose steroid treatment, he showed marked clinical improvement. CSF pleocytosis was normalized and the lesions were gradually reduced in size and were no longer enhanced with Gd-DTPA. MRI findings are well correlated with clinical features.(ABSTRACT TRUNCATED AT 250 WORDS)

Renotropic functions of hepatocyte growth factor in renal regeneration after unilateral nephrectomy.

Hepatocyte growth factor (HGF), a most potent growth factor for mature hepatocytes may act as a trigger for liver regeneration. We reported that HGF strongly stimulates DNA synthesis of rabbit renal tubular cells in secondary culture (Igawa, T., Kanda, S., Kanetake, H., Saitoh, Y., Ichihara, A., Tomita, Y., and Nakamura, T. (1991) Biochem. Biophys. Res. Commun. 174, 831-838). To investigate whether or not HGF is involved in renal regeneration, we examined changes in HGF mRNA, HGF activity, and HGF receptor in the rat kidney following unilateral nephrectomy or treatment with carbon tetrachloride (CCl4). In the intact kidney, the HGF mRNA increased markedly reaching a maximum 6 h after unilateral nephrectomy, followed by an increase of HGF activity at 12 h after the surgery. The marked increase in HGF mRNA and HGF activity was also found in the kidney of rats treated with CCl4. Results of in situ hybridization suggested that cells producing HGF in the kidney are endothelial cells. The number of HGF receptors on renal plasma membranes decreased to 30% of the normal value 12 h after unilateral nephrectomy, with no change in the Kd value. The HGF receptor was greatly diminished 24 h after the operation, and recovery to 60% of the normal level was evident 1 week after the operation. Because the decrease in HGF binding may result from internalization of the HGF receptor, the HGF may bind to its receptor in vivo and act as a mitogen for renal epithelial cells. HGF may function as a renotropic factor during renal regeneration after kidney injury.

Hepatocyte growth factor has potent anti-proliferative activity in various tumor cell lines.

Hepatocyte growth factor (HGF) has potent mitogenic activity for mature hepatocytes and various normal epithelial cells. We now have evidence that HGF at 1-10 ng/ml, strongly inhibits the growth of HepG2 hepatocellular carcinoma cells, B6/F1 melanoma cells and KB squamous carcinoma cells. These tumor cells express high affinity receptors for HGF with a Kd of 25-28 pM, similar to findings with hepatocytes. HGF at 1-100 ng/ml had no significant cytolytic effect on tumor cells. Therefore, the anti-proliferative effect of HGF on tumor cells seems to be cytostatic, not cytolytic. As HGF apparently has bidirectional effects on cell growth, the possibility that it can serve as an anti-tumor agent merits attention.

[Value of "additional arterial embolization" to bilateral internal iliac TAE in patients with severe pelvic fractures].

Transcatheter arterial embolization (TAE) of the bilateral internal iliac arteries has been shown to be of great value in controlling retroperitoneal massive hemorrhage associated with pelvic fractures. It has recently been reported, however, that TAE of only the bilateral internal iliac arteries sometimes fails to stabilize the patient's hemodynamics. The purpose of the present study was to evaluate "additional arterial embolization" to the bilateral internal iliac TAE in hemodynamically unstable cases. Sixty-eight patients who underwent emergency TAE following severe pelvic fractures were reviewed. Additional arterial embolization of the lumbar arteries in four patients, middle sacral arteries in two and inferior epigastric artery in one was carried out following bilateral internal iliac TAE. The damage to these arteries was associated with fractures of the lumbar costal process, sacrum, pubis, etc. The patients were in stable condition after the procedure, and no significant complication was experienced. When fractures and dislocations of the above-mentioned bones are seen on plain X-ray films and arterial bleeding is identified by angiography, additional embolization of the arteries should be performed immediately.

Hepatocyte growth factor is a potent stimulator of human melanocyte DNA synthesis and growth.

Hepatocyte growth factor (HGF) is a potent mitogen for adult rat hepatocytes in primary culture. HGF stimulates growth and DNA synthesis of normal human epidermal melanocytes in culture. The maximal stimulation of DNA synthesis by 4.0-fold occurred with 10 ng/ml HGF. This stimulatory effect was additive with both acidic and basic fibroblast growth factors, while it was inhibited by transforming growth factor-beta 1. Melanocytes expressed a single class of specific, high-affinity receptors for HGF with a Kd of 22 pM and approximately 120 receptors/cell. Thus, HGF is a potent mitogen for normal human epidermal melanocytes.

[An approach to tuberculosis in newcomers from Asia and Africa].

In recent years, it has been documented that tuberculosis frequently occurs among recently entered foreigners dominantly from Asia. We studied 85 cases admitted to our hospital for active tuberculosis from 1986 to 1990. Many of those cases, we believe, were infected in countries of origin and were reactivated soon after or before entering Japan. In spite of higher rate of involving recurrent cases and resistance to anti-mycobacterial drug agents, chemotherapy has been generally efficient. In some cases, continuance of treatment was difficult because of their illegal stay, misunderstanding of the disease or other problems such as customs and economic difficulties.

[A case of mediastinal mature teratoma with elevated of CA 19-9 in tumor fluid].

A 20-year-old female was diagnosed as mediastinal tumor and admitted to our hospital for the operation. Among the preoperative examinations, the serum level of CA 19-9 was within normal limits. The fluid inside the cystic tumor which was removed during the operation showed high levels of CA 19-9. We discussed the relationship between CA 19-9 and cystic lesions. Histopathological study revealed that CA 19-9 was produced in only epithelial cell of the cystic lesion. And we speculated that examination of serum CA 19-9 helps the diagnosis of cystic lesions.

Effect of an iso-osmolar contrast medium on pulmonary arterial pressure at pulmonary angiography.

A clinical comparison of the effects on pulmonary arterial pressure induced by contrast media with various osmolalities, iohexol 140 mg I/ml (300 mosm/kg H2O), iohexol 300 mg I/ml (690 mosm/kg H2O), and diatrizoate 292 mg I/ml (1480 mosm/kg H2O) following selective pulmonary angiography was made in 12 patients with normal pulmonary arterial pressure. A double-blind crossover study was performed and the contrast media were administered in random order. The pulmonary arterial pressure was recorded continuously before, during, and for 3 min after the injection. The effect of iohexol 140 on the pulmonary arterial pressure was significantly less marked than that of diatrizoate 292, whereas no statistical significance was shown between iohexol 140 and iohexol 300. These results indicate that iso-osmolar contrast medium (iohexol 140), as well as iohexol 300, would be better tolerated than diatrizoate 292, and is therefore a safer contrast medium for selective pulmonary angiography.

[Clinical assessment of rotational digital angiography for the diagnosis of aortic dissection].

Rotational digital angiography was performed in five patients with aortic dissection. Intimal tears, intimal flap and involvement of major branches were clearly demonstrated in two patients with opacified false lumen. Ulcer-like projections were also identified in three patients with thrombosed false lumen. The device has, therefore, been found to be of great value in diagnosing aortic dissection particularly for the detection of intimal tear sites and ulcer-like projections.

[Change of renal function immediately after angiography].

Abdominal angiography using ioxaglate (3.0-4.5 ml/kg) was performed in 10 patients with normal renal function. Ccr, FENa, CH2O were compared before and immediately after angiography. There was no significant change in Ccr and CH2O level, while FENa increased remarkably after angiography. This study suggested that the osmotic diuresis and the proximal tubular stress were induced by administrated low osmolality contrast material during conventional abdominal angiography.

Changes in HBsAg carrier rate in Goto Islands, Nagasaki Prefecture, Japan.

Annual mass examinations in an area where hepatitis B virus (HBV) infection is very prevalent revealed that 12.1% of inhabitants born during 1946-50 were positive for hepatitis B surface antigen (HBsAg), compared with only 0.6% of those born during 1971-75. To find out why the HBV carrier rate has fallen, changes in the modes of HBV infection were examined. The HBsAg positivity rate among mothers who gave birth to HBV carrier children in 1965 and before was 26.8% and that for such mothers whose babies were born in 1966 and after was 66.7%, whereas the HBsAg positivity rate among children born to HBV carrier mothers in 1965 and before and in 1966 and after were 30.8% and 28.3%, respectively. None of the 503 inhabitants who had no HBV markers in 1976 had become carriers by 1981. These findings indicate that the decrease in the prevalence of HBV carriage is caused mainly by the reduction in occurrence of horizontal transmission of HBV in infancy.

Inhibition of epidermal growth factor-induced DNA synthesis by tyrosine kinase inhibitors.

We prepared methyl 2,5-dihydroxycinnamate as a stable analogue of erbstatin, a tyrosine kinase inhibitor. This analogue was about 4 times more stable than erbstatin in calf serum. It inhibited epidermal growth factor receptor-associated tyrosine kinase in vitro with an IC50 of 0.15 micrograms/ml. It also inhibited in situ autophosphorylation of epidermal growth factor receptor in A431 cells. Methyl 2,5-dihydroxycinnamate was shown to delay the S-phase induction by epidermal growth factor in quiescent normal rat kidney cells, without affecting the total amount of DNA synthesis. The effect of erbstatin on S-phase induction was smaller, possibly because of its shorter life time.

[Histologic characterization of late asthmatic response in guinea pigs].

We examined lung tissues in a guinea pig model actively sensitized by inhalation of aerosolized ovalbumin (OA) and found reproducible late bronchial responses (LBR) 7 h and 24 h after OA challenge. Light microscopic examination revealed bronchoconstriction, damage to the epithelium, eosinophil infiltration in both the epithelium and subepithelium, retention of mucous in the pulmonary bronchial lumens and a decrease in the mucous content of Periodic Acid-Schiff (PAS) stain positive cells in the bronchial epithelium in LBR. An increase in the number of PAS-stain positive cells of in the bronchial epithelium was also observed in LBR. Electron microscopic examination revealed eosinophil migration around mast cell at 7 hs after OA challenge. Two different mechanisms of degranulation of eosinophil specific granules were observed in LBR. In one mechanism, the granule core changes after its matrix has changed. In the other mechanism, the granules were exocytosed as a whole, accompanied by the cell membrane and densely clumped chromatin, suggesting burst of eosinophil.

[An approach to the control of the so-called vagrant patients with tuberculosis].

In recent 3 years, we experienced 46 cases of so-called vagrants, who were admitted for active tuberculosis to our hospital at the time of detection, their disease was in advanced and severe state, and 35 out of 46 patients were admitted for emergency and 7 cases died of tuberculosis soon after admission. In the remaining cases, anti-tuberculosis agents were effective, however, the incidence of dropout cases was much higher. How to improve compliance of these cases to treatment is one of the most difficult but important problem in present tuberculosis control.

Portal hypertension secondary to intrahepatic arterio-portal shunt in primary amyloidosis: a case report.

Portal hypertension is a rare complication in hepatic amyloidosis. We experienced a case of a 40-year-old man with primary amyloidosis and advanced esophageal varices. Angiographic procedures clearly demonstrated portal hypertension secondary to intrahepatic arterio-portal shunting. Hepatic arterial embolization brought about a disappearance of the A-P shunt and portal hypertension, though rebleeding occurred. To our knowledge, this is the first case report in which the pathogenesis of portal hypertension in hepatic amyloidosis was elucidated.

[Combination with medroxyprogesterone acetate and chemotherapy making possible complete removal of uterine corpus carcinoma].

A case of uterine corpus carcinoma (stage III) after surgical resection of her primary lesion was treated with a medroxyprogesterone acetate (MPA) and PAE (CDDP, adriamycin and etoposide) regimen. The patient has been given MPA during a three-month period and received four courses of PAE regimen. The residual tumor was decreased and removed completely at the second-look operation. Pathologically, the carcinoma cells were clearly changed in loss of atrophy or degeneration by MPA or PAE regimen. After the second-look operation, the patient underwent 4 courses of PAE regimen and was given MPA continuously. We have not recognized any recurrence of carcinoma for seven months after the second look operation.
