Post-traumatic cyst of the liver found on CT scan--a new concept.

Six patients with traumatic cyst of liver are reported. Among them three patients without clinical manifestation had cysts detected by followup study with CT scan, which has been routinely performed since 1978 in 14 patients with blunt injuries of liver. Two cases of spontaneous cure of traumatic cyst are also reported. There was no relationship between the severity of the injury and cyst formation. Clinical signs, such as abdominal pain, tenderness, or distention were observed in only two of the six patients. We consider, therefore, that traumatic cysts of liver may occur much oftener than has been commonly accepted and that most of them may cure spontaneously. Elevation of alkaline phosphatase later after injury may suggest expansion of the cyst. In such cases CT examination is mandatory.

[A new indicator to determine the optimal PEEP (author's transl)].

To elucidate the optimal PEEP for respiratory treatment, respiratory and haemodynamic studies were performed on 12 normovolemic patients with ARDS for various levels of PEEP (PEEP = 0, 5, 10, 20 cm H2O). In this study, it became clear, that Suter's best PEEP (maximal O2 transport should be taken as a reference for optimal PEEP) cannot be used in practice because O2 transport is usually maximal in ZEEP (PEEP=0) and changes with the alteration of FIO2. We propose "intrapulmonary nonshunt flow ((Qt-Qs)" as a new indicator to determine the optimal PEEP. The level of PEEP to achieve the maximal intrapulmonary non-shunt flow must be such that the decrease in intrapulmonary shunt flow is attained with minimal decrease of cardiac output.

Novel peptidoaminobenzophenones, terminal N-substituted peptidoaminobenzophenones, and N-(acylglycyl)aminobenzophenones as open-ring derivatives of benzodiazepines.

Peptidoaminobenzophenones (1), terminal N-substituted peptidoaminobenzophenones (14), and acylglycylaminobenzophenones (16) were prepared as a novel series of ring-opened derivatives of 1,4-benzodiazepine. Z-Gly- and Z-Ala-N-methylaminobenzophenones (4) were treated with HBr-HOAc to give Gly- and Ala-N-methylaminobenzophenone hydrobromides (8). Reaction of 8 with chloroacetyl chloride in dimethylformamide (DMF) or hexamethylphosphoramide (HMPA) gave chloracetamide (13), which was allowed to react with various amines to afford a number of terminal N-substituted derivatives (14). Reaction of 8 with various acyl halides in HMPA or DMF gave a number of acylglycyl-N-methylaminobenzophenones (16). Peptidoaminobenzophenones (1) were also prepared by several convenient methods. Many of these compounds exhibited high CNS activity in animals when given orally. In antianxiety activity the potency of some compounds is equal to or higher than that of diazepam.

Effect of HLA-DR matching between living kidney donors and recipients and of pretransplant blood transfusions on graft survival.

The effect of HLA-DR matching between living kidney donors and related recipients on 1-year graft survival was examined in 35 transplant recipients excluding 2-haplotype identical siblings. All 11 DR-compatible grafts survived 1 year; 7 of 24 DR-incompatible grafts ceased to function within 1 year (p less than 0.05). Among 24 DR-incompatible recipients, 6 of 11 who had preoperatively been transfused with less than 1000 ml blood lost their graft function. On the other hand, 12 of 13 who had received at least 1200 ml blood maintained their graft function beyond 1 year (p less than 0.05). All DR-compatible recipients received pretransplant blood transfusions and all produced anti-DR antibodies against random panel B cells after transplantation, but not against the graft donor's B lymphocytes. The anti-DR antibodies disappeared within 3 months. In recipients whose sera broadly reacted to random panel B cells at 1 years after transplantation, only 4 of 10 grafts survived. In recipients whose sera had lost their anti-DR reactivity within 1 year, all 20 grafts survived (p less than 0.05).

Immunosuppressive effect of bredinin in the management of patients with renal transplantation.

Clinical effects of bredinin were evaluated in 18 renal transplant patients. Of 12 cases in which administration of azathioprine was suspended due to liver dysfunction and leucopenia, treatment with bredinin resulted in amelioration of these symptoms without harm to the transplanted kidneys in 10 cases. In six other cases, bredinin was used from the time of renal transplantation, and the postoperative course in four of these cases was uneventful, enabling these patients to return to society. The remaining two patients have had to undergo haemodialysis again as their transplanted kidneys failed to function and they showed severe stomatitis in the early postoperative period. The results of clinical use of bredinin in renal transplant patients suggest that this drug is an effective immunosuppressive agent which is without the drawbacks seen with azathioprine. Nonetheless, one must bear in mind that the function of the transplanted kidney should be evaluated carefully before using this drug.

1H-NMR and rosonance Raman spectra of octaethylporphyrinatoiron(III) perchlorate and its mono imidazole adduct.

The 1H-NMR spectra and the resonance Raman spectra of intermediate spin complex, octaethylporphyrinatoiron (III) perchlorate (OEP-Fe(III)ClO4) and its mono imidazole adduct have been recorded and analyzed. The perchlorate complex was determined to be an intermediate-spin state (S = 3/2) in dichloromethane. The mono imidazole and 2-methylimidazole adducts of OEP-Fe(III)ClO4 were of the high-spin state in dichloromethane, which is a good model for the ferrihemoproteins such as metmyoglobins. The spin state of OEP-Fe(III)ClO4 varies the polarity of solvent from typical high-spin (S = 5/2) to typical low-spin (S = 1/2) state including intermediate-spin state (S = 3/2). The resonance Raman studies of the intermediate-spin complex in various solvents indicate that the complex is a plausible model to reproduce anomalous physico-chemical properties of the ferricytochrome c' at physiological condition.

Peptidoaminobenzophenones, a novel class of ring-opened derivatives of 1,4-benzoidazepines.

A series of noval peptidoaminobenzophenones has been prepared via several routes and was evaluated for CNS activity. The structure--activity relationships in the series are discussed. In general, dipeptido-N-methylaminobenzophenones showed higher activities than the corresponding NH derivatives. Some compounds had very high activities in antipentylenetetrazole and antifighting tests in mice when orally administered. Very weak toxicity was also found in these compounds. Water solubility of the peptidoaminobenzophenones and their salts were tested. Possible in these compounds. Water solubility of the peptidoaminobenzophenones and their salts were tested. Possible in vivo conversion of peptidoaminobenzophenone by enzymatic cleavage of the terminal amino acid, followed by chemical cyclization to 1,4-benzodiazepine, is also discussed. Such novel open-ring derivatives of 1,4-benzodiazepine may serve as useful CNS agents.
