Ion-sensitive field effect transistor as a monovalent cation detector for ion chromatography and its application to the measurement of Na+ and K+ concentrations in serum.

An ion-sensitive field effect transistor, which is a small potentiometric ion-sensing probe, was applied as a detector in the ion chromatography of alkali-metal cations. The cation-sensing transistor was prepared by coating the gate part of the probe to form a poly(vinyl chloride) matrix membrane containing tetranactin, which enables detection of alkali-metal ions such as Na+, K+, Rb+, and Cs+. To be able to analyse amounts less than 1 microliter and make full use of the small ion-sensing probe, we constructed a miniature cation-exchange separation column (support 10 microns polystyrene) with a PTFE tube (20 mm x 1.5 mm I.D. x 2.1 mm O.D). As an application of the system, Na+ and K+ concentrations in serum were determined. The analytical results for these two cations were good agreement with those obtained by flame photometry.

Glucocorticoids suppress and oestrogens enhance the lipopolysaccharide-induced increase in putrescine and N1-acetylspermidine in mouse liver.

Previously we reported that administration of lipopolysaccharide (LPS) to mice increased the hepatic levels of putrescine (PUT) and N1-acetylspermidine (N1-acetyl-SPD). In the current study, we examined the in vivo effects of some steroid hormones on the LPS-induced increase in PUT and N1-acetyl-SPD. Corticosterone, hydrocortisone and dexamethasone suppressed the LPS-induced increase in PUT and N1-acetyl-SPD in mouse liver in a dose-dependent manner, dexamethasone being the most effective among them. On the other hand, oestrone and oestradiol-17 beta enhanced the LPS-induced increase in PUT and N1-acetyl-SPD in a dose-dependent manner. Oestradiol-17 alpha and 16 beta-ethyl-oestradiol, as an inactive oestradiol isomer and an antioestrogen, respectively, likewise enhanced the increase in PUT and N1-acetyl-SPD concentrations induced by LPS. 16 alpha-hydroxy-oestradiol (oestriol), 16 alpha-hydroxyestrone, 2-hydroxyoestradiol, 2-hydroxyoesterone, progesterone, testosterone, diethylstilboestrol and nonsteroidal antioestrogens such as tamoxifen and nafoxidine had no effect on the increase. Oestradiol-17 beta enhanced and corticosterone had little effect on the carbon tetrachloride-induced increase in PUT and N1-acetyl-SPD. These results suggest that glucocorticoids suppress the increase by preventing the immunological injury by Kupffer cells on hepatocytes and that the stimulatory effect of oestrogens may not be associated with their oestrogenic activities mediated by the oestrogen receptor system.

Metabolic changes in patients severely affected by tetanus.

Metabolic changes in six severely affected tetanus patients suffering from characteristic labile hypertension (maximum systolic blood pressure greater than 200 mmHG, maximum diurnal change in systolic pressure greater than 100 mmHg) were investigated. Daily urinary excretion of urea nitrogen increased gradually from the onset of opisthotonus, reached a peak value (10.4 to 15.4 g/m2) in 8 to 20 days, and decreased subsequently. Average cumulative excretion in 30 days reached 239.6 +/- 32.7 g/m2. Urine catecholamine excretion was elevated in each patient and remained elevated during this period. Plasma cortisol and glucagon concentrations were not increased markedly except in a case complicated other systemic bacterial infection. Increased protein catabolism in these patients could not be explained by the metabolic effects of 'stressed hormones' alone, and neurologic factors must be considered.

Role of granulocyte elastase in tissue injury in patients with septic shock complicated by multiple-organ failure.

To better understand the role of granulocyte elastase (GE) in mediating tissue injury during sepsis, GE levels were measured in plasma and bronchoalveolar lavage fluid (BALF) in patients with septic shock (n = 16) and hemorrhagic shock (n = 30). Granulocyte elastase levels were compared to levels of alpha 1-protease inhibitor (alpha 1-PI). Results show that although plasma GE-alpha 1-PI complex was initially elevated in patients with hemorrhagic and septic shock, elevations in plasma GE-alpha 1-PI complex (831 +/- 241 micrograms/L) persisted in septic shock patients. alpha 1-Protease inhibitor levels in serum were increased, resulting in an inhibition of serum GE activity. Granulocyte elastase activity in BALF, however, was significantly higher in those patients with septic, as compared to hemorrhagic shock (31.4 +/- 25.8 versus 3.7 +/- 4.0 U/L, respectively). In addition GE levels were compared to other parameters, including respiratory index, blood neutrophil count, and plasma levels of endotoxin, fibronectin, and coagulation factor XIII. Significant correlations were observed between GE-alpha 1-PI and increased endotoxin concentration and decreased fibronectin and coagulation factor XIII levels. Significant correlation was found also between GE activity in BALF and respiratory index. These findings suggest that severe tissue damage occurred in patients with septic shock complicated by multiple-organ failure. Although GE activity appeared to be adequately inhibited by alpha 1-PI in blood, increased GE activity in local tissues, such as lung alveoli, may be responsible for significant local tissue injury during septic shock.

Acceleration of superoxide production from leukocytes in trauma patients.

Superoxide (O2-) and granulocyte elastase (GE) from neutrophils mediate host defense and tissue injury in inflammation. To determine alterations in leukocyte function after trauma, O2- production and GE secretion from neutrophils were studied in trauma patients (n = 20) and healthy controls (n = 15). The priming effect of tumor necrosis factor (TNF), interleukin-1 alpha (IL-1 alpha), and lipopolysaccharide (LPS) on O2- or GE release also was evaluated. Superoxide production (nmole/10 minutes) was elevated significantly in trauma patients at days 0 (9.5 +/- 4.8), 1 (14.2 +/- 7.3), and 3 (12.2 +/- 5.9) and returned to control levels (4.2 +/- 1.6) by day 7. There was no difference in GE secretion between trauma patients and healthy controls. Incubation of neutrophils with TNF induced release of both O2- and GE. Superoxide production was induced by TNF at concentrations at or greater than 10(-11) mol/L. Granulocyte elastase secretion was induced in a time- and dose-dependent manner by TNF at concentrations between 10(-10) and 10(-7) mol/L. In contrast IL-1 alpha and LPS did not potentiate O2- or GE release. These results suggest that neutrophil O2- production increases acutely in trauma. Tumor necrosis factor may mediate this O2- and GE production by neutrophils involved in the inflammatory response.

Intrahepatic arterioportal fistula after blunt hepatic trauma: case reports.

Intrahepatic arterioportal fistula (APF) was found in five out of 65 consecutive patients following blunt hepatic trauma. In four patients the fistula was located peripherally and the blood flow was small. These fistulas closed spontaneously within 3 months. However, a centrally located fistula with early visualization of the trunk of the portal vein persisted in one patient and necessitated transcatheter embolization. The APF in this patient caused portal dilatation which was detectable by CT scan. We conclude that spontaneous closure can be expected when an APF is located peripherally and the shunt flow is small, while centrally located APF with large flow require active treatment, preferably by transcatheter embolization. An APF detectable by CT scan suggests the need of intervention.

Pulmonary contusion causes long-term respiratory dysfunction with decreased functional residual capacity.

To elucidate the mechanism of persistent dyspnea after blunt chest trauma, we prospectively studied the pulmonary function of 18 patients with blunt chest trauma for 6 months. Nine of the patients had flail chest and 12 had pulmonary contusion (PC). Pulmonary function was evaluated using spirometry, arterial blood gas analysis, chest x-ray studies and CT scans. Functional residual capacity (FRC) remained significantly reduced throughout the 6 months in patients with PC. Such patients experienced a fall in Pao2 when changed from a sitting position to a supine position and they had fibrous changes in the contused lung as demonstrated by CT scans at 6 months after injury. These findings were supported in an additional study of another 20 patients who had suffered PC 1 to 4 years previously. This study demonstrated that pulmonary function recovered within 6 months in patients without PC even with a residual deformity of the thoracic wall caused by flail chest, while patients with PC had decreased FRC and a fall in Pao2 when moved to the supine position even several years after injury. This might be related to the persistent dyspnea seen after blunt chest trauma.

Hemodynamic response with passive neck flexion in brain death.

Twelve brain-dead patients admitted to the Department of Traumatology, Osaka University Hospital, from July 1988 to August 1989 were studied. A hemodynamic response elicited by passive neck flexion was observed in 10 of the 12 patients. After passive neck flexion, blood pressure began to rise, and the heart rate increased slightly. Blood pressure peaked about 2 minutes after the initiation of neck flexion and then decreased gradually to the baseline level within a few minutes. These responses were suppressed completely by administration of the ganglion blocker trimethaphan camsilate, which suggests that the efferent pathway of the response is mediated by the sympathetic nervous system.

Immunological abnormalities in Kawasaki disease with coronary artery lesions.

A review of our previous immunological studies on Kawasaki disease (KD) was undertaken. The results showed that peripheral blood macrophages/monocytes, T-cells and B-cells become activated during acute KD in terms of numerical changes in immunocompetent cells, expression of activated antigens on the cell surfaces and cytokine production. Also, during acute KD with coronary artery lesions (CALs) the numbers of macrophages/monocytes are increased. In addition, both the increased levels of tumor necrosis factor-alpha and shed intercellular adhesion molecule-1 in serum are more evident in KD patients with CALs than in those without. Our results further suggest that the main characteristics of the pathogenesis of KD are increased numbers of peripheral blood macrophages/monocytes with the secretion of monokines by these activated cells, and the expression of adhesion molecules on immunocompetent cells. These immune responses develop more vigorously in KD patients with CALs.

Effect of sodium D-3-hydroxybutyrate on amino acidemia in hemorrhagic hypotension.

The effect of sodium D-3-hydroxybutyrate (3-OHB) on the amino acidemia caused by hemorrhagic stress was studied. Rats were infused 3-OHB (3-OHB group, n = 11) at a rate of 30 mumol/min.kg or the same volume of saline (control group, n = 11) for 1 h prior to hemorrhage. Mean arterial pressure was maintained at 40 mm Hg using reserved bottle for 30 min. While ketosis was suppressed in control group, plasma ketone body concentration increased from 2,062.9 +/- 223.7 to 3,151.4 +/- 794.8 mumol/l in the 3-OHB group during hemorrhage. Although significant difference in plasma concentrations of amino acids were not shown between two groups at the onset of shock, it was shown after 30 min of shock in alanine, proline, and threonine. Particularly severity of hyperalaninemia was clear. In the control group plasma alanine concentration increased as shock proceeded (p less than 0.01), and no significant change was shown in the 3-OHB group. These results indicate that pretreatment with 3-OHB suppress the hyperalaninemia seen in hemorrhagic hypotension. Ketone body administration may inhibit the protein catabolism in stressed states, such as hemorrhagic hypotension.

[Compression of body by clothing--increase in urinary norepinephrine excretion caused by foundation garments].

The urinary excretions of norepinephrine (NE) and epinephrine (EP) were measured to investigate the physiological effects of girdles worn by women to look attractive. 1) The urinary excretion of NE was remarkably increased by the wearing of a girdle in all the subjects examined, whether living conditions and stimulations were changed or remained unchanged. 2) The urinary excretion of NE was increased by wearing a girdle, regardless of the type of girdle or its intensity of compression. 3) The urinary excretion of NE tended to increase more by the wearing of a bodysuit having a large area of body compression than by the wearing of a girdle having a narrow area of compression. 4) The urinary excretion of NE showed no specific pattern of change, even when the subjects were accustomed to the use of a girdle. These findings may indicate that a girdle of even the best-fitting size exerts a kind of stress on the human body.

Effect of cell density on growth hormone release from, cyclic AMP levels in and peptide alpha-amidation activity of primary cultured rat anterior pituitary cells.

To investigate the effect of cell density on rat somatotrophs, dispersed adult rat anterior pituitary cells were plated at several densities (0.6, 1.2, 2.4 and 6.0 x 10(5) cells/cm2) or at two densities (6.07 x 10(5) and 1.21 x 10(5) cells/cm2) as high and low densities respectively. A static incubation system was used to study the release of GH and peptidyl glycine alpha-amidating enzyme (PAM; a component of secretory granules) and the cellular concentration of cyclic AMP (cAMP) in basal and stimulated cells. In addition, a perifusion system was used to characterize the sensitivity to GH-releasing factor (GRF) at both high and low densities. The high density of cells in both the static incubation and perifusion systems caused a low basal secretion rate of GH and PAM. When the cultured cells were stimulated with human GRF (hGRF) in perifusion, GH secretion from cells at high density was markedly higher than that from cells at low density. The cAMP content in the static incubation system showed a similar tendency to that observed for basal and stimulated GH secretion; that is, the basal cAMP content was increased as the cell density decreased. The cellular concentration of cAMP was increased by about threefold by 1 nmol hGFR/l and by more than tenfold by 10 nmol hGRF/l. When the medium from cells cultured at low density was replaced by that from the cells at high density, there was no change in the basal cellular cAMP content of the cells at low density.(ABSTRACT TRUNCATED AT 250 WORDS)

Advantages of triple therapy with mizoribine, cyclosporine and prednisolone over other types of triple and/or double therapy including cyclosporine for renal transplantation.

Mizoribine (Mz) is an analogue of azathioprine (Az) with less hepatotoxicity, being extensively used as immunosuppressant in place of the latter agent especially in Japan. However, careful comparative studies of mizoribine (Mz), cyclosporine (Cy), and prednisolone (Pr) versus azathioprine (Az), Cy and Pr or Cy and Pr in renal allotranspalnt patients have not been reported. Retrospectively we compared triple therapy with Mz, Cy, and Pr (group I, n = 50) to triple therapy with Az, Cy and Pr (group II, n = 13) and/or double therapy with Cy and Pr (group III, n = 11) in one-haplotype-identical living related renal transplantations performed between Oct. 1984 through March 1989. Initial and maintenance doses of Cy in groups I and II were largely two thirds of those in group III. Patient and graft survival rates at 3 years in each group are 100% and 92% (group I), 100% and 91% (group II), and 91% and 82% (group III). There were no statistical differences in patient and graft survival rates between these three groups. The incidences of miscellaneous complications were the same in the groups. Bone marrow suppression, however, was significantly less in group I than in group II (P less than 0.005). Cy related nephrotoxicity was apparently less in groups I and II than in group III. Estimated US $5,000 in a year can be saved by immunosuppressive treatment in a patient of group I as compared to a patient in group III. Therefore, we conclude that triple therapy with Mz, Cy and Pr is superior to those with Az, Cy and Pr, and/or double therapy with Cy and Pr.

Muscle morphometry in amyotrophic lateral sclerosis.

Quadriceps muscle biopsies from 24 patients with amyotrophic lateral sclerosis (ALS) and 15 age-matched controls were prepared for histochemistry and analyzed morphometrically. Pathological features for denervation and reinnervation were observed in most ALS patients, although considerable variation between patients was noted. Myopathic changes were also seen in one-third of the cases. The morphometric data were not only related to the duration and mean diameter of type I fiber, but also to the duration and hypertrophy factor of type II fiber, suggesting that the progression and severity of ALS depends on the preservation of both fibers.

Alpha-tocopherol and superoxide dismutase suppress and diethyldithiocarbamate and phorone enhance the lipopolysaccharide-induced increase in N1-acetylspermidine concentrations in mouse liver.

Previously, we have shown the close association between hepatic concentrations of N1-acetylspermidine and the radical-producing potency of several drugs. Since vitamin E, superoxide dismutase (SOD), and reduced glutathione (GSH) are known to scavenge free radicals, in this study we tested the effect of alpha-tocopherol, one of the most potent vitamin E isomers, and SOD on the lipopolysaccharide (LPS)-induced increase in hepatic concentrations of N1-acetylspermidine. The LPS-induced increase in hepatic N1-acetylspermidine was more than twice as great in vitamin E-deficient mice as in vitamin E-supplemented mice. Pretreatment with alpha-tocopherol suppressed the LPS-induced increase in hepatic N1-acetylspermidine in vitamin E-deficient mice. Alpha-tocopherol and SOD given to mice maintained on a usual diet likewise suppressed the LPS-induced increase in hepatic N1-acetylspermidine and putrescine. The hepatic concentrations of alpha-tocopherol and GSH were lower in LPS-treated mice than in control animals. Diethyldithiocarbamate, an inhibitor of SOD, and diisopropylidene (phorone), a GSH-depleting agent, enhanced the LPS-induced increase in hepatic N1-acetylspermidine. These results suggest that the LPS-induced hepatic increase in N1-acetylspermidine is connected with radical-induced injury in vivo and that superoxide anion is produced in the liver of LPS-treated mice.

[Growth of the aortic anastomosis in puppies--comparison of monofilament suture materials, whether absorbable or nonabsorbable, and of suture techniques, whether continuous or interrupted].

This study compared the growth of vascular anastomoses performed with either polypropylene or polydioxanone sutures and with either continuous running suture or interrupted suture technique. Primary end-to-end anastomoses of the infrarenal aorta were performed in 38 puppies. Nineteen were performed with 5-0 polypropylene sutures; in ten all sutures were placed in continuous fashion, and in nine in interrupted fashion. The other nineteen were performed with 5-0 polydioxanone sutures; in ten the sutures were placed in continuous fashion, and in nine in interrupted fashion. The animals were subjected to the repeated aortograms at 2, 4, 8 weeks, 6 months and 1 year following operation. Each abdominal aorta was subjected to burst test, and pathological examination. All anastomoses were patent and tolerated burst-test (300 mmHg). Neither aneurysm nor dilatation of anastomotic site was observed. Anastomotic area was significantly smaller and more stenotic in the continuous polypropylene suture group than in other three groups at 8 weeks, 6 months and 1 year following operation. There was no significant difference among these three groups. No suture materials were observed in the polydioxanone suture groups after 6 months following operation. This study suggests that polydioxanone suture will be useful for the repair of cardiovascular anomalies where growth of the suture line is required.

[A case report of pseudoxanthoma elasticum with upper gastrointestinal hemorrhage].

A gastric hemorrhage in a patient with pseudoxanthoma elasticum (PXE) is reported. A 44-year-old woman was admitted to our hospital with hematemesis. Bleeding point was located at upper body of the stomach. As neither endoscopic alcohol injection nor operative hemostasis by over sewing under gastrotomy were unsuccessful, total gastrectomy was performed. Histologic examination of the stomach revealed fragmentation of elastic fiber in the internal lamina of arteriole and degeneration of muscle layer. This change was observed fundamentally in all specimens obtained from different region of the stomach. In addition to describing clinical and pathological feature of this case, the management of gastric hemorrhage in patients with PXE is discussed.

[Correlation between the findings of magnetic resonance imaging and prognosis of patients with predisposing factors of aseptic necrosis of the femoral head].

Eighteen patients with the predisposing factors for aseptic necrosis of the femoral head were followed up for more than two years after the initial magnetic resonance (MR) imaging. At the time of the initial examination, 24 femoral heads showed abnormal low-intensity areas on MR images without abnormal findings on plain radiographs. Among them, six femoral heads collapsed in the follow-up period. The initial mid-coronal T1-weighted MR images of these femoral heads had shown characteristic findings, that is, band-shaped low-intensity areas with the lateral end not covered by the acetabulum. Subchondral fracture of the femoral head occurred in the vicinity of the lateral end of the band. The findings of initial MR imaging seem to predict subsequent collapse of the femoral head.
